DR. ROBERT GARELICK- MEDICATION AND SLEEP APNEA
Over the last thirty six years, Dr. Robert Garelick has built an enormously successful dental practice based in Lindenhurst, New York. The practice has thrived, resting on a foundation of clinical excellence and a profound commitment to the care and overall wellbeing of patients. Sleep apnea can become very serious if not diagnosed and treated properly. If you are having difficulty falling asleep, come talk to Lindenhurst dentist Dr. Robert Garelick.
Did you know that most prescription medications can have various side effects, with many causing sleep and weight irregularities? If you are a person who already suffers from sleep apnea or are unable to get proper rest at night, then this is one of the areas you might want to investigate about.
Tumblr media
Prescription drugs like those for high blood pressure, steroids, seizure medication like benzodiazpines may be responsible for such side effects. In addition, even non-prescription medication like cough and cold medicines can exacerbate sleep apnea. Oral contraceptives, inhaled respiratory drugs, diet pills, medications with caffeine, ADHD stimulant medicines and also certain anti-depressants can have a debilitating effect on one's sleep apnea. Barbiturates, which are used for sedation, can cause a state of coma in toxic doses. Apart from a number of other degrading effects, illegal drugs like cocaine and methamphetamines as well as nicotine can disrupt sleep and increase daytime sleepiness.
Much like other intoxicating substances, alcohol too, ultimately, is most likely to cause sleep disruptions. Though calming and sleep-inducing in the beginning, a repeated pattern of alcohol consumption before bedtime reduces this effect and inversely, results in low sleep quality and fatigue during the day.
Dr. Robert B. Garelick, DDS, is committed to providing excellent dental care in Lindenhurst, NY across the greater Suffolk County, Long Island area. As part of that dedication, we provide comprehensive sleep apnea treatment. Our goal is not only to help you improve your dental health, but to improve your overall health. We will work with you to determine which oral appliance will best treat your obstructive sleep apnea, ultimately helping you to get a healthier and better night’s sleep.

DR ROBERT GARELICK - ORAL APPLIANCE OVER CPAP
A highly accomplished, award-winning dentist, Dr. Robert B. Garelick, DDS has his practice extended all over long island - Nassau and Suffolk counties as well as parts of Queens and Manhattan. Dr. Robert Garelick D.D.S. has over 30 years of experience in the field of dentistry including exciting Sleep Apnea treatments. Call 516-506-0000 for immediate assistance.
For over 32 years as a dentist, Dr. Robert B. Garelick D.D.S. has been providing comprehensive dental services to patients of all ages in Lindenhurst, Copiague, Babylon and Amityville, NY. As you enter our office, we hope that our homelike atmosphere will make you feel welcomed.
Dr Robert Garelick himself believes that Oral Appliances are more beneficial and comfortable as compared to CPAP. He says, "More than 50% of the patients that have been prescribed the CPAP can't really tolerate it. They find it uncomfortable, noisy, restrictive. So we make oral appliances which treats it differently. Instead of forcing air in to prevent an obstruction, we prevent the obstruction from happening in the first place by holding the lower jaw just a little bit forward. It prevents the tongue from sliding forward and keeps your airway open."
An oral appliance is extremely simple to use and clean. Travelling with a CPAP device means you need to have an extra bulky bag. On the other hand oral appliances fit the palm of your hand which means you don't need to skip treatment when you're not at home. Additionally, there’s no mask to strap to the face and no hose connecting the mask to the pump. It’s much more like sleeping naturally, without a huge apparatus attached to your face.
Tumblr media
Moreover, Oral apnea appliances are subtler than CPAP machines. If you’re re-imagining the scene from Brooklyn Nine-Nine in which, much to Gina Linetti's horror she's surrounded by her cousins, each with a buzzing, bulky CPAP machine, a mouth splint may be more to your liking. A sleep apnea mouthpiece creates no additional sound that might keep light sleepers awake (either yourself or a bed partner). A CPAP, on the other hand, blows air continuously all night. It’s not noisy, but it does emit sound.
Our office utilizes state-of-art technology in order to detect any problem areas early. By detecting decay and other problems early, we are able to help our patients avoid costly dental procedures. Dr. Robert Garelick and his staff keep themselves updated on any changes occurring in the world of dentistry in order to provide the highest quality of services.
If you are having difficulty falling asleep, come talk to us. Sleep apnea can become very serious if not diagnosed and treated properly.

