Even as PFAS has turned up in wastewater, the government has continued to promote the use of sewage sludge as fertilizer. And while Donald J. Trump’s election raises the prospect that PFAS restrictions might be rolled back, alongside other environmental rules, Synagro is pressing ahead in the effort to protect itself from expensive lawsuits.
The company is already facing all manner of legal challenges. This year a group of ranchers in Johnson County, Texas, stopped sending their cattle to market and sued Synagro for damages after a neighboring farm used sludge fertilizer on its fields. County investigators found 32 types of PFAS in the ranchers’ soil and water. Synagro has contested those allegations.
Residents of San Bernardino County, Calif., have also sued the company, over exposure to PFAS and other pollutants when an open-air pit of biosolids caught fire at a Synagro subsidiary. In October, a cattle farmer in Randolph County, Mo., filed an intent to sue Synagro and another biosolids company, saying the companies had acknowledged that PFAS chemicals may be present in the sludge but continued to provide it to farmers.
“I think it’s terrible,” said Donald Craig, the Missouri farmer, who alongside two local environmental groups is demanding that Synagro cease supplying sludge fertilizer in the state. He and the coalition have also petitioned the state to ban the use of the fertilizer altogether. “It’s disgusting. It needs to be outlawed.”
The current lawsuits against Synagro don’t make claims under America’s Superfund law, which requires corporations to clean up toxic contamination. But that landmark law is likely to be central to future cases, because of the E.P.A.’s decision this year to designate two major kinds of PFAS as hazardous substances under the law. The industry’s lobbying seeks to inoculate Synagro and others from lawsuits, even over decades-old contamination.
“What the biosolids companies are doing is attempting to buy themselves a ‘get out of jail free’ card,” said Mary Whittle, an attorney with Guerrero & Whittle who is representing the Texas ranchers. All this protects a business model that “makes Synagro rich while destroying America’s farmland,” she said.
How PFAS Ended Up on Farmland
The federal government has long encouraged the use of sludge as fertilizer in part because it would otherwise need to be disposed of another way — dumped in landfills, or burned — potentially releasing greenhouse gases and other pollution. In addition, sludge contains nutrients that encourage plant growth, and helps reduce use of fertilizers made from fossil fuels.
But a growing body of research shows that, unbeknown to farmers, fertilizer made from the sewage that flows from homes and factories can contain heavy concentrations of PFAS, which can then contaminate farmland.
It’s difficult to know how much fertilizer sludge is used nationwide, and E.P.A. data is incomplete. The industry says that more than two million dry tons were used on 4.6 million acres of farmland in 2018. And it estimates that farmers have obtained permits to use sewage sludge on nearly 70 million acres, or about a fifth of all U.S. agricultural land.
Researchers have found the chemicals in products as varied as milk, eggs, fruit juice and seafood.
Only one state, Maine, has begun systematically testing agricultural land for PFAS. So far, it has found 68 farms with significant contamination. In 2022, the state banned the use of sludge fertilizer and has since set up a fund to support affected farms.
Lawmakers in Washington are only starting to take notice. The Senate version of a stalled farm bill would have created a $500 million fund to be used to clean up farms, buy out farmers, monitor health, and fund testing for PFAS, essentially shifting the costs onto taxpayers. The fate of the measures, modeled on Maine’s approach, remained unclear.
“It could be that ultimately millions of acres of farmland is contaminated with biosolids, and may no longer be suitable for agriculture unless they are cleaned up,” said Scott Faber, an attorney with the Environmental Working Group and adjunct professor of law at Georgetown University. But there’s another possibility, he said: “They may simply be too expensive to clean up.”
Smelly but Profitable
Sewage sludge is a lucrative business.
When local water utilities treat sewage, and filter out the water, it leaves behind a thick sludge. Companies like Synagro take the sludge from more than 1,000 wastewater facilities in North America, which is then sold as inexpensive fertilizer.
