Dead to You (Are You at the Wake?) - Chapter 4: You Had to Kill Me but It Killed You Just the Same
“Luke Skywalker, in the flesh.” says Luthen Rael, who if all goes according to plan, will be Luke's new boss. “-delighted to meet you.”
Luke laughs and shakes his hand.
“You sound surprised.”
“Well, my boy.” He says, “-look at it from my perspective. A young, extremely qualified and experienced Mortician calls me, an aging one man show, to tell me he's moving to the middle of nowhere, would I have a job for him? Sounds too good to be true. You can never tell with the internet.”
“I suppose you have a point.” Luke says, as they walk through the tiny funeral home.
“Tell me son,” Luthen says, “-why did you become a mortician?”
Luke gives him the same spiel he gave Din, about being eminently qualified, tells him about Wedge. He knows that Wedge would never tell anyone about the inquiry unless Luke told him to, so he's not too worried that this guy would dig.
He also leaves out the bit about the pull toward death he’s felt his whole life, as he's really only coming to terms with it now.
“I see.” Luthen says, “-why move all the way out here?”
Luke puts on a sheepish grin.
“Ah, well, I… followed my boyfriend.” He says, “-I know how that sounds, but-”
Luthen laughs.
“-and you're not even married? He’d better put a ring on it soon.”
Luke snorts but his stomach twists.
Him and Din are bound together now, in arguably a more permanent way than a marriage vow. But.
Luke pushes that thought aside.

Din’s son is dead: To begin with, there's no doubt whatsoever about that.
What there is doubt about, however, is how Din can possibly go on; having loved and lost in a way he never expected he would.
The only thing keeping him moving is the thought that Grogu would want him to. So, he soldiers forth, through the numbing pain and the aching grief; just trying to get through one day at a time.
Until he meets Luke.
Luke, who refuses to leave Din’s loneliness unbroken and makes him feel like, maybe, he could live again.
It's only fitting, then, when Luke reveals that he is a Mortician. It's his life’s mission to help distraught families send off their loved ones to whatever awaits in the undiscovered country from which no traveller returns.
Din thinks that being surrounded by death must be his life’s new purpose.
Din is more right than he knows.

I've seen a lot of OFMD fans pointing at other fan campaigns recently, especially the failed campaign to save Anne with an E, and getting disheartened by what they see, and I think something that's missing in those discussions (particularly after the weirdly negative slant at the end of that Guardian article) is that not all fan campaigns are the same.
The campaigns to save shows that started out on Netflix (Warrior Nun, Sense8, Anne with an E, Shadow and Bone, etc.) are very different to what we are facing with OFMD, because Netflix is much more protective of its IP - and yes, they even hold the IP of shows that they've cancelled tightly to their chests and very rarely let them go. Some have been able to make the jump to different platforms, but not many, and it's actually pretty hard to do so (“For most shows, shopping away from Netflix is reportedly very difficult when Netflix owns the underlying IP”).
Because of this, fans of shows cancelled by Netflix are often campaigning directly for Netflix to reverse that decision, rather than campaigning to get it picked up elsewhere. Warrior Nun (I saw people panicking at the length of effort and the numbers they had to put in before they got their success) and Sense8 were both granted final films by Netflix to wrap up their stories after long-running successful fan campaigns - they were not saved by a different platform, and likely couldn't have been because of Netflix's strict grip on its IP. So comparing ourselves to fan campaigns for shows cancelled by Netflix isn't accurate or fair to us or to those other campaigns.
It's also worth pointing out that Anne with an E was a joint venture between Netflix and CBC, and that relationship appears to have fallen apart, which may have been a factor in its cancellation (see more at this tweet). This is also likely a contributing factor as to why, despite the campaign to save it being huge, it didn't get the reprieve from Netflix that other shows have received. That is baggage that OFMD simply doesn't have.
OFMD is in a very different position to many of the shows that were cancelled by Netflix and then went on to have fan campaigns around them, in that we have confirmation from HBO Max that they are happy for it to be shopped around. HBO Max is not holding a firm grip on the IP the way that Netflix does. We are able to try to find OFMD a new home. And that's a very different sort of campaign and trajectory than trying to convince the platform that cancelled us to reverse/amend their decision (I'd also argue it's slightly easier - nothing is certain, nothing is easy, but I think we have a better chance being allowed to look for someone else to pick us up rather than relying on the company that cancelled us to show us mercy).
Yes, some fan campaigns have had many more petition signatures than us (don't sniff at 80,000 signatures in two weeks, that's huge and fast), but that is only one part of the story. Petition signatures alone won't be the thing that clinches it. There are fan campaigns with hundreds of thousands of signatures that have failed, but there are also fan campaigns with way less signatures than us that have seen their show saved. There's so many elements at play, and every single one is unique. It's very tempting to play 'like for like' with these sorts of campaigns, but it's simply not a truthful reflection of the situation.
Every show is different. Every fan campaign is different.
We have every reason to be hopeful, and every reason to keep fighting. OFMD's numbers are good, it's already incredibly marketable to other streamers, and then ON TOP of that the fan reaction has been loud enough for a lot of people to take notice.
Yes, we're in a dip right now because that's what happens - every campaign has peaks and troughs because nothing sustains at the same level forever, and, again, it's only been just over two weeks and look at what we've done - but the impact we had, and the noise we made, happened at exactly the right time and kept happening for long enough to make an impression. A lot of things will be happening behind the scenes right now that we don't know about, and there's so many factors at play that we have no control over. It's a long game.
So don't look at other fan campaigns and be disheartened by what you see. Because what you see on the surface is nowhere near close to the full picture.
Even if we don't get our S3, lets celebrate the fact that we have made a huge impression, kicked up a huge stink, and been the thorn in the side of some people who absolutely fucking needed a couple of thorns in their side. And I think that's the best legacy that our rebellious little gay pirate show could possibly have.

