re: breast cancer ai, is the testing that would need to be done to make sure it's an accurate result invasive or expensive or both?
It seems I jumped the gun when I reblogged that article. Despite them using a mammogram on the post's images, this AI innovation is for pathology, not radiology. I was talking about computer-aided detection for mammography, but this is a different beast entirely.

So some background, DCIS is known as ductal carcinoma in situ and is a pre-cancerous or a non-invasive cancerous lesion of the breast, depending on who you ask. Approximately 1/3 of people who have this develop actual breaat cancer aka it leaves the duct and start invading other things. So to make sure cancer doesn't develop, people are often recommended to excise it or even get a mastectomy - the removal of the whole breast - to treat it.
But doesn't that sound a little extreme when 67% of people may not even develop it?
I mean, look at prostate cancer, where urologists watch and wait to see if cancer will progress using labs, imaging, and occasionally biopsy. Because by the time you are diagnosed with prostate cancer, you're more likely to die from something else like a heart attack, old age, etc than from the cancer itself because it tends to grow so slowly. And getting a preemptive removal of the prostate - prostatectomy - has a lot of unwanted consequences including urinary incontinence and erectile dysfunction.
Wouldn't it be nice then if we could figure out what forms of DCIS would turn into full-blown cancer and what sort would behave more like prostate cancer?
Well, we do have some tech in the works, I believe, in the form of RNA sequencing, where you look at the cancer's genetic profile to see if it will go bad quickly. But RNA sequencing is expensive, and I don't believe it's common place yet.
And that's what the MIT research comes in. They are stating they can predict the behavior of DCIS, whether it'll act good or bad, by taking tissue samples of past known DCIS (and already having documented their behavior if they turned into cancer or not and how fast they do), using a chromatin stain to create slides, and feeding them into an AI to generate a predictive model that could tell from a tissue slide how DCIS will behave based on the model.
Staining and creating slides is cheaper than doing RNA sequencing according to the article ( I do not have the numbers on hand, but it seems like it'd be right), so this is a big deal as it both is cheaper for the patient and may allow a patient to avoid unneeded surgery.
Of course, the model needs to be validated to see if it works in practice, but it sounds promising.

As for what I was taking about erroneously, it's called computer-aided detection, and it analyzes mammogram images.
The CAD software can detect areas that are suspicious on a mammogram and alert a radiologist to them. The problem is that it can mark areas that may seem suspicious to the computer but seems normal to the mammographer. But what does a mammographer do with that information then?
The normal workflow of mammography is that you find a suspicious lesion on screening mammogram, ask the patient to come back to take diagnostic / magnified / compression images aka the breast getting smooshed views (more cost and discomfort), and if it's still suspicious with additional views you do a biopsy (invasive).
If the CAD is wrong but you test, you have done unnecessary imaging or worse even an unnecessary biopsy.
But if you the mammographer say that the CAD is wrong when it's right, you just missed cancer and put yourself in a very tight spot if they run the CAD software on your images.
To answer your question from what I was referring to, yes, the testing to confirm all the findings that the CAD points out would add a lot of unnecessary tests and biopsies for the patient.

Last night was... interesting! Adam has all sorts of bizarre health issues, as some of you may know. He has gout, likely EDS, one of his lungs spontaneously collapsed in February of 2021, unrelated to COVID (which miraculously he has probably never contracted). When they did the surgery to re-inflate his lung they of course ran all sorts of tests and imaging beforehand and saw that he had kidney stones, which was unsurprising given the gout diagnosis. They have been causing him chronic discomfort ever since, mostly manageable with Nabumetone/Colchicine and regular degular acetaminophen. Typically he fluctuates between a 1-3 on the pain scale.
Well, this past weekend he shot up to a consistent 6 and was bedridden. Yesterday we started calling around to urologists in the area and all of them are booked straight through until mid-July, so they advised him to just go to the ER. And that's where he spent the night! Apparently there are 20 size gradations on the kidney stone scale and his is fourth from the largest (!!!). The on-call urologist in the WPH ER department was shocked he didn't have an infection. I'm not, because the guy never gets sick, even when I'm deathly ill and sleeping right next to him; his immune system is fucking diesel. But they took one look at his scans and were like, "You get Oxycodone now!" He's feeling a lot better, thank god.
They wanted to admit him, but he refused — he has a project to deliver this afternoon, and was afraid of the cost because his high-deductible health plan sucks. Sickening that he's even in this position, where he's scared of a health emergency affecting both our income and his continued marketability as a freelancer at [REDACTED], but the hospital didn't give him that much of a hard time since he's got no fever and until we know whether or not he needs surgery there's really not much they can do except give him fluids and pain meds.

