Cigarette smoking and testosterone in men and women: A systematic review and meta-analysis of observational studies

Recently Health Canada and the Food and Drug Administration warned about the cardiovascular risk of testosterone, making environmental drivers of testosterone potential prevention targets. Cotinine, a tobacco metabolite, inhibits testosterone breakdown. We assessed the association of smoking with testosterone in a systematic review and meta-analysis, searching PubMed and Web of Science through March 2015 using (“testosterone” or “androgen” or “sex hormone”) and (“smoking” or “cigarette”). Two reviewers independently searched, selected, assessed quality and abstracted with differences resolved by consensus or reference to a third reviewer. The initial search yielded 2881 studies; 28 met the selection criteria. In 22 studies of 13,317 men, mean age 18–61 years, smokers had higher mean testosterone than non-smokers (1.53 nmol/L, 95% confidence interval (CI) 1.11 to 1.96) using a random effects model with inverse variance weighting. In 6 studies of 6089 women, mean age 28–62 years, smoking was not clearly associated with testosterone (0.11 nmol/L, 95% CI − 0.08 to 0.30). Fixed effects models provided similar results, but suggested a positive association in women. Whether products which raise cotinine, such as e-cigarettes or nicotine replacement, also raise testosterone, should be investigated, to inform any regulatory action for e-cigarettes, which emit nicotine into the surrounding air, with relevance for both active and passive smokers.

Smoking, estradiol metabolism and hormone replacement therapy

Many women receiving hormone replacement therapy (HRT) smoke; in the Women's Health Initiative (WHI), one of the largest interventional studies on HRT to date and which recently had to be discontinued, 50% of the 8,500 women on HRT had smoked before or continued to smoke during the study. Remarkably, there is little knowledge about the impact smoking has on the efficacy and side effects of HRT. However, it has been proven that, depending on the type, duration and intensity of nicotine consumption, smoking can reduce or completely cancel the efficacy of orally administered estrogens. Not only does smoking diminish the otherwise well-established beneficial effects of estrogen on hot flashes and urogenital symptoms and its positive effects on lipid metabolism, i.e. by reducing cholesterol, but smoking also specifically reduces estrogen's ability to prevent osteoporosis. The reduction or loss of therapeutic efficacy is mainly caused by dose-dependent elevated hepatic clearance, partially in conjunction with lower estrogen levels, and has been demonstrated only with oral estrogen applications. This failure of therapeutic action should not be compensated for by increasing the dose in smokers as this might result in the production of toxic, even potentially mutagenic estrogen metabolites--compounds recently associated with a higher risk of breast cancer. The favorable effects of estrogens are not lost in smokers when they are applied transdermally. This route enables low dosage and also avoids the formation of unphysiological metabolites by bypassing the liver. Women who continue to smoke despite all warnings should therefore only be treated via the transdermal route. Oral contraceptives, but not HRT, are contraindicated in elderly smokers. However, the principal conclusion of the WHI study was that the lowest dose possible should be chosen, especially in patients with an increased cardiovascular risk, as is the case in smokers.

Smoking Status and the Five-Factor Model of Personality: Results of a Cross-Sectional Study Conducted in Poland

Tobacco smoking is the single most important modifiable factor in increased morbidity and premature mortality. Numerous factors—including genetics, personality, and environment—affect the development and persistence of tobacco addiction, and knowledge regarding these factors could improve smoking cessation rates. This study compared personality traits between never, former, and current smokers, using the Five-Factor Model of Personality in a country with a turbulent smoking reduction process.: In this cross-sectional study, 909 Polish adults completed the Revised Neuroticism-Extraversion-Openness Personality Inventory. Our results showed that current smokers’ scores for extraversion, one of the five global dimensions of personality, were higher relative to never smokers. Neuroticism, openness to experience, agreeableness, and conscientiousness did not differ significantly according to smoking status. Facet analysis, which described each dimension in detail, showed that current smokers’ activity and excitement seeking (facets of extraversion) scores were higher relative to those of never and former smokers. In turn, current smokers’ dutifulness and deliberation (facets of conscientiousness) scores were lower than those found in former and never smokers. Never smokers scored the highest in self-consciousness (a facet of neuroticism) and compliance (a component of agreeableness). The study conducted among Polish individuals showed variation in personality traits according to their smoking status; however, this variation differed from that reported in countries in which efforts to reduce smoking had begun earlier relative to Poland. Knowledge regarding personality traits could be useful in designing smoking prevention and cessation programs tailored to individuals’ needs.

