I went and found the original post in question, and I read all of the expectedly negative responses, so now I feel compelled to add this. 
I’ve found almost universally that any nonmedical person talking about a failure of our medical system chooses to blame that failure on either the malice or the incompetence of individual doctors.
Earlier today I was on instagram and came across a reel mocking of the idea of hospitals having a physician week the way they have nurses week, and I think I finally realized why this happens.
Historically, physicians have been the owner/employer class. It is ingrained in our cultural consciousness that doctors are in charge. They own practices, they hire (and abuse and exploit and underpay and undervalue, etc etc) nurses, they make their own schedules, they choose whether they are willing to be there for you as a patient. Why would you hold an appreciation week for your CEO?
Except that’s very much not the case anymore. Doctors are employees now. They’re cogs in the machine. They’re told what to do and how to do it and spend mandatory meetings learning how to meet their metrics and get emails every month telling whether if they did it fast enough and cheap enough, from someone who couldn’t tell you the first thing about why. 
As of 2023 a huge percentage of doctors - 77.6% - are employees of hospitals or corporations. I couldn’t find whether this number includes “independent contractors” like ER doctors, who are functionally employees. If it doesn’t, the number is even higher. And, because of how corporate medicine practices, I guarantee you these employee physicians account for an even higher percentage of patient encounters. 
Since we’re almost 6 months into 2024, it has also almost certainly already gone up. This is not a casual shift. It’s an active corporate takeover. In 2018, that percentage was 47.4. Corporations and hospitals bought up 30% of private practice physicians in the past 6 years.
I’ve seen a lot of people mock the idea that working in medicine can be akin to a customer service job. But when such a majority of doctors are employees, it absolutely is. And much like blaming your waitress for being slow when the restaurant is understaffed, these corporations are counting on you blaming the doctor. If they demand your doctor perform an impossible amount of work caring for an impossible number of patients and you walk away thinking “my doctor just sucks” you’ve fallen for their grift. 
I’m not saying this system doesn’t produce bad care, or that you shouldn’t be mad about receiving it. Quite the opposite. It provides horrible care! You should be mad!
But we have to be mad at the system which has deprived you of the quality care that your doctor wanted to give you. We can’t be so mad at the individual doctors. Even the ones who are so burnt out they aren’t trying anymore. Not because they should be allowed to be horrible, but because treating a symptom does not cure the disease.
I don’t know what the solution is here. I look at our healthcare system and I fear for our future. But I do know that blaming the corporations instead of the individuals and supporting doctors the way you would support any other exploited worker is a step in the right direction.
Got reblogged by Seanan McGuire again; can safely assume my notes will be a nightmare of people accusing me of being a lazy, incompetent, distracted, inhumane doctor, assuming they notice I’m a doctor. May take a week off.
People will simultaneously describe all the elements of the current US medical system that lead to burnout in service of making the very few shareholders very rich, and then pretend that doctors aren’t also profoundly negatively affected by it, and then decide the system is also actually our fault.

25 Things You Need to Know About Becoming a Holistic Health Coach

Taking the leap to become a holistic health coach can seem intimidating—especially if you’re leaving the stability of a 9-5 job.
Whether you’re just starting to consider the possibility of health coaching or you’ve already received your certification, there are a few things you should know before jumping in. In honor of our 25th anniversary, we’ve put together 25 things you need to know about becoming a holistic health coach.
1. It’s not only about nutrition
While nutrition is an important factor in holistic health coaching, it’s not the whole (veggie) enchilada. Instead, your job will be to take mind, body, and spirit into consideration when working with clients. This can mean anything from meal planning to meditation practice.
2. There are prerequisites
Keep in mind that you must be at least 18 years of age and have your high school diploma, GED or equivalent in order to become a certified holistic health coach through AFPA.
3. You can hail from any professional background
You don’t need to be a doctor or even a health professional to become a holistic health coach. From yoga instructors to business professionals, anyone can find success in the industry; it all comes down to the determination to learn, teach and grow.
4. You need an entrepreneurial spirit
Making a career out of holistic health coaching is not for the faint of heart. It requires a strong will and determination to succeed. Much of your career will be predicated on your ability to sell yourself and your services to people who may not even know they need them yet. It takes a passionate entrepreneur to attract clients and build a portfolio.
5. It takes strong communication skills
As with most entrepreneurial positions, you must be an excellent communicator in order to excel in this space. From in-person interactions with clients to digital correspondence, you must ensure that clients maintain a clear understanding of your recommendations, reasoning, and boundaries. By prioritizing clarity, you’ll keep your clients (and yourself) happy.
6. You can go your own way
Whether you want to start your own business or work for an established wellness center, you can take your career in many different directions. There are places for holistic health coaches at physicians’ offices, spas, schools, corporations and more.
7. You’ll be more likely to adopt healthy habits
Something about learning the dangers of processed meat and excess sugar makes you want to be a better person. As you teach the benefits of holistic health, you’re likely to catch on to some of your own advice.
8. You’ll always be a student
The learning doesn’t stop once you become a certified coach. Science is always revealing new and improved ways to nourish both the mind and body. By keeping up with the latest in holistic health research, you’ll be able to provide your clients with the best experience possible.
9. You’ll also be a teacher
While part of your job is continuing education, the other part is teaching. It’s important that your clients know why you’re recommending specific foods, physical activities or sleep schedules. Arming them with this knowledge will boost their confidence in you as a coach and make them more likely to follow through with their programs.
10. One size does not fit all
Never forget that what works for one client may be a detriment to another (and vice versa). You must approach each client with a fresh mindset and avoid preconceived notions. Only by getting to know the client individually will you be able to architect a program that works for their needs.
11. Mental health is part of the package
From aging gracefully to maintaining healthy relationships, the mental aspect of holistic health is just as important as the physical. Science shows a strong correlation between mind and body, with mental stress contributing to an unsettling number of illnesses. Keeping your clients mentally nourished will help them to thrive physically.
12. It’s about prevention, not prescription
Holistic health is about taking the measures to prevent disease, rather than treat it. The idea is that taking care of mind, body, and soul will help people to avoid the illnesses that often accompany poor diet, stress, lack of sleep, and so on.