Surgery for my little brother
Good day po. The fundraising is for my little brother (13) circumcision surgery. He has a penile torsion condition. This condition causes the penis to appear to be rotated – or twisted – on its axis. Almost always, this twist occurs to the left (counter-clockwise). (Image on the cover which is the 45 degree)
The doctor (Pedia Urologist) said my brother's condition will be difficult as it is rotated to 45 degree. This made my mother anxious for the surgery and the cost. The surgery will cost us 80k which is expensive for her who also fund my tuition fee that is also 80k a year. We are currently preparing for the medical assistance that we could reach for help.
My mother hopes that the surgery will start on August 1st, so the healing would be done in one month before class starts.
I hope to help my family in this financial crisis.

Prologue
It is currently 6:21 AM and I've awoken from my melatonin induced dreams. I haven't really dreamed since diagnosis so it was weird to experience 3-4 different dreams in a row. But... maybe that's another blog. Anyways here's the story so far. I'm still fuzzy about the dates and timeline so bear with me.
The Lump
I'm not sure when it grew or how long it was growing for but I noticed something hard at the end of my right testicle around June 2024 or so. Part of me knew it cancer so I just brushed it off for a little. I was scared for it to become a reality. Even if it was potentially life threatening, I didn't want to acknowledge it. I would rather live in ignorance for a little bit.
I started to spend a little more and started doing more things as if I wasn't going to be able to do them soon. That KBBQ place? Hell yeah let's go. That sushi place? Yup, we're eating there. How much does that cost...? Doesn't matter, money ain't an issue. I felt like I had to live little more than usual before I started down this road, because I knew once I started it wouldn't be fun and I won't be able to stop until it's done. So for about a month or two I ignored it and just went on with life.
Eventually... I built up the courage to schedule a doctor's appointment. Unfortunately, my primary care physician was really busy. His only appointment was like well into August and I needed to check it out now. I chose someone else within the care team and luckily I got seen a lot quicker. July 17th. The doctor asked me about pain and I said I didn't have any. Honestly, I would not have noticed it there if not for me touching my balls. The doctor then examined it and they also felt the lump. An ultrasound was ordered.
I believe I got the ultrasound like a week later and it cost like 300+ dollars!!!! I was only told how much when I got there and signed in. Brazy. Anyways, I checked in and waited until it was my turn. I went into a dark moody room with a man and a student. I forget her name but the man asked me if it was okay for the student to be there and to do the ultrasound. I said yeah that's fine. I felt a little funny because she would be sounding me balls. The man asked me a couple of questions and then they both left for me to get ready. I laid down on the bed and pulled my pants down. They gave me a towel to cover up my groin so I'm not just dicks out for Harambe. The ultrasound lasted for about 30 mins or so. Nothing special.
A week later I get a call from me doctor saying the lump is confirmed. The size was like 6cm X 1.5cm X 2cm or something and that there was a second lump. The doctor expressed her concern and referred me to a urologist. Of course, I put off meeting him for a week or two.
The Urologist
My girlfriend and I were driving home one day and she had mentioned something she needed to go to the doctors for. I figured this was a good time to bring up I needed to go the urologist. I had never mentioned this journey so far with anyone. I've just been going to appointments secretly. I figured if I told someone about needing to go to the urologist I would actually schedule and go. It worked.
Tuesday 9/10/2024 I met with my urologist. I didn't know what to expect going in but it definitely wasn't that. He came in and introduced himself, asked a couple questions, felt my testicle, and then said "yeah I'm taking your testicle". Well it more was along the lines of "It's got to come out".
What.
"My scheduler will help you schedule the surgery. I'm thinking next week."
What.
"Any questions?"
I'm too shocked to think of any.
I still needed to produce a urine sample so I go back to the lobby and chug water while I tell my girlfriend what happened. She is obviously shocked as I'm giving her the play by play.
Looking back and her telling me, it wasn't the best idea to tell her I had cancer over text but oh whale.
At the time, we didn't know it was cancer. We do know it is a lump and that it should come out. The urologist said anything inside the body we treat as a cancer until proven not and anything outside of the body we treat as not a cancer until proven.
Lo and behold I put of scheduling the surgery for a couple days because I'm scared out of my mind. This was my first surgery ever.