Zayn signed with a live show booking agency!!! Like is it possible that he just is always signed with someone and needed to switch it up, sure, BUT IT'S ALSO POSSIBLE HE'S GOING TO PLAY LIVE SOUND THE FUCKING ALARM! They POSTED ABOUT SIGNING HIM... maaaybe??? Wow and you thought having OT4 out there all playing shows at the same time this year was gonna be a lot OT5 are really coming for our necks (and wallets jesus.)
Lights Up video director Vincent Haycock tells us that we will never (ever ever) see the director's cut of the video. BOOOOO WTF that's terrible news! And WHY NOT? Many turned their sadness over this news to anger, asserting that it was a conspiracy on the part of HSHQ because the full length was too subversive. Well it could be, what do I know! If people wanna blame Jeff and boo him about it you know I'm always down for that. The full length video of the Howard Stern interview and performance had been posted, though (speaking of conspiracy theories) someone said it wasn't full length even though it says that ("full length"), that maybe the show had edited? Idk. And a long CBC interview (recorded last week) aired. Harry says the album is called Fine Line because it's about "the fragility of emotion and how fast that can change," and that the idea that you can't make certain kinds of music because the fans "won't get it" isn't worth limiting yourself for because (to paraphrase heavily) the fans don't know less about how to listen to music than people in the industry do.
Louis' Madrid show was added for Mar 10 ("a small show" according to Louis) but sadly the Milan show (Mar 11) was cancelled (along with all other large public events in Italy it's not just Louis.) Fuck! I hate this for Louis! It's the TOUR the biggest most anticipated thing that should be the happiest moment and this hanging over the whole thing! I mean I hate the possiblity of a global pandemic and people dying quite a bit as well, yes, but LT TOUR, COME THE FUCK ON HIGHER POWERS. Anyway some do but I don't think it's weird he added the show last minute personally. He said a while back that he'd be adding more shows probably, and I wouldn't be at all surprised if they do more very close to the dates (assuming we aren't living in some kind of post apocalyptic hellscape that prevents that by then.)
Niall posted video and pics from the Spotify singles recording, and today's HBBW song is Everywhere, yes: another single common word song title that's confusing to talk about because it isn't clear you mean the song necessarily. The lyrics today are, "you walk into the room I go quiet. I catch your eyes and don't blink an eyelid. Feels like the world locked us on an island. An island without waves." He said the lyrics he's sharing aren't his favorite ones, that he's saving those for next week, but idk if I believe him about this song, he was at least excited enough about the island part to tweet it a while back. The daily snippets are going backwards through the track list, counting down to the first song and the release, so tomorrow's is Arms of a Stranger, which we learned is the song Niall wrote in a dream! Looking forward to that one. Taylor Swift praised the Lover cover ("absolutely stunning") and you know Niall was pumped for that so he's having a good day I betcha.

'Boys have role models, why shouldn't we?' How female superheroes like Captain Marvel are filling a void
There's a scene in the new film Captain Marvel in which the titular Captain, played by Brie Larson, is facing certain adversity. To draw strength, she experiences a flashback to all the times in her life when she was knocked down and told to quit because, well, she was just a girl.

