1 - When was the last time you had a roast dinner? What kind of meat or vegetarian option did you have with it? That’s not really a thing we do here, but for our noche buena dinner we did have roast chicken with rosemary as one of our dishes. It was surrounded with bangus (milkfish), lumpia with cheese, callos, baked macaroni with cheese, and mashed potatoes with gravy. My family has never been traditional when it comes to Christmas food and I’ve never tried hamon or queso de bola.

2 - When was the last time you drove or travelled for over an hour? Where did you go? It is for sure going to be today because we’ll be driving to Laguna to visit my dad’s family wearing our face masks, face shields, equipped with alcohol and hand sanitizer, and ready to follow social distancing.

3 - What’s your favourite kind of coffee to order (eg. cappuccino, latte etc.)? I’ve been ordering caramel macchiato in the last...6–7 years. If a coffee shop doesn’t serve it, the next thing I go for is whatever sounds like the sweetest drink on their menu, like a chocolate chip truffle frappe or something like that.

4 - When you get old, are you going to let your hair go grey or dye it instead? I will probably dye it for a bit, like what my grandma used to do. But I guess it’ll also depend; I might end up liking grey hair and not feel the need to change how it looks.

5 - What genre was the last book you read? Was it any good? I can’t remember which one it was but it was definitely a wrestler’s memoir; I must’ve read around four this year. The best ones I read are always of Chris Jericho since he has fantastic stories but he also knows how to write well. A lot of wrestlers have great stories, but are so-so in writing.

6 - Did you ever wear braces on your teeth? Yeah, briefly from the end of middle school to freshman year of high school. I need to get braces again, though.

7 - When was the last time you were relieved about something? What caused you to feel that way? Continued from earlier today. We brought both dogs to my dad’s family’s place today and I was glad that they were fast asleep on our drive back home and that neither felt bad and puked.

8 - Where was the last place you went that required you to wear a mask? Are you used to having to wear one now? Everyone is required to wear masks as long as they find themselves outdoors, period. I wear one even if I’m just walking one of my dogs in our backyard. I’m definitely more used to it now, compared to months ago when I had just started going out again and would be forgetful when it came to masks.

9 - How often do you receive calls from unknown numbers? Do you ever answer them? I get them a lot more often now that I’m working and have to coordinate with third-party people...I get unknown numbers once or twice a week. As much as it irritates me, I have no choice but to answer all of them now.

10 - What’s your favourite condiment to have with sausages or hot dogs (or the vegetarian equivalent)? Whenever I have a sausage/hotdog in a bun, I’d be glad to simply have mayonnaise on it.

11 - Which fictional character can you relate to the most? Is this a character from film, TV or a book? Monica Geller from Friends.

12 - Do you groom your eyebrows? If so, how? Nah, haven’t in a long time. It’s just not something I’ve ever particularly cared about. Whenever I have to, I just shave them. I also haven’t had a trichotillomania episode with my eyebrows, which is a relieving thing to realize.

13 - Did you get “told off” for anything the last time you went to the dentist or was everything okay? My last trip went smoothly. I just had a tooth extracted and there wasn’t really anything to reprimand me for since the tooth was already damaged.

14 - Would you rather get a starter or dessert? Have always been more of a savory girl than sweet, so I’ll go with the starter.

15 - Have you ever been involved with the police? Did you find them to be helpful? I mean...I guess? Kind of? I wasn’t arrested by any means, but I was a journalism student, so *shrug* pretty self-explanatory lol. There was one time in one class where we were all required to write a story covering the police beat, so I had to stay at a police station in Manila and wait for any tips or leads to come in. I was in freshman year and was absolutely scared, but I did have a classmate/friend with me ‘cause we were in the same class and she needed a lead as well.

16 - Are you tired at the moment? Is there a specific reason you feel that way? I am extremely tired and would pass out in a second if I let myself. But I had been out all day and didn’t have the chance to do anything I wanted to do whether it was doing embroidery, watching 2 Days 1 Night, or taking a survey, so I’m making up for all the lost time and powering through this survey.

17 - Are you big on colour coordination? Does that just apply to your outfits or to the rest of your life as well? It rarely comes to mind. It’s not something I feel the need to maintain, precisely because I’m very inconsistent with maintenance. Even though I know I possess the necessary effort and patience to organize a group of items by color, I also know it would be disorganized in less than a week.

18 - What shoes did you last wear? How long have you had them? Puma sneakers. Yep, I’ve had them for the past two years.

19 - When was the last time you wore make-up? What kind of make-up was it? September, for my first job interview. My items were simple; just an eyeliner pencil and some lip gloss.

20 - Have you ever slipped or skidded on the ice? Did you end up getting hurt? Continued from last night because I’ve been so sloppy at taking surveys lately, loooool. I’ve slipped a few times on ice skating rinks before. I don’t know if it’s the same experience, but whenever it happened to me I usually felt more embarrassed than hurt. It only hurt quite a bit when the cause of my fall was crashing into someone on the rink.

21 - Do you wear glasses or contacts? I wear glasses, but I already need to change mine since my eyesight has gotten worse and the lens that are on my current pair aren’t for me anymore.

22 - Do you own any photo albums? Are they dedicated to special occasions or just a random selection of photos? My mom made several photo albums for her kids from our childhood days so each of us have lots of photos from age 0 to about 7 or 8. I haven’t made a photo album just for myself, though I really should. Making memories with a film camera and having them developed still sounds very appealing to me.

23 - What was the last reason for you using a spoon? I was mixing my coffee to make sure all the granules are mixed into the water.

24 - Did your state/region go into lockdown or similar back when Covid hit in March? What did you do to pass the time while you were stuck at home? Of course. I would be more surprised to hear of a city or region anywhere in the world that did not go into lockdown. March was a livelier, more optimistic time, so I tried out lots of new things while the lockdown was still fresh and no one had any clue for how long it would actually pan out, and continue to pan out until the literal end of the year. I played the Switch for longer hours, tried making dalgona coffee, watched Descendants of the Sun, pulled more all-nighters, revisited computer games from my childhood, and did my thesis chapter by chapter with Andi.

25 - What’s your favourite meal of the day - breakfast, lunch or dinner? In my family, lunch and dinner are very identical so I’d go with either of them.

26 - Who was the last person you texted? How do you know that person? I have not touched my phone in a while, believe it or not...I haven’t needed it much during the holiday break. I think it was my cousin? He’s a relative, so I’ve known him since birth.

27 - What was the last thing you put in a sandwich? A hotdog.

28 - What was the reason behind the last time you shouted or raised your voice? I was at my dad’s family’s last night and I had ordered four boxes of empanadas from their business, and I was trying to hand over the P500 bill to my aunt (the total was like P300, but I gave them bonus to serve as my treat and my gift). I kept trying to give it and she kept rejecting it and putting it back in my pocket hahaha, so it became like a game for me to see which tiny space in the house I can squeeze the bill in so she can stop retorting. It turned into a funny cat and mouse situation and I ended up raising my voice a few times.

29 - Are you a citizen of more than one country? Would you ever use that advantage to move abroad? No.

30 - Do you know how to change a tyre? Could you do it without help? HAH, no. I am fucked if it ever happens and when it does, I really hope luck would be on my side that day and have a kind stranger that knows how to change tyres walk or drive by.

I won’t take up 30 posts with this but rather do it all in one sitting. It’ll be good to evaluate myself properly for the first time in awhile.

5. I miss how I felt when I was in the depths of this years ago. I know that sounds crazy. But that time is almost romanticized. I had little real world responsbilities, I was reading and creating things, and in school, and meeting my favorite people, and thin, and working out, and excelling in my sports, and it was all so perfect. I want some of that back. I believe I’d feel more confident for myself and for my boyfriend if I was thinner now too. Elegantly thin. A sharp jawline and sleek collar bones, highlighted from the light leaking in from the window. Like being thin would suddenly make me profoundly interesting too...

7. No, They are aware of my past with restrictive eating and would be very concerned. They know I’m not eating sugar for health reasons but thats it. 

8. Yoga, Spinning, Hiking, and try to run but Im not very good at it. (I aim for daily workouts, but I don’t always get that in. Although I just ordered a spin bike for my home!!!)

9. The only comment I can really recall was pre ED when I was less conscious of my body and my mom kindly pointed out my jeans were too tight because it was squeezing my hips out the top. I know it wasn’t anything critical or negative. She was trying to do me a kindness so I didn’t dress unflatteringly. And I am grateful for that. But more than any negative comments, I always was the skinny friend, and I longed for those comments to continue. 

10. Eating out with friends. ED’s are incredibly lonely. 

11. I can’t recall the blog or her username, but I can see her profile photo in my mind. She was quite popular on blogger back in the day. Looking for new blogs to follow currently if you have any great suggestions. 

12. Nothing for breakfast. Smoothies or 1/2 veggie sandwich or granola bar for lunch. Veggies or small grilled chicken or eggwhite omlette for dinner. 1 small square of dark chocolate to satisfy sweet tooth. 

13. Both. My therapist always told me I liked to play both doctor and patient. I like to skirt a fine line, I restrict (currently just minimal daily food, but in the past would fast for a day, or days, or a week, the longest was just over 2 weeks). I was unhealthy but never dipped into a place that was too dangerous. I never wanted to die. I just want to be thin. I also love to work out, so I find a balance between restricting and working out. 

14. I think I’d like to be around 115 or 110. When I was down there previously of course I wanted to keep on losing. But I think thats a reasonable goal for myself. 

15. Im mostly vegetarian. We only eat meat on occasion. I don’t notice too much of a difference between diets for me. I feel less heavy when I dont eat meat which I prefer. Eating meat is more of a rare indulgence. 

