There are many types of blood pressure medications, each with a different mechanism of action and possible side effects. Some of the most common ones are:

Renin-inhibitors interfere with the production of an enzyme that initiates a chain of reactions that lead to the formation of the hormone that ACE inhibitors and ARBs block, thus lowering blood pressure. An example is aliskiren.

What do I need to tell my doctor BEFORE I take this drug?
If you are allergic to this drug; any part of this drug; or any other drugs, foods, or substances. Tell your doctor about the allergy and what signs you had. If you have a sulfa allergy.
If you are taking dofetilide. If you have any of these health problems: Kidney disease or liver disease. If you are not able to pass urine. Types of Blood Pressure Medications
Blood pressure medications come in many different forms. The types include ACE inhibitors, Aldosterone antagonists, and Potassium-sparing diuretics. Each type has its own distinct benefits and drawbacks. Ultimately, the decision of which medications to use depends on the individual and his or her medical needs.
Alpha blockers are a class of drugs that are used to reduce blood pressure. They work by inhibiting the action of a chemical called norepinephrine. These blockers are usually given along with other medications. Some people may experience side effects, but the majority do not.
Although alpha blockers can help lower blood pressure, they may not be effective for some individuals. For instance, pregnant women or those who are breastfeeding should not take these medicines. And they should not be given to patients with liver or kidney problems.
A leaflet that comes with these medicines lists the possible side effects of these medicines. Some of these side effects include dizziness and fainting. Others include stress incontinence and bladder discomfort. If these side effects do not go away, you can change the dose of your medication.
Many people take potassium-sparing diuretics to help control their blood pressure. Although these pills are not as effective as other types of diuretics, they can still lower systolic blood pressure, thereby making it easier for the heart to pump. However, they may also cause high levels of potassium in the blood.

Inibitori diretti della renina: Aliskiren
L'Aliskiren (INN) è il primo principio attivo della classe di farmaci noti come inibitori diretti della renina. È commercializzato in U.S.A con il nome di Tekturna e in Europa, compresa l'Italia, con il nome di Rasilez. È stato approvato nel 2007 dalla Food and Drug Administration per il trattamento dell'ipertensione primaria.

La renina è il primo enzima del sistema renina-angiotensina-aldosterone che ha un ruolo nel controllo della pressione arteriosa. Viene secreto dall'apparato iuxtaglomerulare renale in seguito a stimolazione simpatica (la noradrenalina attiva i recettori di tipo beta) in genere provocata da un abbassamento della pressione arteriosa, converte l'angiotensinogeno (proteina inattiva prodotta dal fegato e normalmente circolante) in Angiotensina 1, che viene poi convertita dall'enzima ACE ("enzima che converte l'Angiotensina") in Angiotensina 2, proteina attiva dalle capacità vasocostrittrici e stimolante la produzione di aldosterone dalla corteccia surrenale, che a sua volta stimola il riassorbimento di sodio con incremento della ritenzione idrica, espansione del volume plasmatico e così aumento dei valori pressori.
L'Aliskiren si lega alla tasca secondaria S3bp della renina.
Diversi farmaci controllano la pressione arteriosa interferendo con l'angiotensina o l'aldosterone, tuttavia quando questi sono usati in cronico, vi può essere un aumento della produzione di renina come risposta del nostro organismo. L'aumento della renina nel sangue può re-innalzare i livelli di pressione arteriosa: la ricerca si è indirizzata verso la produzione di un farmaco che inibisse direttamente la renina e l'Aliskiren è il primo di questi farmaci.
L'assunzione del farmaco riduce significativamente l'albuminuria in pazienti trattati con terapia antipertensiva ottimale.
La Commissione Europea per il Farmaco ha approvato la commercializzazione delle associazioni di Aliskiren con Idroclortiazide, con Amlodipina e con entrambi i farmaci.
Reazioni avverse
Le evidenze suggeriscono rischi di eventi avversi (ipotensione, sincope, ictus, iperkaliemia e alterazioni della funzionalità renale, inclusa insufficienza renale acuta) quando aliskiren è assunto in combinazione con ACE-inibitori o ARB (farmaci antagonisti del recettore per l'angiotensina).  Specialmente in pazienti con diabete di tipo I e II o con insufficienza renale. Sebbene, per altri gruppi di pazienti, siano disponibili minori evidenze non si possono escludere eventi avversi e quindi il CHMP (Committee for Medicinal Products for Human Use) non raccomanda l'uso di questa associazione.

We report a "macrocycle route" toward aliskiren, a drug presently marketed for the treatment of hypertension, using a highly stereocontrolled approach starting from a common "isopropyl chiron". Highlights of the synthesis include a challenging RCM reaction to produce a nine-membered unsaturated lactone, a highly stereoselective catalytic Du Bois aziridination, and a regio- and diastereoselective aziridine ring-opening to a vicinal amino alcohol.
