It was supported by Nnoitra's Hollow Mask that lay underneath. It was not uncommon for Hollow to cover up their hole. Nnoitra's mask just happened to be right around where his hole was, and so he covered that up as well. The insides of his hollow hole differed from the rest of his body. His hierro covered the insides, of course - but unlike the rest of his body, the insides of the walls of his hollow hole were sensitive. Nnoitra could hardly even feel it when others touched him, because of how strong his hierro was. But inside his head? It was different. So - he covered it up.
