June 2011.

Casey had been at NYU for a year now. She loved dorm life and had made good friends. This was her first major exposure to the lgbtqi+ community. She knew people who identified as such existed, she knew that those who identified suffered both systemic and non systemic abuse. To see the extent was an eye opener and not long before pride month she saw first hand some of the non systemic abuse. Her and a few friends were out for lunch, enjoying a gorgeous day in New York when her companions, who were only holding hands, were verbally abused and threatened. This experience was an eye opener for Casey and right then and there she decided she needed to be the change she wanted to see. A few weeks later there was pride and there was a March. It felt empowering attending, being with people who were open about who they were and who they loved and the fight that everyone had in them. Casey had only just decided that she was going to pursue medicine but she wanted to be the doctor that anyone who identified as lgbtqi would feel comfortable seeing.

June 2015.

It was the pride between undergrad and medical school and around campus there was an air of excitement. There had been a long time push for marriage equality and even though it was allowed in 36 states, all eyes were on SCOTUS to recognize it across all states. June 26th was a massive celebration but they knew there was a lot of work to do. She knew of the systemic abuse that still occurred and she was entering a health system that showed not the best outcomes for those who were lgbtqi+.

June 2019.

Casey was now a resident. She still considered herself an ally but work was busy, now that she was a junior fellow on the world famous Diagnostic team. She still attended events and she went with Jackie for most of them. Jackie had to hide it for most of her life, due to her parents and when she was away from home was still guarded about her sexuality due to her upbringing. Children were not on Casey’s radar yet, if ever but hearing Jackie’s experience made her determined to be the parent who would be understanding of her own child’s sexual orientation.

June 2022.

Casey was now an Attending and after the news broke that Roe was going to be repealed she was awed by Caroline’s proactivity. Casey knew that if women’s rights in health care were attacked, those who identified as lgbtqi were definitely going to be in the firing line. Caroline instigated a whole suite of measures and education aimed at the public and physicians so we knew where we stood legally, putting out education programs at community clinics in schools around women’s and sexual health and consent. Edenbrook fast became seen as an ally and Casey could not be any more proud of the organization she was working for.

June 2026.

There was more change at Edenbrook again but again there was much excitement. Caroline had not long announced her retirement from being the CEO of Edenbrook Hospital and her parting gift was a Medical centre that despite being apart of the hospital was going to be set in one of the more diverse areas of Boston both socially and economically and the primary focus was women’s health and the lgbtqi+ community. Casey was excited and hoped that at a local level in time there would be better health outcomes.

June 2034

Pride had become a family affair for the Ramsey’s. There was always plenty happening at the hospital but they always made an effort to go to a parade together. Hudson was nearly 9 and Alexis 5. Even though they were young both Ethan and Casey agreed it was good to educate them in an age appropriate way that the love that those who identified are people too, who love and are a part of society. This pride was bittersweet. Hudson enjoyed it but he was starting to not like being seen with his parents. Ethan and Casey did not want Hudson attending by himself just yet but they did not want one child missing out either. Ethan and Casey made the decision to continue coming to pride but leave the kids with family.

June 2047

Pride was special in more ways than one this year. It was first Pride for Alexis after coming out as bi-sexual and the first pride where Casey and Ethan were a parent to someone in that community. They were very proud of Alexis regardless, graduating top of her class and heading to UPenn but they were relieved when Alexis came out. She was less stressed and seemed to be really happy that she was a true version of herself.

June 2048

This was Alexis’s first pride out of home. She was loving college and had joined the lgbtqi club on campus. It was here that she was introduced to Polyamory and she enjoyed the freedom it allowed to explore her sexuality. Ethan and Casey were still very proud of her. Ethan and Casey were still very active at work and they had been noticing, at the local level any way even more and more better outcomes for those who identified as lgbtqi and both were really happy about that.

Yeah, he's been busy.

Empty nest syndrome had nothing to do with it, no. No way. His getting emotional over pasta and cookies mean nothing, he's fine.

Better Therapeutics begins ‘real world’ study of digital diabetes therapy

After announcing plans to go public, prescription digital therapeutics (DTx) firm Better Therapeutics has begun a “real world” study to test whether its BT-001 prescription cognitive behavioural therapy tool could change behaviours that are the root cause of type 2 diabetes.