13. You can make good money
As with most professions, pay is commensurate with experience in holistic health coaching. While pay can range from about $50 per hour to $200 per hour (or more), the longer you work in the field, the more clients are willing to pay you for your expertise.
14. You need to take sleep seriously
Sleep is crucial to maintaining holistic health. Much like stress, failing to catch those Z’s can lead to a host of health problems. By advising your clients to make time for adequate sleep, you may save them the headache of a doctor’s visit later.
15. Listening is key
As a holistic health coach, you’ll partially be playing the role of detective. Active listening is the key to uncovering your clients’ problems and devising the right program to solve them.
16. You won’t always have the answers (and that’s okay)
Part of being a holistic health coach is knowing when it’s time to refer a patient to their physician. While preventative measures are effective, they don’t make your clients immune to illness. When in doubt, it’s always best to play it safe and get a medical professional involved.
17. You have to be willing to network
Networking isn’t just for CEOs in suits. If you want to make it as a holistic health coach, it’s vital to connect with peers in your industry in order to grow your career. By building a solid network, you’ll learn from like-minded pros and increase your referral business.
18. It’s your job to set attainable goals
Setting attainable goals is literally in your job description, but don’t forget about setting realistic goals for yourself. By breaking down your overarching goals into smaller, more manageable chunks, you’ll be able to take the baby steps that’ll get you to where you want to go.
19. Detoxes aren’t just for the body
While we��re big fans of dietary detoxes, there are many types of programs that can clear out the junk in clients’ lives. Don’t be afraid to think outside the box when it comes to wellness. Think home detoxes or even relationships detoxes – anything to keep your clients healthy and positive.
20. Things will change
Client needs are ever-evolving; what’s right for them today might be wrong for them next week. Stay cognizant of your clients’ goals and progress, quickly adapting their programs to accommodate their needs along the way.
21. Corporations are waiting for you
Workplace wellness is a $6 billion industry in the United States. Corporations often have built-in budgets for wellness perks, such as – you guessed it – holistic health coaches. This makes large companies a good starting point for job seekers in the health coaching space.
22. You can get certified for supplemental services
If you want to expand your offerings, getting your certification for related services, such as counseling or reiki (healing touch), can help you to boost your business and your pay.
23. You must diversify your income
If you use a single powerhouse as the source of your clientele, you could get yourself into trouble. It’s easy to get comfortable with (seemingly) reliable sources of business and say “to heck with the rest – I’ve got a good thing going here.” The problem comes when that source dries up; now you have to scramble to create a portfolio from scratch in order to support yourself. This problem can be avoided if you intentionally garner business from a wide range of sources.
24. It takes a love of people
Above all else, you must possess a hunger to help others. Holistic health coaching (or any personal coaching) is all about taking pride in the success of others. If this is what gets you up in the morning, it’s a surefire sign you’re headed down the right path.
25. Nobody’s perfect
Not you, not your clients, not Jennifer Lopez. Keep your standards high, but don’t beat yourself up if you fail. By adopting this attitude and encouraging your clients to do the same, there will be no limits to what you can achieve.

