Gvozdi (Nails)
C'è un tipo che soffre d'ansia. Tanto, tanto, tanto.

You were looking for films similar to the work of Fred Vogel and Lucifer Valentine, yes?
I've been researching extreme cinema for a few years now, and I've found a few helpful resources on the topic:
Horror News has a very long list of disturbing films that I used as a starting point. There are a lot of films here, which is why I still have much to research.
"Unboxed, Watched, and Reviewed" is a channel I subscribe to on Youtube (that honestly deserves way more subs than it has). Tanner Tooboch reviews extreme horror films, as well as notorious pornographic/fetish films (works often mentioned in discussions of "disturbing" films). Thank goodness he does, so I don't have to! (Admittedly, despite supporting and having an intellectual fascination with extreme material, I don't tend to find it too personally entertaining)
There are plenty of lists on imdb for extreme cinema and "disturbing" films, though one's mileage may vary, as I wouldn't place Cronenberg's Naked Lunch (1991) on the same level as, say, Sella Turcica (2010). Here are three lists I have bookmarked: [1] [2] [3]
That aside, I will list a few notable directors and films myself that I feel are closest to your criteria.
Anything by German director Marian Dora, as his work tends to be on the extreme side, such as in Meloncholie der Engel (2009).
Jörg Buttgereit is a trailblazer in the field of extreme cinema, beginning with Nekromantik (1987). His latest work is a short film titled Schweinchen (2020).
Gaspar Noé is a French director that frequently comes up in discussions of extreme cinema, mostly for Irreversible (2002) and I Stand Alone (1998).
Olaf Ittenbach's special effects experience has resulted in a slewcof notoriously gruesome films, such as Black Past (1989), The Burning Moon (1992), and Premutos: Der gefallene Engel (1997).
Andreas Schnaas is much like Olaf Ittenbach, though his works come off as a bit more goofy. Still, his gore effects are maniacally over the top, with kills you will only see in his work. He is perhaps best known for his Violent Shit trilogy, but has many more films under his belt. Violent Shit III should be watched with friends and plenty of shots.
Brian Paulin quickly carved a place for himself in extreme horror; his most notable films are Bone Sickness (2004), Fetus (2008), and Blood Pigs (2010).
Andrey Iskanov is a former medical student and photographer from Russia, most known for the long, grueling documentary Philosophy of a Knife (2008), which recreates many of the horrifying and inhumane experiments carried out on Russian and Chinese prisoners by Japan's infamous Unit 731 during World War II. His other work is similarly grotesque.
Eric Stanze began his career shooting on video. His most notable films are Ice from the Sun (1999) and Scrapbook (2000).
Ron Atkins has a few extreme titles to his name, including Mutilation Mile (2009), Schizophreniac: The Whore Mangler (1997), and Eat the Rich: The Cannibal Murders (2000).
Harmony Korine is not particularly well-liked as a director, mainly because he comes off as try-hard and pretentious. Still, he produced at least one decent unsettling film (Gummo), wrote the controversial and highly disturbing Kids (1995), and made the bizarre and bluntly-titled Trash Humpers (2009).
Otto Muehl is known for his fairly extreme short films, most notably Sodoma (1970), Stille Nacht (1969), and Wehrertüchtigung (1967). Günter Brus worked with Muehl, and his work is similarly notable.
Giuseppe Andrews makes...a lot of fucking weird experimental short films. Coffee Cake alone is enough for me to put him here. No clue as to gore content, but with titles like Cat Piss, Garbanzo Gas, Vagrant Womb, and Who Flung Poo?, there has to be some reason why Tanner Toobach thinks so highly of this man.
Individual films (I'll try to limit this to maybe 20. I have not seen every film I've encountered in my research (and a couple are not technically horror) , so I can really only list ones I either have seen, or have enough knowledge of to make a judgment call. I also feel compelled to feature the most notable/controversial films).
I want to make this very clear:
These films are considered "extreme cinema" for a reason. They contain material that is not for the squeamish, nor those who are easily upset. It is the filmmakers' intention to shock and disgust the viewer. These films are not made to entertain.
Pink Flamingos (1974)
Sweet Movie (1974)
Salò, or the 120 Days of Sodom (1975)
Island of Death (1976)
SXXX-80 (1980)(short film)
Guinea Pig: Devil's Experiment (1985)
Begotten (1989)
The Baby of Mâcon (1993)
Aftermath (1994)
The Bride of Frank (1996)
Cutting Moments (1997)
Nekro (1998)(short film)
The Green Elephant (1999)
Subconscious Cruelty (2000)
Visitor Q (2001)
Murder-Set-Pieces (2004)
Antichrist (2009)
The Life and Death of a Porno Gang (2009)
Madness of Many (2013)
A Serbian Film (2010)
I hope this list will be a good start down the road of extreme cinema!

151118 JazzaJ EXTRA / Inconsolable Ghost (H-HR-NL)
INCONSOLABLE GHOST: SŐRÉS Zsolt (Budapest) – mélyhegedű, elektronika, hang Miodrag GLADOVIĆ (Zágráb) – gitár, elektronika Gideon KIERS (Rotterdam) – élő elektronika Ajánlott adomány: 1500 Ft ________________________________________________________ INCONSOLABLE GHOST (H-HR-NL) – BUDAPEST CHANNELLING 2018 * Hilary Jeffery and Gideon Kiers, former members of the legendary The Kilimanjaro Darkjazz Ensemble and The Mount Fuji Doomjazz Corporation, created INCONSOLABLE GHOST in 2014, an occult audiovisual group that transmits otherworldly signals in haunted, tumbledown cinemas. The project incorporates numerous well-known musicians and artists who reckon themselves as conveyors of the Inconsolable Ghost. (That is why Inconsolable Ghost can also be a solo project; Zsolt Sőrés, who has been a permanent member of the project since 2014, acted as a medium of the alternative spectral dimension on two occasions in the last few years – at UH Fest and HaviZaj series.) Inconsolable Ghost collaborated with guest artists such as Anthea Caddy, Charlotte Cegarra, Colin Hacklander, Farahnaz Hatam, Alfredo Genovesi, Ralf Meinz, Gianpaolo Peres, Makino Takashi, Raphael Vanoli, Claus Löser, András Halmos and many others. They summon the 21st century vision of spiritism and occult science; secret messages discovered in distorted telephone signals that transmit from another dimension, aura photographies, blurry images of old analogue TV screens, enigmatic scripts and files programmed on archaic computers, self-destructing hieroglyphic emails, fast-changing pictures of long-forgotten websites... Their first album was released in September 2018 on Fourth Dimension Records titled „Sokolowsko Channelling” that is a recording of their concert at the Sokolowsko Sanatorium of Sound festival from 2017, performed in an abandoned movie theatre once important for Krzysztof Kieslowski in his childhood. The ensemble’s music is also featured in Russian far-eastern filmmaker Andrey Iskanov’s Visions of Suffering – Final Director’s Cut that was being shot for 13 years (!) before finally released on DVD and an accompanying soundtrack CD by Last Exit Entertainment in the spring of 2018. The last time they performed before a Hungarian audience was at the Capa Centre and not long afterwards on an UH event few years ago. Now, they arrive to Kék Ló from Graz and heading to the Audio Art festival in Krakow where they – together with Hilary Jeffery – will present their new audiovisual performance with 360° projection. INCONSOLABLE GHOST: Zsolt SŐRÉS (Budapest) – viola, electronics, voice Miodrag GLADOVIĆ (Zagreb) – guitar, electronics Gideon KIERS (Rotterdam) – live electronics Suggested donation: 1500.-
