What Benefits Can You Get From Poziotinib on NSCLC?
The antitumor activity of poziotinib was found to be significant in patients with EGFR exon 20 positive non–small cell lung cancer (NSCLC). The efficacy of the treatment was found to be highly dependent on the location of the exon 20 loop insertion. These findings were published in Cancer Cell. The results were published in Cancer Cell.
The study, designated as phase 2 (NCT03066206), included 50 patients with advanced non-small cell lung cancer (NSCLC) who exhibited point mutations or insertions in the EGFR exon 20. The study achieved its primary end point with a confirmed objective response rate (ORR) of 32% (95% CI, 20.7%-45.8%) and 31% (95% CI, 19.1%-46%) by investigator and blinded independent review, respectively.
Moreover, researchers found a median progression-free survival (PFS) of 5.5 months (95% CI, 5.4-10.4) with a PFS rate of 43% (95% CI, 30%-60%) at 6 months, and 29% (95% CI, 18%-46%) at 12 months. Median overall survival also appeared to be beneficial at 19.2 months (95% CI, 11.8-24.1) with a disease control rate of 84% (95% CI, 71.5%-92%).
“EGFR exon 20 mutant lung cancers typically don’t respond well to the types of tyrosine kinase inhibitors that have been largely successful in targeting classical EGFR mutations, leaving this patient population with few effective treatment options,” said senior author of the study, John Heymach, MD, PhD, chair of the Thoracic/Head & Neck Medical Oncology at the MD Anderson Cancer Center, in a press release.2 “Our study gives hope for not only a potentially beneficial treatment option, but for a new level of precision to better target EGFR exon 20 mutations and to design more effective clinical trials.”
A new drug application for poziotinib was accepted by the FDA for patients with advanced or metastatic NSCLC harboring HER2 exon 20 insertion mutations in February 2022. Poziotinib also received fast track designation from the FDA in March 2021 for patients with HER2 exon 20 insertion–mutated NSCLC.

#JamesDonaldson On #MentalHealth – #COVID-19 Survivors More Likely To Suffer From #MentalHealthProblems