You can also see the moment in a teaser trailer, which shows the young character getting knocked down over and over again, only to turn and give the camera that defiant superhero stare: a look that says, no matter how big the odds stacked against the character, they will overcome them. It's an image we've all seen before, but for such a cliché, it's a scene we don't see given enough to women onscreen, let alone female superheroes.
"You don't often see a movie in general with such a strong female lead, so seeing that really hit home for me," says Saara Chaudry, 14, a contributor to CBC Kids News who saw an advance screening of Captain Marvel. "It made me feel warm, knowing that women were being represented in this light, but at the same time it spoke to personal experiences that I've had. It had me on the edge of my seat really just thinking about every time that I've been knocked down."
That sentiment was certainly intentional, according to Anna Boden, who co-wrote the film with Ryan Fleck. "As she gets to know herself and embrace what makes her her, she really achieves her true power," Boden told the New York Times, of the character. "Part of that means rejecting the voices of people who tell her she's not strong enough and doesn't belong. I feel like a lot of people will be able to relate to that, particularly women."
Captain Marvel is the 21st movie in the Marvel Cinematic Universe, and yet is only the first film to be centred around a female superhero. DC Entertainment beat Marvel to the punch by two years with Wonder Woman, the Gal Gadot-starring box office and critical juggernaut that is still one of the top-grossing films to come from the DC Universe (a sequel is already slotted for 2020).
Two films in two years may not seem like a lot — it's not — but to put it into perspective, it was only in 2015 that a leaked email from Marvel chairman Isaac Perlmutter revealed his distaste for female superhero films, citing Supergirl (1984), Catwoman (2004) and Elektra (2005), blaming them for poor box-office performances and calling them a "disaster."
Wonder Woman's success certainly helped to open the door a crack, but the fact is, in the 10-plus years that we've been immersed in the current superhero boom, the lack of films centred on female characters has had a negative effect on young women.
SuperPowering Girls
In a study published last year called "SuperPowering Girls," conducted by the Women's Media Center and BBC America, researchers found that representation on film matters, especially when it comes to superheroes. Seeing female superheroes makes girls feel more "strong, brave, confident, inspired, positive and motivated," it states. Almost 2,500 participants were surveyed, male and female, including kids between the ages of 10 and 19 and parents of children aged five to nine, who answered on behalf of their children.
Among young boys, almost half (49 per cent) cited male sci-fi/supeheroes as a top role model. For girls, that number dropped to just 21 per cent. And yet, outside of the family, parents of young girls said they were most likely to look to superheroes as role models, with Wonder Woman being the top choice for girls, followed by Supergirl. A full 65 per cent of girls said there were simply not enough female superheroes. In fact, one question that both boys and girls overwhelmingly agreed on was that there needs to be more female heroes.
Big heroes, small screens
While the big screen has been slow to give female heroes their spotlight, it's a different story on the small screen. DC's Supergirl has been on the air since 2015, and is currently in its fourth season on the CW Network. Batwoman, another CW show, is also in the works, while Marvel's Jessica Jones will see its third and final series air on Netflix this year.
While all those shows appeal to a decidedly older audience — anywhere from young adult to adult — an animated series geared towards even younger fans has been successfully running on YouTube since 2015. DC Super Hero Girls features short episodes, often under five minutes, focused on the young lives of various female heroes, such as Wonder Woman, Batgirl and Supergirl, as they navigate their way through school. On March 8, DC Super Hero Girls will relaunch as a full-length series on the Cartoon Network, created by Lauren Faust (My Little Pony: Friendship is Magic, The Powerpuff Girls).

Tara Strong, who was born in Toronto but lives in Los Angeles, has been a voice actor on the show since the beginning, and will voice Batgirl and Harley Quinn for the series relaunch.
"I was always drawn to these strong female characters, so it feels like such an honour to be able to play them and inspire young girls and boys," she says. "These are super strong characters that are smart and conscientious and care about friendships and, you know, use their brains."

Strong says that representation of female heroes has come a long way since she was a kid, and points to the current boom as having a positive effect on young fans.
"I remember loving the Wonder Woman show with Lynda Carter, I loved The Wonder Twinscartoon, and I remember that what we had was good, but it's so nice to have what we have now," she says. "It's a huge improvement, especially on the female empowered characters."

Strong adds that female superheroes have "become more sophisticated" over the years, and "as a whole been given a lot more to do, more powers, more voice in everything that they're doing. I think it's just sort of how the times have changed. DC has always been pretty conscientious about changing with those times."
While DC was certainly the first to give more screen time to prominent female heroes, if Captain Marvel is also a success, we can only expect to see more from that camp as well. Judging by future projects, the studio already seems to be banking on female heroes for its post Avengers: End Game rollout, hiring director Cate Shortland to helm a long-rumoured standalone movie for Scarlett Johansson's character Black Widow, which includes a script from Jac Schaeffer, one of the women on the writing team for Captain Marvel.
For young women like Chaudry, it couldn't be soon enough.

2019 - Asian Championships Aurora Games
2018 - Looks like we lost the 2018 Commonwealth Games vault final which was the only part of that competition we have. It’s a good time to point out again that the Commonwealth Games are incredibly aggressive only behind the Olympics about striking videos. If you haven’t watched the current year competition yet and you want to you should make it a priority.

