Mediums don’t possess supernatural abilities. Sorry to burst your bubble kids. There are no magic powers. You aren’t getting your Hogwarts acceptance letter this fall. The reality of what mediums are is a lot less fantastical. Mediums fit into one of three categories: 
A. Your medium might be someone with a mental illness who percieves hallucinations as ghosts. 
B. Your medium might not technically be suffering from a diagnosable mental illness. Instead, he or she may have deluded themselves into thinking they really talk to the dead out of some ego-driven desire to be special. Belief is a powerful force in guiding our perception of ourselves and others. But believing you talk to the dead and talking to the dead is not the same thing. The deluded medium might incorrectly percieve internal thoughts as coming from an external spirit. If they want to be a medium bad enough, their minds can play tricks on them to convince them that they are. Think confirmation bias on steriods. Let me explain with a hypothetical. The so-called medium holds a séance and she repeatedly says out loud, “If a spirit is with me, give me a sign.” On the fifth time that she  says this, she hears a faint noise. She unilaterally determines that the noise is confirmation that a spirit is with them. She ignores the fact that there was no noise the first four times she asked for a sign. She also ignores the possibility that there is a rational explanation for the noise. It could be the cat. It could be the heater about to kick on. It could be the house settling. The wind could be making the house creak. The noise could even be coming from outside the house but still audible inside. Perhaps the other person at the séance shifted in their seat. A million different things could cause the noise. But she ignores or doesn’t seek out any explanation that might make this situation not a paranormal one because she wants to believe she is a medium.
C. But usually your "medium" is just someone who knows how to read people really well and they do what they do for money and attention. Mediums often use a technique known as cold-reading. Cold reading often starts with a vague question or statement that applies to a lot of people. Like for example, “Who was the family member who had heart problems?” It’s an open-ended question which often leads people to volunteer information and most people have a relative who died of heart problems. The medium watches your reaction to that question, both verbal and nonverbal. People who aren't accustomed to reading body language aren't aware just how much people say without speaking. You give them the cues they need to determine if that vague guess was accurate or not. If they were right, they’ll continue down that train of thought. If your one of the few people that doesn’t have a dead relative who had heart problems, the medium will try to gracefully pivot away from that statement. One way to do that would be for the medium to say, “Not literal heart problems. They died with a broken heart. Someone really emotionally wounded them before they died.” Everyone has been emotionally wounded by someone. Keep in mind, people who show up to see mediums often want to believe the reading is real. If you want to believe it, you are a million times easier to manipulate than a skeptic. (I'll post some links below that go into more detail about how cold-reading is done and common tricks "mediums" use). 
A lot of "mediums" use other methods to cheat as well. You have to already know that reality TV is not that real. So, a lot of shows like "Long Island Medium" may use scripted scenes. Reality shows are also well-known for using editing tricks. For example, the producers may cut out all or part of a reading if the "medium" isn’t doing too hot.  There are also ways to alter a clip of the person recieving a reading so that they sound like they are saying something different than what was actually said. Mediums have also been known to pay people to appear in the audience when they are guests on talk shows. That way they  can zero in on someone they already know details about when they do a live reading. 
Then there are what is known as hot reading techniques. If you call a psychic hotline or you pay to go to a show or set up a one-on-one appointment with a psychic. You are going to give them all the information they need about you before you walk in the room. All they need is your name and the medium (or the little voice in their ear that is feeding them real-time information) can search you online and find out enough about you to sound "psychic." Someone with good computer skills can find out just about anything they want to know with very, very little information about you. I once located my friends birth mom for him. All I had was a very common first/last name. It turned out to be her maiden name but I found her- along with her picture, every address she'd lived at in the last 20 years, her email address, her phone number, where she worked, a picture of her house, a complete criminal history, a list of every civil case she'd been involved in, a list of the children she'd had since giving my friend up for adoption, her parents names, what kind of dog she had, the fact that she was in recovery, what music she liked, her favorite movies, etc. etc. etc. I had less information to go on than they'd have. They cannot do what they do without you either unintentionally giving them the information they need or being gullible enough to buy into vague guesses.
