The first-ever negotiations between the federal government and pharmaceutical companies have led to agreements that will lower the prices of 10 treatments, reducing costs for the Medicare program and for some individual seniors, the Biden administration announced early Thursday morning. This round of negotiations began in 2023 and took place because of the Inflation Reduction Act, the law that Democrats in Congress passed on a party-line vote and that President Joe Biden signed two years ago. The new prices are for drugs covering a variety of conditions, including diabetes and inflammatory illnesses, and are set to take effect in January 2026. The negotiation process is going to happen each year, with a new set of drugs each time. If all goes to plan, that means the scope of drugs subject to negotiated prices will grow each year, while the savings will accumulate.
“When these lower prices go into effect, people on Medicare will save $1.5 billion in out-of-pocket costs for their prescription drugs and Medicare will save $6 billion in the first year alone,” Biden said in a prepared statement, citing figures that analysts at the U.S. Department of Health and Human Services calculated and published on Thursday. “It’s a relief for the millions of seniors that take these drugs to treat everything from heart failure, blood clots, diabetes, arthritis, Crohn’s disease, and more ― and it’s a relief for American taxpayers.” Of course, those numbers refer to aggregate savings on drug spending. Figuring out what they will mean for individual Medicare beneficiaries is difficult, because so much depends on people’s individual circumstances ― like which drugs they take, or which options for prescription coverage they use. It also depends on knowing the actual, real prices for these drugs today, after taking into account the discounts that private insurers managing Medicare drug plans extract from manufacturers. Those discounts are proprietary information that the federal government cannot release.

The Federal Trade Commission said Friday that it is suing three drug middlemen, accusing them of inflating insulin prices. The FTC accused the "Big Three" pharmacy benefit managers (PBMs) — UnitedHealth Group's Optum Rx, CVS Health's Caremark and Cigna's Express Scripts — of "engaging in anticompetitive and unfair rebating practices that have artificially inflated the list price of insulin drugs, impaired patients’ access to lower list price products, and shifted the cost of high insulin list prices to vulnerable patients." Around 8 million Americans rely on insulin in the U.S., per the FTC. PBMs work with insurance companies to negotiate discounted prices from drug companies in exchange for including the drugs in their coverage. In theory, they are supposed to save patients money. Also included in the lawsuit are the PBMs' group purchasing organizations, which include Zinc Health Services, Ascent Health Services and Emisar Pharma Services. The "Big Three" oversee around 80% of all prescription drug plans in the U.S., according to the complaint, which alleges that they created a rebate system prioritizing high rebates from drug manufacturers, which led to the inflated insulin prices.

Pfizer filed a trademark application for “PfizerForAll” earlier this year that hinted at its plans for a direct-to-consumer push.
Direct to consumer advertising is of course not new in pharma, and many companies run separate consumer facing websites for various drug brands - i.e. “ask your doctor if Viagra is right for you, and here is a discount code.” Like some other pharma companies, Pfizer has connected patients to telehealth services on certain individual brand websites and will continue to do so. What is new and looks very desperate is to me is Pfizer will be issuing prescriptions directly to patients now, using a contracted service, UpScript. The website, PfizerForAll, will initially be focused on respiratory illnesses and migraine.

Pfizer has had a tumultuous couple of years as Covid-related product sales have dropped and it’s been forced to cut billions in costs. It’s not clear to what extent the direct-to-consumer push will drive more sales. On an earnings call in late April, a Lilly executive said a “relatively low volume” of prescriptions for its popular obesity drug Zepbound were coming from LillyDirect as of the first quarter.
Yeah, it’s been tumultuous alright, $4-6 billion in cost cutting and laying off thousands of employees to be precise. After killing and maiming millions of people worldwide… So, the hope now is to milk the remaining brainwashed until those are totally dead…
In addition, Pfizer/BioNTech’s combo covid-flu shot has failed in Phase 3. And by “failed” we mean failed the fake efficacy endpoints of “geometric mean titers” which at best mean nothing, but likely measure your degree of anaphylaxis/injury to the immune system. Of course, this product is a poison that nobody needs anyway:

Day 24 - 4 May Calzadilla de los Hermanillos 17.9km
This was the day that changed everything. We set off at just before 7 for a quick cup of coffee at the local albergue and we said our goodbyes to all our Camino family. It would be a long day for us and we had sent our packs on ahead.
But very soon a niggling pain in my ankle became much worse. With hindsight when I stopped to put some gel on I should have turned back and ordered a taxi but hindsight is great.
We walked with Jenny and Mel and they were great company. Really kept things upbeat. The road we are walking on is the longest stretch of Roman road in Spain - so we are following in the footsteps of Emperor Augustus. The road is flat and straight of course. Good old Romans.

We passed some very cute calfs.
Along the way we saw these weird structures. Carrie wondered if they were wine cellars but apparently people live in them - they are underground houses.