While Synagro does not publicly report financial results, its earnings hit $100 million to $120 million last year, analysts estimated. An investment fund run by Goldman Sachs, West Street Infrastructure Partners III, acquired Synagro in 2020 in a deal reported to be worth at least $600 million.
As concerns over PFAS risks have grown, Synagro has stepped up its lobbying.
In 2022, the company set up a nonprofit, the Coalition of Recyclers of Residual Organics by Practitioners of Sustainability at Synagro’s corporate headquarters, and installed the company’s chief executive, Bob Preston, as chairman, according to the group’s tax filings. Since its founding, the group has spent $220,000 on federal lobbying, disclosure forms show.
In a statement, the nonprofit said the bills it lobbied for would “ensure liability resides with the manufacturer of these chemicals.”
Mr. Trump’s return to office introduces a new complication: The E.P.A.’s designation of some PFAS as hazardous under the Superfund law could be rolled back. Project 2025 calls for removing the hazardous-substance designation, and a major industry group has challenged the E.P.A.’s move in court.
Ryan McManus, government-affairs manager at the American Public Works Association, which represents water utilities and wastewater-treatment plants nationwide, and which has been key player in the effort to lobby Congress, said his group remained “very focused on a legislative solution because ultimately you could have another administration four years from now that decides to reverse course.”
Risks Hidden for Years
The argument that sludge companies aren’t liable because the chemicals were already in the sludge is based on the fact that PFAS manufacturers for years had minimized the dangers. Lawsuits, news articles and peer-reviewed research have chronicled how chemical giants 3M and DuPont, the original manufacturers of PFAS, for decades hid evidence of the chemicals’ dangers.
The chemicals are now so ubiquitous in the environment that nearly all Americans carry PFAS in their bloodstream. As many as 200 million Americans are exposed to PFAS through tap water.
In a statement, 3M said it was starting to exit PFAS manufacturing.
Under a sweeping settlement last year, 3M is paying $10 billion to cities and counties to test for and clean up PFAS in public water supplies. The E.P.A. has said that almost no level of exposure is safe, and this year imposed strict limits on drinking-water contamination for six types of PFAS.
Synagro and the sewage plants say they are simply at the end of that chain of contamination. We “do not manufacture or profit from PFAS,” Michael Witt, general counsel at Newark’s Passaic Valley Sewerage Commission, said at a hearing this year. “Industry did that.”
But critics point to research that for years has detected PFAS in wastewater. Recent studies have also explored how the chemicals can move from the soil into water and plants, and then to the livestock that feed on them.
Regarding Synagro, “it seems crazy to be able to say they’re a passive receiver and they shouldn’t be liable, that they know it’s harmful but they’re going to continue to sell it,” said Laura Dumais, an attorney with Public Employees for Environmental Responsibility, a group that assisted the Texas ranchers with PFAS testing. “It’s like CVS selling a tainted medicine and saying, ‘We can’t be liable and we’re just going to continue to sell it.’”
Wastewater treatment plants say they are finding it increasingly difficult to find landfills to accept sludge, partly because landfill operators are themselves wary of contamination. “They don’t want their landfill to potentially become a Superfund site,” said Mr. McManus of the public-works association.
‘They’re Not Even Aware’
The E.P.A. continues to promote sludge as fertilizer. It regulates harmful pathogens and some heavy metals in biosolids, but not PFAS. The agency is working on a risk assessment that it intends to release this year, the first step in determining whether new standards are necessary.
Some farming groups support the Senate bill, saying it would also offer them protection against potential lawsuits. “Your neighbor or anybody out there could sue you for the price of a postage stamp,” said Courtney Briggs, senior director for government affairs at the American Farm Bureau. Farmers, she said, are “victims, and often they’re not even aware” of the danger.
But experts point out that the Superfund law already exempts farmers from cleanup responsibilities. So farmers would gain little, while becoming unable to themselves sue, Mr. Faber said.
Neither can farmers easily sue the PFAS manufacturers, unless there is evidence that the manufacturer intended for the chemicals to be released onto farmland, Kate R. Bowers, legislative attorney at the Congressional Research Service, testified at a recent hearing.