SLEEP APNEA NEWS WITH DR. ROBERT GARELICK

Dr. Garelick believes that the more informative a patient has, the easier it is to diagnose problems and the smoother the transition to prevention and remedy, not to mention the added benefit that comes for the patient who is knowledgeable.
Tumblr media
In a study conducted by the Oxford Radcliffe Hospital Charitable Funds and the ResMedUK and the the National Institute for Health Research Oxford Biomedical Research Centre, 25 adults with OSA included in the study had all been using CPAP successfully for over a year at the time of recruitment. The double-blinded, cross-over design required that CPAP be withdrawn for 14 nights during each treatment arm, during which time participants received supplemental oxygen or regular air (sham) overnight via a face mask or nasal cannula.
The primary outcome was the change in home morning blood pressure following CPAP withdrawal for 14 nights, oxygen versus air. Secondary outcomes included oxygen desaturation index (ODI), apnea hypopnea index (AHI), and subjective (Epworth sleepiness score) and objective (Oxford sleep resistance test) sleepiness.
“As expected, supplemental oxygen significantly and markedly attenuated measures of intermittent hypoxia, with a median reduction in ODI of -23.8/h (P<0.001; IQR -31.0 to -16.3), and an absolute median reduction of the percentage time with oxygen saturations <90% of -9.8% (P<0.001; IQR -16.7 to -4.3), compared to air," the researchers wrote.
"There was a small significant reduction in the heart rate rises index (a measure of autonomic arousals), with a median reduction of -3.7/h (P=0.006; IQR -9.1 to -0.8), oxygen versus air. The small reduction in median AHI of -3.6/h (P=0.98; IQR -10.2 to 10.1), oxygen versus air, was not statistically significant."
No significant differences in either subjective or objective sleepiness were seen with oxygen versus air, and neither were there any significant differences seen in change in mean overnight urinary volume or overnight urinary creatinine concentration.
Thus, while it can be said that Oxygen eliminates morning BP spike in Sleep Apnea, it is yet to be determined through additional study, if the most likely candidates for supplemental oxygen therapy - those with the most severe hypoxia, will benefit from it.

DR ROBERT GARELICK – BENEFITS OF ORAL APPLIANCES
One of the hallmarks of Dr. Robert Garelick’s career and practice has been his vision of dentistry and its expansive role in promoting a patients quality of life and general health. This career long vision and commitment provided the impetus for his growing focus on dental sleep health.
He says, “More than 50% of the patients that have been prescribed the CPAP can’t really tolerate it. They find it uncomfortable, noisy, restrictive. So we make oral appliances which treats it differently. Instead of forcing air in to prevent an obstruction, we prevent the obstruction from happening in the first place by holding the lower jaw just a little bit forward. It prevents the tongue from sliding forward and keeps your airway open.”
Tumblr media
Oral appliances has various benefits over CPAP. Some of them are listed below.
Some patients who use CPAP complain of dry nasal passages, sore noses, or congestion; but oral devices don’t supply forced air to your nose and throat, so they don’t present the same issues.
There’s guaranteed comfort, regardless of one’s sleep position. Because your apnea mouthpiece is custom fit to your mouth and jaw, it doesn’t interfere with your sleep position, skin, or facial hair the way a CPAP mask might.
Less to zero interference with colds and allergies is another advantage of Oral appliances. Wearing an oral appliance has no effect on a stuffed-up nose during cold and allergy season. (Whereas CPAP requires a clear nasal airway for you to get the full benefit.)
Moreover, Oral apnea appliances are subtler than CPAP machines. If you’re worried about wearing a “Darth Vader” mask to bed or keeping a large medical device on your bedside table, a mouth splint may be more to your liking.
A sleep apnea mouthpiece creates no additional sound that might keep light sleepers awake (either yourself or a bed partner). A CPAP, on the other hand, blows air continuously all night. It’s not noisy, but it does emit sound.
Oral OSA appliances are simple: they’re small and portable. They don’t require replacement parts. You don’t need to plug them in or use a battery. Care and upkeep is easy: you don’t need to clean out tubing or replace filters.
Oral appliances are totally portable and come with their own travel cases. They are small and lightweight and won’t trigger TSA inspections at airports. You can even wear them if you sleep on the plane, eliminating embarrassing snoring during red-eye flights and naps. If you travel routinely for work, an oral device presents less hassle than a CPAP.
If you pay out of pocket for OSA treatment, anti-snoring oral appliances are often less costly than CPAP.
Perhaps most importantly, you may be more likely to use an oral appliance than a CPAP. Many people find CPAP uncomfortable or intrusive and end up abandoning their therapy or being inconsistent with it. You can’t reap the health benefits of a treatment if you don’t use it. Oral therapy has a high rate of compliance in part because it’s so comfortable and easy to use.