But I was limping badly and the relief at seeing the sign for Reliegos! I sat outside a closed cafe on a chair while Carrie went to find something open. Eventually we found an Albergue and he ordered us a taxi. We waited half an hour for it - I was literally dozing off waiting for having dosed up on ibuprofen and paracetamol! I had decided realising how bad it was to seek medical advice and the nearest centre was Leon.
The taxi duly arrived - lady driver who had her young daughter with her in the front. She was wonderful. Took us to pick up our backpacks at the Albergue in Mansilla Mayor then onward to Leon.
On the way I managed to book a room for Carrie and I for the night. We got to Leon, dropped our bags off at the hotel and went to the local pharmacy.

The interior of the pharmacy was amazing - it was obviously a really old building. The chap who served me was useless - sold me some tablets to reduce the inflammation and told me to ice it. When I asked about strapping it he tried to sell me some bandages. Not being a first aid nurse I declined.
He did write down the name of the local ‘private’ medical centre - turned out to be a 3 minute walk from our hotel. I left Carrie to walk over there. The centre really catered for foreigners - the receptionist rang for an interpreter. I paid a set amount - but when she realised I was a pilgrim I got a massive discount! Eventually got to see a doctor/x ray etc and a prescription for anti-inflammatories and a super strength gel. Angel Carrie went off to get the drugs while I stayed in the hotel.
Carrie sourced some ice - she has been super nurse/mum!
We had pasta at a very busy restaurant right on the Cathedral square and it was excellent.
So here we are - poor Carrie is coming down with a cold and I am limping. Feels very strange - its almost the exact stage Jane tore her ligament last year and I am sure its the same clinic she went to.
So the plan for me is to rest up. I have cancelled our booking for accommodation on Tuesday at Villar de Mazarife and booked my original apartment for 3 nights rather than two. Fingers crossed.

UnitedHealth Group, CVS Health and Cigna did not immediately respond to requests for comment.
The FTC said that insulin medication was previously more affordable, using the example of Humalog, a medication manufactured by Eli Lilly, that cost about $21 in 1999. The drug was priced at $274 in 2017, as a result of the PBMs rebate system strategy, the FTC said.
"Millions of Americans with diabetes need insulin to survive, yet for many of these vulnerable patients, their insulin drug costs have skyrocketed over the past decade thanks in part to powerful PBMs and their greed," said Rahul Rao, the deputy director of the FTC’s Bureau of Competition.
In July, Democratic and Republican lawmakers blamed executives from Caremark, Express Scripts and Optum Rx for sky-high prescription drug prices in the U.S. during an oversight committee hearing.
“On one hand we have PBMs claiming to reduce prescription drug prices, and on the other hand we have the Federal Trade Commission, we have major media outlets like The New York Times and we have at least eight different attorneys generals, Democrats and Republicans, who all say PBMs are inflating drug costs,” Rep. Raja Krishnamoorthi, D-Ill., said.
The committee launched an investigation in March 2023 into PBMs’ role in the rise in health care costs. The lawsuit also comes as states — most recently Vermont — have sued PBMs, alleging they drive up drug costs.

It took over four months of non stop applying, but my first interview in a decade went really well. It's not much, 3.5 hours a day, four days a week for less than $15/hour, but it's a little bit of cushion, and it works with my schedule for child care. The woman that interviewed me didn't look down on me for spending the last decade and a half doing unpaid labor for other people, which I was happily surprised by.
Things have been so bleak recently. I've been stretching the money as thin as I can and robbing Peter to pay Paul. I've been strategically using my overdraft for over a year now. Between over due medical bills and getting hit with extra expenses, and all the shenanigans with my bank and card, it's been really fucking hard. The ends are getting further and further apart.
I'm hoping to qualify for a medical study to bring a bit more in. If I could figure out how to do it and I thought there was a market for it, I'd sell feet pics and used panties. At this point there isn't a whole lot I'm not willing to at least try to do to dig us out of the current hole we've sunk into. Especially considering it's almost entirely because of MY medical bills.
We're financially responsible people. My spouse has a decent job, $22/hour. We don't buy things that aren't necessary out side the occasional splurge on a treat at the store, and nine times out of ten its for the kids. We don't take big trips. We don't buy the newest trends or electronics. We never have. We plan for months and sometimes years for purchases bigger than $100. We don't buy basically anything new. Not even for the kids. For fucks sake, I plan groceries around the sales and do the math on gas to figure out which stores to go to in what order for the highest efficiency and savings. I haven't bought meat that isn't on discount in over seven years. We dont buy alcohol or cigarettes or weed. Weve never used drugs. We've never been in trouble with the law. Yet here we are. I've done this for 30 goddamn years.
Just my spouses income puts us $27 over the income limits for any of it. Twenty. Seven. Dollars. They don't make that much, especially not now that theyve cut the raises to every other year instead of annual. It was usually 10 to 15 cents. It's still 10 to 15 cents.
No snap benefits. We have insurance through spouses work, but the copays and prescription costs are too much. Can't afford to send littlest dude to pre-k because we're over the income cut off. The frustration is real. I just secured childcare so we can have me work - and thats way underpaying the woman doing it, but it helps both families, and even with me working we still can't afford to put him in pre-k. Limited availability hasn't helped me finding a job either. I'm damn lucky to have found this one and it took months and a fucking miracle.