The company has begun the open-label, non-randomised, controlled study to test whether the durability of treatment effect and total cost of care impact of the treatment.

San-Francisco-based Better will work with Steward Health Care in the year-long trial and expects top line data to be available in the second half of next year.

The company’s lead therapy candidate, BT-001 is also being investigated in a pivotal study as a prescribed treatment used under physician supervision for people with uncontrolled type 2 diabetes. If positive the data could support a filing with the FDA.

The real world study will involve around 1,000 patients with type 2 diabetes, with A1c levels between 7% and 11% across Medicare, Medicaid and commercial health insurance plans.

The primary objective of the study is to test the change in A1c and total healthcare costs after one year of treatment compared to the control group.

One year changes in blood pressure, blood lipids, weight, cardiometabolic medications, cardiometabolic medication costs and medical costs will also be evaluated.

Better’s technology works by blending clinical, behavioural and psychological inputs into a series of therapy lessons and skill-building modules.

These are designed to isolate and shift the underlying thoughts and beliefs which guide diet and lifestyle behaviours that cause a wide range of cardiometabolic diseases, including type 2 diabetes.

As sleep navigator for two hospital locations, Andrea Ramberg, BA, CCSH, RPSGT, refers high-risk patients for sleep studies before they wind up in the hospital again.

In her work with Illinois-based Centegra Health System, Andrea Ramberg, BA, CCSH, RPSGT, prides herself on asking the right questions at the right time. It’s a talent that has taken her from night technologist to sleep navigator for Centegra’s multi-hospital inpatient sleep screening program.

Similar to a breast health navigator or oncology nurse navigator, sleep navigators guide patients to make informed decisions. In this role, Ramberg combines a healthy respect for the science of sleep with a keen bedside manner that alleviates patient anxieties.

Splitting her time between Centegra hospital locations in McHenry and Huntley, Ramberg sees patients who are flagged after responses from the STOP-Bang questionnaire raise concerns. A recent case involving a woman in her 50s illustrates Ramberg’s willingness to dig deeper.

The woman had uncontrolled high blood pressure and type 2 diabetes, and physicians admitted her to the hospital for chest pains. Add in new onset atrial fibrillation and doctors agreed that a face-to-face screening with Ramberg was warranted.

“I went into the room and immediately saw how anxious she was,” says Ramberg, who this year earned a bachelor’s degree in psychology. “This was only her third hospitalization, and her comorbid conditions had not caused complications. I noted by her bedside that she had cute little drawings probably done by a grandchild.”

The observation sparked communication, and the conversation helped the patient to begin the process of really listening—instead of simply nodding reflexively. Ramberg introduced the idea that some of the symptoms could be caused by the quality of sleep. The physician had asked, “Do you snore?” but little else at that point.

Ramberg says, “By the end of the visit, she was so grateful. She gave me a hug and said nobody had ever taken the time to dig deeper. If [patients] are tired, particularly women, they assume it’s because they are taking care of everyone else in their life. They believe they are excessively fatigued because women are often the caregivers.”

The encounter took about 40 minutes. Ramberg contends that the time can often pay dividends in the form of reduced hospital readmissions. It’s a contention backed by statistical analysis.1-2

The woman eventually took an in-lab sleep study, and she did indeed have severe sleep apnea. She is now compliant on CPAP and is “thrilled” with it. “She was blown away that nobody else had discovered her sleep apnea,” Ramberg says. “She could not believe that a hospital visit was able to bring such positivity into her life.”

Ramberg combines the science of sleep with a relatable bedside manner.

Similar testimonials have flowed in since Ramberg began her position as sleep navigator 3 years ago, a role that followed 7 years in a variety of clinical sleep capacities. Part of the inpatient sleep screening program’s success is due to a concrete set of steps that bring high-risk patients into a room with Ramberg.

Referrals come to each respiratory department after the STOP-Bang questionnaire—asked to every patient upon admission—yields four or more positive responses. From there, Ramberg triages by reviewing health histories. “When I am assessing patients, I use a document I created in the electronic medical record [EMR] that satisfies insurance’s face-to-face requirements and documentation, and also serves as the prescription,” Ramberg explains. “The physician working with the patient then co-signs the document, and it also acts as the prescription, with specification on follow-up.” She then schedules either the overnight polysomnography or home sleep testing, as appropriate.

