Ministers have announced a national NHS plan to tackle birth injuries and trauma that rob the “joy of pregnancy and birth” from tens of thousands of women.
Hospitals have been told to offer greater aftercare to women who have suffered birth injuries, which can have a devastating long-term effect on their mental health, relationships and ability to work.
Maternity services will also be required to assess women’s pelvic health as “early as possible” in pregnancy. This will provide support to those at high-risk, in an effort to reduce the prevalence of common birth injuries such as difficulties controlling the bladder and bowel, chronic pain and prolapse.
The new perinatal pelvic health services — run by specialist midwives and physiotherapists — will be implemented across England by April, backed by £11 million in government funding.
Maria Caulfield, the minister for women’s health, said: “For too many women, the joy of pregnancy and birth is tarnished by trauma, and pelvic health problems can have devastating ramifications for women’s lives.
“It’s vital women receive appropriate support, treatment and information. From initial antenatal appointments, right through to mental health care after birth, we are determined to support women throughout their pregnancy and birth journey.”
It comes after a campaign by Theo Clarke, the Conservative MP for Stafford, and Rosie Duffield, the Labour MP for Canterbury, for improvements in care for women who have had traumatic births.
Speaking in the House of Commons yesterday, Clarke said the taboo around the subject must change as she described the “terrifying” moment she believed she was going to die in childbirth. She described being separated from her baby and rushed into emergency surgery, adding: “I spent over two hours awake without a general anaesthetic, and I could hear them talking about me, and obviously it was not looking good. It was the most terrifying experience of my life.”
Clarke suffered a third-degree tear, a birth injury that affects 3 per cent of women and can lead to chronic pain and incontinence.
The Birth Trauma Association charity says 20,000 women a year suffer post-traumatic stress disorder after childbirth. Roughly one in three experience urinary incontinence three months after pregnancy, and about one in seven women experience anal incontinence six months after birth.
One in 12 women report symptoms of pelvic organ prolapse, when one or more of the organs in the pelvis slip down from their normal position.
Kate Brintworth, NHS England’s chief midwifery officer, said: “For any women that have experienced a traumatic birth, timely access to support is crucial to help address any issues like incontinence that can occur.
“To increase the support available, the NHS is rolling out dedicated pelvic health clinics nationally, bringing together expert clinicians under one roof, so women can seek help quickly and easily — and already thousands of women have been supported through our pilot sites.
“As part of these plans, women will be supported by maternity teams to recognise pelvic health problems and offered a self-assessment of their pelvic health as early as possible in pregnancy, and the NHS is here for women if they need support.”
An MP has described the “terrifying” moment she believed she was going to die in childbirth as she called on the Government to do more to help women who experience traumatic births.
Conservative MP Theo Clarke (Stafford) received a rare round of applause from the public gallery in the House of Commons from mothers and campaigners as she gave what she described as “probably the most personal speech” she will make in Parliament.
Describing her own experience, she said: “After a difficult 40 hours of labour I began bleeding very heavily after delivery.
“I was separated from my baby and rushed into the emergency room for surgery.
“I remember the trolley bumping into the walls and the medical staff taking me into theatre, and being slid onto the operating table. I spent over two hours awake without a general anaesthetic, and I could hear them talking about me, and obviously it was not looking good.
“It was the most terrifying experience of my life.”
Visibly upset, the MP broke off and took an intervention, before continuing: “I genuinely thought I was going to die.”
The MP was opening what she described as the first debate on birth trauma in the history of the Commons.
She said her own experience “completely opened my eyes to challenges with post-natal care”.
At one point, while in recovery, she said she was lying next to her baby, who was screaming, and unable to pick her up she pressed a call button for help, only for the person to respond by saying “not my baby, not my problem”, before leaving.
The MP said such a moment was “unacceptable”, and said there is a need to address the “postcode lottery” of services available across England.
She described her own birth injury, a third degree tear, which she said occurs in about three in 100 women.
The consequences of an obstetric anal sphincter injury, she said, can include urinary and faecal incontinence, chronic pain, sexual dysfunction and other mental and physical effects, adding: “It is very clear, we must do more to help women.
“We need to break the taboo by talking about this,” she said.
Outlining her requests to the Government, she said: “It is so clear to me that so much more needs to be done to support women who experience traumatic births.
“Today I call for the Government to add birth trauma to the women’s health strategy, recruit more midwives, ensure perinatal mental health services are available across the UK, provide appropriate and mandatory training for midwives with a focus both on mental and physical health, ensure that the post-natal check with your GP is provided to all mothers and will include separate questions both for the mother’s physical health and mental health.”
She also called for an improvement in continuity of care and national post-birth services, for the obstetric and anal sphincter care bundle to be rolled out to all hospital trusts in England, and to provide better support for partners and better education for women on birth choices and risks.
Responding to the debate, health minister Maria Caulfield said: “By early part of next year, every integrated care system in England, and I can’t comment what’s happening in Labour-run Wales, but in England we will have a fully working maternal mental health service to support mothers experiencing moderate, severe or complex mental health difficulties.
“It is true that the number of women accessing perinatal mental health services has risen almost 50% over two years, but that’s actually good news because we want women to come forward, our challenge as a Government in England is to be able to meet that demand.”
On the women’s health strategy, she said: “We will be updating our year-two strategy fairly soon and we’ll be setting out our priorities and I can let members know about that as soon as possible.
“This is a priority for the Government and we are seeing change, but more change needs to happen.”
On Thursday, the Department of Health and Social Care also unveiled plans for how it intends to implement NHS pelvic health clinics across all areas of England – first announced in 2021 – which follows a number of pilot schemes, and which the Government said will provide better information for pregnant women, and improved aftercare.