16. End of high school. I worked at a cafe, and in the beginning it was a perk to be able to snack on things throughout the day for free. A muffin here, some chicken strips there, a small scoop of ice cream, grilled chicken salad...  At some point after eating during the work day, I grabbed a plasic spoon and went on break and tried to purge. (Its not for me). After this, I decided my path would be focused on restricting. I remember going to my high school boyfriends house and telling him so proudly that I had only had a smoothie and a cliff bar the entire day. I was hooked from there. 

17. Yes. My therapist was not one who liked the make an official diagnosis. Although she categorized me somewhere along the lines of EDNOS with Anorexia Nervosa tendencies. 

18. Certain foods really stand out to me. My friends’ moms fried chicken. My fried tacos. Almost anything sweet. Chips. Perfectly buttered toast. They all represent something to me, but I’m relatively good at distracting myself now. Only rarely do I give in to these temptations. 

19. It’s been over a year since I’ve had fast food (as in a drive thru business). The only one I really liked was Taco Bell (and chic fil a but stopped going there years and years ago for political reasons). Ever since I really focused on clean and organic foods, fast food hasn’t appealed to me. 

20. A healthy diet of veggies and fruits is always nice and refreshing. A diet program I turn to most often is the ABC diet. 

21. Pants (used to be 25, currently a 27/28). Shirt (a S but I prefer M or L for a baggy fit). 

22. My lowest weight was around 110-113. I gained while in therapy several days a week and focusing on my passions in life. Things were going really well in my life and my need to control things was redirected into my athletic and creative goals. 

23. Media has a large role in motivating me to remain sick. Im not so sure it was a role in me wanting to lose weight. Once I was ill I rewatched Girl Interrupted, Thin, Intervention, Whats Eating You, Super Slim Me, Supersize vs Superskinny, Biggest Loser etc. over and over and over again. 

24. I don’t believe anyone who suffers from an ED is For any of this. I think it more accurately describes an individual as “actively-Ana” or “acitvely-Mia.” 

25. I’ve only successfully purged once. As I said earlier, it’s not my thing. My friend who struggles more with bulimia suggested I could try using a toothbrush and tickle my tonsils. After a lot of gagging and sweat, I was able to purge a good amount of my meal. But I hated the idea of eroding my esophageal lining. And in the end, restricting was just easier for me. Tidier. More control. More discipline. Reflected my personality more. 

26. A different kind of confidence, and a certain mentality that I truly miss in my creative process. 

27.  If I can avoid it like, avoid going into the kitchen to grab something, then its very easy. It tests me when sitting around a table filled with snacks. So if I can keep my hands occupied with a hydroflask of water, thats what I focus on. And I’m always in my head from the beginning of meal. Who am I wish. What are we eating. What else have I eaten today. What else do I have to eat today. How many calories is the meal. How many calories can I afford to eat. etc etc. And when I have a plan, I stick to it. 

28. Yes. I’ve always had a thigh gap growing up and even through my 20s. Only in very late 20s and 30 have I felt the very top of my thighs rub together if I walk a certain way and it makes me feel seriously ill. I miss the days of standing with my feet tightly together, upright, butt tucked (not sticking out like IG influencers for better angles), and still having a gap.

Delpad Prompt 2: Holiday, Part 1

Della: Okay kids, we're about to head out. Launchpad you got our bags?

Launchpad: Yup. though this ones' kind of heavy (Dewey falls out of the bag, having not really zipped it that well) Oh hey Dewey you coming with us?

Della: No he's not.

Dewey: But why. What's more romantic than the result of your last relationship huh huh?

Della: Sweetie I know you want to come but we need some time to ourselves. I haven't really had ANY since I got back. I tried to put it all on you.

Donald: And as much I don't want your mom to go either she NEEDS IT. She needs time to herself that isn't terrifying isolation.. isolation... no no you had melon your fine.

Louie: We'll get Uncle Donald therapy while your gone.

Della: (Nods) We're just going to a small tropical island for two days. We'll be fine.

Huey: We know it's just.. last time one of our loved ones went on vacation they ended up on the moon and then stranded for months

Donald: Months months... melon where's melon?! (Dewey slaps him) Thank you. (Dewey slaps him again)

Dewey: That's for not naming me Turbo!

Huey: Let it go Dewey.

Dewey: Never!

Della: boys please i've taken every precaution. We should have cell reception, and if you don't hear from me in 24 hours, Launchpad has been chipped and I have an emergency radio in my hollowed out tooth. I'm NEVER loosing you boys again. Never. (hugs them tight.. for about half an hour)

Scrooge: (walks in) I just came to say goodbye, make sure the percautions are over.. .and .. (joins in group hug, which donald and launchpad have joined int he interim. )

Della: Okay we really gotta go or we'll loose our reservation. I love you all. Stay safe.

(The family waves them off0

Donald: She'll be back this time right?

Scrooge: you can count on it... I guarantee it

(The rental plane, a small seaplane)

Della: Okay... we're fine.. it's fine i'm just.. heading off.. without my family. I"m .. okay. My therapist said I need this. (Sighs) I'm sorry i'm babbling

Launchpad: it's okay I do that all the time. But are you you know okay? We can turn back. We can always use this plane to make loops over the mansion

Della: Oh we're certainly making loops in this I just.. I haven't left the kids behind since I got back. I put up my best front back there but.. I worry about never coming back again.

Launchpad: Look you can't worry about that. Eventually their going to grow up and leave. Dewey to become a sucessful talk show host/ indiania jones, Louie to buy Japan and Huey to write a sucessful book series.

Della: you REALLY know those kids don't you

Launchpad: I'm a good listener. My point is, you can't focus on what your missing you just have to focus on what's ahead. The boys will be fine and if their not NOTHING will stop you from getting back to them. It didn't before, it won't this time.

Della: (Smooches his cheek before turning her attention back to the controls) When did you get so smart?

Launchpad: About the time I got around to asking you out finally.

Della:... Sweet too. We should be landing soon. You know as much as i'm worried.. Donald is right.. I need this.. just some Della time... but you know not alone so my reflection dosen't murder me in my sleep. YOU DON'T GET ME DENISE.

Launchpad: DOn't worry i'll break all our mirrors.

Della:... (Blushes) Your really sweet you know that?

Launchpad: I know you say it often enough.

(One landing and a boring check in we're skipping over later. )

Della: (Collapses on the bed) OH god that's refreshing... my bed usually has a kid or a brother in it these days (Does snow angels)

Launchpad: Should I stay in the tub?

Della: No no no I just want to.. enjoy this. (Smiles happily)

Launchpad: Okay i'm just going to watch the tv.

(Turns on the tv, and flips through it eventually stopping on)

Launchpad: OH boy I didn't know they were airing this again. (Sings along) Daring duck of mystery, champion of right, leaps out of the shadows darkwing of the nighttttt. Somewhere some villian schemes but his number's up. Three two one darkwing duck. When there's.

Della: (Cracks eyes open) Wait wait.. don't you know this guy?

Launchpad: yeah. He's one of my best friends along with Dewey and Donald.

Della: But he's fictional.. but I saw him. I even met him. He got stuck on our roof for a good hour.

Launchpad: Oh he grew up watching the show but then I convinced him to become him for real after we stopped darkings actor from kiling a bunch of people.. then he died. (Puts hat over heart) I still miss ya jim.

Della: Wait THAT'S the jim who's name you keep saying at night? I had no idea.

Launchpad: Yeah he just.. didn't want to give up the role> They were making a movie, Scrooge was.

Della: But he refuses to watch those. I had to tie him down just to get him to join us for movie night.

Launchpad: Yeahhhhh there's a reason it didn't get finished.. and that reason is my hero, darkwing's actor jim starling, sorta decided to get rid of Drake.. or wanted me to. it took talking with him to realize he wasn't a bad guy and I tried talking jim out of it but well. he kinda snapped, threatned the set including Dewey and Scrooge and had to be stopped.

Della: He threatanted my babY?!

Launchpad: (Gets up and sits down on the bed) not on purpose I just.. I think something in him broke? I dunno.. but then I ALMOST had him.. I just.. like the last pickle in the jar just out of reach. But instead he died trying to save me instead. He pushed Drake out of the way but .. he could've taken it. He just.. wanted to be darkwing and if i'd just TALKED to him I could've convinced him he was only hurting people.

Della: Oh sweetie (hugs him tight) it's not your fault. Okay? You tried but he was too far gone. It's not your fault okay? He died saving you... but he went out of his WAY to not because it was you, but because he didn't want the other guy to do it. But the fact you feel so bad about it... it proves what a great guy you are.

Launchpad: Awwwwwwww (Smooches her)

Della:(Smooches back) Now come on let's go get something to eat, then maybe enjoy this resort a little huh? Get your mind off this

Launchpad: yeah.. get my mind off this... you always know what to do. (Smiles and takes her hand as they head out

Can I Bar-row Your Attend-tion (ch. 5)

Pairing: Roman/Patton

Summary: Roman and Patton are two bartenders at a local bar & grill with some serious chemistry. However, the only people who don’t realize it are each other, and one of them is in a relationship…on the rocks.

Chapter 5: Make it a Double (POV- Roman)

“Greetings hungry and thirsty patrons! Logan, I got your text and lucky for you my rehearsal got out early tonight. Apparently the director ate a bad—“

Roman had expected it to be a busy night at the bar & grill. He had expected everyone to be a bit wound up and stressed from work. The last thing he ever expected to see when he walked through the door was Patton in front of the bar crying in Virgil’s arms. The sweet man before him, the object of his affections and who always wore a smile, looked so broken and vulnerable before him.