That leaves ranchers like Tony Coleman, one of the plaintiffs in the Texas case against Synagro, in limbo. They have now taken legal action against the E.P.A., saying that it failed to properly regulate PFAS in fertilizer. The agency is pushing to dismiss the lawsuit.

3. Yes, my perception of the art has changed since the first time I looked at it. When I first looked at the art, I was amazed by its vibrant colors and details. But after spending more time with it, I started to see the deeper themes the artist was expressing. I noticed the brushstrokes that create a sense of movement and the way the light interacts with different parts of the piece. This has made me appreciate and understand the art more deeply.
About seven years ago, I had the chance to be part of a school club that took us to Painting with a Twist, where I created a memorable piece of art. Using a regular canvas and acrylic paint, I painted an elephant with a stunning sunset within its silhouette. This painting holds a special place in my heart and hangs proudly in my room, as it’s one of the few paintings I've done that I actually like. I think it's beautiful because the vibrant colors of the sunset blend seamlessly with the majestic form of the elephant, creating a harmonious and captivating scene that always brings me joy.
When I look at art, I think I'm bringing along a mix of my own experiences, memories, and emotions. It's like every piece of art has a way of connecting with different parts of my life. I am a 21-year-old female, originally from Bradenton, Florida, but raised in Venezuela. My ethnicity is Hispanic. I enjoy spending time with friends and family, going fishing, and visiting clubs. I am a member of the American Dental Hygienists' Association (ADHA) and currently work at a dental office. What makes me uniquely me is my deep love for animals; if I could, I would save all the strays. I consider myself to be very caring.
As a Dental hygienist I often find fascination in several aspects of my daily work. I am intrigued by advancements in dental technology, which introduce new tools and techniques that make procedures more efficient and comfortable for patients. Building relationships with patients and witnessing the improvement in their oral health over time is another rewarding aspect. I also cherish the opportunity to educate patients about maintaining good oral hygiene and seeing them take that advice to heart. “Hyejoo. Come join us for some Rainbow Six.”
For once, Hyejoo is interested, but there’s a blank document in front of her and its paired midnight deadline convinced her not to play.
“Sorry, Soo. I have a paper due at midnight. Next time?” The college student genuinely wishes she didn’t procrastinate this bad.
I know it looks this way, but the three sisters aren’t that distant with each other. After Sooyoung and Hyunjin left for university, they didn’t see each other as often, but still bonded through online gaming.
It all started when they were younger. Sooyoung had granted Hyunjin the responsibility of taking care of her Nintendogs when she went to a summer camp. Usually Hyejoo, too young to play, would sit next to her and sometimes they switched off taking care of the Sharpei they virtually adopted.
As they grew up, their tastes for video games changed as well. Sooyoung mostly grew out of them, having to take care of her younger siblings and started pursuing dancing. Of course, during college, she wouldn’t hesitate to join in party games, like Just Dance and trivia games. Hyunjin became a sports star, but still played video games. She enjoyed low-stress, life simulation games, but also occasionally played FIFA with her teammates.
Hyejoo had a funny relationship with extracurricular activities. Growing up, she was always dragged into watching her sisters’ dance recitals or soccer games, so she tended to have low energy. She tried dance and soccer and actually showed potential in both hobbies, but had no interest or motivation in either. By the time she was in high school, she only continued what she enjoyed, which was just playing video games. She specifically enjoyed shooting and battle royale games, but she could play anything.
As both Sooyoung and Hyunjin transitioned into university and adult life, they tried their best keeping in touch with their youngest sister. When video calls became awkward, Hyunjin brought up the idea of playing Mario Kart online together and talking through Discord. Sooyoung had to go buy herself a Switch and Hyejoo didn’t like the idea of spending more than 30 minutes talking to her sisters, but the experience was a pleasant surprise. The two older siblings couldn’t help enjoying the serotonin boost they would get when hearing competitive Hyejoo screaming and threatening them with embarrassing secrets from the past when Hyunjin threw a Blue Shell or Sooyoung Bullet Bill-ed past her. An hour passed and Hyunjin was ready to log out, but couldn’t resist it when Hyejoo asked, “One more round?”