The Queen has died, ending the longest reign in British history.
She died peacefully, surrounded by her family at Balmoral Castle in Scotland.
In the days ahead, here's what we expect for her lying in state and her state funeral, as the nation pays its respects.
Scottish journey
The Queen's coffin will depart Balmoral estate in the coming days for the Palace of Holyroodhouse in Edinburgh - the official residence of the British monarch in Scotland.
From here, it will be taken in procession to St Giles' Cathedral, where the Queen will lie at rest for 24 hours, allowing the public to view her coffin.

The coffin will then move to London, ready for the lying in state.
The Queen's lying in state
The Queen will lie in state in Westminster Hall for about four days before her funeral, allowing members of the public to file past and pay their respects.
The grand hall is the oldest part of the Palace of Westminster, at the heart of British government.
The last member of the Royal Family to lie in state in the hall was the Queen Mother in 2002, when more than 200,000 people queued to view her coffin.

The Queen's coffin will rest on a raised platform, known as a catafalque, beneath the 11th Century hall's medieval timber roof.
Each corner of the platform will be guarded by soldiers from units that serve the Royal Household.
She will be brought to Westminster Hall from Buckingham Palace in a slow procession, accompanied by a military parade and members of the Royal Family.
People will also be able to watch the procession as it passes through the streets and big screens broadcasting events are likely to be set up in London's Royal Parks.
Her coffin will be draped in the Royal Standard, and once in Westminster Hall, it will be topped with the Imperial State Crown, orb and sceptre.

Once the coffin is placed in position in the hall, a short service will be held. Afterwards, the public will be allowed to enter.
When is the Queen's funeral?
The Queen's state funeral is expected to take place at Westminster Abbey in less than two weeks.
The exact day will be confirmed by Buckingham Palace.
The Abbey is the historic church where Britain's kings and queens are crowned, including the Queen's coronation in 1953, and where she had married Prince Philip in 1947.

There hasn't been a monarch's funeral service in the Abbey since the 18th Century, although the funeral of the Queen's mother was held there in 2002.
Heads of state from across the world will be flying in to join members of the Royal Family to remember the life and service of the Queen. Senior UK politicians and former prime ministers will also be there.
The day will begin as the Queen's coffin is carried from Westminster Hall to Westminster Abbey on the State Gun Carriage of the Royal Navy.

The gun carriage was last seen in 1979 for the funeral of Prince Philip's uncle, Lord Mountbatten, drawn by 142 sailors from the Royal Navy.
Senior members of the Royal Family, including the new King, are likely to follow in procession.
The service will likely be conducted by the Dean of Westminster David Hoyle, with the Archbishop of Canterbury Justin Welby giving the sermon. Prime Minister Liz Truss may be called on to read a lesson.

Following the funeral service, the Queen's coffin will be drawn in a walking procession from the abbey to Wellington Arch at London's Hyde Park Corner, before heading to Windsor by hearse.
The Queen's coffin will make its final journey that afternoon to St George's Chapel in Windsor Castle.

