Shoulder Dislocation Treatment in Delhi
Shoulder is a unique joint to have a 360-degree range of motion. The Humeral head makes a joint with a socket – called the glenoid. The shape of this socket is flat with the surrounding labrum making it deep. The labrum tissue of the shoulder joint is the thickened part of the shoulder capsule which makes the socket deep and has all the nerve endings with connection to the brain for the joint position sense of the joint. This joint position sense from the brain makes the shoulder joint always centric during the motion of the shoulder joint. The 360-degree range of the shoulder joint is possible due to inherent laxity in the joint. This arrangement makes it a highly mobile joint of the body which is essential for all day-to-day activities of the upper limb. But the laxity and mobility of this joint renders it prone for joint dislocation unlike other deep-seated joints of the body example the hip joint. Joint dislocation means that the head of the humerus comes out of the socket of the shoulder joint. This happens when the force of Abduction and External rotation is applied to the head of the humerus. Meaning when the arm is lifted and is at or above the shoulder joint, example when you are trying to throw a ball. In this position of abduction and external rotation the joint can get dislocated if the force applied in this position is greater than what the shoulder joint can sustain. This can happen during a fall, accident or while playing a sport with the arm in abduction and external rotation.
Shoulder Joint dislocation is a very violent movement of the joint causing the stabilizing ligaments of the shoulder joint to tear. The shoulder dislocation causes anterior labral tear along with injury to other structures. The force of dislocation can also at times cause fracture of the Humerus head or more frequently indentation in to the head of the humerus called the Hill Sach’s lesion. This is a sign of how violent the force is, causing the head to collapse at a localized area. Since this is a permanent insult to the head of the humerus, this further causes the head to engage with the socket every time the head is lifted and taken beyond a certain range. The patient with this pathology of the shoulder joint starts to feel apprehensive about the joint feeling loose or unstable in this position. The Fracture can also be seen at the anterior margin of the socket – called the glenoid. In anatomical terms it is called Bony Bankart. People with this kind of injury to the shoulder joint are prone to easily dislocate their joints with mild provocative maneuver of the shoulder joint. Especially the patients with history of dislocation before 20 years of age have been found to have recurrent episodes of shoulder dislocation due to the above explained factors.
Shoulder joints can dislocate in the elderly people due to a different mechanism of the injury to the shoulder. In the elderly people if the fall or jerk to the shoulder causes them to dislocate then the likely cause is tear of their Rotator cuff. This tear of the Rotator cuff causes a permanent tent through which they easily dislocate in a provocative position of the shoulder joint. This position of abduction and external rotation meaning the position of the arm like you are throwing a ball causes the head to escape through the tent of the torn rotator cuff to dislocate. This process can become recurrent and cause further damage to the surrounding structures of the joint.