As the #COVID-19 #pandemic stretches into its third year, countless people have experienced varying degrees of uncertainty, #isolation and #mentalhealthchallenges. However, those who have had #COVID-19 have a significantly higher chance of experiencing #mentalhealthproblems, according to researchers at #WashingtonUniversity School of Medicine in St. Louis and the #VeteransAffairs St. Louis Health Care System. Such disorders include #anxiety, #depression, and #suicideideation, as well as opioid use disorder, illicit #drug and #alcohol use disorders, and disturbances in sleep and cognition. In a large, comprehensive study of #mentalhealth outcomes in people with SARS-CoV-2 infections, researchers found that such disorders arose within a year after recovery from the #virus in people who had serious as well as mild infections. Overall, the study found that people who had #COVID-19 were 60% more likely to suffer from #mentalhealthproblems than those who were not infected, leading to an increased use of prescription medication to treat such problems and increased risks of substance use disorders including opioids and nonopioids such as #alcohol and illicit #drugs. The findings are published Feb. 16 in the journal The BMJ. We know from previous studies and personal experiences that the immense challenges of the past two years of the #pandemic have had a profound effect on our collective #mentalhealth. But while we've all suffered during the #pandemic, people who have had #COVID-19 fare far worse mentally. We need to acknowledge this reality and address these conditions now before they balloon into a much larger #mentalhealthcrisis."Ziyad Al-Aly, MD, senior author, clinical epidemiologist, #WashingtonUniversity More than 403 million people globally and 77 million in the U.S. have been infected with the #virus since the #pandemic started. "To put this in perspective, #COVID-19 infections likely have contributed to more than 14.8 million new cases of #mentalhealthdisorders worldwide and 2.8 million in the U.S.," Al-Aly said, referring to data from the study. "Our calculations do not account for the untold number of people, likely in the millions, who suffer in silence due to #mentalhealth #stigma or a lack of resources or support. Further, we expect the problem to grow because cases seem to be increasing over time. Frankly, the scope of this #mentalhealthcrisis is jarring, frightful and sad. "Our goal was to provide a comprehensive analysis that will help improve our understanding of the long-term risk of #mentalhealthdisorders in people with #COVID-19 and guide their post-infection health care," added Al-Aly, who treats #patients within the VA St. Louis Health Care System. "To date, studies on #COVID-19 and #mentalhealth have been limited by a maximum of six months of follow-up data and by a narrow selection of #mentalhealth outcomes -; for example, examining #depression and #anxiety but not substance use disorders." The researchers analyzed de-identified medical records in a database maintained by the U.S. Department of #VeteransAffairs, the nation's largest integrated health-care delivery system. The researchers created a controlled dataset that included health information of 153,848 #adults who had tested positive for #COVID-19 sometime from March 1, 2020, through Jan. 15, 2021, and who had survived the first 30 days of the disease. Few people in the study were vaccinated prior to developing #COVID-19, as vaccines were not yet widely available at the time of enrollment. Statistical modeling was used to compare #mentalhealth outcomes in the #COVID-19 dataset with two other groups of people not infected with the virus: a control group of more than 5.6 million #patients who did not have #COVID-19 during the same time frame; and a control group of more than 5.8 million people who were #patients from March 2018 through January 2019, well before the #pandemic began. The majority of study participants were older white #males. However, because of its large size, the study included more than 1.3 million #females, more than 2.1 million #Black participants, and large numbers of people of various ages. Compared with those in the control groups without any infections, people who contracted #COVID-19 were 35% more likely to suffer from #anxietydisorders and nearly 40% more likely to experience #depression or #stress-related disorders that can affect #behavior and emotions. This coincided with a 55% increase in the use of antidepressants and a 65% growth in the use of benzodiazepines to treat anxiety. Similarly, people who had recovered from #COVID-19 were 41% more likely to have #sleepdisorders and 80% more likely to experience neurocognitive decline. The latter refers to forgetfulness, confusion, a lack of focus, and other impairments commonly known together as brain fog. More worrisome, compared with people without #COVID-19, those infected with the #virus were 34% more likely to develop opioid use disorders and 20% more likely to develop nonopioid substance use disorders involving #alcohol or illegal #drugs. They were also 46% more likely to have #suicidalthoughts. "People need to know that if they have had #COVID-19 and are struggling mentally, they're not alone, and they should seek help immediately and without shame," Al-Aly said. "It's critical that we recognize this now, diagnose it and address it before the opioid crisis snowballs and we start losing more people to #suicide. "There needs to be greater recognition of these issues by governments, public and private health insurance providers, and health systems to ensure that we offer people equitable access to resources for diagnosis and treatment," he added. To better understand whether the increased risk of #mentalhealthdisorders is specific to SARS-CoV-2 virus, the researchers also compared the #COVID-19 #patients with 72,207 flu #patients, including 11,924 who were hospitalized, from October 2017 through February 2020. Again, the risk was significantly higher -; 27% and 45% -; in those who had mild and serious #COVID-19 infections, respectively. "My hope is that this dispels the notion that #COVID-19 is like the flu," Al-Aly said. "Our findings suggest a specific link between SARS-Co-V-2 and #mentalhealthdisorders," Al-Aly continued. "We're not certain why this is, but one of the leading hypotheses is that the #virus can enter the brain and disturb cellular and neuron pathways, leading to #mentalhealthdisorders. 50 years of progress in women's health

Debates over women’s health have long been contentious, but have also resulted in significant improvements in areas like equitable access to health care and survivorship. But the overall picture remains far from perfect. For example, the United States still has the highest rate of maternal death among high-income countries, particularly among African American women.
As the United States Supreme Court prepares to hear a Mississippi abortion case challenging the landmark 1973 Roe v. Wade decision, some experts are questioning whether women’s health may be reversing course.
Cynthia A. Stuenkel, MD, clinical professor of medicine at University of California San Diego School of Medicine, and JoAnn E. Manson, MD, DrPH, professor of epidemiology at Harvard T.H. Chan School Of Public Health, review 50 years of progress in women’s health in a perspective article published in the May 29, 2021 online issue of the New England Journal of Medicine.
“Reproductive justice is broader than the pro-choice movement and encompasses equity and accessibility of reproductive health care, as well as enhanced pathways to parenthood,” wrote the authors.
In addition to Roe v. Wade, advances in reproductive health include:

Advances in women’s health go beyond reproduction, said the authors. As interest and focus has expanded to all stages of a woman’s life, science has begun to catch up to the specialized needs of women and sex-specific risk factors for chronic diseases that disproportionately affect women’s health, such as autoimmune diseases, mental health, osteoporosis and coronary heart disease.

“Moving forward, it will be essential to recognize and study intersectional health disparities, including disparities based on sex, race, ethnicity, gender identity, sexual orientation, income and disability status. Overcoming these challenges and addressing these inequities will contribute to improved health for everyone,” wrote the authors.

