How Is Glaucoma Diagnosed?
To diagnose glaucoma, Ophthalmologist needs to perform a comprehensive eye examination along with detailed medical family history of glaucoma. They’ll check for signs of deterioration, including loss of nerve tissue. They may also use one or more of the following tests and procedures:
Puff test – It is a basic glaucoma screening test to estimate eye pressure (normal range - 12 to 22 mmHg).
Tonometry – It measures inner eye pressure using eye numbing drops.
Pachymetry - People with thin corneas have an increased risk of developing glaucoma, so pachymetry test can be used to determine cornea thickness.
Ophthalmoscopy – It is used to look at the shape and color of your optic nerve
Perimetry - a visual field test that checks your peripheral and central vision.
Gonioscopy – It examines the angle in your eye where the iris and the cornea meet
Treatment
Glaucoma is a disease basically due to increased eye pressure so the main focus in early stages of glaucoma is to to reduce pressure in the eye somehow. Regular use of prescription eye drops is the first line of treatment. Some cases with associated medical conditions may require systemic medications, laser treatment, or other surgery. While there is not yet a cure for glaucoma, early diagnosis and continuing treatment can preserve eyesight.
Medications
Nowadays lot of medications is currently available to treat glaucoma. Typically, medications are used to reduce elevated pressure in the eye. A single medication or a combination of medications may be prescribed. The type of medication may change if it is not reducing pressure enough or if the patient is experiencing side effects.
Surgical Procedures
Surgical procedures include laser treatment, making a drainage flap in the eye, inserting a drainage valve, or destroying the tissue that creates the fluid in the eye. All procedures aim to reduce the pressure inside the eye when medication is not sufficient. Even though, Surgery cannot reverse vision loss.
Laser surgery
Although few treatments are temporary and need to be repeated but for patient comfort, Laser trabeculoplasty helps fluid drain out of the eye. A high-energy laser beam stimulates the structure that drains fluid from the eye (the trabecular meshwork) so that fluid drains more efficiently.
Conventional surgery
If all other treatments like eye drops and laser surgery aren't controlling eye pressure, the patient may need a trabeculectomy. This filtering microsurgery creates a drainage flap. Fluid can then percolate into the flap and later drain into the vascular system.
Drainage implants
Drainage valve implant surgery may be an option for adults with uncontrolled glaucoma or secondary glaucoma or for children with glaucoma. A small silicone tube is inserted in the eye to help drain fluid.
Treatment for acute angle-closure glaucoma
Acute angle-closure glaucoma is a medical emergency. Those affected can take medication to reduce eye pressure as quickly as possible. They will also likely undergo a laser procedure called laser peripheral iridotomy. In this procedure, a laser beam creates a small hole in the iris to allow fluid to flow more freely into the front chamber of the eye where it then can drain.
Prevention
There is no known way to prevent glaucoma, but catching it early means it can be treated more effectively and vision loss can be minimized. Because, often, there are no symptoms, getting your eyes regularly checked is important; especially for people with a greater risk. Moderate activity like walking or jogging on regular basis might help lower eye pressure. Use protective eyewear when playing sports, outdoor activities in sunny weather or working on home improvement projects.

What causes eye floaters
 It has been observed as age-related change in your vitreous is the cause of eye floaters. As your eyes get older, the vitreous jelly deteriorates: it becomes more liquid, starts to sag and pull away from the inside of your eyeball, and some of the former gel clumps. The shadows of these stringy bits are what you see.
In case of vitreous detachment from retina, floaters can be seen. The stimulation of the retina during this process will often cause flashes in the eye. The moment the vitreous pulls away from the head of your optic nerve, it can make a ring-shaped floater.
Rarely, this detachment will pull a bit of your retina with it. This retinal detachment leaks blood into your vitreous, which appears as a scatter of small dots and needs immediate attention from your eye doctor.
Bleeding and inflammation in the eye, from retinal tears, blood vessel problems or other injuries, tends to cause floaters in general. Floaters can also be small specks of protein and other material that was trapped in your eye as it was forming before birth.