Fitness trackers’ heart rate monitoring accurate enough for most, study says

(CNN)The heart rate monitor inside your fitness tracker may not be as precise as the equipment used in doctors offices and hospitals, but researchers say the smartwatches and wristbands are accurate enough for most consumers’ needs.
When tested alongside electrocardiograph (ECG) technology, devices from Fitbit and Mio performed reasonably well at measuring resting and active heart rates, according to a study published Monday in the Annals of Internal Medicine.
“It’s very exciting because we’ve had so much advance in technology during such a short period,” lead researcher Lisa Cadmus-Bertram said. “These trackers are such an enormous improvement over what we used to have.”

For the small study, Cadmus-Bertram and her team at the University of Wisconsin looked at how the trackers worked for 40 healthy middle-age adults, compared with an ECG and against each other. The participants wore four devices during the test: a Fitbit Surge, a Fitbit Charge, a Mio Fuse and a Basis Peak smartwatch.
Compared with the ECG reading, wearable products varied in their accuracy. The Fitbit Charge performed the best at rest, measuring within 5 beats per minute of the ECG reading 95% of the time. The Basis Peak activity tracker was shown to be within 22.6 bpm of the ECG reading during the 10-minute resting test.

Accuracy lessened in all of the tested devices during increased activity. The monitors were off by a range of 20 to 40 beats per minute compared with ECG measurements. The findings that devices were more accurate during rest are similar to what previous research has found.
Although data were not provided to show readings at each measured time, Cadmus-Bertram suggested that outliers — numbers well outside normal readings — may have caused a wider range.
Critics question heart rate monitors
Critics, however, say that the devices are not performing up to their advertised promises and could supply users with dangerously inaccurate information.

Questions about the devices and their accuracy sparked a class-action lawsuit in 2016 over the technology in Fitbit trackers. The lawsuit was filed on behalf of people who bought Fitbits especially to help track their heart rates, whether for health reasons or to make sure they are getting the most out of their workouts. The lawsuit is ongoing.
“However accurate they may be at rest, the Fitbits are wildly inaccurate as heart rate monitors when worn during moderate- and high-intensity exercise, which is precisely the purpose for which Fitbit (in particular) markets them to consumers,” said Jonathan Selbin, one of the attorneys who filed the lawsuit.
Physicians and researchers note that the trackers aren’t medical devices. Selbin and others point out that Fitbit, in particular, “claims to be a ‘Digital Healthcare Company’ and is actively trying to get corporations and insurers to make health care decisions based upon data they collect.”

Researchers and physicians said the technology has a long way to go. Dr. Nisha Jhalani, a cardiologist at NewYork-Presbyterian/Columbia University Medical Center, who was not involved in the new study, noted that the technology in ECG machines and LED-based activity trackers is entirely different.
“Electrocardiography, or ECG, involves placing electrodes on the skin surface to measure electrical impulses generated by the heart muscle itself. The LED technology used in fitness trackers is an indirect measurement looking at the changes in light reflection through the skin during each heartbeat,” she said.
If you’ve ever worn a wrist heart monitor, you may have noticed a time or two during exercise when your heart rate read inaccurately low for a few ticks before getting it right. These hiccups are one reason, researchers say, why the range of accuracy of these devices is so wide.
“A tracker will give you the occasional crazy reading, but most of the readings will be quite a bit closer than that,” Cadmus-Bertram said.
Tracking your health
Overall, researchers and experts are encouraged by the improving technology and continue to promote the use of fitness trackers to provide more information about a user’s health.
“Fitness trackers are a great way to make people aware of their activity level throughout the day,” Jhalani said. “Oftentimes, our perceived activity is much more than actual time spent we spend moving, especially people whose jobs involve sitting at a desk for hours at a time.”

Heart rate data, she says, can be an empowering tool that can allow users to be more aware of their heart health and overall fitness.
“It is good for people to see their heart rate, both at rest and with exercise. Low resting heart rates, in the 60 to 70s bpm range, are considered generally healthy. High resting heart rates, especially when close to 100 bpm or higher, can be a sign of high stress levels or other medical conditions,” Jhalani said.
“How quickly the heart rate goes back to normal after exercise also holds a lot of information. Quicker indicates better cardiopulmonary fitness level.”
The compact, convenient and more stylish design achieved by using LED technology versus ECG comes with a few tradeoffs.
“LED technology can be affected by variables such as how loosely or tightly the tracker is worn, the user’s skin tone or any other interference between the sensor and the skin. It can also become inaccurate with motion, which is why it doesn’t fare as well as ECG with exercise,” Jhalani said.
In response to the new study, Fitbit said in a statement that its trackers “are not intended to be medical devices and, unlike chest straps, wrist-based trackers fit conveniently and comfortably into everyday life, providing continuous heart rate for up to several days without recharging (vs. a couple hours at a time) to give a more informed picture of your overall health.”