It shattered Roman’s heart.

He rushed to Patton’s side immediately and gently cupped the other bartender’s face in his hands.  Roman tried to wipe away some of the stray tears with his thumbs. They did not belong there.

“Patton, what’s wrong?” Roman asked. “Are you hurt? Did someone hurt you? Are you sick?”

Patton didn’t seem to be injured yet his wracked sobs continued as he shook his head. The sight of his beloved so distraught twisted Roman’s chest and made him want nothing more than to fight whatever had cause Patton so much pain.

“Virgil, what on earth happened!?” he asked, turning anxiously towards his friend.

Virgil signed. “You’re not gonna like this…”

Roman listened as Virgil filled him in on what he’d witnessed in Archwood Park. How he’d spotted Dio kissing someone else, proof that he’d been cheating on Patton for who knows how long, and how Virgil immediately went to Sanders’ to tell Patton the truth as gently as possible. It wasn’t that Roman couldn’t believe that Dio was capable of cheating. Far from it to be honest; after all, both Roman and especially Virgil had good instincts about people and neither had ever liked the dancer. What shocked and revolted Roman was the fact that Dio had been so villainous as to do such a thing to someone as kindhearted and loving as Patton. Poor sweet Patton who would never hurt so much as a fly; heck, he would likely nurse one back to health. How DARE he betray the trust of my precious Patton!

It took every ounce of strength not to erupt in righteous fury then and there. Right now Patton needed him. He took a breath to quell his anger for the moment and stepped back to properly look Patton in the eyes with all the tenderness he could convey. Although his honey eyes were waters and his freckled face blotchy, Patton still looked to Roman like the most sublime creature in existence. Roman searched for the right words to best comfort Patton, to reassure him that he deserved so much better that this act of betrayal, that we was worthy of the most true and faithful love.

Before he could however, Patton caught him in an embrace with such force that he had to take a step or two to regain footing. The heat rose to Roman’s cheeks. Sure they’d hugged before, but never so intensely as this. He could feel Patton trembling as he cried. Screw not crossing any lines. Roman wrapped his arms around Patton and held his trembling figure close, stroking fingers through his soft wavy hair in the hopes of soothing his wounded heart. Patton…can you feel how much I love you? And he did love Patton, so very much…and he would avenge his love’s broken heart even if it was the last thing he did.

Reluctantly, he pulled away. Roman took Patton’s hand and kissed it before nodding to Virgil. ‘Take care of him for me,’ he mouthed, and Virgil nodded back ‘I will’. Reassured that Patton would be in good hands, Roman turned to the door, clenching his fists.

“I’m going to kill that creep.” Roman marched forward, a man on a mission.

“Roman where are you going? You just got here,” said Logan.

“Sorry boss. I’ve got a knuckle sandwich to deliver!”

Roman held tight to his anger as he walked out the door, barely registering that Patton had called out to him.

He held onto his anger as he got into his car and drove over to Archwood Park where his co-worker has often mentioned that his boyfriend (ugh! he doesn’t deserve that honor!) often performed at this time of night.

He held onto his anger when he parked his car in a 24 hour spot, just in case things got ugly and he hoped they would get ugly for a certain snaked faced prick.

He held onto his anger when he marched through the park until he found his target in question, surrounded by a small audience that watched him breakdance and contorted his shoulder in a crude fashion. Roman shuddered, disturbed. I’ll never understand what Patton saw in him.

He just barely held onto his coiled up rage until he forced his way through the crowd and finally unleashed the full force of it through his fist onto Dio’s tattooed face.

“THAT IS FOR PATTON!” Roman shouted.

The crowd around them gasped. Some took out their phones. Roman took great pleasure in seeing the look of surprise and pain on the dancer’s face as the force of the blow knocked him to the ground.

“What the HELL was that about, Prince?” asked Dio, wiping the blood from his lip. A bruise was already blossoming on his cheekbone.

“You STAY AWAY from my Patton, you lying, cheating, slimy SCUMBAG!”

Dio stood wobbly back up and straightened his bowler hat.  “Why does everybody call me slimy? I’m not! I just have large sweat glands.”

“You betrayed the trust of your boyfriend, the most pure hearted person on the planet, and cheated on him with some other man. You’re a two faces LIAR and that is what makes you a slimy boi, you villainous serpent!”

“Look, I don’t know what you’re talking about,” he said. Roman saw with satisfaction that he’d managed to chip a tooth. “But he isn’t your Patton, and you’re interrupting my performance. So why don’t you do yourself a favor and—“

“The only favor I’m doing,” said Roman, grabbing him by the shirt, “is telling you one last time. Stay. Away. From Patton!”

“Wait, Patton from that old dive bar downtown?” said an audience member.

“That really sweet guy from Sanders’?”

“This guy cheated on him?”

Dio looked nervously around as his audience went from cheering for him and tossing dollars into his hat to whispering about him. He was really sweating now.

“No way to save face now.” Roman smirked up at Dio as he looked around him frantically. “Can’t lie your way out of this one you son of a—”

A fast and painful knee to the side from Dio silenced Roman. He let the other guy go and clutched his stomach in pain. That was definitely going to bruise later. At least he didn’t get me in the face.

“What I do in my private life and relationship is non of your business. Besides, Patton will never believe you over me. He trusts me too much. It’s not like we both don’t know he’s a naïve lovesick—“

Roman tackled him to the ground. “YOU AREN’T WORTHY OF PATTON’S LOVE!”

Roman and Dio rolled on the hard concrete ground trading blows. Dio was fast but the actor was stronger and madder, so he landed more hits than the dancer beneath him. He got some good shots to the ribs that would surely leave his pale form black and blue for days, as well as a nice shot to the eye. Dio got in some scrapes and gave Roman a split lip (he did manage to get the face), but by the time someone pulled Roman off him Dio was the worse for wear.

“Alright, alright, break it up!” Roman turned and, to his dread, saw that the voice belonged to a park police officer. “What the hell is going on here?”

“Officer,” said Dio, who was also being held up by another policeman. “This crazy stranger just came out of nowhere and assaulted me! Arrest—ow.”

“Stranger? You’ve known me for months you liar!” Roman scoffed. “And I did not assault him for no reason, officer. I came here to defend the honor of my friend, whom he has been cheating on! If anyone deserves to be arrested it’s him for crimes against love and all that is decent!”

“Enough!” said the female officer holding him back. “Did anyone see who threw the first punch?”

The crowd of onlookers seemed, for the most part, to be on Roman’s side more than Dio’s. Roman even recognized one or two faces from the bar. However, no one wanted to lie to the police so they mumbled and reluctantly pointed to Roman. That was enough for the officer.

“That settles it. Looks like I have to take you down to the station. Officer Hobs, looks like that one needs a doctor, so make sure he finds a ride. Show’s over folks.”

The officer placed her handcuffs on him, lead him to her car nearby, and read Roman his rights. Sufficed to say, it was the most terrifying drive of Roman’s life. Ohhh sweet Sondheim, I’ve really gotten myself into trouble this time. Even so, he couldn’t help thinking that it was worth it. At least, he hoped it would be…

…They soon arrived at the police station, yet to Roman it all felt a bit unreal. Having his mug shot taken (worst headshots ever), taking his thumb print, giving them his personal information, and having the handcuffs removed when brought to a grey walled room. It was only when he was sitting beneath those fluorescent lights and saw his battered reflection in the double sided window/mirror (sweet mother of hairbrushes, what was his hair!?) that it hit Roman just how much shit he’d landed himself into.

“Listen, I can explain—I didn’t mean—don’t I get a phone call or something?” Roman rambled nervously.

“No,” said Officer (according to her badge) Rivera. “Personal calls are restricted to persons in custody and questioning…Relax kid, this isn’t Law & Order. I’m just going to ask you a few simple questions and have you explain to me in full detail what happened and why. Clear?”

Roman gulped. “Crystal.”

So he answered her questions first. Then he told Officer Rivera about his day, told her about Patton and his relationship with Dio as well as his own personal feelings for Patton. She took notes in her pad as he spoke and didn’t seem very partial to whatever he said in his defense. However, when he got to the part about Virgil and his discovery of Dio’s infidelity, it seemed to strike a chord with her. True, the officer hid it behind a professional demeanor, but Roman was a good enough actor to catch subtle expression changes in other people.

“So that’s what happened,” Roman finished with a deep breath. “Am I…going to jail?”

Officer Rivera finished jotting her notes and steepled her fingers before answering. Good lord this woman knows how to leave you in suspense.

“Ordinarily you’d be tried under charges of assault.” That sent a chill up his spike. “However, the other guy didn’t make any official charges against you and since there were no reliable witnesses or officers on scene at the time, this could be classified as a misdemeanor. Which we don’t make arrests for. So you won’t be prosecuted.”

Roman released the breath he’d held. “Oh thank goodness!”

“That being said we’ll still have to keep you here overnight as protocol. Unless you can pay the fine for bail.”

“I’m a part-time bartender and community theater actor. Do I seem like I have that kind of money?”

“Wait, community theatre?” At this she let her impartial police demeanor slip. She looked down at the file with his man on it. “Roman Prince…You wouldn’t happen to be part of the RENT production going on, would you?”

“Uhh I am?”

“You wouldn’t happen to be friends with my daughter Krystal, would you?”

“Why yes, she’s out Mimi—Wait, you’re Krystal’s mother?” Didn’t see that coming.

“I am. So you’re the new friend that she keeps talking about. My daughter says you’ve got quite the acting chops, and that you’re a pretty stand-up guy.”