Plastic Surgery Practice and Hologic™ Celebrate National Cancer Survivors Month
In Part 1 of our PSP breast cancer series, Dr Valerie Gorman explains the benefits of using the Hologic LOCalizer

Now in its 34th year, National Cancer Survivors Day (NCSD), which took place on June 6, 2021, initially launched to unite people around the world in recognition of cancer survivors, and to raise awareness of the ongoing challenges cancer survivors face. This annual celebration of life that kicks-off the first Sunday each June and extends further into National Cancer Survivors Month (NCSM), marks an important reminder for us to share resources, research, and the legislation being implemented to strengthen the survival rate of cancer patients everywhere.
At the time of this publication, the American Cancer Society estimated that in 2021 the United States will have 1,898,160 new cancer cases and 608,570 cancer-related deaths. That equals 5,200 new cases and 1,670 deaths every day, the Society notes. In addition, the statistics predict a total of 284,200 new male and female breast cancer cases on the rise, with 44,130 related deaths. But according to the American Association for Cancer Research, there are also more than 16.9 million people living with and beyond cancer in the United States alone.
In Part 1 of our Plastic Surgery Practice breast cancer series, we shine a spotlight on one of the latest technological advancements in surgical cancer treatments, the Hologic LOCalizer

To learn more about the wire-free guidance system that is currently being used to identify cancerous lesions and improve surgical outcomes for breast cancer patients, we spoke with Valerie Gorman, MD, FACS, chief of surgery and medical director of surgical services at the Baylor Scott and White Waxahachie, Texas Surgical Specialists/Texas Breast Center. Gorman, who typically performs 15 to 25 lumpectomy procedures per month, also discussed best practices in oncoplastic surgery.
She transitioned away from the wire (needle) localization technique two years ago and began using a new system, designed to mark and guide to non-palpable breast lesions using a miniature radiofrequency identification (RFID) Tag. Another purpose of NCSD is to acknowledge the contributions of families, friends, healthcare providers—like Gorman and her medical team, and those engaged in the cancer research developments necessary to keep patients alive.
Plastic Surgery Practice: Why did you transition from using wire (needle) localization to Hologic’s LOCalizer

device, and how has it improved your work flow?
Gorman: The wire-free miniature radiofrequency identification (RFID) Tag (“clip”) makes it easier for breast surgeons to pinpoint the exact distance and location of the cancerous lesion/s to be removed. It also enables multiple clips to be placed in the breast at one time with separate identification numbers. So, if there is a larger area of breast calcifications you’ll want to add two or three tags to triangulate the affected area being targeted, especially if there is more than one lateral, or medial lesion to be taken out.
Another reason I transitioned over to using the new wire-free device is because wire localization could only be done on the day of surgery, so my breast cancer patients had to arrive extra early to prepare for two different procedures in the same day—the wire placement to detect the affected tissue, and the breast cancer surgery to remove it. Depending on the complexity of radiology and operating room (OR) timelines, patients were required to wait hours inside of the day surgery area before we can finally schedule their case, because they could not go home wearing a wire. Using the miniature radiofrequency identification (RFID) Tag (“clip”), we’re now able to place the clip a week or two before surgery—or longer if needed, so there is much more flexibility with no limitations. This saves time for the radiologist, doctor, and patient.
In fact, some patients receive chemotherapy treatments prior to breast cancer surgery. The doctor can place the clip in the breast at the time of biopsy or before chemo to have it ready for upcoming breast surgery. And, if additional health issues arise that prevent the scheduled breast surgery, you don’t have to replace the clip.
For example, one of my cancer patients had a clip placement, and between the time we placed the clip and the time she was scheduled to have breast surgery, she broke her hip. It was a very protracted recovery period, but we didn’t have to do anything because the clip stayed in place until we were ready to do the surgery. As soon as she recovered from hip surgery, we were able to operate because the clip was still in place for many weeks.
PSP: Describe your oncoplastic surgery experience collaborating with plastic and reconstructive surgeons on patient cases.
Gorman: We have a supportive plastic surgeon, and our entire medical team is very close. When I partner with our plastic surgeon on lumpectomy procedures at Baylor Scott and White Waxahachie, Texas Surgical Specialists/Texas Breast Center, he’ll do a breast reduction at the same time. The LOCalizer

helps me identify the location I need to put the incision, so he can plan the pedicle method for breast reduction.
During my previous training for the wire localization method, we were taught to go in right over the wire to pull the lesion and address any cosmetic issues once the cancer was removed. As doctors we can now try our best to make breast surgery outcomes cosmetically favorable— rebuilding the breast for each patient helping them during a difficult time. Hologic LOCalizer