Sleep Apnea Specialist at Lindenhurst: Dr. Robert Garelick
Dr. Robert Garelick graduated in 1980 from New York University with a Doctorate of Dental Surgery Degree. He completed his residency at Montefiore Hospital and Medical Center in 1981. Over the last thirty six years, Dr. Robert Garelick has built an enormously successful dental practice based in Lindenhurst, New York. The practice has thrived, resting on a foundation of clinical excellence and a profound commitment to the care and overall wellbeing of patients.
One of the hallmarks of Dr. Robert Garelick’s career and practice has been his vision of dentistry and its expansive role in promoting a patients quality of life and general health. This career long vision and commitment provided the impetus for his growing focus on dental sleep health.
Sleep apnea is a condition in which one’s breathing pauses or even stops when they’re asleep. These disruptions can occur hundreds of times over the course of a single sleep. Obstructive sleep apnea is the most commonly found form in which the throat muscles collapse while one is asleep, resulting in snoring. Around 22 million Americans are estimated to have sleep apnea, indicating how common it really is. It is often passed on across generations, but can also be caused due to environmental factors like obesity and alcohol addiction. While a person is resting, their airways become blocked by structures such as oversized tonsils, a receding jaw, or an oversized tongue. Moreover, the likelihood of men suffering from sleep apnea is higher than women. Undiagnosed, untreated sleep apnea can be fatal as it deprives the brain and heart of sufficient oxygen leading to strokes and heart conditions.
Sleep Apnea can have overall unhealthy effects on your body systems.
Endocrine system
Imagine your cells not being able to respond sufficiently to insulin, and important hormone that helps regulate blood sugar levels in your body. People suffering from sleep apnea are highly likely to develop a condition in which the body cells do not respond very well to insulin, called insulin resistance. This greatly increases the likelihood of developing type 2 diabetes due to the rise of blood sugar levels. Moreover, sleep apnea has also been associated with a cluster of heart disease risk factors called metabolic syndrome which includes high blood pressure, high LDL cholesterol levels, high blood sugar levels, and a larger-than-normal waist circumference.
The Respiratory System
If you are already suffering from asthma or COPD (Chronic Obstructive Pulmonary Disease), sleep apnea can aggravate an already existing problem. Obstructive Sleep Apnea deprives your body of sleep and given the number of hours one is expected to sleep every day, you may find symptoms of asthma or COPD worsening. This might make mundane tasks like exercising, difficult.
Digestive system
Sleep Apnea can worsen heartburn and other symptoms of gastroesophageal reflux disease (GERD), which can interrupt your sleep even more. Also, the likelihood of having fatty liver disease, liver scarring, and higher-than-normal levels of liver enzymes shoots up.
Circulatory and cardiovascular systems
One may notice a reduced efficiency in cardiovascular functioning since sleep apnea, which has been linked to obesity and high blood pressure, increases the strain on your heart. If you are a sleep apnea sufferer, you’re more likely to have an abnormal heart rhythm such as atrial fibrillation, which could increase your risk of a stroke. Heart failure is also not uncommon in people with sleep apnea.
Reproductive system
Apart from common symptoms like fatigue and a general bad mood, sleep apnea also can reduce your desire to have sex at all. In men, it could contribute to erectile dysfunction and affect your ability to have children.
Other systems
Other common symptoms of sleep apnea include:
• dry mouth or sore throat in the morning
• headache
• trouble paying attention
• irritability