“Well, I think you’re daughter is pretty fantastic too. Probably one of the best actors I’ve had the pleasure of working with. Not to mention one of the best singers I’ve ever heard. I mean seriously, she can hit an A about high C! Do you know how HARD that is? She’s SO talented!”

The policewoman chuckled. “She did say you were pretty over the top. And yeah, that she is.”

Officer Rivera seemed to be regarding Roman in a new light. He shifted awkwardly in his metal chair as she stared him down, drumming her fingers on the table. Then she sighed loudly.

“So, technically you aren’t allowed to make any calls…I on the other had am free to,” she said. “So, if I happened to know the number of a certain family member of yours or a friends or…”

She slyly slid a piece of paper and her pen across the table to him. Ohhh! Catching on, Roman wrote down the number of Sanders. It was one of the few phone numbers he’d remembered off the top of his head (they’d confiscated his cell phone along with his wallet), and surely Thomas or Logan would still be there at least. Ah geez, he is not going to be happy about this. He finished and slid the paper back over.

“I can’t thank you enough Officer Rivera ma’am.”

“Don’t thank me yet. I’ll see what I can do. Meanwhile, I’ve gotta take you to a holding cell here for a little while. Don’t panic, it’s not a jail cell. Think of it as a really small boring waiting room.”

Roman nodded his understanding and allowed her to take him to a small room with one crosshatched window on the metal door. With an apologetic face, Officer Rivera locked him in. He looked around at the grey stained walls and dented metal bench against the wall. Might as well have a seat. The bench was cold and uncomfortable, yet as tired as Roman felt he could’ve probably taken a nap then and there. That is, if he wasn’t feeling the aches from his fight setting in on him finally.

All the adrenaline and anger form before was gone, leaving him all aware of the bruise on his side, the scraped skin of his knuckles and the iron taste of dried blood on his cut lip. It was worth it. It was SO worth getting to punch Dio in his stupid face. At least, that’s what Roman kept telling himself. Being in such a solitary quiet place gave him too much time to think about whether or not he had acted too rashly. Yes he has done it with the intent to defend Patton’s honor, but was there a bit of selfishness to it as well? After all, he’d never liked that Dio got to be Patton’s boyfriend and not him. Was it really right of him to have stepped in on Patton’s personal life like that? And what would Patton think of him after he found out Roman had basically kicked his boyfriend’s ass, even if it was for good reason? The first thing Roman would do once he got out of this place is apologize to Patton.

If I get out of this place…

…Roman must have dozed off at some point for a bit. He was suddenly jarred awake by the screeching sound of a metal door unlocking. He sat up with a grunt.

“Prince, you made bail,” said Officer Rivera.

“Wait, really? But who—“

Before he could finish Logan stepped in from behind the policewoman’s back. Roman had never been happier to see his stoic face.

“Logan?”

“Roman. How are you?” asked Logan, scanning Roman’s injuries. “I apologize I was unable to come sooner. I had to stop at the bank first.”

The tone of his voice revealed how concerned he truly was. No doubt Roman must have looked a wreck. He gave Logan a guilty half smile as he stood up from the hard bench and met him at the door.

“Trust me, I look worse than I feel. A lot better than I left Dio looking,” he said.

Logan nodded to him and, to Roman’s surprise, gave a flickered grin. “Good.”

Logan turned to Officer Rivera. “Since I made the payment in cash to your chief officer, I trust he won’t be called in again?”

“Given that there wasn’t any police witness at the scene it could be classified as a minor offence. Besides, the other guy neglected to place any official charges against your friend, so as far as we’re concerned he’s free to go.”

“Thanks again Officer,” said Roman. “If there’s any way I can repay you, I shall.”

“It was no trouble. Just tell my daughter to call her mother more often,” she said with a wink.

“Consider it done.”

He gave a mock bow that made her chuckle deeply and Logan rolled his eyes. That is, until they saw how Roman winced from the motion. Guess Dio had put more of a dent in him that he’d realized.

“We should get going Roman,” said Logan. “Thomas, Virgil and Patton are waiting back at the bar & grill. I have my car.”

They retrieved Romans things before leaving the police station and headed to Logan’s parked Volkswagen. Logan filled him in on what had occurred at Sanders after he’d stormed out. How he’d closed up early, the way their regulars had stood up and vouched for Logan and getting the call from the police station. It left Roman both surprised and in awe. He knew that their customers were loyal, but it warmed his heart to hear just how much. Not to mention the way Logan had put himself out there for his employees. Guess the cyborg had a heart after all. Roman always suspected as much.

When Officer Rivera said she’s make a call to Sanders’ for him, he’d thought that Thomas or maybe Virgil would be the ones to come to the station, even if the couldn’t get him out. Roman never expected his boss and friend to be the one to come to his rescue, let alone go above and beyond by paying what must have been quite the pricey bail. He’d never be able to repay him enough.

“Logan,” said Roman. “I didn’t get to properly thank you back there. For what you did, I mean. Paying my bail like that, it was, I can’t thank you eno—“

“Just for the record, the price of bail will be coming incrementally out of your next few paychecks,” said Logan. “And I do not care if we are friends. If you ever pull such an impulsive stunt like that again, even if it was for a noble reason, I will NOT let it slide and I WILL be forced to have you find work elsewhere. Understood?”

There was a stern bite to each word that came out of his mouth, and Roman couldn’t help shrinking a bit in the passengers seat. It felt like when he was scolded by his mother as a young boy for standing on the dinning room table reciting soliloquies.

Roman gulped. “Clear as crystal.”

“Good,” said Logan. After a pause he turned to Roman on a stoplight and his face actually softened up again. “Speaking outside of being your boss, however…that was a noble thing you did. Defending Patton’s honor.”

“Wow…Thanks specs,” said Roman, humbled.

“Of course,” Logan said, with a smile. Then he turned back to the road. “Once I had learned the details of the reason behind your sudden departure and Patton’s distress at the hands of Dio, I too had the urge to smite him down like the hand of God. Albeit in a much less physical manner than you opted for.”

Roman gulped when he caught the icy glint behind his glasses. He made a mental note to never get on Logan’s bad side.

“So, we are friends then?” Roman asked teasingly.

“That was the impression I’ve been operating under. However much I attempt to remain impartial during business hours.”

“Come on poindexter, you love us. Admit it. You’re a big ol’ softy.”

“Falsehood.” Logan couldn’t hide his faint blush.

Roman laughed. “So, as friends, can I ask you something that I’ve always been curious about?”

“Of course,” said Logan, keeping one hand on the steering wheel as he pushed back up his glasses.

“Why did you decide to own a bar and grill? I mean with your brains I would’ve imagined you’d want to be some sort of teacher or a chemist or something.”

Roman was afraid he had stepped too far into personal territory. However, Logan simply smiled and even gave a low chuckle.

“I can understand why you would think that, and truth be told I had considered being a mathematician for a long time.”

“So why a restaurant owner?”

After a pause Logan said, “As you know, I have aspergers syndrome, and as such I have never been the best at picking up on social cues or reading people. That doesn’t mean I have no desire to connect with people, it just makes it more difficult. Growing up, people were like a puzzle that I need to solve—that I wanted to understand and connect with better.”

Logan made a turn signal before continuing. Gee, the ride from Sanders to the park to the police station hadn’t felt half as long to Roman the first time around. Then again, I guess time moves a bit differently while in the back of a police car.

“Something I learned from growing up in a large family–aside from the fact that little brothers are demons–was that people seem to connect more openly over a shared meal in a comfortable environment. Thus over time I thought to myself, what better way to combine my love of mathematics and my desire to better observe and understand others than by operating my own business, specifically a food establishment? So I earned my GED, bought Sanders from an old family friend, and turned it into a bar & grill.”

“And…did you get what you wanted out of it?”

“Indeed. Running Sanders’ and familiarizing myself with our customers has helped me to connect with people better than I ever could have. Tonight was proof of this.  Granted the lot of you exacerbate me to no end some days but…becoming friends with you, Joan, Talyn, Virgil, Thomas and Patton especially has helped me tremendously in this regard.”

He noticed how Logan had made no attempt to hide the warmth behind those words and even smiled more openly than he’d ever seen. Perhaps it was because they were speaking outside of work that he’d let himself do so. Here he wasn’t Logan the boss man, he was Logan the friend. Either way, Roman had never felt closer to the big nerd. However, the mention of Patton brought Roman back to reality.

“How…how is Patton? Did he say anything when I left?” Roman asked. “About…me?”

Logan breathed out through his nose. “It’s difficult to say. He was a 9 on the emotional pain scale regarding Dio’s betrayal, and he seemed distraught over you after hearing about your temporary incarceration. I can only imagine how he will react upon seeing your current state.”

Roman gulped at hearing his fears confirmed and silently cursed his own impulsiveness. He didn’t regret throttling Dio, but the last thing Roman had wanted was to cause Patton further pain, yet he did just that. Still, he had to face Patton eventually. They rode in silence the rest of the way.

“Oh Patton,” he whispered to himself, “please forgive me.”

They got back to Sanders about 15 minutes later. It was strange going back into the bar & grill when it was all closed up, yet the light peeking out from inside through the blinds reminded Roman that his friends and crush were waiting for them inside. Logan unlocked the door and Roman followed him inside.

The bar had been cleared, the floors mopped and the chairs stacked atop the tables. Virgil was nervously scribbling on a napkin while Thomas was telling him something, likely going over breathing exercises judging by the rise and fall of their chests. Patton was pacing over by the bar and nervously fiddling with the sleeves of his favorite grey cardigan. Roman guessed he likely had changed out of uniform into his own clothes, given that Patton was also wearing a blue polo instead of his black t-shirt. When Logan shut the door with a clank all three heads shot up.