Between the two hospitals, it adds up to 250 to 300 referrals per month. Ramberg prioritizes in-person visits with patients who have comorbid conditions possibly linked to undiagnosed sleep apnea. Some patients are deprioritized because they are not good candidates for a sleep study, for example, if they are hospitalized with a terminal illness. Regarding the remaining patients, she says, “We are currently working on the protocol for those I do not screen face to face that alerts the patient they were flagged to be at higher risk for sleep apnea and to discuss it with their primary care physician or a sleep medicine specialist.”

It all works well now, but hurdles arose at first. Physicians were hesitant to write a prescription for an outpatient study, knowing they would not be the ones to follow up with the patient. “I follow up with the patient’s primary care physician or our sleep medicine physician,” Ramberg says. “Once physicians saw that transition of care was happening, they were more apt to sign my assessment.”

Insurance companies require certain aspects to be documented to approve sleep studies. Before Ramberg’s inpatient sleep screening program, some physicians would write prescriptions for sleep studies but not document the steps as required. In response, insurance companies would kick the prescription back. “If that happened, some patients would have the sleep study, but they would be stuck with the bill,” Ramberg says. “Or things would be denied when they got set up with their CPAP units. I was able to make sure that certain items were documented with every single patient, no matter what. It makes the physician’s job easier.”

Ramberg created her screening assessment as a way to get patients sleep studies scheduled faster. Many physicians did not know what to cover on a face-to-face assessment, which led to delays. “I was scheduling patients for a consult post-discharge with our sleep physician, but patients would not show up for it, or it would end up being two months past discharge before finally getting in for a sleep study,” Ramberg says. “That is too long. We needed to get these patients in sooner than later if we were going to prevent that readmission.”

Ramberg interfaces with a multidisciplinary team of hospital staff.

Patients could not be scheduled right away because the face-to-face consult was not taking place in a timely manner or was not appropriately documented. Ramberg looked into Medicare guidelines and created the screening form partially based on those strictures. The assessment form “has a free form box that I type in all my notes,” she says. “Whenever I speak with a patient, I include anything I think will help the physician or technologist with the sleep study. Underneath I have any type of sleep study I could order and who the follow-up will be done with, because that’s also something that insurance requires.

“Then I input the signing physician and it goes to the physician’s e-mail inbox. The signing physician must only click a button to sign, instead of having to wonder if they are writing the prescription correctly….We made it a much simpler process.”

It adds up to a smooth workflow, particularly if the right person is at the helm. “Andrea is very passionate about what she does,” says Rebecca Rockwood, MT (ASCP), MHA, director of operations for Laboratory, Respiratory and Neurology, at Centegra. “She is focused on our patients and how sleep is an integral part in a person’s well-being. She is very personable and discusses the process that patients go through from scheduling, through working with DME [durable medical equipment companies]. She is patient and persistent with patients because she genuinely believes that completing a sleep study can make a positive change in a person’s life.”

Benjamin J. Nager, MD, medical director, Sleep Center and Neurology, Centegra Health System, says, “Andrea is a highly intelligent, goal-directed, and motivated person who had the foresight and courage to pursue an idea that she thought would help patients, reduce overall complications, and improve outcomes.”

A philosophical transition may still need to happen if Ramberg’s model is to be replicated successfully. “People often think of sleep as something you can and should handle after the fact,” she says. “They see it as disposable in a sense that it’s not as important as other things. I think the inpatient sleep screening model is showing that it is as important. Yes, we must treat patients right now for why they are in the hospital, but we need to make sure we are offering the best care plan possible. Teaching patients how untreated sleep apnea could potentially be causing atrial fibrillation, congestive heart failure, and fluid retention is vital.”

Extra education for staff members is one way to get buy-in, but Ramberg acknowledges that pushback is inevitable from techs who have already completed a significant chunk of learning. “There is a need for our field to increase their educational base, and the educator role helps us to evolve to the next level,” says Ramberg, who earned her CCSH credential and bachelor’s degree in what she says was one small step at a time, all while raising a son and daughter. “I think techs can get frustrated at times hearing they need to increase their education base when they have already met the requirements to work in a field they love. However, my mission while I’m working on the program committee with the AAST [American Association of Sleep Technologists] and on the Board of Directors with the BRPT [Board of Registered Polysomnographic Technologists] is to show how higher education will help them take care of patients in the very best way possible.”