helps doctors plan effectively for patient cases, and in the OR to make sure we have all the advantages to perform the best oncoplastic reconstructive lumpectomy type techniques.
PSP: Why is it crucial for oncologists and plastic surgeons to support the needs of cancer patients in unison?
Gorman: Historically, I’ve worked with other plastic surgeons who didn’t have the same vision or care for the patient. It’s important to partner with a plastic surgeon who is willing to listen to the patient—and together as a team, and who also has the same goal for your patient—which is to treat the patient’s cancer in the best way possible, because we want our patients to do well. In addition, for patients who have always been unsatisfied with their breast size because it has kept them from doing certain activities, we can do a breast reduction with the lumpectomy and meet their goal while we’re taking good care of the cancer. A plastic surgeon who is not limited to only one approach and has many tools available to patients will be able to offer a menu of reconstruction options as needed, such a breast reduction with free nipple graft, nipple-sparing mastectomy, or a DIEP flap.
We want to treat the cancer and take it away from them in the best oncological way, so they don’t have any long-term effects from the disease or any recurring cancer. We aim to maintain the goals of each patient’s cosmetic results, which is different for each patient depending on their preference. Sensitivity is key to figuring out what that is, evaluating the resources, and making sure the whole team is on board.
PSP: What other features does this device offer that you appreciate most?
Gorman: I really like the pencil-size of the Surgical Probe. It’s smaller than my pinky, disposable, and doesn’t need a cover! It can be used during the lumpectomy within the cavity to identify when you’re getting close to the lesion and when you’re traveling from your incision over to it. The entire device is compact, so I don’t have to worry about another big device that needs to be hooked on a stand and have a wire connected to it. This just sits on the table, and the price is right. Surgery waiting room times and OR scheduling time crunches can also be costly, so the savings on that by using the Hologic LOCalizer

Reducing Plastic Use: We also go deep on the subject of plastic use and encourage everyone listening to reduce their use of plastics (especially single-use plastics) both for the sake of health and of the environment. With floating islands of plastic in the ocean that are the size of Texas, and plastic chemicals found several kilometers deep in the ice of the Arctic, this is now a global problem and we can all pitch in to reduce our dependency. Check out this post on reducing plastic use for some ideas.
Health Trends for 2019:
We delve into some health trends we anticipate continuing to grow in popularity in 2019. Specifically:
Elderberry
All things elderberry seem to be on the rise, and with good reason. While both of us have been using homemade elderberry syrup for years, we’re starting to see all types of remedies and even skin care that involves elderberries. Want to get creative? Try these elderberry marshmallows or elderberry tea.
CBD
Another health trend that seems to be exploding right now! Be careful, because there are many types out there and some are not as effective as others. I personally prefer this one because it is water soluble and colloidal so it’s highly effective and doesn’t use solvents to extract.
Digital Detox
After years of increasing technology use, we’re seeing a trend toward intentional time off from technology and seeking out quiet and focus. Personally, we love taking digital days off for a mental reset.
Rethinking Sunscreen
With Hawaii leading the charge, many states and countries are moving to reduce or ban chemical sunscreens that can harm ocean life:
In a world first, last May Hawaii passed legislation that will ban the sale and distribution of any non-prescription sunscreen containing these chemicals (found in 78% of the most popular brands) from 2021. The Caribbean island of Bonaire followed in the same month. The most comprehensive ban so far is in the small Pacific island nation of Palau, which is to ban the buying, selling, importing or manufacturing of sunscreen and skincare products that contain these chemicals and eight additional ones, from 2020.
Integration of Conventional and Alternative Medicine
We’re excited to see an increasing amount of integration between conventional and alternative medicine and doctors from each category being willing to work together. Specifically, we’re even seeing research about concepts from traditional Chinese medicine being used in conventional cancer treatment:
Yung-Chi Cheng, a pharmacology professor at Yale School of Medicine, worked on studies to help reduce side effects of chemo for cancer patients. He realized that those who finished treatment lives longest, so he figured that if they could increase the number of patients who didn’t discontinue treatment because of side effects like nausea and digestive problems, more people would survive. He used traditional herbs used in Chinese medicine to help these patients and while more did finish treatment, he noticed another side effect: their cancers shrunk faster than those who didn’t.
This has led to a potential cancer treatment based on Chinese medicine called PHY906 that shows promising results.
Placenta Research
Thanks to some new breakthroughs we’re learning much more about the amazing placenta. Researchers think this new data will give them an early window and possible way to help at-risk pregnancies and create new tests to help detect and remedy problems early.

In addition, at a recent meeting in Bethesda, Md., of the Human Placenta Project, several teams of researchers showed off sophisticated new techniques that enable the placenta to be studied in real time. That work could help doctors diagnose dangerous complications in pregnancy — including pre-eclampsia (a form of high blood pressure), preterm birth and fetal growth restriction — early enough to intervene. It might also help to reveal why boys are much more vulnerable than girls to disorders of brain development, including schizophrenia, A.D.H.D., autism, dyslexia and Tourette syndrome.