Roman only had eyes for Patton, who looked both relieved and about to cry. He felt so guilty for causing Patton further grief. Surely he must be mad or at least disappointed in Roman for going off to fight for him. Roman prepared for the worst.

“Hey, everyone…” Roman rubbed the back of his neck. “Patton… I’m so—“

Patton’s arms were around him before he could finish. The force of his hug could’ve knocked the wind out of a football player. He pulled back when he heard Roman groan. Patton’s hands gently probed over him as though he were made of fine china.

“Roman, thank goodness you’re alright! Are you hurt? Did Dio hurt you? Did they press any charges? I’m SO glad you’re not in jail anymore! Oh, what happened to your lip? And just look at your knuckles, they’re swollen!”

Roman blushed at being fussed over in such a way, but at least Patton wasn’t angry. “Patton, I’m alright, I promise. Just a couple of bruises and some scrapes here and there, nothing major. Logan took care of things with the police, so it’s all good padre. I promise.”

At this Patton finally relaxed a bit. Virgil and Thomas came over to him as well once Logan caught them up on everything that happened back at the station. They were clearly irritated at Roman but seemed just as relieved.

“That was a really stupid thing you did dude,” said Virgil.

“Roman, buddy, I know you’re all about the chivalry, but don’t go running off and getting into fights like that,” said Thomas.

“Not without us as backup at least,” said Virgil.

“Indeed,” said Logan.

“Okay?” asked Thomas.

Roman knew full well that neither of his friends were fighters in the slightest. Still, the gesture meant a lot to him. Looking at Logan, Virgil and Thomas standing around him, he felt incredibly grateful to know they’d always have his back; even when he was being an impulsive love struck idiot.

“I swear,” said Roman. “And listen, Patton, about…about Dio—“

“I don’t care about him. Not anymore,” said Patton. “I’m done with him. Officially and for good.”

There was a hard edge to his voice that Roman had never heard before. He looked to Virgil for confirmation if this was true and the artist gave him a nod, smiling at his stepbrother proudly. Although they weren’t good circumstances by any means, Roman couldn’t help but feel proud of Patton for standing up for himself like that. The sweet bartender was a lot stronger than he let on. Aaand Roman wasn’t ashamed to admit to himself that he was incredibly happy that Patton was now free to court. Of course, all that could wait till much later. They had time. He could wait.

“Well then,” said Roman, “I’m proud of you Patton.”

“As am I,” said Logan, placing a hand on Patton’s shoulder. “I don’t know very much about the complexity of your relationship, but I think it’s a fair assessment that Dio was a raging douchbag.”

Virgil laughed. “Couldn’t have said it better myself. He was never good enough for my brother.”

“Heck yeah. You deserve way better Patton,” said Thomas. “We’re all proud of you.”

“Yeah, I’m proud of me too,” said Patton.

Roman caught a flicker of sadness in Patton’s eyes. He was about to ask if Patton was truly all right, but before he could Roman found his hands gently being cradled in the other mans. A blush crept up his face.

“I’m just glad that you’re safe,” said Patton, kissing his scraped knuckles. Roman winced, but he’d never felt happier. “Come on. Let’s get you fixed up.”

Patton gently dragged him by the hand to Logan’s office in the back. Roman would have followed that man anywhere he’d asked. The others didn’t follow and frankly he was glad of it. Roman wanted to talk with Patton in private.

Patton closed the door behind them and gently sat Roman down while he got out the first aid kit from their bosses/friend’s desk. Logan’s office much like his personality: clean, organized, and precise. Roman did however notice there were three framed photos on the desk next to a stack of documents. One was of a rather large family standing in front of a Christmas tree with Logan amongst them. Another was of Logan standing with a proud smile in front of a newly opened Sanders bar & grill, and the last one looked to be a candid photo (no doubt taken by Patton) of Logan with every one of the Sanders’ employees as they laughed at some statement or other. The photographer had even managed to capture the rare laugh from Logan frozen in time.

“Alrighty, now lets get you cleaned up,” said Patton as he took a seat across from Roman. He took out antiseptics and bandages from the kit. “This might sting a little.”

The swabs did sting a bit, but Roman sat patiently as he let Patton nurse his battle wounds with all the focus and tenderness of Florence Nightingale.

“So…it’s truly over between you and Dio now?” Roman tentatively asked.

“Yeah,” said Patton as he wrapped bandages around Romans knuckles. “I called him up after I found out about your guys’ fight and ended things.”

“Oh...” Roman gulped. “I’m sorry. I-I din’t mean to be the cause of—”

“Oh no, no, it wasn’t about you Ro!” Patton said quickly, placing a reassuring hand atop Roman’s. “It was about Dio. After he cheated and hurt you—hurt someone I cared about, well, that was the last straw.”

Roman’s shoulders relaxed. “Ah, then…good. So, how are you feeling?”

Patton paused for a moment and then took a deep breath. “Lighter. Like I can breathe again. Honestly, I hadn’t let myself realize how unhappy I started to feel towards the end of the relationship, how over it truly was, until tonight.”

Patton finished taking care of Roman. Then he put the first aid kit on the desk and scooted his chair closer to Roman before continuing.

“I think part of me suspected he’d been cheating, but I…just thought it’d be easier to…avoid all those nasty feelings, y’know? I hate to admit it, but I’m not sad about it being over…which makes me feel guilty because I’m the one that ended it and it-it was my first real relationship so i—so what does that say about me?”

Roman used the pad of his thumb to gently brush away the tear that streaked down Patton’ cheek. Hesitantly, he cradled it in his hands. To his pleasant surprise Patton actually leaned into the touch. The gesture choked Roman up a little.

“Patton, you have every right to feel however you feel…but if you think it means there’s something wrong with your heart, then you could not be farther from the truth.” said Roman, looking him in the eyes. “You have the strongest, most pure heart of anyone I’ve ever known. You deserve someone who recognizes that. Who treasures your heart and treats you like the most sublime creature in all of existence! And if someone isn’t doing that or isn’t meeting your heart’s needs, then you have every right to walk away without guilt. You deserve to be happy!”

Patton let out a sad laugh, but at least the silent tears ceased. Roman saw a sad smile creep up the other’s face.

“Thanks Roman. I don’t need all that wooing though. All I want is someone to be present with me, to spend time with. Someone who I can be playful with and share things with and we’d take care of each other. Someone who, maybe, finds me…desirable, somehow? Someone…”

Patton looked up at Roman with, perhaps, something akin to hope in his eyes. It made Roman blush, the intensity of that hopeful yet open look humbled the young man as he waited patiently for his secret love to finish.

“Someone who…loves me…as much as I love him. Do you…think there someone out there like that for me?”

Roman’s face was hot as a hearth now and he couldn’t stop the frantic flutter of his heartbeat. It’s not like he was new to the ways of love and he certainly had never been the shy type. Yet Patton’s words had submitted something within him and he couldn’t for the life of him find the right words or poetics to say to the man before him. So, he went with instinct.

“There is…”

Roman leaned in slowly. Patton seemed to have edged in as well, until finally Roman closed the distance between them. He felt Patton stiffen only for the slightest of seconds before melting into the kiss with equal measure. Perhaps it was his overactive imagination but Roman could’ve sworn he also heard a small whimper from the other. Patton’s lips were softer than Roman could have ever imagines, and there was a faint lingering flavor of chocolate sweetness there as well. No kiss had ever felt like this before, with so much underlying knowingness that this thing between them was right. It felt like a drop of honeyed milk on the tongue of his parched heart. Kissing Patton felt like coming Home.

I’m going to be by this man’s side for as long as he wants me, Roman vowed.

Sadly though, breathing was still a thing. Patton was the first to pull away. He touched his bottom lip with the tips of his fingers. A rosy blush colored his freckly face and he gaped at Roman, which made him panic.

“I—I’m sorry! I thought you—I should’ve asked your consent first—I—“

“Chai…”

“W-what?”

“Your lips, they…they taste like chai. Spiced and sweet and warm.” Patton smiled warmly at him. Roman melted. “I like it. I like you Roman. More thank like you, I…love you.”

“I love you too Patton,” Roman beamed. “So very much!”

A giddy laugh spilled from Patton’s lips and Roman just wanted to drink it in with his own mouth. They’d have all the time in the world for that though. Roman beamed at his beloved and they touched their foreheads together. Their silent reverence of each other was interrupted however by the sound of a slow clap coming from the door.

They both turned to find their friends peeking in through the office door. (Roman could have sworn they’d closed that) Thomas peeked in from the right beaming at them, Virgil leaned against the doorframe arms crossed and grinning cheekily, and right in the middle giving the slow applause was Logan; a satisfied smile upon his face as well. They really are a bunch of snoops, Roman thought slightly irked. Still, he couldn’t help smiling as well. He was just too gosh darn happy to care.

“How long have you all been there?” asked Patton.

“Long enough bro,” said Virgil.

“No way we were gonna miss this!” said Thomas. “Virgil, you owe me twenty bucks.”

“Damnit,” Virgil grumbled.

“Language, kiddo.” Patton said, eliciting a laugh form them all.

“How about you Logan. Do you have anything to say about this?” asked Roman.

“Well,” Logan said, adjusting his glasses, “it is certainly about time!”

Roman barked a laugh at that. Yet he knew now that it came from a place of caring. In less than 24 hours he’d been called form rehearsals, gotten in a fight, taken to jail, bailed out, and now here he was in the afterglow of his first—and hopefully not last—kiss with the love of his life surrounded by his dearest friends. In the back of an old hole-in-the-wall bar and grill no less! It was almost too much drama even for the actor. Yet when he felt Patton slide a hand into his and saw his beloved angel smiling back at him, he knew that it had all been worth it.