Ramberg entered the sleep field with an associate’s degree and took classes one at a time, with yearlong breaks at times, and recently earned her bachelor’s degree. One of her instructors for the bachelor’s program, Glenn Gelman, PsyD, says, “Columbia College courses are 8 weeks long and intensive; they cover an entire semester of material in half the traditional time. To do well, students must work hard. To excel, students must possess a high degree of personal initiative, self-discipline, and determination.” Ramberg’s determination got her to graduation with magna cum laude honors and she believes other techs can do the same.

Learning the full breadth of sleep treatments is also important, Ramberg says, and it demands a willingness to embrace oral appliances, surgical alternatives, or whatever new technologies may come along. Prior to starting at Centegra, Ramberg knew the DME side and how to download data from machines to gauge compliance. “We also brought in a dentist,” she says. “He saw patients alongside our sleep medicine physician. It [oral appliances] worked well for mild to moderate patients who could not tolerate CPAP.”

In addition to her reliance on science and questionnaires, Ramberg’s old-school willingness to listen is her “secret weapon.” “Personally, I like the touch of looking a patient in the eye,” she says. “I jot down notes on a clipboard, while always trying to engage. If a patient sees you are not looking, they may not tell you as much. I usually sit, instead of staying standing, because it puts patients at ease a little more.

“This job is part sales. I sell to patients the concept of their own health. We all should focus on finding the right motivation to keep patients compliant and get them to want to invest in their own health….Obviously, sleep is not going away. We all need to evolve with the science and figure out how we fit in. As long as sleep techs keep their minds open, they will be fine, and patients will benefit.”

Physicians were hesitant to write a prescription for an outpatient study, knowing they would not be the ones to follow up with the patient. “I follow up with the patient’s primary care physician or our sleep medicine physician,” Ramberg says. “Once physicians saw that transition of care was happening, they were more apt to sign my assessment.”

Insurance companies require certain aspects to be documented to approve sleep studies. Before Ramberg’s inpatient sleep screening program, some physicians would write prescriptions for sleep studies but not document the steps as required. In response, insurance companies would kick the prescription back. “If that happened, some patients would have the sleep study, but they would be stuck with the bill,” Ramberg says. “Or things would be denied when they got set up with their CPAP units. I was able to make sure that certain items were documented with every single patient, no matter what. It makes the physician’s job easier.”

Ramberg created her screening assessment as a way to get patients sleep studies scheduled faster. Many physicians did not know what to cover on a face-to-face assessment, which led to delays. “I was scheduling patients for a consult post-discharge with our sleep physician, but patients would not show up for it, or it would end up being two months past discharge before finally getting in for a sleep study,” Ramberg says. “That is too long. We needed to get these patients in sooner than later if we were going to prevent that readmission.”

Ramberg interfaces with a multidisciplinary team of hospital staff.

Patients could not be scheduled right away because the face-to-face consult was not taking place in a timely manner or was not appropriately documented. Ramberg looked into Medicare guidelines and created the screening form partially based on those strictures. The assessment form “has a free form box that I type in all my notes,” she says. “Whenever I speak with a patient, I include anything I think will help the physician or technologist with the sleep study. Underneath I have any type of sleep study I could order and who the follow-up will be done with, because that’s also something that insurance requires.

“Then I input the signing physician and it goes to the physician’s e-mail inbox. The signing physician must only click a button to sign, instead of having to wonder if they are writing the prescription correctly….We made it a much simpler process.”

It adds up to a smooth workflow, particularly if the right person is at the helm. “Andrea is very passionate about what she does,” says Rebecca Rockwood, MT (ASCP), MHA, director of operations for Laboratory, Respiratory and Neurology, at Centegra. “She is focused on our patients and how sleep is an integral part in a person’s well-being. She is very personable and discusses the process that patients go through from scheduling, through working with DME [durable medical equipment companies]. She is patient and persistent with patients because she genuinely believes that completing a sleep study can make a positive change in a person’s life.”