PETER LOMBARD AND THE EMERGENCE OF THE PARISIAN BOOK TRADE
From 1200 to 1500, the textbook required for the study of Scholastic theology at the universities was the Libri Quattor Sententiarum, or Sentences, of Peter Lombard. The success of that work made its author the most widely read and influential theologian of the later Middle Ages. Peter was not a profound thinker, but he had genius for systematization, an inherent diligence, and a far-sighted understanding of the types of information resources the nascent discipline of Scholasticism and the nascent institution of the university would require.
His glosses of the Psalms, Commentarium in Psalmos, and the Pauline Epistles, the Collectanea in Epistolas Pauli, which were subsequently published together as the Magna Glossatura, are compendia of theological and exegetical commentary from the patristic period up to the moment their composition (c. 1140). Even more ambitious, the Libri Quattor Sententiarum is an encyclopedic survey of all Christian theological thought (available to Peter) on over 400 topics, such as the nature of the soul, the sacraments, the Trinity, and the afterlife.
The Sentences consist almost entirely of quotations from, paraphrases of and and references to texts by established authorities–Peter’s original thoughts amount to 11 lines of text. The real originality of the Sentences is formal. The uniform application of a dialectical method of inquiry, in which views were contrasted with opposing views which are then rationally synthesized, to the entire body of Christian thought launched a wave of theological writing the culminated in the great scholastic Summae of 13th and 14th centuries.
Given their central role in higher education and primary importance to ecclesiastical culture, Peter Lombard’s works were copied thousands of times. According to the medieval inventories, the larger cathedral, priory, monastic and university libraries often had as many as four copies of the Sentences. Hundreds of manuscript copies of that work alone survive, including Peter’s original copy, which was acquired by the Archbishop of Canterbury, Stephen Langton, who in turn donated the book to the library of Nôtre-Dame in Paris in the early thirteenth century.
The manuscripts of Peter’s works seen here were produced around 1160/1180. Befitting their learned audiences, the books are handsomely, but not lavishly, decorated with large initials on gold grounds marking major textual divisions. Figural representation is restricted to an occasional author portrait, inscribed in an initial, showing Peter as Archbishop of Paris, scribe, and/or lecturer. The crisp, stylized vegetal and foliate forms (characterized by the distinctive “fleshy” acanthus), inhabited vinescroll, and deeply saturated palette are hallmarks of the late Romanesque style preferred by scholarly patrons in the later 12th century.
Equally impressive is the text itself. The evenly-spaced lines and clearly formed letters of invariable height of the early Gothic bookhand facilitate the legibility of the highly-abbreviated text. The complex mise-en-page, developed in the later 12th century, is not the one that Peter Lombard used. In this system, the double-spaced biblical text appears in short blocks to the left of each column of single-spaced gloss; the gloss surrounding the biblical text is extended above and below the scripture passage when necessary. Biblical quotations within the commentary are underlined in red; running-titles and initials, added after the main text was in place are in red or blue ink.
The uniformity and high quality of the Peter Lombard manuscripts strongly suggests they were produced by professional scribes, illuminators, and rubricators working together either under the same roof or in close proximity to each other. These teams were almost certainly based in Paris, where their services were most in demand. In the early 12th century, monastic scriptoria produced books for in-house use, as they had done for 500 years. The lay bookmakers of the later 12th century independently produced works for a wide range of institutions. In the 13th century, the Parisian book trade grew into a full blown industry, wherein the components of the book (vellum, illumination, copying, binding) were created separately in specialized workshops and assembled by the bookseller. This industry, which supplied texts to students, scholars and teachers across western Europe facilitated the rapid expansion of theological scholarship and higher education that took place in the 13th century.

Tim Ronalds Architects converts 19th-century Canterbury malt house into theatre
British practice Tim Ronalds Architects has converted a 19th-century malt house in Canterbury into a theatre for The King’s School, using an "as found" approach that celebrates the industrial building's original steel, brick and timber structure.
The London-based firm completed The Malthouse in 2019, following an invited competition by the school to create a host of new spaces, including a 400-seat theatre, dance and drama studios, dressing rooms and wardrobe facilities.
Adopting an approach of "doing no more than was necessary", much of the existing layout and finish of the original 1898 building has been retained, with what the practice call "structural surgery" used to create the new spaces.

"The Malthouse looks monolithic, but within has three distinct parts which mirror the [malting] process," explained the practice.
"In each part, the aim was to preserve the quality of the found space – the warm, rough texture of the thick brick walls, the muscular cast iron columns and beams, the heavy timber floors and trusses."
In the centre of the building, the large central kiln was cut away to open up a foyer space, centred around a weathering steel and wood staircase illuminated by a skylight in what would have been one of the malt house's ventilation stacks.
The interior celebrates the industrial history of the building
Perforated terracotta tiles designed for drying grains above the kiln have been retained as the floor surface on the landings, with areas of the steel grid supporting them left open to allow light from the skylight to pass through.
With no existing area large enough to support an auditorium, new steel trusses were inserted into the roof to allow the existing columns and floors to be cut away. This created a triple-height space with flying facilities and an orchestra pit.

The roof structure in the auditorium has been left exposed. Where new additions have been made for the raked seating and balconies, robust materials such as wood and steel that mirror those of the industrial structure were chosen.
"The new elements were to be neither distinct nor in 'period' style, but to have the same quality as the original building," said the practice.
"Everything was made plain and built to last. Theatre balconies were the simplest construction of boards and brackets, services were concealed, structures left exposed."

On the other side of the foyer in the former warehouse space, two large studios occupy the top floor beneath sloping wooden ceilings, with the original large wooden trusses replaced by lattice steel beams to increase the head height.
Below these studios is a double-height dining hall and a series of smaller classrooms on the first floor, and a kitchen and toilets in the vaulted ground floor spaces.

Externally, a series of later additions to the building have been stripped back. The practice reinstated the original white-painted chimneys and hoists, and painted "Malthouse" on the exterior in a nod to historic industrial signage.
The new entrances are set back from a row of three unadorned brick openings at the building's base, which the practice kept to create a sense of "mystery" around the building's new use.

The Malthouse sits alongside the recently completed King's International College by Walters & Cohen, which also references the site's industrial heritage through its use of exposed concrete and weathered steel.