“Sure is specs,” Roman said, yet his eyes were all on Patton. “It sure it.”

1. “foreign object”

“shit--fuck--”

kate breaks his nose.

guard #10--tall, lithe, baggy uniform, the shade of blond that calls to mind hospital lights--manages, “go to hell.”  

she smiles, blood in her mouth. spits it onto the cold concrete floor. the iron taste lingers. “you gonna tell me where mrs. regina goldfeather iii is being held?” 

more muffled cursing. jesus, this guy’s a real mumbler. she reaches up to squeeze his neck, just a little. leans in real close. her noise wrinkles at the axe body spray. “did you hear me? or am i going to have to repeat myself?”

finally, after he flinches, adam’s apple bobbing under her palm, voice thick: “down the hall, make a left. last door on the right.”

“there.” kate knocks him out--easy, really, when they’re this close. “was that so hard?”

“minola,” her earpiece crackles, nasally voice low and grating in the static. “get a move on. we don’t have time for this--”

“h,” kate huffs as she jogs down the hallway, ankle throbbing, right eye swelling shut. “c’mon. i’m a professional.”

“you keep saying that, and yet--”

“and yet you need to shut the hell up. i need to find the holding cell.”

there’s breathing of some sort cutting into her ear--horatio is an expert in long-suffering sighs--and after a few minutes of breathing hard, of sweat dripping down her back, of coughing up god knows what, kate gets to the door. 

“fuck you,” she tells the door, and kicks it wide open. 

the room is tiny and dirty, with one cracked lamp flickering from the ceiling, dust floating in the air, rusted file cabinets lining the walls. there are three men and two women unconscious on the ground.

none of that matters. 

kate grins wide, front tooth chipped, voice hoarse. “i win.”

portia rolls her eyes as she finishes untying herself. “bless your heart. this was all me.” 

“how d’you think we’re getting out, hm? the guards didn’t neutralize themselves, sweetheart.”

“we’re not undercover anymore.” portia fixes her wig, fake red ringlets spilling behind her in graceful arcs. “you don’t have to call me that.”

kate shrugs, ignoring the protest of her bruised shoulder. “regardless, darling, you have to admit that i saved your sorry ass. that’s....what, twenty to nineteen?”

“no, you--” portia’s round face changes, frowns slightly. she’s such a sore fucking loser. “next time, you’re the one who’s going to need saving--”

“ladies,” horatio snaps into their earpieces. “we are on a schedule here. i can’t break into their databases if i don’t know what i’m looking for. you have the intel, you get out and get to the safe-house. now.” 

“of course,” portia says, slipping into her real voice, new yorker drawl back in place. “we’ll be perfectly punctual. won’t we, mrs. pauline goldfeather?” 

kate raises an eyebrow. “yes,” she says, and refuses to break eye contact. it’s been a long two weeks of pretending to sip wine, of getting government gossip out of marcellus’s cousin, of showing off her stolen diamond ring, of holding hands and linking arms and selling every last piece of it. stuff like that was portia’s thing, usually. kate’s never been any good at it. she had really tried, once, to be what her father wanted her to be, to be what her husband wanted her to be. still. she had smiled when their bodies were lowered into the ground. car accident on the way to a men’s rights protest. in the end, she’d had the last fucking laugh.  

it’s been good to finally break some knee caps.  

kate glances at portia’s lipstick, bright red, designed to attract attention, to distract form the cold and merciless glint in her eyes. 

every last piece. kate shakes her head. it’s strange to know what your co-worker tastes like. “let’s go.”

2. “animal mask”

it’s 11:30am in the 24-hour diner on a friday. traffic had been shit, as usual. the service is slow, as usual. the music is too loud, as usual. nerissa is late, as usual.

“sorry,” she says, not sounding very sorry at all. as usual. “work ran later than i thought.”

“it always does.”

“yeah, yeah. you order for me?”

portia raises an eyebrow. “turkey BLT on rye, no mayo, extra bacon.”

nerissa nearly cackles. “i’ve got you trained.”

“you wish.”

they get their coffees. there’s a couple of families seated near them--a baby’s giggles pierce through the top 40, an old woman chides a kid who must be her grandson about the way he holds his fork. there’s a lot of chatter about work, the kids, the new apartment, the news. 

portia stirs in cream. “can you help us?” 

nerissa grabs another sugar packet. “with what? espionage? smuggling drugs? identity theft? murder? world domination?”

“world domination, definitely,” portia says, smiling despite herself. 

“excellent. you’ll pay me my fee, right?”

“of course.”

“good.” nerissa sips her coffee, sweetened to her liking. “but seriously, what do you need?”

“i need you to look into cases involving nathaniel fortinbras and claudius hansen.”

“those guys? aren’t they the ones who--?”

the waiter brings them their food, uncommonly on time. nerissa immediately starts eating the fries that come with her sandwich, and portia’s glad that her eggs benedict has enough sauce. 

after a while-- “yeah, but i gotta watch the game tonight,” a woman tells her husband, jabbing a finger at her yankees hat-- portia asks, “you know who they are, right? there was that big scandal and everything?”

“yeah,” nerissa says with her mouth full. “yeah, i remember our office had bets going on whether or not they could all get away with it. huge legal fallout. i got fifty bucks out of it though.”

“mhm.” portia wipes her mouth with the cheap napkin. “look into him, bother your lawyer coworkers about it, dig up whatever you can. i’ll let you know specifics over email.”

“right, yeah. the super secret one.”

“yes, that.”

“god, you were always a bit shady--don’t give me that look, you know it’s true--but this is next-level.” nerissa laughs incredulously. “portia the vigilante. never thought i’d see the day.”

portia sighs. she can’t believe it either, on most days. every morning, she looks at herself in the mirror, and every morning she can never quite recognize the face that stares back at her. she picks at her eggs. “i can’t--after what happened...you know i had to start over.”

nerissa nods. she stops bouncing her leg. “yeah,” she says, brown eyes softening. “yeah, i get it. well, i don’t get it, i have no idea what you’re up to half the time, but. you know what i mean.”

portia smiles. “i know.”

The question concerning me is that my son did not pay anything for the car he was driving because it was given to him by my daughter. The car has a salvage title because it had apparently been totalled before but had been restored. The car probably would not be worth very much even had it never been in an accident because it is a 1986 year model Pontiac 6000. Now he is without transportation unless I provide it. Will they only be required to pay a salvage price for damages or what?

Progressive auto insurance problem !?

I need an opinion on what should i do in this case. 3 weeks ago , as I was driving home , on the freeway , from the truck in front of me fell big peace of ice and hit my car from underneath. Less than 1 minute later i noticed blue smoke behind , luckily there was and exit with gas station right there. By the time i pulled up to the gas station car was barely moving. After staying parked for 20 min , it leaked oil and tranny fluid . I called towing , had them pick the car up , called progressive made the claim. All of that happened in WI near Milwaukee , before i used to leave in Chicago , so I never dealt with progressive from WI. After couple days adjuster came out and gave shop 1100$ to start the repairs , shop diagnosed everything and came to a conclusion that its the transmission leaking because of the damage inside. Adjuster was extremely slow to give shop any kind of approvals to do anything , So after 3 weeks they finally allowed them to take transmission apart , another adjuster came out took pictures , then shop owner want around , called me and said that he will call me next day. Next day shop owner calls me asking if I heard anything from progressive ? I call that guy back , but he went on vacation. Call my first adjuster , she says that the guy who went on vacation concluded that it wasn't related to impact , so they wont pay for anything and I owe 500 for taking down transmission. I called shop owner right away asking if its possible , not being related to impact , he says of course not , otherwise your car wouldn't break after impact. He also said some technical explanation what caused it to break , i just don't know what. he also said that no one ever called him and asked him what was the cause if that. I dealt with progressive in Chicago twice , when i lived there , and i had greatest experience ever , but it seems that in Milwaukee , they are playing games , making me pay , and not taking responsibilities. I figured somebody might know what should I do in this case , I just want them to do what they should do , especially when Im paying 200$ a month.""

There are individual plans out there, directly through the insurance companies, also family plans. You can pick and choose your premiums/deductibles/co-pays, hunt around for what suits your needs best. The single plans especially, can be and most of the time are, affordable. I don't understand why people do not apply for basic health insurance that would cover them in an emergency, or for general routine health-care. After signing the car from the used car dealer, it came to my knowledge that i will have to get FULL coverage auto insurance. Frances Oldham Kelsey by all accounts was not mean to have a consequential life.  She was born in Canada in 1914, at a time women were meant to be seen and not heard.  Nonetheless, an affinity for science eventually lead to a masters in pharmacology from the prestigious McGill University.  Her first real break came after she was accepted for PhD level work in the pharmacology lab of a professor at the University of Chicago.  An esteemed professor was starting a pharmacology lab and needed assistants, and the man from Canada seemed to have a perfect resume to fit.  That’s right, I said man.  Frances was thought to be a man’s name, and the acceptance letter accepted Mr. Frances Oldham.  Given the times, her Canadian mentor advised the young Frances she not write to inform the Chicago professor of the mistake but to simply sign the acceptance letter as Miss Oldham.  The rest, as they say, is history.  Ms. Frances Oldham arrived in Chicago in 1936, and just two years later was asked to work on figuring out what caused one of the worst poisonings in American history by the nascent US Food and Drug Administration (FDA).