Benjamin J. Nager, MD, medical director, Sleep Center and Neurology, Centegra Health System, says, “Andrea is a highly intelligent, goal-directed, and motivated person who had the foresight and courage to pursue an idea that she thought would help patients, reduce overall complications, and improve outcomes.”

A philosophical transition may still need to happen if Ramberg’s model is to be replicated successfully. “People often think of sleep as something you can and should handle after the fact,” she says. “They see it as disposable in a sense that it’s not as important as other things. I think the inpatient sleep screening model is showing that it is as important. Yes, we must treat patients right now for why they are in the hospital, but we need to make sure we are offering the best care plan possible. Teaching patients how untreated sleep apnea could potentially be causing atrial fibrillation, congestive heart failure, and fluid retention is vital.”

Extra education for staff members is one way to get buy-in, but Ramberg acknowledges that pushback is inevitable from techs who have already completed a significant chunk of learning. “There is a need for our field to increase their educational base, and the educator role helps us to evolve to the next level,” says Ramberg, who earned her CCSH credential and bachelor’s degree in what she says was one small step at a time, all while raising a son and daughter. “I think techs can get frustrated at times hearing they need to increase their education base when they have already met the requirements to work in a field they love. However, my mission while I’m working on the program committee with the AAST [American Association of Sleep Technologists] and on the Board of Directors with the BRPT [Board of Registered Polysomnographic Technologists] is to show how higher education will help them take care of patients in the very best way possible.”

Ramberg entered the sleep field with an associate’s degree and took classes one at a time, with yearlong breaks at times, and recently earned her bachelor’s degree. One of her instructors for the bachelor’s program, Glenn Gelman, PsyD, says, “Columbia College courses are 8 weeks long and intensive; they cover an entire semester of material in half the traditional time. To do well, students must work hard. To excel, students must possess a high degree of personal initiative, self-discipline, and determination.” Ramberg’s determination got her to graduation with magna cum laude honors and she believes other techs can do the same.

Learning the full breadth of sleep treatments is also important, Ramberg says, and it demands a willingness to embrace oral appliances, surgical alternatives, or whatever new technologies may come along. Prior to starting at Centegra, Ramberg knew the DME side and how to download data from machines to gauge compliance. “We also brought in a dentist,” she says. “He saw patients alongside our sleep medicine physician. It [oral appliances] worked well for mild to moderate patients who could not tolerate CPAP.”

In addition to her reliance on science and questionnaires, Ramberg’s old-school willingness to listen is her “secret weapon.” “Personally, I like the touch of looking a patient in the eye,” she says. “I jot down notes on a clipboard, while always trying to engage. If a patient sees you are not looking, they may not tell you as much. I usually sit, instead of staying standing, because it puts patients at ease a little more.

“This job is part sales. I sell to patients the concept of their own health. We all should focus on finding the right motivation to keep patients compliant and get them to want to invest in their own health….Obviously, sleep is not going away. We all need to evolve with the science and figure out how we fit in. As long as sleep techs keep their minds open, they will be fine, and patients will benefit.”

Helen was 47 years old when she felt a lump in her lower neck that was later diagnosed as thyroid cancer. She lived an hour from New York City and came to Memorial Sloan Kettering for a second opinion. When she heard another patient tell a treatment success story, Helen felt confident in making the commute for surgery and long-term surveillance with the thyroid cancer team.

One summer afternoon, while waiting in her car at an intersection, Helen Courtney, 47, was adjusting her necklace when she felt a lump in her lower neck. A few days later, her primary care physician confirmed that she had six lumps on her thyroid.

Helen got a referral to a surgeon, who said the nodules were most likely benign but sent her for a fine needle aspiration. (This is a type of biopsy during which tissue or fluid is removed using a thin needle for examination under a microscope.) “I was still thinking that it wasn’t going to be anything,” Helen recalls. But a week later, she got a call from the doctor’s office saying that she needed to come in to talk about the test results.

“It was almost surreal when he said, ’We found cancer and we have to take your thyroid out. But you don’t have to do it with me. You can get a second opinion.’” Helen had been diagnosed with papillary carcinoma, the most common type of thyroid cancer.

A friend whose father was treated at Memorial Sloan Kettering Cancer Center urged Helen to go there, if only for a consultation. Taking her friend’s advice, Helen called and scheduled an appointment with Memorial Sloan Kettering head and neck surgeon Ashok Shaha.