Dr. Robert Garelick D.D.S. has over 30 years of experience in the field of dentistry including exciting Sleep Apnea treatments. Call 516-506-0000 for immediate assistance.
Sleep apnea can become very serious if not diagnosed and treated properly. If you are having difficulty falling asleep, come talk to Lindenhurst dentist Dr. Robert Garelick. For over 32 years as a dentist, Robert B. Garelick D.D.S. has been providing comprehensive dental services to patients of all ages in Lindenhurst, Copiague, Babylon and Amityville, NY. As you enter our office, we hope that our homelike atmosphere will make you feel welcomed. Dr. Garelick and his staff keep themselves updated on any changes occurring in the world of dentistry in order to provide the highest quality of services.
Having worked in Lindenhurst for so long, he has built a reputation within the community for excellence in the world of dentistry. Dr. Garelick believes that the more informative a patient has, the easier it is to diagnose problems and the smoother the transition to prevention and remedy, not to mention the added benefit that comes for the patient who is knowledgeable.
Do you ever feel that, no matter how much sleep you get, you never find yourself completely rested? Sleep apnea may not immediately have absolutely overt or apparent consequences like chest pain. The patient may be a little sleepy or confused. People often fail to make the correlation that the daytime sleepiness may be related to disrupted sleep at night. Most aren't diagnosed until something terrible like a car accident has already taken place. Obstructive Sleep Apnea is the cessation of airflow during sleep which henceforth prevents air from entering the lungs due to an obstruction. In simple words, sleep apnea is a serious medical condition, a sleep disorder which not only affects the patient’s sleep but also his/her health in the long run.
Millions of Americans are affected by this same feeling every year, often due to obstructed breathing known as sleep apnea. Usually caused by soft tissue in the back of the throat collapsing and impeding oxygen flow, obstructive sleep apnea affects a significant portion of the population. Central sleep apnea, a similar but less prevalent form of the disorder, isn’t caused by airway blockage but because the brain fails to signal the muscles to breathe. While sleep apnea is a common disorder affecting men far more frequently than women, many people are unaware that they suffer from obstructed breathing because the halted breathing doesn’t trigger a full awakening.
If you are suffering from OSA, there are luckily many options for treatment. You can treat your sleep apnea with surgery, a CPAP machine or oral appliance therapy. Oral appliance therapy is recognized as the least invasive treatment option available. Dr. Garelick works with sleep physicians to treat obstructive sleep apnea with oral appliance therapy.
Oral appliance therapy involves the selection, fitting, and use of an appliance that effectively holds the jaw forward to maintain an open airway while the patient sleeps. Dentists with specialized training in oral appliance therapy to treat obstructive sleep apnea are uniquely qualified to treat OSA with custom appliances. Since oral appliances may affect the teeth and jaw, a dentist’s expertise in these areas are required in order for the treatment to be successful.
Dr. Garelick will evaluate your specific symptoms and then will select the most appropriate oral appliance to correct the airway obstruction and breathing conditions that will ultimately make breathing easier, helping you to finally get the night’s sleep you deserve. These oral appliances are so discrete that they can fit into the palm of your hand.
Dr. We will work with you to determine which oral appliance will best treat your obstructive sleep apnea, ultimately helping you to get a healthier, better night’s sleep.

How Risky is Undiagnosed Sleep Apnea
Dr. Robert Garelick graduated in 1980 from New York University with a Doctorate of Dental Surgery Degree. He completed his residency at Montefiore Hospital and Medical Center in 1981. Over the last thirty six years Dr. Garelick has built an enormously successful dental practice based in Lindenhurst, NY. One of the hallmarks of Dr. Garelick’s career and practice has been his vision of dentistry and its expansive role in promoting a patients quality of life and general health. This career long vision and commitment provided the impetus for his growing focus on dental and sleep health.
If you’re suffering from obstructive sleep apnea, Robert B Garelick, DDS may be able to help you treat your symptoms. Dr. Garelick will evaluate your specific symptoms and then will select the most appropriate oral appliance to correct the airway obstruction and breathing conditions that will ultimately make breathing easier, helping you to finally get the night’s sleep you deserve. Dr. Robert Garelick offers a variety of Oral appliances to treat snoring and obstructive sleep apnea. These appliances are FDA-cleared therapeutic devices that are durable, comfortable, discreet and highly effective. It is absolutely imperative to get the required treatment as the cumulative effect of symptoms could prove fatal.
Research suggests that there’s a fairly strong link between sleep apnea and increased risk of death. A study from the Yale Center for Sleep Medicine in 2005 found that “sleep apnea doubles the risk for the development of stroke and death, and severe sleep apnea more than triples the risk,” lead author H. Klar Yaggi, assistant professor of medicine at Yale, said in the study’s press release. There’s also evidence that sleep apnea can cause heart problems, as this Cleveland Clinic article details. In the article Reena Mehra, MD, director of Sleep Disorders Research in the Sleep Center of the Neurologic Institute at the Cleveland Clinic, said “there is a very strong association between sleep apnea and cardiac arrhythmia,” adding that research shows that repeated episodes of “upper airway collapse in sleep apnea may trigger arrhythmia events.”
The total number of deaths related to sleep apnea is something difficult to determine, but thanks to research (American Academy of Sleep Medicine), we now know that this disorder can lead to the following complications:
• An increased risk of having a stroke
• Death due to heart disease
• Insulin resistance and glucose tolerance, leading to diabetes
• Gastric Reflux (also called GERD)
• Weight gain and inability to lose weight
• Increased risk of cancer
• Higher blood pressure
• Heart attack
, Neither of these is conducive to a good night's sleep. Since studies indicate that sleep deprivation itself contributes to the development of heart disease, this obviously worsens the situation.
Dr.