The Chinese National Games... and I mean a LOT of it. It’s broken in parts so it’s missing at least one bit of one sub from qualification but otherwise it’s largely complete with qualification, team final, AA, and EFs.
2004 - A little over half of Athens qualification (2 full subs, half of another though nothing from the American sub), a bit of the Athens team final from the BBC, 3 rotations of the AA from the BBC, and the entire AA from Brazilian TV.
Unfortunately it looks like the only copy of the Athens FX EF was nuked. In trying to see if there was any other version of it still on youtube I found some partial coverage from NBC and have put it in it’s place. It’s not ideal but I really can’t tell you how much building an olympics from youtube coverage can be a bit like filling the holes in swiss cheese.
2003 - Universiade Team Final
A full length no commentary version of the Worlds TF ... but it is deeply potato.
2002 - Asian Games AA - I don’t know how I missed this one the first time because i grabbed the team final and event finals from the same source.

Canadian junior and senior championships. This is video from the stands which I often pass on but I make exceptions and non-US championships is often something I’ll go with if it seems complete which this does.

Ahh editing to fit a time slot. The European Masters of Gymnastics is a now defunct meet format that involved teams of WAG/MAG/RG not unlike USAG’s 3 on 3 but... also so much more complicated.

1992 - Two versions of the Worlds semi finals (both are Eurosport so likely only differ in commentary one in english and one in I believe dutch). Canadian Olympic Trials. The Spanish version of the Olympic AA final.

1980 - 21 minutes of the Moscow AA with Japanese commentary. It’s in 3 parts but I suspect that is all of that one since that is about the length the sometimes would edit down an AA.
1976 - 8 of 10 parts of the Olympic AA. As is the general rule if something was uploaded in a million parts more than a decade ago some amount of it is missing.

Ultimate fanboys P@gey+Em3rgency Awesom3 think the Purple Lightning used for Iris as Flash in 4x16 is solely for contrast and has no connection to the Tornado Twins. However, parallel+symbolism are big on ⚡️In FP when Wally+Barry fight the Rival, Cisco specifically mentions (2) distinct funnel patterns, tornadoes 🌪 measuring f13 on the Fujita scale. They were Purple like the outfits that the original TT wore. It was a nod to WA’s Twins. I think the lightning is too!
I also agree the purple is in connection to the Tornado Twins and Jenni, and I’m hard-pressed to understand how anyone could think otherwise lol.
I’m not really sure what to tweet for Run Iris Run but if I retweet & like everything I see will that help keep the momentum going and help improve the overall sm presence??

I’m so excited for Run Iris Run. Candice will kill it with her performance as usual and her fans are going to show up in huge numbers but I worry a bit about the writing. If the writing falls through the episode may not be good. I don’t want too much forced humor. Honestly how much faith do you have in Eric Wallace?

Joni Mitchell is a legend. She’s also a bitch. Which is cool. I’m a bitch. But I’m also something that Joni is not: A proud feminist.
In a 2013 CBC interview with Jian Gomeshi (a real piece of human shit, who got canned in 2014 for sexual assault of his gf, which he conveniently documented on his work phone – call him patient zero of #metoo), Joni distanced herself from the movement. Here’s the exchange, cut and pasted from her archive:
JG: Well, do you feel like – do you feel then, and especially doing this all as a woman too and what you just talked about in terms of the terms of your contract that Neil and others were followed in your footsteps, do you accept that you were a pioneer?