A Team Approach to Care

When Helen and her husband, George, met with Dr. Shaha a few weeks later, he agreed that surgery to remove her thyroid in a procedure called thyroidectomy was necessary. He explained to Helen and George that if she came to Memorial Sloan Kettering, she would be regularly monitored after the operation by him and his colleague, an endocrinologist, Dr. Michael Tuttle, and that she would need to take replacement thyroid hormone medication. A regimen of radioactive iodine might also be necessary, he told her, to ensure that all the cancer was destroyed. (Radioactive iodine, a form of internal radiation therapy, is taken orally. The radioactive iodine is taken up by the thyroid gland and destroys thyroid cells.)

Even though they liked Dr. Shaha and felt good about his approach, Helen and George were uncertain whether to come to Memorial Sloan Kettering for her treatment, or to continue with her surgeon closer to their home in the Hudson Valley, about an hour north of Manhattan. As they stood outside Dr. Shaha’s clinic discussing their options, a woman with a noticeable scar on her neck approached them.

“The woman told us that she had overheard our conversation,” Helen recalls. “She said that she also hadn’t known where to go for her thyroid cancer treatment, but she’d done all her research and had great results with Dr. Shaha.” Helen and George took her positive endorsement as a good sign and decided to proceed with the team approach of Dr. Shaha and Dr. Tuttle.

Throughout this process, the couple decided not to give their 15-year-old daughter, Allison, many details about Helen’s diagnosis. With Allison so busy with schoolwork and athletics, they didn’t want to worry her until they felt confident that everything would be fine. But they quietly alerted her friends’ parents about Helen’s surgery, just to be sure she would have a few other people looking out for her. Thinking back, Allison says, “I didn’t understand why all of my friends kept inviting me over!”

“It was interesting how our friends would give us different advice on whether we should tell her or not,” says Helen. “But I wanted to do it in my own time and in my own way.” George continues: “Even though it was serious, we had all of the faith in the world in Dr. Shaha and Dr. Tuttle and that the cancer would be gone after her surgery.”

Surgery & Follow-Up Care

In late October 2007, a few weeks after meeting Dr. Shaha, Helen underwent surgery at Memorial Sloan Kettering. “I came home on Halloween with my own Frankenstein scar!” Helen jokes. About a week after her operation, she had a follow-up appointment with Dr. Shaha and met with Dr. Tuttle for the first time. Based upon the results of surgery and the pathology report, her doctors did not feel that radioactive iodine was necessary, but Dr. Tuttle started Helen on thyroid hormone therapy to replace the hormones that the thyroid normally makes and to decrease the risk of recurrence.

In the months that have followed, Helen’s recovery from surgery has gone well and she has become used to the hormone medication. “I’ve been able to more or less maintain my daily activities with only slight modifications,” she reports. Helen has resumed her full-time job as a pharmaceutical representative, and she and her family enjoyed a weeklong vacation to the Grand Canyon in April, 2008.

No Regrets

Even though the hospital was an hour away from their home, Helen and George have no regrets about their decision to come to Memorial Sloan Kettering. “Some people don’t want to go there because of the distance,” says George, “but I always tell them that they should go to the best. Don’t worry about driving back and forth — your friends will get you down there.”

 I think because I have a great family and everyone around me is so positive, I feel like nothing’s going to happen to me!

One of the biggest surprises for Helen has been the positive attention her scar receives from the people around her. “When Dr. Shaha told me about the incision he was going to make, he said that I should ’wear it with pride’ — meaning that I didn’t have to be bashful,” says Helen, touching her neck lovingly. “I find that I hardly wear necklaces anymore. I’m not afraid to show my scar and I don’t try to hide it. I’m not embarrassed by it. This is where I am, this is what I’ve been through, and if I can help anyone else by them noticing it, then that’s good.”

Although she realizes that hers was a treatable type of cancer, Helen has come away from her diagnosis with a newfound appreciation for her life as well as for her family and friends. “I had one or two thoughts early on when I wondered if this was it, if I was going to die,” she admits. “But you kind of have to go through all of that — what’s going to happen to my family, that kind of thing. Cancer is such a scary word, but I think because I have a great family and everyone around me is so positive, I feel like nothing’s going to happen to me!”
