To me he looks like he's gotten thinner, more tired, he's stopped shaving his face daily, and started keeping his hair longer. To me it feels like he's trying to avoid attention, but another part if it is changing his mindset, not working for a fight any longer, more so trying to survive, learn to run, and manage what he's got left.

So it drives me nuts when Dean comes back from Purgatory, and understandably Dean is a bit more focused on his ptsd at that moment, but the fact that he doesn't look at Sam and see the sheer difference from how he was before. And how he'd rather believe any change he does see is because Sam gave up because he didn't love Dean enough rather than Sam loved Dean so much and Dean's death was his last straw.
