From Watching to Working…. James and Elli’s Story

James found himself in a place he had never been before in the back of an ambulance as he watched the paramedic and EMT load his wife Elli in the ambulance. James sat on the jump seat in front facing backwards. Paramedic climbed up on the left side of EMT on the right side of Elli, who now been secured on the cot in the back of the ambulance with her torso elevated to help her breath. As the paramedic ready the Zoll heart monitor EKG unit.The EMT open a bag holding a nasal cannula with capnography and he place the two prongs of the nasal came in Elli‘s nose. Her shortness of breath and extreme tightness in her chest was making her feel like gonna pass out. She hadn’t felt well when she told James that she wanted him to call an ambulance. She thought she was dying as her breathing was erratic and shallow. James is sure that she was having a heart attack as she also thought she too. The EMT hook the nasal cane tubing into the oxygen supply on the ambulance and set it to 8 L per minute the paramedic lifted Elli’s loose fitting shirt attaching the electrodes to Elli‘s chest for the 12 lead EKG. He also placed the 4 lead EKG one on each leg and one on each arm he pushed analyze and the heart monitor screen came to life with all sorts of lines and spikes indicating the condition of Elli‘s heart. The EMT placed the BP cuff and pulse ox on Elli’s arm. Pulse was running about 180 and her blood pressure on the monitor was showing it to be 200/120 both the paramedic and EMT was concerned with Elli‘s condition being that she was so young and having this kind of episode. Both EMS workers were concerned as Elli’s skin was gray and her lips purple. James was concerned because they let him riding in the back. He wasn’t sure if it was because she was so anxious and the paramedic was hoping that James could keep her calm or that maybe there was something else going on and he knew that Elli might be in the last minutes of life and would at least be able to spend them with her husband. The EMT got the nod from the paramedic to start the transport of the ambulance.

The ambulance pulled away from James and Elli‘s home. Elli set upright on the ambulance cot holding onto James‘s hand trying to catch her breath. Suddenly she let out wild gasp and her grip stiffened so hard that James thought she was gonna break his hand. Then she let out another grown louder higher pitch, and her voice began to trail off as she faded from consciousness. James felt her grip relax as an alarm began sounding. The paramedic look at the heart monitor and suddenly she went from sinus tachycardia into ventricular tachycardia. He quickly lowered the torso at the end of the cot so that Elli was now flat he tore open her T-shirt, exposing her bare chest. Her bra, the medic then took his trauma shears and snapped the center part of her bra and began to do chest compressions. He looked at James and said can you help me and he said yeah, what do I need to do? He looked and nodded towards the bag on the side right side of the jump seat “that’s a bag mask place the mask part over your wife face hold it against your face as hard as you can when I tell you to squeeze the bag”. So James took the bag mask he put the mask over Elli‘s face empty and grim. Then he squeezed the bag as he squeeze the bag. The air from the bag was forced into her face, causing her cheeks to quickly swell, and then her chest arise then he heard her exhale. James watched the medics gloves hands compress Elli’s chest cause her breasts to bounce. James did it again, and then a paramedic started compressing her chest again. He hollered for the EMT to pull the ambulance to the curb in the assist him. The ambulance slowed to the curb. The EMT got in the back and see what we got. He said she’s had a cardiac arrest. Can you get her on some defib pads? He said “yes” he reaching for the Defib pads. He placed one above a right breast and one blower left breast. Once he applied the Medline brand pads the paramedic looked at her heart rhythm once again. The more spiky pattern of erratic heartbeats indicated that she was still in ventricular tachycardia. He pushed charge on the heart monitor defibrillator as it begin to charging the series of tones told the medic it was ready. The cardio averting shock was delivered to Elli‘s chest. When the tone sounded the medic looked at James and said pull the mask up off her face and watch out. He then hit the shock Button at that second 125 J of energy travel through the wires into the pads shooting like a bolt of lightning through Elli‘s chest to her heart that lay quivering in her chest and that electricity cause her heart to instantly stop. The monitor displayed a flatline as it traced across the screen. James fearing he had just watched his wife die and was struck by her nude torsos reaction. The paramedic told him to keep giving her ventilations so he gave her another two ventilations The EMT started doing chest compressions again. The paramedic went ahead and got an epinephrine box from the drug bag. He quickly assembled it then he injected the Epi into Elli‘s IV tubing As the EMT continued his CPR James still giving her ventilations every 30 compressions the paramedic watch the monitor after about a minute, he asked the EMT to stop the CPR compressions to see what the rhythm now look like. That rhythm was the same erratic pattern as before so he set the unit to 200 J energy, hit charge and it began to charge. The same sequence of tones was heard as the unit charged. The paramedic hollered clear when he did so James pull the mask of back away from Elli‘s face the EMT took his arms off Elli‘s chest. The paramedic hit the shock button, again the electricity course through her torso and into a body which arched off the ambulance cot, causing her arms and her legs to throw their selves against the mattress. They all looked at the screen, which now displayed a sinister flatline across the monitor. The EMT continue to CPR compressions as the paramedic grabbed the laryngoscope from the airway back on the 7ETube. He asked James to move off the jumpsuit so he could set there . This would provide better ventilations.