The gaming continued when Hyejoo went to university. Even though it wasn’t their style, the three of them would play whatever was popular and multiplayer, whether it be PUBG or Smash.
Eventually, scheduling time to play with Hyejoo was a little unsuccessful. In her last year of university, she became busy with school and actually became more social. You could catch her playing intramural soccer or at the dance studio. She was even dating someone and worked at a nearby bakery. Hyejoo could’ve made time for her siblings, but replaying the same courses and campaigns for hours wasn’t stimulating enough for her.
Tonight’s a little different. It’d been nearly 2 months since she spoke to her sisters and Gowon was scolding her to spend time with them. Even though her computer was starting to run slow, she downloaded the new zombie game Sooyoung mentioned and logged onto her Discord account.
“Nice of you to finally join us, Hyejoo.” Sooyoung naturally scolds her first.
“I know. I thought you were going to abandon us.” Hyunjin spoke in a higher tone and it made Hyejoo cringe.
“Stop being fucking dramatic, Soo. I’m busy with school.”
Hyunjin scoffs. “You’re busy dating your Minecraft girlfriend. I see you online.”
Hyejoo scowls. She turns around, making sure said girlfriend wasn’t in the room. “She’s my real girlfriend, too, you know?”
Sooyoung just chuckles. “I’ll see it when I believe it. When are you introducing her to mom and dad?”
��I don’t know. Maybe during the next holiday?”
“Can you bring some snacks from your work?” Hyunjin shoves herself in front of the camera, as if trying to corner her little sister.
“Hyun, that’s not important.”
“What's more important than bread?”
“Kim Jiwoo. That’s what’s more important than bread.”
Hyejoo pinches the bridge of her nose. This was why she didn’t like talking to her sisters for more than 30 minutes.
“As if, Sooyoung. Jeon Heejin is more superior than Jiwoo, but bread is always on top.”
“You say that now, once you get married, you’re going to change your minds.”
There are two gunshots.
“Hye! Why’d you shoot us?! We’re on the same team?”
“Can you two simps shut up? I just sacrificed 6 gigabytes of space to play this game with you two.”
To Hyejoo’s surprise, Hyunjin’s avatar punches her character and Sooyoung takes her out with a headshot. Hyejoo’s about to seek revenge until Sooyoung starts talking again.
“Yah. I’ve seen your Instagram story posts. Once you two get together, you’re gonna be whipped for her, too. It’s in the Ha genetics.”
“For fucks sake, I am dating Gowon! And why would anyone be proud of being whipped?” Hyejoo starts shooting at her sisters but they run away from the lobby before starting a campaign.
It always starts with small talk. It usually consists of their jobs, what they’re doing on the weekends, and each other’s girlfriends. Hyejoo liked hearing from her sisters, but she couldn’t help but feel a little pathetic hearing about their numerous plans while she just wanted to get through school. Conversations like this is where they found out that Sooyoung became a manager at a well known dance school. The last time they talked, Hyunjin was talking about taking Heejin to Disneyland. Hyejoo was slightly jealous because she wants to spoil Gowon, too, but with the minimum wage she’s earning, she can only afford the new gaming mouse Gowon wanted.
Like always, she’ll just listen. Listen and shoot. Maybe she’ll learn a new lesson from the mistakes her sisters laugh about, but she’s too busying carrying the team for the most part.
The three have just finished a round when Hyunjin asks for a break so she can eat. Hyejoo checks her phone to see a few texts from Gowon.
Sooyoung opens a conversation. “Hey guys, I need to tell you something.”
“Go right ahead,” Hyejoo says before chugging some water. Hyunjin just hums before biting into some pasta that Heejin brought in about 15 minutes ago.
“I haven’t told mom and dad, yet because we’re still thinking about it,” Sooyoung takes a deep breath, “Jiwoo and I want to try for a kid.”