Together, we decided that maybe making her flight, rather than her self-imposed deadline, was more important. And she decided that I should just cover this record like any other. This is a music blog, so focus on the music?
Right on. Let’s talk about summer and grass.
Joni Mitchell was riding high in 1975. Her 1974 album, Court and Spark, was nominated for four Grammys. She won one – Best Arrangement, whatever that means. Court and Spark is one of the best adult (an adjective that wasn’t as pejorative then, as it is now) records of the ’70s. Impeccably produced, for sure. Critically received, for sure. And popularly consumed. Cooler than Seals & Crofts, and more cerebral, but in the same general ballpark. If you were a kid back then, you heard the single “Help Me” regularly on AM radio. And the record is one that your cool aunt and uncle had in their vinyl collection to play on the hi-fi for the grownups smoking crooked cigs during fondue or curry parties, while you and your cousins were screwing around in the back yard and didn’t notice. Court and Spark was/is foreground and background, depending on your situ and mood. It’s aggressively L.A., like the Eagles’ Hotel California, but totally different. (The culture-change from ’74 to ’76 was seismic. I was there, and I could tell.)
Court and Spark laid a foundation for Joni to explore even further her version of proto-adult-contemporary. Some people call her stuff from this period jazzy – mostly because there are jazz instruments backing her, I think. What’s jazziest, though, is her vocals. Her approach is sui generis. Joni’s most famous song, “Big Yellow Taxi” from 1970’s Ladies of the Canyon, follows a traditional verse-chorus-verse pop-rock structure. So do “California” and “A Case of You” from 1971’s all-time classic Blue. Much of her output from the mid-70s, however, is an extended argument against that form.
Take a sorta well-known track like “You Turn Me on I’m a Radio” from 1972’s For the Roses. It’s a vocal riff over a strummy guitar and harmonica background. The melody stays the same, but she refuses to give us anything lyrically to grab and hold. Nothing repeats. She sounds like she’s making shit up in a heady, controlled way. (A compliment, not a slam.) She never scats; her words in her voice are just light as air, and they float like the sound from a sax or a trumpet. The Hissing of Summer Lawns isn’t the culmination of that style (her 1978 album Hejira, featuring bass virtuoso Jaco Pastorius, probably is), but it’s a statement of purpose. As in, you guys liked “Help Me”? Well here’s more of that, only weirder. Light up and sit back. She’s got you.
I’ve grown to love this record. I’ve listened it at least ten times for this post. It’s not as good as Blue or Court and Spark. (On a road trip last weekend, my best friend checked out on THoSL around the middle of what would’ve been side two.) But it’s more interesting, and it’s thematic for our purposes here – whose lawns aren’t hissing for more water in late August? Worth a spin. Key tracks: “In France They Kiss on Main Street,” which is a deeper dive in to the vibe of Court and Spark‘s “Free Man in Paris;” “The Jungle Line,” which samples field recordings of drummers from Burundi; the title track, which oozes cool; and “Harry’s House – Centerpiece,” which is a single-track song-cycle that’s almost indescribable. In a buffer overflow attack, the data is intentionally made to overflow into areas where important executable code is stored, where it can be overwritten with malicious code, or altered to change a program’s behaviour.
In C, there is no built-in protection against accessing data outside of certain boundaries, so it is essential to implement boundary checking when dealing with arrays.

Block Cipher – to secure a msg (encrypt / decrypt)
An algorithm which takes a fixed-length of bits called a block and applies a symmetric key and cryptographic algorithm (rather than applying it to one bit at a time – stream cipher). The previous block is added to the next block so that identical blocks are not encrypted the same. (I think this is not true of all block ciphers but good ones do this - ECB doesn’t that’s why it sucks) An initialisation vector also help prevent identical ciphertexts – they are a randomly generated set of bits, added to the first block and key.
Block Modes – for authentication
An algo that uses a block cipher to provide confidentiality / authenticity. It describes how to repeatedly apply a single-block op to securely transform data greater than the size of a block. They usually require an IV which must be non-repeating. Data greater than a single block is padded to make it a full block size – some modes don’t pad they act more like stream cipher.
ECB – Electronic Codebook: The simplest. Do not use for cryptographic protocol
Each message divided into blocks and encrypted separately -> this means identical plaintext becomes identical ciphertext leaving it vulnerable to pattern searching attacks. It is also susceptible to replay attacks as each block is decrypted the same way
CBC – Cipher Block Chaining
In cipher block chaining each message is XOR-d with the previous block (kind of like Merkle-Damgard ?) This makes it more secure than ECP mode as you cannot decrypt individual blocks – they are all dependent on the previous block. The initialisation vector is unique for every message, it does not have to be secret but keeping it secret probably adds a bit of extra security
CTR (Counter Mode)
CTR mode changes a block cipher into a stream cipher (i.e every bit is encrypted rather than every block). Kestream blocks are generated by encrypting a nonce with a counter ( I think - come back to this )
Moore’s law
Processor speeds will double every 2 years – i.e. the number of transistors on an affordable CPU double. This was a prediction made in 1965 and despite many others predicting this rate would slow, so far it has been pretty accurate
Cold Boot Attacks
A side channel attack used to gain access to the encryption keys of an OS. The attacker needs physical access to the computer to perform a cold reboot to restart the machine. After a computer is switched off there is a small window where the RAM holds onto data. In a cold boot attack, the contents of the RAM can be copied to an external drive in this time.
Attackers can also lower the temperature, giving them more time before the RAM fades due to lack of electricity. The computer must be forcibly turned off so that the processor does not dismount important data. Most modern systems wipe memory early in a boot process to help prevent these attacks. Of course this attack can be stopped entirely by remaining with your computer for a few minutes after it’s switched off, ensuring no one has access to your machine.