James moved over on the bench seat next to the EMT. “Can you take over compressions”? Figuring James had never done CPR. James crossed his hands up on top of his wife’s chest and began to push on her chest as hard as fast as he could harder and faster than he ever thought he could. EMT prepared another injection of epinephrine as James watched the paramedic slide to Laryngoscope blade into Elli‘s mouth and down into the throat where after he visualizing her vocal cords he passed in the endotracheal tube into her airway. He withdrew the scope blade once he done that he attached the bag mask to the end of the endotracheal too. He squeezed it. He watched Elli‘s chest rise as he watched her chest. He listened to her chest with the stethoscope, listening for air to be filling her lungs. Yes he had gotten her endotracheal to placed correctly, squeezed a syringe filled with air which filled the air bags both in her mouth and at the end of the tube to keep it from moving around and to keep her from having any kind of aspiration. Once he done that he secured the endotracheal tube with an ET to holder the ET tube holder secured that and connected the bag mask giving her two more ventilation. James was still doing compressions on Elli as fast and as hard as he could. The EMT looked at the medic saying they probably need to start for the hospital.

The EMT moved back to the front and the ambulance pulled away from the curb. Heading five minutes to the hospital James continue to pump His wife’s chest. The paramedic would deliver two breaths every every five seconds or every time James did 30 compressions. The medic would push a different kinds of cardiac medications into the IV tubing. He gave Elli epinephrine then he gave her a 300 mg injection of amiodarone to attempt to alleviate her erratic and irregular heartbeat. After a minute or so the ambulance siren could be heard in the back of the ambulance the paramedic charge on the defibrillator unit once again this time he set it for 360 J the most energy that is used on a human being.Before he hit the button to shock he called clear, James pulled his hands away. Elli’s body contorted, an erotic spasm of jerking movements and extrimity contraction. The monitor screen went off the chart and then the line came back down flat. They stopped the electrical activity but now she had no electrical activity in her heart. James continue to push down on her chest as hard as fast as he could. Fueled by the fear of losing Elli he continued as she lay lifeless and clinically dead. The paramedic then administered another 5 mg of epinephrine. He also went ahead and started amiodarone drip by the attempt to stabilize the erratic heartbeat if they got it again, he then administer 3 mg of atropine push IV. James was beginning to tire out from doing compressions so the paramedic switched. James took over ventilating his wife. The paramedic slid over on the bench seat place his hands in the middle of Elli‘s chest now bruise from all the chest compressions and began to push hard and fast again. James would give her two ventilations by squeezing the bag every 30 compressions. The medic shocked Elli 3 more times at 360, James watched the empty expressionless face of Elli each time. Each time the shock only reinforced his fear that Elli was dead. At this point the ambulance begin to slow and through the opening of the ambulance the medic could see the emergency room at UL South Medical Center beginning to pull into view.

Elli was still in cardiac arrest and at the time they rolled her out of the back of the ambulance. The paramedic got up on the bottom cot rails continuing to do one arm compressions on Elli as the EMT bagged her rolling into the emergency room. James was following along when a nurse asked him to step out to the waiting room as they tend to do his wife. As the EMS crew rolled Elli into the ER James look into the treatment room. The ER staff was already taking over the resuscitation as the doors closed. Once in the emergency room they quickly lifted Elli over onto the treatment table where they continue to resuscitate her. Her heart was still in arrest, but they continued as they continued as the ER physician assessed Elli. Nurses hooked the leads and pads into the monitor. The heart begin to beat in an erratic fashion again, it was ventricular fibrillation. The same thing that she had before it happened again attending, the emergency room physician was overseeing her care and a team of nurses were around each side of something doing CPR, one ventilations, and one administering drugs. The physician called “stop” he charged the defibrillator unit to 360. He then called “Clear”. He pressed the shock button, Elli’s body naked now erupted in another electrically spasm. Instantly Elli relaxed. The staff continued treatment. The doctor studied to EKG monitor trying to get an idea of what he wanted to do next to her. He asked the nurse for another shot of epinephrine which she did. They continue CPR for about five minutes. It was at that time Elli‘s heart began then stopped again and she went back into VF again. They continued CPR and administered some more epinephrine. This injection seemed to do something because her heart began to beat at first it was really a weak amplitude on the monitor, but the doctor felt for femoral. The pulse began to feed blood circulating in her body. The nurse continued CPR for another minute and after that minute, Elli‘s heart continue to strengthen her pulse begin to speed up and her blood pressure begin to increase. The doctor looked at the nurse and so you go ahead to stop the CPR now Elli lay there in the in the emergency room treatment. The Doctor asked the nurse to start an IV drip to support blood pressure as he check extremity reflexes. Now being revived with a pulse running 60 and her blood pressure the same 60 they increased medication to stabilizer blood pressure and increase her heart rate which were successful over the next hours. They assessed Elli‘s condition she was kept in the medically induced coma. The heavy sedation was first in case she had some kind of anoxic brain injury from the lack of oxygen during her cardiac arrest. Secondly this coma allowed they ICU to preform post resuscitation hypothermia therapy. It was determined after a day that she had not received any kind of neurological side effects. It was then at that point that she was removed from the ventilator. After a few hours she was taken to the Cardiac Cath Lab, where a cardiac catheterization was performed this cardiac catheterization showed no blockages. They determine that she had suffered a coronary artery spasm myocardial infraction from anxiety.

Elli and James live happily ever after.