Hyejoo stops looking at her phone. She stares at Sooyoung through the screen. “Oh damn. Really?”
Sooyoung offers a smile. Hyejoo can’t help but soften up at the honey dripping out of her sister’s eyes. “Yeah. She keeps talking about how cute her students are so I brought up the conversation. We’re gonna start going to fertilization clinics and adoption centers to see our options. I don’t think anything’s going to happen in the coming year or two, but I just wanted to let you guys know.” She sighs. “I’m kinda excited actually.”
“Congrats, unnie!” Hyunjin lets out a chuckle. “The idea of a little Jiwoo running around and doing taekwondo flips sounds adorable.”
Sooyoung whines. “What about a little Th-ooyoungie dancing in the living room?”
“Don’t ever use that fucking voice again.” Hyejoo grimaces in disgust.
Hyunjin didn’t spare a pause for disagreement. “I second that.” Sooyoung never used aegyo so it was slightly unbearable hearing that.
“Whatever.” Sooyoung sighs. “It’s just— it’s kinda surreal thinking about having a kid.”
Hyejoo also agrees. Maybe it’s because she’s looking forward to building her career or the fact that she doesn’t know where to begin in taking care of a kid that the idea of her own children seems foreign. She’s thought about adopting a dog, but a human being?
Hyunjin asks Sooyoung about the logistics, like the future of their current jobs and who would carry the child if they decided to not adopt.
Hyejoo listens, but her mind gets clouded thinking about a little Sooyoung or Jiwoo running around and giggling and maybe even playing with a little controller. Hyunjin will compete to be the favorite aunt, but she’ll share with Heejin. She knows Gowon will be a better aunt than her, but she still wants to be a part of that kid’s life. Even though she wasn’t the most active, she wants to take that future kid to the park and kick a ball around and teach them pranks.
The idea warmed Hyejoo’s heart.
For a second, Hyejoo zones back into the conversation. Sooyoung eggs Hyunjin about finishing dental school so she can start her own family, too.
Then it hits Hyejoo.
It’ll still be in the far future, but maybe she needs to grow up a little more. There’s going to be little Sooyoung and soon enough there will be a little Hyunjin.
Hyejoo needs to stop cursing. Maybe she needs to go home more and keep her parents company. She’ll learn how to cook something besides instant ramen and fried rice.
Things are going to be different. They’re all growing up. Sooyoung’s moving up positions. Hyunjin is going to finish dental school. Hyejoo was going to graduate soon.
Hyejoo was more than used to being away from her family during the school year, but then it hits her.
The two neighboring rooms back home were going to be empty. Her two sisters are eventually going to stop coming home for holidays.
The three of them are not going to play video games.
It’ll no longer be just the three of them.
Hyejoo let’s that sink in a little. The days of shooting zombies and racing around Nintendo tracks and visiting each other’s islands will significantly lesson.
The maknae suddenly feels guilty of declining all those game requests. Although all reasons were valid, she feels like she should’ve made more time for her sisters.
She hears Hyunjin whining at something Sooyoung said. Hyejoo flinches a smile, remembering how fifteen years ago, they were bullying each other at the playground.
Some things didn’t change.
And maybe some things still won’t change.
The future is uncertain and maybe she shouldn’t think too much about it.
At least she has the present. She can still play with her sisters now. She can play with them next week, next month.
“Hye? Are you okay? You’ve been pretty quiet.”
Hyejoo tried to think of a snarky comeback, but couldn't think of anything. Surprisingly, her ego doesn’t get to her and just comes clean. “Yeah. I’m just thinking about a little Sooyoung. It’s kinda cute actually. Congrats, Soo.”
Her two older sisters just stare at her through the camera. “Wow. It must be late. Hyejoo’s being nice. Let’s just do one more round and call it a night.”
Hyejoo scoffs. She wants to curse, but holds herself back.

“You guys down for a few rounds of Smash?”
“Sounds good. Is Hye joining?”