The FDA at the time was a small organization within the federal government that had come into being a few decades earlier after the passage of one of many progressive laws passed to protect consumers from rapacious pharmaceutical companies of the day.  At the time there was no standard for claims that could be made to an unsuspecting public and no requirement that drug companies specify what ingredients were being consumed by the public.  The companies of the day would take alcohol, water and coloring mixed together – give the formulation a name, get a US patent, and make millions by heavily marketing testimonials of cures to all that ails directly to the consumer.  At one point, it was estimated that there was more alcohol being sold via ‘patent medicines’ than at liquor stores.

Sadly, it was not medical professionals that put a stop to this, but muckraking journalists like Samuel Hopkins Adams who exposed the seamy underside in a series of articles for Collier’s Weekly entitled The Great American Fraud.   Popular outrage followed publication of this series in 1905 and in response, the first draft of the FDA came into being by order of Congress in 1906.

The initial purpose of the FDA was a small, but important one – ensure the correct labeling of drugs being sold to the public.  The 1906 act – fought tooth and nail by industry – mandated that ingredients such as alcohol, cocaine, heroin, morphine and cannabis be accurately labeled with contents and dosage.  To understand the scope of the problem at the turn of the century, understand that Coca-Cola (Coke) was sold initially as a ‘patent medicine’ that was marketed as a cure for headaches, impotence, morphine addiction as well as a number of other ills.  The main ingredients?  Cocaine and Caffeine.  Current drinkers of Coke will be happy to know that cocaine was eliminated from the formula in 1903.

The FDA as we know it now is a result of the passage of the Federal, Food, Drug and Cosmetic Act (FFDCA) of 1938 that came as a reaction to national tragedy in 1937 that once again drove home the point that the public needed protection from private corporations.   The matter was a simple one that involved an ingredient that actually worked – sulfanilamide.  Sulfanilamide was an antibiotic that killed bacteria by preventing the synthesis of Folic Acid.  Humans are relatively primitive and don’t make Folic acid – so it isn’t toxic to humans.  That is, unless you’re the chief chemist of  S. E. Massengill Company, and you create a preparation of sulfinilamide that is dissolved in di-ethylene glycol (DEG).  DEG is toxic to humans – the first case reports of toxicity had been reported six years earlier.  Unfortunately the chemist, Harold Watkins, who worked for the company was unaware as this was not widely known at the time.  Watkins simply added some raspberry flavor to the sulfa dissolved in DEG, and the company founded by a Tennessee medical student who saw more opportunity in selling drugs to doctors than practicing medicine started distributing the drug widely.  No animal studies or any type of premarket testing was required at the time, and so the drug went straight from the lab to the consumer.  More than a hundred people died.

Figuring out what was happening fell to the FDA, and it was investigative work by Kelsey working in her Chicago lab that ultimately lead to the identification of DEG as the culprit.  Months later, the FFDCA act passed, effectively changing the scope of the FDA from ensuring proper labeling, to safeguarding the public from known and unknown formulations of drugs.

The nation found itself engulphed in World War 2 shortly after, and all the resources of the nation were diverted to the war effort.  Pharmacists like Kelsey worked to solve the problem of the day – create a synthetic anti-malarial for troops fighting in mosquito infested lands.  There was no synthetic anti-malarial that came of the effort, but in working with a pregnant rabbit model Kelsey learned drugs could affect mothers very differently from their fetuses.

The experience with sulfilinimide and the work with antimalarials would soon be brought to bear after Kelsey was hired in 1960 by the FDA to help oversee the approval of new drugs.  A few months after starting, an application from the William S Morrel company in Cincinnati lay on Kelsey’s desk.  There was a new German drug – thalidomide – that the Morell company sought to sell in the United States. The drug was meant as a mild sedative for pregnant women with morning sickness.  It was already approved in multiple other countries and being prescribed by doctors widely.   This was a chip shot.  A stamp was all that was needed.  But, the three-person team was not happy with the animal studies presented or the clinical studies which essentially amounted to testimonials from doctors. While Kelsey delayed the application asking for more data, the company grew more and more irate, pressing approval of the drug for what they felt was going to be a profitable Christmas season. The bureaucrats won the day.  Slowly a trickle , and then like a flood came stories from pediatricians of babies born deformed with tiny limbs.  The final toll was gruesome – 10,000 affected in 46 countries.  There were only 17 reported cases in the United States.  And so it came to pass that a bureaucratic delay saved thousands of lives.

The story was reportedly leaked to the Washington Post by politicians attempting to pass legislation to further expand the FDA’s scope and ran under the headline “Heroine of the FDA keeps bad drug off market”.  Shortly after, the Kefauver-Harris drug amendment was signed into law by John F Kennedy, creating the FDA we would recognize now.  Drug companies had to prove efficacy in addition to safety, and it was going to take a lot more than physician/patient testimonials to prove it.  The law also put the FDA in charge of advertising, as well as manufacturing quality.

It is a beautiful story.  But there are other stories.

Diane

Diane came to the emergency room with unbearable chest pain.  She was 52, had struggled with controlling her blood pressure, but was otherwise well, until she suddenly wasn’t.  It was like there was something ripping and tearing in her chest.  The main pipe that serves as the conduit from heart to the rest of the body is called the aorta.  The walls of the aorta are made of multiple layers to withstand the pressure wave generated by each heartbeat. Making it to 70 years of age means the aorta will withstand over 30 million pulsations.  It is a remarkable thing that we don’t all die from exploding aortas once we hit the 10 million mark.  Some aren’t as lucky as others – the higher the stress on the aorta the greater the chance a microscopic tear forms in the innermost layer with one of those pulsations. The whole wonderful system begins to fail with that small tear. Each pulsation now makes the tear bigger.  Eventually blood forces it’s way into the tear, and the innermost layer starts to be torn away.  It’s like your insides are being ripped apart.  This was Diane’s problem.  Mortality for the patients lucky enough to make it to the hospital is measured in hours- the oft-quoted number is 2%/hour.  The technical term is an ascending aortic dissection.  There is little in medicine that is more acute.  Something needs to be done to stabilize the aorta and usually this means rapid transport to an operating room where a cardiothoracic surgeon can saw through the sternum, arrest the heart, and replace the defective portion of the aorta.  That patients like this survive is one of the miracles of modern medicine.

Diane was in the operating room an hour after coming to the Emergency Department.  Minutes from starting, a problem emerged.  Diane was on a blood thinner to treat a specific arrhythmia called atrial fibrillation. The anticoagulant was called apixaban, and it is commonly used by cardiologists to prevent the strokes normally associated with atrial fibrillation.   There are three anticoagulants in wide use – 2 belong to the class of medications called Factor Xa inhibitors, one is a direct thrombin inhibitor.   The problem here is that the direct thrombin inhibitor has an FDA approved antidote, the factor Xa inhibitors do not. Apixaban is a Factor Xa inhibitor.  There is no FDA approved antidote, no effective reversal agent.  All one can do is wait for the drug to be metabolized. The instructions are to wait 24 hours for minor surgery, 48 hours for major surgery.  Diane didn’t have 48 hours, and only had a flip of a coins chance of making it 24 hours.  But cutting through the aorta when any cut made has little hope of clotting is a daunting proposition in its own right.

The team elected to wait.  Diane was moved to intensive care. Analgesics brought some relief from the chest pain. Medicines were started to help reduce the shear stress from every heartbeat on the aorta to buy time.  The next day came and went.   36 hours later she began to have more chest pain. Suddenly and abnormal heart rhythm developed. Minutes later she became unresponsive and lost her pulse.  Half an hour later Diane was pronounced dead.

The proximate cause of death was paralysis on the part of the surgical team worried about uncontrollable bleeding.  The surgical team pointed to the class of blood thinners Diane was on.  Factor Xa inhibitors are a class of medications started somewhat gleefully every day by cardiologists.  The underlying reason is typically an arrhythmia of the heart called atrial fibrillation that predisposes patients afflicted to strokes. It is believed small blood clots form in the heart of patients with atrial fibrillation, and are ultimately pumped into the brain, where occlusion of a vessel results in a stroke.  The treatment shown to reduce the risk of this for years now has been the blood thinner known as coumadin – an inhibitor of Vitamin K dependent factors the body needs to clot blood.  Coumadin is also known as rat poison – take too much, make the blood too thin, and death from internal hemorrhage results.  Coumadin is a bear for patients and their physicians to deal with.  Blood tests are sometimes needed weekly, and eating a varying amount of vitamin K in your diet causes dangerous fluctuations in blood thinning.  The only upside of bleeding while on coumadin is that the rapid reversal of this effect is provided by transfusion of factors.

The difficulties of coumadin meant there was a market for alternative anticoagulants that were hopefully safer and easier to use. After decades of failure, three such agents hit the market just within the last 10 years.  Pradaxa (dabigatran) inhibited the procoagulant thrombin,  while Eliquis (Apixaban), and Xarelto (Rivaroxaban) bound and inhibited the procoagulant Factor X (ten).  In each of the three randomized controlled trials patients given assigned to the new class of medications did just as well (Xarelto) or better (Pradaxa,Apixaban) than Coumadin at preventing clots being thrown from the heart.  Remarkably, they did this despite being on a fixed dose of medication that didn’t require weekly/monthly blood tests to check the level of blood thinning.  They also importantly reduced the feared complication of bleeds into the brain by half relative to Coumadin.  The small problem, though, was that none of the new agents had an effective antidote.  The drugs were approved knowing patients would bleed on them or need emergent surgery for some other reason with no effective way of reversing the effects of the drug.  You couldn’t just give plasma or Factor concentrates as you did with Coumadin because the administered thrombin / Factor X would be inactivated by the circulating drug.    The only thing to do was stop the drug and wait the 48 hours it typically took for the drug to lose effectiveness.