Block Modes:
From Richard: Read up on block modes - ECB, CBC, CTR. These are define ways of joining blocks of data like in Merkel-Damgard.
Block cipher modes: since DES and AES only take 64 and 128 bit size blocks respectively, we use Block Cipher Modes to divide files (greater than 128 bits) into chunks so they can be encrypted.
Electronic Code Block (ECB) - a simple ‘divide a file into blocks of 128 bits and then encrypt them all separately’ method.
why this sucks: since we are encrypting chunks as individual blocks, that are not mixed in with each other, the same block of plaintext repeated in a message will lead to repeated blocks of ciphertext. This is like with vignere when the key repeats and the text and key line up so you can start to do analysis to break the key.
This is why, in the penguin picture, large chunks of the photo with the same colour get encrypted to the same “cypher colour”, since the original colour is repeated.
Cipher Block Chaining (CBC) - fixes the problem with ECB by encrypting 1 block, then including that in the next block’s ‘plaintext’ which is then encrypted as all one chunk and repeated until the final block emerges.
This means every block is a result of all the blocks before it
Unfortunately must be sequential and so is inefficient
Counter Mode (CTR) - generate a random NONCE as well as counter value, then encrypt those 2 things to get a long stream of pseudo-random bits. These random bits are then used in stream-cipher fashion in that they are XOR’d with the plaintext to get the final cipher for that block.
The uniqueness of the counter means that, with diffusion in AES, there is no repetition problem that occurs with ECB
The fact that blocks are calculated separately means this method is able to be done with parallelism and so is faster than CBC.

- Next, if you keep pressing keys, it takes 16 keys for a new block to occur, so then by writing a 16 key string twice, you should be able to see what gets repeated and hence find the ECB blocks, my string will be:

Abnormal Uterine Bleeding – Currae Hospitals
Today, We are going to talk about a very common topic, for which we see the patients in the OPD, that is abnormal uterine bleeding. Now, what is abnormal uterine bleeding? Abnormal uterine bleeding is when there is an abnormality in the duration of the cycle, the menstrual cycle, amount of flow and the cycle length.
To know what is abnormal, you should know what is normal first. So, what is a normal menstrual cycle? There’s a lot of doubts in the people like, you know, when they start, what is the normal menstrual cycle has to be? A normal menstrual cycle lasts somewhere between 4 to 6 days, the cycle length that is the duration of the cycle should be between 21 days to 35 days, and the blood loss should not be more than 30 to 35 cc. Now, when it is abnormal is when the cycle length, the length of the cycle is less than 21 days or above 35 days, if the bleeding is above 80 cc, i.e. more than 80 cc of blood loss, and the duration is more than 7 days. So, this is basically what we call as an abnormal uterine bleeding.
Now, there are a few terms which you must be noticing it when you come to an OPD or when the doctors discuss your case, i.e. dysmenorrhoea. In the day-to-day life, you know, you must be listening to terms like dysmenorrhoea. What is dysmenorrhoea? Dysmenorrhoea is painful menses or painful menstrual cycle, same way, polymenorrhea is when the cycle length is short, i.e., it is you get menses in less than 3 weeks. Menorrhagia is other term which usually we hear is when the flow is more, i.e. more than 80 cc of blood loss or more than 7 days of bleeding. Amenorrhea that means there is no periods at all. And metrorrhagia, i.e. in between periods when you get bleeding, that is these are the common terms which I used.
Now, what are the causes of abnormal uterine bleeding? We always think that now, according to the age, the causes of bleeding have been divided. A young girl, i.e., she is in her puberty can come with menorrhagia. Menorrhagia means heavy flow or abnormal uterine bleeding. Puberty menorrhagia what we call as… so, these girls basically have got a history of early menarche that means they start their periods less than 10 years of age or they have got precocious puberty that is early puberty, so their management is accordingly. The second thing is when it is in the adolescence that means they get their periods at the normal age, mean when they menarche that is around 12 to 14 years of age, but they have heavy periods. Now, why, what are the causes? What we suspect is that either they are anovulatory cycles, i.e., because the hormonal axis is not yet developed, so these girls deal a lot, or the other causes are any coagulation defect. Some girls have got or some families have got genetic history of, you know, bleeding dyscrasias, so these patients have to be treated accordingly.
The other age group is again the reproductive age group. So, in reproductive age group, they can be abnormal uterine bleeding as a cause of complication of pregnancy, like either abortions or ectopic or other conditions like either fibroids or adenomyosis, polyp, and dysfunctional uterine bleeding, i.e., an abnormal hormonal cycle.
In perimenopausal age group that means around 40 to 60 years of age, again, the patient presents to us with abnormal uterine bleeding. So, in these patients, what we suspect is, first of all, a dysfunctional uterine bleeding, i.e., hormonal imbalance or endometrial hyperplasia that means the endometrium formation is more. Other conditions like either fibroid or adenomyosis, polyps, and even carcinoma to that matter.
In postmenopausal ladies, again once the menses are stopped, we are not supposed to bleed. At least when we label the patient as menopause that means 1 year there is no period and then again she starts bleeding that is a matter to be worried about. These patients have to be definitely assessed with regards to their endometrial hyperplasia, carcinoma, polyp, so and dysfunctional uterine bleeding.
And last but not the least, is iatrogenic that means some factor which is causing problem introduced by yourself. For example, if you have Copper T or an intrauterine device which can cause an abnormal uterine bleeding. For example, now the festive season is coming up, so most of the ladies come to our OPD, we want to postpone our periods, so some medicines that is oral and injectable steroids; or inbuilt medications like psychotropic drugs or patients who are already on medications, they also can present with abnormal uterine bleeding.
Now, what is the commonest cause of abnormal uterine bleeding, that is DUB, dysfunctional uterine bleeding. Now, what is the DUB? DUB is a bleeding condition when there is no organic pathology involved. Organic pathology, as in there is no fibroid, polyp, or any other inbuilt cause which is involved. There is just a hormonal imbalance. There is no iatrogenic factors like any Copper T or any medicines, which are on, or any other, other hormonal problems like thyroid, you know pituitary causes and all that. So, commonest causes are hormonal imbalance that is DUB. Now, in DUB, in 90% of the patients, it is either anovulatory cycle that is where the oestrogen which is the hormone, which supports the whole menstrual cycle, is produced in excess and there is no progesterone support to it. In such patients what happens, the patient starts bleeding and she continues to bleed because there is no support of progesterone which is there. And the other 10% have got ovulatory cycles, where they have excess of progesterone, where the progesterone is irregularly supplied in their body and that causes irregular bleeding or excessive bleeding because of irregular shedding of the endometrium.
The other causes are thyroid disorders, which is again one of the causes for abnormal uterine bleeding where the patient does not even realize whether the patient has thyroid or not, unless a blood test is done. When the patient comes to our OPD that is the first time when a thyroid has been sent and it comes to a very high level. Only correcting the thyroid can make her periods normal. So, that is thyroid or pituitary conditions.
Secondly, the most most most common is PCOD. Nowadays, 20% of our OPD patients come with complaints of PCOD. They usually present with complaints of amenorrhea that is no periods at all for 3 to 4 months, but once they start bleeding, they bleed a lot. They bleed for 15 days, 20 days continuously at stretch, which is not good. Now, PCOD that is polycystic ovarian disease is a condition where there is a hormonal imbalance in the patient and these patients have anovulatory cycles. So, these patients usually present with the symptoms of obesity that is excessive weight, more than the BMI. They have features of acne, hirsutism, and irregular cycles. Obesity, one more major factor for periods becoming irregular and with heavy bleeding in between, like they have amenorrhea of 3 to 4 months and then they again present with… when they start bleeding, they bleed, profusely they bleed. So, obesity also has to be controlled. I will be talking about the management later.
Now, when the patient comes to the OPD, first of all, we have to do an examination, a general examination, which includes weight, as I said, obesity is one of the factors for heavy periods. We have to check her BMI. What is her BMI? If she is in the high BMI then accordingly lifestyle modification has to be advised. Similarly patients with PCOD, when they come, they have to be counselled about weight loss, about lifestyle modifications, about maintaining a healthy lifestyle, losing weight. Acne, hirsutism is for cosmetic reasons, but for the Gynaec point of view, we have to monitor their weight.