Hyejoo puts down her phone. “Gowonnie.” She nudges the girl next to her. They were watching a variety show together while snuggling.
Gowon just hums, a little engrossed in the show.
“I’m gonna go play with my sisters. Is that okay?”
Her girlfriend just smiles and rolls her eyes. “Why do you even ask? You know I’m going to say yes.”
Hyejoo doesn’t want to admit that she just wants to be chummy. “If you had plans in mind, I was just going to reschedule.”
Gowon just shrugs. “I mean I’m craving fried chicken, but that’s not worth rescheduling.” She then shoves Hyejoo off of her. “Go.”
The younger one grumbles, but makes her way to their gaming room.
As she turns on the light, she admires the room they have created. On one side, her girlfriend has a pastel setup. It contrasts her black black setup.
An idea finally sparks in her head. She walks back outside and peaks back into their small living room.
“Gowonnie, do you want to join us?”

“Are you really her girlfriend?”
“Of course she’s my girlfriend. She wouldn’t have been playing with us for the past hour if we’re not dating.” Hyejoo puts her head on Gowon’s shoulder. Hyunjin and Sooyoung are entertained by their little sister’s webcam showing an abandoned chair while Gowon’s webcam showed Hyejoo being soft and protective of a bright and rather crunchy lady.
“She’s not paying you with credits or diamonds or something?”
Gowon giggles. “That sounds like a better bargain.” She turns to her girlfriend and pats her head. “I’m tired of carrying you. Starting next month, I’m expecting 2,000 Battle Points or I’m going to stop doing your laundry.”
Hyunjin chokes on her water. Sooyoung’s jaw dropped.
Hyejoo pouts. “Park Chaewon.”
“Wow. I like her already. I think mom will like her, too.”
“How did you meet such a nice person, Hye? She’s pretty, too. I didn’t know you had it in you.”
Gowon smiles. “Thank you. It’s nice to know Hyejoo’s siblings are nice.” She turns to Hyejoo. “You should learn something from your sisters. Forget it. I’m upping the payment to 4,000 Battle Points.”
Hyejoo pouts and gives a quick peck on Gowon’s cheek. “Can I pay you with love instead?”
Hyunjin gags. “I don’t know if I’ll get used to this.”
“She got that from you, Hyun. Don’t deny it.” Sooyoung scolds her younger sister.
“You’ve been dating, Jiwoo-unnie longer! She got that from you!”
Gowon giggles. Hyejoo can’t help, but admire her girlfriend. She pleased how well things were coming along.

For many of us, even as adults, the waiting room can be one of the most nerve-wracking parts of a visit to the dentist. Preparation is key for a worry-free dental visit, but what about once you arrive?

Hygienists Brace for Pitched Battles With Dentists in Fights Over Practice Laws
This year, the Illinois legislature was considering measures to expand oral health treatment in a state where millions of people live in dental care deserts.
But when the Illinois State Dental Society met with key lawmakers virtually for its annual lobbying day in the spring, the proposals to allow dental hygienists to clean the teeth of certain underprivileged patients without a dentist seemed doomed.
State Sen. Dave Syverson, a Republican legislative leader, warned against the bills even if they sounded minor. “It’s just getting the camel’s nose under the tent,” he said in an audio recording of the meeting obtained by KHN. “We’ll have, before long, hygienists doing the work that, if they wanted to do, they should have gone to dental school for.”
The senator added that he missed “the reception and the dinners that you guys host” and the “nice softball questions that I usually get” from the dental society’s past president, who happens to be his first cousin.
The bills never made it out of committee.
The situation in Illinois is indicative of the types of legislative dynamics that play out when lower-level health care providers such as dental hygienists, nurse practitioners and optometrists try to gain greater autonomy and access to patients. And the fate of those Illinois bills illustrates the power that lobbying groups such as the Illinois dental society have in shaping policies on where health professionals can practice and who keeps the profits.