The eye is a very complex functional and anatomic organ. The retina is a thin, delicate and transparent sheet of tissue that lines the inside of the back of the eye. Directly in front of the retina is also a cavity that contains a gel called vitreous. The structure responsible for the bulk and shape of our eye is Vitreous part. Diabetic retinopathy occurs in three stages
Background retinopathy - Background retinopathy is said to occur if you have developed micro aneurysms on your retina. Micro aneurysms are when there is a swelling of the capillaries (very small blood vessels) that feed the retina. It is an early warning sign that your diabetes has lead to some damage of the small blood vessels of your retina.
Diabetic maculopathy - The macula is the part of the eye that helps to provide us with our central vision. Diabetic maculopathy is when the macula sustains some form of damage. One such cause of macular damage is from diabetic macular oedema whereby blood vessels near to the macula leak fluid or protein onto the macula.
Proliferative retinopathy - If a significant number of blood vessels on the retina become damaged, your body will respond by releasing a growth hormone known as Vascular Endothelial Cell Growth Factor (VEGF). The growth hormone stimulates the growth of new blood vessels. However, these new blood vessels are particularly weak and prone to leaking. Proliferative retinopathy is the body’s attempt to save its retina but it can often lead scarring of the retina and can cause the retina to detach, leading to blindness.
Symptoms
You might not have any signs of diabetic retinopathy until it becomes serious. When you do have symptoms, you might notice:
Loss of central vision, which is used when you read or drive
Not being able to see colors
Blurry vision
Holes or black spots in your vision
Floaters, or small spots in your vision caused by bleeding
Poor night vision
Risk factors
Anyone who has diabetes can develop diabetic retinopathy. Risk of developing the eye condition can increase as a result of:
Duration of diabetes — the longer you have diabetes, the greater your risk of developing diabetic retinopathy
Poor control of your blood sugar level
High blood pressure
High cholesterol

Age Related Macular Degeneration, What Treatments Are Available for Macular Degeneration?
Human eye has various important parts like Cornea, Pupil, Iris, Lens and Retina. The macula is located in the center of the retina, the light-sensitive tissue at the back of the eye. The retina instantly converts light, or an image, into electrical impulses. The retina then sends these impulses, or nerve signals, to the brain. When the cells of the macula deteriorate, images are not received correctly. In early stages, macular degeneration does not affect vision. Later, if the disease progresses, people experience wavy or blurred vision, and, if the condition continues to worsen, central vision may be completely lost. People with very advanced macular degeneration are considered legally blind. Macular Degeneration is the leading cause of vision loss, more than cataracts and glaucoma combined.
What Treatments Are Available for Macular Degeneration?
There’s no cure for macular degeneration. Treatment may slow it down or keep you from losing too much of your vision. Your options might include:
Lifestyle changes - like dieting, exercise, avoiding smoking, and protecting your eyes from ultraviolet light.
Anti-angiogenesis drugs - These medications – aflibercept (Eylea), bevacizumab (Avastin), pegaptanib (Macugen), and ranibizumab (Lucentis) -- block the creation of blood vessels and leaking from the vessels in your eye that cause wet macular degeneration. Many people who’ve taken these drugs got back vision that was lost. You might need to have this treatment multiple times.
Laser therapy - High-energy laser light can destroy abnormal blood vessels growing in your eye.
Photodynamic laser therapy - Your doctor injects a light-sensitive drug verteporfin (Visudyne) into your bloodstream, and it’s absorbed by the abnormal blood vessels. Your doctor then shines a laser into your eye to trigger the medication to damage those blood vessels.
Low vision aids - These are the devices that have special lenses or electronic systems to create larger images of nearby things. They help people who have vision loss from macular degeneration make the most of their remaining vision.
Submacular surgery - This removes abnormal blood vessels or blood.
Retinal translocation - A procedure to destroy abnormal blood vessels under the center of your macula, where your doctor can’t use a laser beam safely. In this procedure, your doctor rotates the center of your macula away from the abnormal blood vessels to a healthy area of your retina. This keeps you from having scar tissue and more damage to your retina. Then, your doctor uses a laser to treat the abnormal blood vessels.

What are symptoms of Glaucoma?
Open-angle glaucoma is asymptomatic. It develops slowly and sometimes without noticeable sight loss for many years. Most people who have open-angle glaucoma feel fine because the initial loss of vision is of side or peripheral vision, and the visual acuity or sharpness of vision is maintained.