Other features like thyroid problem or any other blood tests, blood tests have to be sent, of the patient. Haemogram , to know their basic CBC that is the basic haemoglobin of the patient. A proper history is the most important thing. When the patient comes to our OPD, they have got a very vague complaint, that I bleed every month but I bleed for a longer period of time. Now, the history what the patient presents is very subjective. For a patient, bleeding for 2 days, maybe she is bleeding for 2 days but maybe she is using maybe 8 to 10 pads in a day. When I take the history of the patient, I just don’t go by the days of bleeding because we Indians have usually take it for granted that it’s good to bleed. No, it is not good to bleed. We don’t have to have a heavy flow or passage of clots because that invariably makes us anaemic. So, when a patient comes in the OPD with saying 2 days of bleeding, I have to definitely ask her about the amount of pad she is using every day, how much is the bleeding, how many pads does she use, does she pass clots or not, how many days does she bleed. So that gives me a basic idea about how much is the blood loss of the patient. The external feature of the patient, she might be looking pale, she might be white as, you know, she must be just pale, too pale, but she says that no, that much, that is a regular menstrual cycle. So, a Haemogram that is a CBC tells me about how much is the blood loss that she is having.
Similarly, with the sonography, sonography usually tells me… after examination, history, investigations; after the CBC, a thyroid profile; in young girls of PCOD, maybe a hormonal profile, and a prolactin level, and a sonography. A sonography tells me about her inbuilt pathology, if any, like a fibroid or a polyp or any other adenomyosis, which is the cause of bleeding. See, if the cause is known, the treatment of the cause is most important.
Similarly, after sonography, there are other procedures like hysteroscopy. Once the cause is detected like a polyp or a fibroid. A hysteroscopy can be both, it can be either a diagnostic hysteroscopy where a scope is introduced through the vagina into the uterine cavity and we have a generalized look, whether there is a polyp involved or any adenomyosis and a submucous fibroid which is causing a bleeding. Similarly, if the cause is ruled out that is there is no clinical cause then we can treat her only with hormonal medicines. At the same time, hysteroscopy also helps us to treat the cause. For example, when we do a hysteroscopy, there is a polyp which is there, we can immediately remove it. If there is a fibroid, we can immediately remove it and send it for testing.
Other things are endometrial biopsy. Whenever we take a patient for hysteroscopy, it is always associated with a hysteroscopy-guided endometrial biopsy because in patients who are perimenopausal or who are postmenopausal, a biopsy always is sent for histopathological examination and it gives us the result whether there is a carcinoma or just a hyperplasia. So our line of management changes accordingly.
Now, if there is some other pathology which is involved, the treatment options I will be going ahead with, first of all, just general haematinic which are given to increase the haemoglobin of the patient because a pale patient or a patient who has bled a lot generally has complains of weakness or you know loss of interest in doing daily activities. So, these patients definitely have to be prescribed with haematinics. A patient with PCOD has to be explained about lifestyle management. Whatever the patient comes, we have to treat what is bothering her more. Most of the patients, when they come, they are not worried about their period, how much is the flow of the period, they are just wanting that it just becomes regular [ki bus regular ho jaye]. They have to be treated according to what is their requirement at that time. If she is a young girl, she is in her puberty and she is bleeding a lot, so right now, the concern for the parents is to control the bleeding. Once all the tests are done and there is no problem, we have to treat her. If she has a coagulating disorder then we have to treat her accordingly. Same thing is with adolescent girls, if she is bleeding continuously, we just can’t leave her like that, we will have to support her with progesterone. So, the hormonal management comes into play where we put her on hormones and we have to control, at the same time, give her something to build up her haemoglobin, so that because these are young girls, they are growing girls.
Same way, in a patient who are in the reproductive age group, if she has complaints of bleeding, first of all, rule out the cause like any iatrogenic cause like a Copper T or a medicine what she has taken, for example, an I-Pill or any other hormonal medicines to postpone the periods, so that has to be removed, regularize her cycle. If she is in the reproductive age group, not wanting for a pregnancy but wanting regular cycles, we can even put her on hormonal medicines that is oestrogen-progesterone combination all birth control pills. Same way, if the patient is bleeding continuously that is an anovulatory cycle where there is a constant oestrogen supply which is there, we have to support it with progesterones. Now, progesterones can either be given in a continuous manner or in a cyclical manner.
If a patient in a postmenopausal age group comes with complaints, we do an endometrial biopsy, there is only hyperplasia, we can wait or advice for a hysterectomy. Same way, we rule out any carcinoma because if there is any carcinoma then the line of management is an extensive surgery. If a patient comes with complaints of fibroid or polyp or adenomyosis on a sonography then the fibroid has to be removed, now that we can do it either laparoscopically or hysteroscopically, which has got a very good patient compliance and much faster recovery.
If the patient is a young girl, family complete, but still she is bleeding, we cannot do a hysterectomy for this patient. So, in such patients, we advise them for progesterone intrauterine device that is Mirena or levonorgestrel IUCD, which gives continuous release of progesterone and that regularizes your cycle. So just an intrauterine device, a progesterone supportive intrauterine device which can be inserted and the bleeding can be controlled, that will give a dual protection, she will have contraceptive help also as well as her uterus is conserved.
If she is in the elderly age group, if the patient is about 45-50 and the family is complete, she is bleeding. On the endometrial biopsy, there is only hyperplasia and she is not wanting to keep her uterus then in such patients, we advise them for a hysterectomy, which can be done again either laparoscopically or an open hysterectomy.
So, whenever we treat a patient, it has to be what the patient wants, it has to be depending upon what is the cause which is causing the abnormal uterine bleeding. If there is just hormonal imbalance, just supporting her in that time or just regularizing her cycles with maybe 1 or 2 cycles of hormones can make her bleeding regular. If there is a thyroid problem, treat her with the thyroid medicines; she does not need any hormones, only correcting the thyroid can make her periods regular. At the same time, if the cause is removed, like a fibroid, polyp or adenomyosis. So, you always treat a patient what she wants, whether it all depends upon what is her lifestyle, what is her reproductive… whether she is in the reproductive age group, what is her family, what does she want.

Someone might ask, how does this happen?
