What is "Case Management"?

The person who helps the individual and/or his/her family to develop a client-specific plan of care is called a case manager. He or she also works closely with the individual and his/her team to ensure satisfaction of services, monitor health, safety, and welfare, and support progress toward identified goals and outcomes.

Now accepting referrals for conflict-free case management!

Our agency is currently accepting individual referrals for conflict-free case management through the Supports for Community Living and Michelle P Medicaid waivers, as well as private-pay services.

Who does Custom Case Management serve?

Our agency is happy to talk with anyone who is in need of services. We are certified to provide supports through the Supports for Community Living and Michelle P Medicaid waivers in Kentucky. We also accept clients on a private-pay basis and gladly make referrals to other professionals and agencies when we are not equipped or certified to meet an individual’s needs. Simply put, we would gladly meet and talk with anyone who needs support.

Our agency is happy to help ensure this for the individuals we serve by providing conflict-free case management. Essentially, we work together with our individuals and their teams in a non-biased manner, putting people first and focusing on goal achievement.