This did not stop cardiologists just like me from prescribing the drugs. After all, the drugs are equally as effective and reduced the rate of bleeding into the brain. Bleeds into the brain are very bad, and that even a reversal agent was no assurance of a good outcome. The best thing is to prevent a bleed, not deal with it after the fact.  It also seemed highly likely that a reversal agent was around the corner.

Dancing with the FDA

Even with accelerated approval, the current FDA process takes years, not months. The first antidote developed was for the first class of novel anticoagulant that had hit the market, the direct thrombin inhibitor Pradaxa.  Idarucizumab  was a  humanized monoclonal antibody fragment that bound  Pradaxa and it’s active metabolites.  The FDA approved the drug based on pharmacokinetic studies in healthy volunteers as well as a case series of patients on Pradaxa with life-threatening bleeding. Idarucizumab  completely reversed the effect of  Pradaxa on clotting times and was not associated with any major adverse events.  The next antidote waiting in the wings for the only other class of anticoagulants – the Factor Xa inhibitors – appeared in the published literature months later.  The new drug was called Andexanet and was ingeniously designed as a mimic to the body’s own Factor Xa, with some important mutations at critical sites that would prevent triggering of the normal coagulation cascade.  Circulating Factor Xa inhibitors would bind the Factor Xa copy instead of the real thing and thus abolish the anticoagulant effect.

The first in man trials of Andexanet took place with healthy volunteers who were given either Apixaban or Rivaroxaban.   After steady state of drugs was achieved, volunteers were infused andexanet or placebo, and anti-Factor Xa levels were measured serially for 12 hours.

The results were remarkable – anti-Factor Xa levels fell immediately after infusion of Andexanet.  A two hour infusion of antidote to follow the initial bolus kept anti-Factor Xa level suppressed for the duration of the infusion.  No serious or severe adverse events or thrombosis was noted.  Anti-Factor Xa is of course a surrogate marker for bleeding.  Though there are strong links between bleeding control and anti-Factor Xa activity  in animal studies, and the correction of Factor levels in Hemophiliacs is a well accepted marker for abolishing coagulopathies, patients and their doctors care about bleeding stopping, not what the anti-Factor Xa level is.  Outcome studies in this space are challenging in part because not every patient may necessarily benefit from andexanet.    Bleeding may stop spontaneously in many cases for many reasons with the conventional approach of just waiting.  After all, the placebo graph shows Anti Factor Xa levels fall 50% in 12 hours doing nothing.  When bleeding into the closed compartment of the brain, the hope is that the amount of brain tissue necrosis that results from the consequent disastrous rise in pressure in the cranium may be ameliorated by decreasing the amount of bleeding.  Unfortunately, we don’t have a great way of quantifying brain cell death.  The Intracranial Hemmorhage (ICH) score is a commonly used system to help predict mortality in patients presenting with bleeding in the brain.  It is a six point scale that incorporates volume of bleeding, age, neurologic dysfunction at presentation, and location of bleeding to predict mortality.

Other than pointing out the information lost when a continuous variable is made categorical, what stands out is the nonlinearity of outcomes relative to ICH score.  An ICH score of 3 is not a little worse than a score of 2, it is a lot worse.  Mortality jumps from 26 to 72%.  Harm increases in a non linear fashion.  While the volume of blood gets only one measly point, it is the central problem – an obtunded patient with 5 ml of blood is probably not in that state because of the blood.  The problem in relating just the volume of blood to outcome is that everyone’s intracranial compartment is different.  80 year olds have more space to accomodate a certain volume of blood than a 40 year old because the brain atrophies with age.  Certain parts of the brain are more critical than others – a small volume of blood in the brain stem may be worse than a larger volume of blood in the frontal lobe.  So a combination of volume of blood, location of blood, and the clinical effect of bleeding determine prognosis.

With these difficulties in mind, the next adenxanet study tested the antidote in 67 patients with severe life-threatening bleeding within 18 hours of administration of a Factor Xa inhibitor.  The coprimary endpoints were anti-factor Xa activity and the rate of good or excellent ‘hemostatic efficacy’.  With regards to head bleeds, a reduction in volume of blood relative to baseline <20% was termed excellent, <35% was termed good.  Of the 47 patients in the efficacy population, 37 patients were adjudicated as having good or excellent hemostasis.  Anti-Xa activity levels decreased significantly after bolus and infusion of andexanet as well.

With regards to safety the concern with any antidote to a drug that causes bleeding is too much clotting.  Recall that Adenxanet is engineered without critical sites that normally catalyze thrombosis, and that in healthy volunteers, no thrombotic events were reported.  The population of patients presenting to ERs with life-threatening bleeding however, are a whole separate ball of wax. The published article helpfully lists everyone who had either a death or complication.

This group of elderly patients arriving at the hospital with any acute illness, let alone life threatening bleeding have a significant background rate of morbidity and mortality with standard of care.  It is, of course, hard to say with certainty without an appropriately matched control group whether the rate of thrombosis is higher after andexanet.  The study was also not designed to assess actual outcome differences.  We don’t know if patients are any less disabled because we were able to reduce their level of anti-Factor Xa activity.

But the data seemed more than good enough, and Portola Pharmeceuticals,  bay area biotech firm that makes Andexanet first submitted a New Drug Application (NDA) in 2015.  The FDA responded in Auguest of 2016 with a surprise rejection in the form of a Complete Response Letter (CRL).  These communications are private, but the company at the time noted that the FDA was primarily concerned about the quality of manufacturing facilities for the drug.  One year after the rejection, Portola reapplied in August 2017 expecting to hear from the FDA in February 2018.  February came and went as regulators pushed their expected response to May 4th, and the CEO of Portola hinted at even further delays on an earnings call.  Casually mentioned on the same earnings call was a net loss for 2017 of $286million , just slightly more than the $269million loss posted in 2016.  Hemorrhaging money, stock price declining, much hung on the May 4th decision by the FDA.

The drug was approved.

Little details are public about what the company did to gain approval. The FDA did make approval conditional on performance of post-marketing RCT of usual care vs Andexanet.  The drug will be available in limited fashion to 40-50 hospitals in late June of this year, with wider distribution and availability expected later in the year.

Unfortunately, all of this was too late for Diane.  The eventual review of the case in a conference of cardiologists amounted to a giant shrug of collective shoulders.  A small case series of patients who were operated on anticoagulation was presented suggesting high mortality of bleeding, and the collective conclusion appeared to be that there were mortalities that just couldn’t be helped. No one expressed any frustration that 3 years had passed after publication of the positive trials, and still, no antidote was clinically available.

We have come a very long way from cocaine laced Coke being sold to patients to cure morphine addiction.  One wonders if we have traveled too far.  An approval process for an antidote to life threatening bleeding now takes hundreds of millions of dollars and years of regulatory approval.  There are clearly patients dying as a result. Is anyone counting?

Even with approval, a post marketing RCT will require clinicians to choose patients with life threatening bleeding to not administer andexanet to.   Will it be a patient like Diane?  I imagine the argument for another trial, and slow approval relates to concerns a  Perhaps some will raise the uncertain safety profile because of the lack of a comparison group in the only trial in a real world setting.  That would be unfortunate because this is a poor way to navigate the real world. Any clinician taking care of Diane would have taken their chances with what is known about Andexanet now.  There is an intuitive understanding about the potential upsides and downsides that doesn’t require a strong mathematical foundation in game theory, and certainly does not require knowing anything about p-values.

Nassim Taleb, the mathematician/philosopher/flaneur conceptualizes what should be clear to physicians uninfected by the average treatment effect virus as decision making under non-linear conditions.

There are large outsize gains here to operating to repair the aorta in a timely fashion with the blood thinner neutralized that vastly outweigh risks.  The best thing about the graph is that it incorporates uncertainty – it does not require one to know exactly which direction from the starting point any given patient may travel.  Because traveling to the right results in outsize gains, one doesn’t have to ‘win’ much to come out ahead.  To be clear, it helps, of course that the gain here is the chance of life with surgery vs. certain death without surgery, and that the probability of pain/harm as suggested by biology and demonstrated in the trials to date suggest harm related to Andexanet is low.

In the parlance of Taleb –

  Regulators should worry more about efficacy than harms in the face of convexity.

(The threshold for approval of an antidote for life threatening bleeding should have a low threshold for rapid approval if the drug has a high probability of being effective and a low probability of harm)

Physicians who have never heard of Taleb do this intuitively.  Every day in some emergency room a trauma surgeon is working to find and stop the bleeding in a patient shot in the chest. Many of these patients will not survive – does this mean surgeons should cease their efforts to find and stop bleeding as soon as possible? Is there no point to innovating quicker ways to stop bleeding in service men and women in the field because most folks stepping on IEDs won’t make it?

None of this takes into account the significant costs that must be recouped for this type of an approval process.  Portola plans to sell the drug for $27,000 per dose.  It is estimated there are 100,000 admissions and 2000 deaths for bleeding related complications in patients taking Factor Xa inhibitors.   The figure makes me groan because I know the drug will be reached for over and over again in EDs across the country, and it is entirely possible that a large proportion will not derive a significant benefit.  The idea is significantly more palatable if the drug cost $2000.  But given that Portola lost $500million dollars in the last 2 years alone, can we grouse for lower prices given the regulatory hurdles erected?

If we really are interested in helping patients , we will need to pay heed to impulses on all sides – that of profit seeking corporations to cut corners, and that of regulatory excess to overwhelm, stifle innovation, and raise cost.
