Close, yeah!  Here’s my specific take for this:

Sylvanas was a big fan of physical relations while alive and particularly a fan of ones with Nathanos.  She just wants to see if the things that still work are still any fun at all with her old friend, but in doing so, they settle into old habits from old memories (like breathing... which is what I imagine they started doing in the colored pic, because that's just what you do after a good SMOUCH).  And while they were never in any "let's run away together" grand romance, they were quite close, maybe more than they'd ever admitted to each other or themselves, and so this causes some Emotions.  Forsaken feel emotions, it's just that usually they're dampened and distant and ignorable, and if certain emotions (in particular ones that have to do with their past lives) do become intense enough to cross a certain threshold, it sort of all gets converted into pain and rage.   So they both malfunction because of this, and Sylvanas basically knows what has happened because of feeling something similar during the events of War Crimes with her sister.  Nathanos however doesn't really fully understand what just happened, but he does know Sylvanas is hurt by it.