“There’s always a struggle,” said Margaret Langelier, a researcher for the Center for Health Workforce Studies at the University of Albany in New York. “We have orthopedists fighting podiatrists over who can take care of the ankle. We have psychiatrists fighting with clinical psychologists about who can prescribe and what they can prescribe. We have nurses fighting pharmacists over injections and vaccinations. It’s the turf battles.”
In 2015, the Illinois Dental Practice Act was revised to let hygienists treat low-income patients on Medicaid or without insurance in “public health settings” — such as schools, safety-net clinics and programs for mothers and children — without a dentist examining them or being on-site. “I just don’t feel anybody with a two-year associate’s degree is medically qualified to correct your health,” Marsh added. “They’re trained to clean teeth. They take a sharp little instrument and scrape your teeth. That’s what they do. That’s all they do.”
He said the problem is not a shortage of dental professionals but, rather, a lack of dentists who can afford to accept Medicaid patients — and “nobody wants to raise taxes to actually be able to reimburse” dentists at higher rates.

“Imagine if you had a toothache and the doctor couldn’t see you that day,” she said, noting that her patients have low incomes. “They can’t afford the services. They wait till something hurts.”
But since becoming a public health dental hygienist, Webster now does immediate cleanings, takes X-rays she sends to teledentists for exams, and applies a solution called silver diamine fluoride that can halt tooth decay.
“The whole thing, start to finish, it’s just a lot more efficient,” she said.

This year, the Illinois legislature was considering measures to expand oral health treatment in a state where millions of people live in dental care deserts.
But when the Illinois State Dental Society met with key lawmakers virtually for its annual lobbying day in the spring, the proposals to allow dental hygienists to clean the teeth of certain underprivileged patients without a dentist seemed doomed.
State Sen. Dave Syverson, a Republican legislative leader, warned against the bills even if they sounded minor. “It’s just getting the camel’s nose under the tent,” he said in an audio recording of the meeting obtained by KHN. “We’ll have, before long, hygienists doing the work that, if they wanted to do, they should have gone to dental school for.”
The senator added that he missed “the reception and the dinners that you guys host” and the “nice softball questions that I usually get” from the dental society’s past president, who happens to be his first cousin.
The bills never made it out of committee.
The situation in Illinois is indicative of the types of legislative dynamics that play out when lower-level health care providers such as dental hygienists, nurse practitioners and optometrists try to gain greater autonomy and access to patients. And the fate of those Illinois bills illustrates the power that lobbying groups such as the Illinois dental society have in shaping policies on where health professionals can practice and who keeps the profits.
“There’s always a struggle,” said Margaret Langelier, a researcher for the Center for Health Workforce Studies at the University of Albany in New York. “We have orthopedists fighting podiatrists over who can take care of the ankle. We have psychiatrists fighting with clinical psychologists about who can prescribe and what they can prescribe. We have nurses fighting pharmacists over injections and vaccinations. It’s the turf battles.”
In 2015, the Illinois Dental Practice Act was revised to let hygienists treat low-income patients on Medicaid or without insurance in “public health settings” — such as schools, safety-net clinics and programs for mothers and children — without a dentist examining them or being on-site. Instead there is By Chloe, a vegan fast food restaurant, which sells a quinoa taco salad for $12.45; Brodo, which sells cups of bone broth for $10; the Good Market, which sells pineapple raspberry kombucha organic slushies for $4.50; Welleco and the Nue Co, both supplement stores; the Hemp Garden, where CBD sheet masks sell for $29.99; the Detox Market, where the Vintner’s Daughter Active Treatment Essence face oil is $225; Lululemon and Nike; boutique fitness establishments like Y-7, New York Pilates (three locations), SLT and SoulCycle, where classes are upward of $30.
In early April, Kind, the snack food company, organized a two-day pop-up highlighting its “Sweeteners Uncovered” campaign, which shows hidden sugars in popular snack foods.
This took the form of a high-tech bodega at 579 Broadway, a space that once housed an Aldo shoe store and whose rent for the space Kind declined to share. At the pop-up, the sugar content from Kind’s competitors could be revealed by scanning a QR code.