Representatives from Mio also emphasized that the product is a consumer device. “We need to make understanding heart rate easier for the average consumer,” the company said in a statement. “This opportunity defocuses the importance of needing to know the exact heart rate at any one time, and rather put emphasis on understanding what getting your heart rate up does for your health over time.”
Representatives from Basis could not be reached; the fitness tracker used in this study was recalled in September. According to the company’s website, the recall is “because the watch can overheat, which could result in burns or blisters on the skin surface.”
As a commercial product, Jhalani said, the overall impact is a positive one. “I think it keeps people from becoming complacent and empowers them to work towards a healthier lifestyle.”
“While you wouldn’t want to rely on a commercial tracker if you need absolute precision during exercise,” Cadmus-Bertram said, “there’s also no reason for the general public not to use it for feedback and motivation.”

In response to the new study, Fitbit said in a statement that its trackers “are not intended to be medical devices and, unlike chest straps, wrist-based trackers fit conveniently and comfortably into everyday life, providing continuous heart rate for up to several days without recharging (vs. a couple hours at a time) to give a more informed picture of your overall health.”

(CNN)The heart rate monitor inside your fitness tracker may not be as precise as the equipment used in doctors offices and hospitals, but researchers say the smartwatches and wristbands are accurate enough for most consumers’ needs.
When tested alongside electrocardiograph (ECG) technology, devices from Fitbit and Mio performed reasonably well at measuring resting and active heart rates, according to a study published Monday in the Annals of Internal Medicine.
“It’s very exciting because we’ve had so much advance in technology during such a short period,” lead researcher Lisa Cadmus-Bertram said. “These trackers are such an enormous improvement over what we used to have.”

For the small study, Cadmus-Bertram and her team at the University of Wisconsin looked at how the trackers worked for 40 healthy middle-age adults, compared with an ECG and against each other. The participants wore four devices during the test: a Fitbit Surge, a Fitbit Charge, a Mio Fuse and a Basis Peak smartwatch.
Compared with the ECG reading, wearable products varied in their accuracy. The Fitbit Charge performed the best at rest, measuring within 5 beats per minute of the ECG reading 95% of the time. The Basis Peak activity tracker was shown to be within 22.6 bpm of the ECG reading during the 10-minute resting test.

Accuracy lessened in all of the tested devices during increased activity. The monitors were off by a range of 20 to 40 beats per minute compared with ECG measurements. The findings that devices were more accurate during rest are similar to what previous research has found.
Although data were not provided to show readings at each measured time, Cadmus-Bertram suggested that outliers — numbers well outside normal readings — may have caused a wider range.

(CNN)The heart rate monitor inside your fitness tracker may not be as precise as the equipment used in doctors offices and hospitals, but researchers say the smartwatches and wristbands are accurate enough for most consumers’ needs.
When tested alongside electrocardiograph (ECG) technology, devices from Fitbit and Mio performed reasonably well at measuring resting and active heart rates, according to a study published Monday in the Annals of Internal Medicine.
“It’s very exciting because we’ve had so much advance in technology during such a short period,” lead researcher Lisa Cadmus-Bertram said. “These trackers are such an enormous improvement over what we used to have.”

For the small study, Cadmus-Bertram and her team at the University of Wisconsin looked at how the trackers worked for 40 healthy middle-age adults, compared with an ECG and against each other. The participants wore four devices during the test: a Fitbit Surge, a Fitbit Charge, a Mio Fuse and a Basis Peak smartwatch.
Compared with the ECG reading, wearable products varied in their accuracy. The Fitbit Charge performed the best at rest, measuring within 5 beats per minute of the ECG reading 95% of the time. The Basis Peak activity tracker was shown to be within 22.6 bpm of the ECG reading during the 10-minute resting test.

Accuracy lessened in all of the tested devices during increased activity. The monitors were off by a range of 20 to 40 beats per minute compared with ECG measurements. The findings that devices were more accurate during rest are similar to what previous research has found.
Although data were not provided to show readings at each measured time, Cadmus-Bertram suggested that outliers — numbers well outside normal readings — may have caused a wider range.

The lawsuit was filed on behalf of people who bought Fitbits especially to help track their heart rates, whether for health reasons or to make sure they are getting the most out of their workouts. “Electrocardiography, or ECG, involves placing electrodes on the skin surface to measure electrical impulses generated by the heart muscle itself. The LED technology used in fitness trackers is an indirect measurement looking at the changes in light reflection through the skin during each heartbeat,” she said.

Do I recommend or agree with everything they say?
No.
But I do enjoy both of their styles of teaching financial freedom using real estate.
The book centers around the 4 quadrants he claims we all fall into:

Which one are you in? 
Kiyosaki claims that most people are on the left side or the “poor side” of the quadrant. They’re either employees or self-employed.
Most physicians and dentists are typically on the left side too as they mainly trade their time for money.

The goal of the book seems to motivate the masses to move from the left to the right side of the quadrant….and he does a pretty darn good job of it too.
