House's signature line is "Everybody Lies," well, the biggest lie House ever tells is that "the puzzle" is all that matters to him, when that couldn't be further from the truth. House cares, he cares a lot. He cares about his patients, he cares about his fellows, he cares about strangers, and he especially cares about Cuddy and Wilson. When his bulimic patient needed a transplant, he lied to the transplant committee, even though the case was already over. When another elderly patient of his who had been diagnosed with brucellosis needed a heart transplant, House went further than anyone else would have to get him. When he found that woman trapped underneath the crane wreckage, he fought for the rescue teams to get her out with her limbs intact until it wasn't an option anymore. When Thirteen got caught sobering herself up in the hospital after partying hard overnight and House covered for her, only to fire her right after, it wasn't because he thought she had screwed up, it was because he thought doing so would let her get closer to the patient she had brought in, and find some kind of connection. That's why when Thirteen looked willing to work for him again in the final season even as she finally found a partner to spend her last days with, he symbolically fired her again, because he cared about Thirteen and wanted her to be happy. He didn't turn in Chase for killing Dibala, he got Foreman to reconcile with his brother, he agreed to go on a date with Cameron so she would rejoin the team, he helped Taub improve his marriage, he bought a whole room of medical equipment for Adams to smash up and blow off steam with, he saved Wilson from ending his career multiple times, and for all the times he joked about Cuddy's "incompetence" as a doctor, when he truly wanted her input he asked for it. But the thing is, House will deny all of this, and that lies the issue. House knows he cares, and he actively tries to avoid doing so. This is why he tries not to meet with patients, why he acts so misanthropic, so flippant and arrogant, it's all protection. House avoids physical pain by abusing vicodin, and he avoids emotional pain by trying to stay distant from as many people in his life that he can. The source of his physical pain is tied to the emotional heartbreak over Stacy, so of course he tries to avoid both, because if he never gets close to anyone again, he can't be hurt like that again. This is why letting himself get close to Lydia, close enough to be heartbroken again is what convinced Dr. Nolan to let him out of Mayfield. Nolan recognized that House was avoiding emotional as well as physical pain, and that the two were linked. That's why when his heart was broken again later by Cuddy, he went back to vicodin. House cares so much, he really does.