Tackling Sleep Apnea
Often misconstrued as snoring, sleep apnea can be a much bigger problem. A person suffering from obstructive sleep apnea is at the risk of asphyxiation during sleep. If you are having difficulty falling asleep or experience fatigue even after a seemingly good night’s sleep visit Lindenhurst dentist, Dr. Robert Garelick for queries regarding the same. Obstructive sleep apnea is diagnosed using a polysomnogram - also known as a “sleep study.” During a polysomnogram, sleep physicians monitor sleep functioning as a patient rests overnight. In order to diagnose sleep disorders and related medical issues like OSA, the sleep physician evaluates the data collected during the study. Sleep Apnea can be treated using an Oral appliance therapy or a CPAP machine.
CPAP
Created in 1981 by the Australian pulmonary specialist Colin Sullivan and his colleagues, Continuous Positive Airway Pressure or CPAP is the most widely used treatment for moderate to severe sleep apnea. It includes a small machine that supplies a constant and steady air pressure, a hose, and a mask or nose piece. It helps increase air pressure in the throat region so that the airway does not collapse when you breathe in. The machine contains a filter and a small tank of water that are designed to remove impunities and increase the humidity level in the air. CPAP is also one of the best available options while combating environmental factors like obesity can progress slowly. However, the bulky apparatus that comes along with this technique makes it difficult to commit to wearing it for a long time, with many abandoning the machine within a few times of use. Fortunately, changes to the technology are making CPAP much easier to tolerate. When CPAP therapy was just introduced, it was one-size-fits-all. However, of late, varied innovations have taken place in the technology used making it more comfortable and hassle free. Some common problems with the device include a leaking mask, trouble falling asleep, and a dry mouth or nose. Despite these problems, the CPAP remains the most commonly used treatment due to its many positive effects. These effects include improved heart function, quality of life, and energy levels. Moreover, CPAPs decrease daytime sleepiness, especially in those who have moderate to severe sleep apnea. There are many different types of CPAP masks to fulfill different needs and face shapes. Some examples of the different styles are nasal, full face, nasal pillow, nasal prong, hybrid, oral, and total face. The best fit for a patient is typically the type of mask that feels most natural. However, Dr. Robert B. Garelick will work closely with your sleep physician to treat your condition using the Oral appliance therapy.
Oral appliance therapy
Dr. Robert Garelick offers a variety of Oral appliances to treat snoring and obstructive sleep apnea. These appliances are FDA-cleared therapeutic devices that are durable, comfortable, discreet and highly effective. In addition, they can be customized to also treat problems like jaw pain and teeth grinding. The SomnoDent Classic or Flex allows patients to speak and drink while wearing the equipment and is customizable. The O2 Oasys treats blockages that cause snoring. ResMed Narval CC is digitally designed for precise fit and is extremely easy to handle.

Diagnosing Sleep Apnea: A Priority
Dr. Robert B. Garelick DDS (Dr. Rob B. Garelick D.D.S) L.I. In atrial fibrillation (Afib), an abnormal heart rhythm increases the risk of other adverse consequences such as stroke. Approximately three to six million Americans have Afib, and according to a review published in the Journal of Atrial Fibrillation, the risk of Afib is two to four times higher for individuals who have obstructive sleep apnea. Besides, 29 million people in the United States have Type 2 diabetes, and approximately seven in 10 of them have untreated sleep apnea, according to a review in Frontiers in Neurology, indicating a probable link between the two. In addition, people with untreated, severe sleep apnea are more than two times more likely to die from heart disease, according to research published in Sleep & Breathing. For more information and consultation on the subject Dr. Garelick remains the expert, who, along with his staff keep themselves updated on any changes occurring in the world of dentistry in order to provide only the best services.

In order to diagnose sleep disorders and related medical issues like OSA, the sleep physicians evaluates the data collected during the study. In the case of OSA, Dr. Robert B. Garelick will work closely with your sleep physician to treat your condition with oral appliance therapy.
The appliances used are FDA-cleared therapeutic devices that are durable, comfortable, discreet and highly effective. In addition, they can be customized to also treat problems like jaw pain and teeth grinding. For more information on the appliances used, click on http://robgarelickdds.com/sleepapnea.html
People, who, for years on end, had woken up more tired and irritable than the day before, who had to use every bit of their willpower to get their tasks accomplished and who suffered from weakening mental health because of sleep apnea have been diagnosed and treated for the same, working along with Dr. Robert Garelick against the stigma associated with it and towards better health and well-being.
