I head cannon that Doll does not really know how to truly smile, and the only type of smile she can do is an evil and michevious one. She has tried to smile on occasion, but she just scares the other drones lol, that is why we rarely see her smile in the show (Lizzy finds Doll's smiling attempts funny)