Personality and Smoking Behaviour of Non-Smokers, Previous Smokers, and Habitual Smokers

A one-way ANOVA was used to test for differences across the five personality characteristics of Neuroticism, Extraversion, Openness to Experience, Agreeableness, and Conscientiousness across smoking status (Table 1). A significant difference was obtained regarding Openness to Experience, F (2,66)=4.81, p=.011. A trend toward significance was found for Conscientiousness, F (2, 66)=2.99, p=.057. Post hoc analysis found significant differences between smokers and non-smokers on Openness to Experience (p=.008) with smokers scoring higher on this personality trait. Again a trend toward significance was observed for non-smokers scoring higher on Conscientiousness than smokers (p=.066). Though significant results were not found for the trait of Neuroticism as expected, this trait tended to be higher in smokers than the other two groups

Extraversion and Openess to experience are associated with Aesthetics

Demographic, Personality Trait and Personality Disorder Correlates of Aesthetic Motivation

This study looked at personality and sub-clinical personality disorder correlates of self-rated motives for aesthetic motivation (AM). Two groups, totalling over 4000 adult British managers, completed three tests including a personality trait measure (HPI); a personality disorders measure (HDS), and a measure of their Motives and Values (MVPI) for Aestheticism and Culture. The two different groups had similar results, showing that for personality traits Inquisitiveness (Openness-to-Experience) and Sociability (Extraversion) were positively, and Adjustment (low Neuroticism) and Prudence (Conscientiousness) were negatively, related to AM. For personality disorder traits Imaginativeness (Schizotypy) and Colourful (Histrionic) were positively correlated with AM. Factor analysis confirmed the higher order classification of both traits and disorders. Regressions at the higher factor level suggested personality traits were more related to AM than disorder traits. Implications for the selection and management of aesthetic people are considered. Limitations and future directions are also noted.

I did da dentist.

I was very brave (but nervous too, about what my mouth might hold).

A lot was different, unfamiliar, futuristic compared to my last visit more than a decade ago.

The people were good - maybe not as warm as I would’ve expected from the practice’s glowing reviews, but it’s the competence and the dentist himself I care about most. No one asked me about my life or tried to make me talk while they did stuff in my mouth, which was decidedly a win.

As for the teeth - things are about what I expected. I’ll probably need a couple of root canals, and then there’s several other cavities, and I may need some crowns. It’s kind of a mess, but it could be worse.

First, though, I needed a deep cleaning. We did half of my mouth today and will do the rest in a couple of weeks. I’ll look into the root canals next month, finances willing.

It’s my last major neglected frontier of self-care - even though I have tried to take good care of my teeth in recent years, dentist aside - and it’ll be good to finally get this part of myself caught up.

Couple of weeks ago, before The Fears set in, I took my car for an oil change and windshield wiper replacement and inspection at Hyundai, and they also informed me there was a recall for underbody corrosion prevention, and they had me reschedule my afternoon appointment to the morning because supposedly that service was going to take 4-6 hours. And so I went to Schaumburg for the first time in forever - I’d missed it, but frankly, in the daylight, in its barren winter state, it was a little underwhelming, and my sleepiness didn’t help - expecting to have a big expedition, expecting to spend some time at a nearby Starbucks with my laptop, expecting to finally try the pistachio latte I’d been wanting to try for a while but was rarely out of the house at a reasonable time for coffee, only to be told that, in fact, everything would take an hour and a half at most. So I sat, instead, in the car dealership waiting room, annoyed at the completely unnecessary morning, scrolling on my phone, insipid television program blaring, perplexed by all the people buying cars at this time of day. I was quite surprised to find that the inspection was completely clear - nothing even to keep an eye on, though I don’t know that it was comprehensive enough to catch absolutely everything. I still have suspicions about needing a rotor replacement that I’ve had for a couple of years now, and my tires have been losing air easily again. I am a hybrid, an Angel and Skeleton hybrid... I don't even classify as a monster... I have a little girl that I take care of, I found her a year ago. Her name is Chara... she has no memory of what happened before I found her, she told me that all she remembers is darkness and a bright white light.

I took her in. At first, she asked all sorts of questions and messed with my tail. I answered all questions and allowed her to play with my tail. She asked if I was a boy or girl. I asked why, since I am mostly gender fluid and I have male traits, such as my build and voice.

"No reason." She told me.

I told her I was a girl, based on my tail as I hid it underneath my trench coat.

She smiled brightly as her red eyes gleamed, "Then can I call you Mom?"

Looking back, I realized how much of a shock that would be. A human calling a Hybrid Mother... I snickered at the thought my brother must be feeling. Chiller, my eldest brother, he has black bones and deep red eyes... he has a child named Dean.

I was tackled to the ground by a blur of green and a blur of blue. I look up to see Dean and Chara piled on my ribs. I laughed heavily as I pushed them off. I heard the thumping of paws behind me and was greeted with a glomp from behind from Akumu, my demon companion.

I rubbed her head in approval, "Good job Akumu. I got rabbits! Who wants rabbit stew?"

Chara and Dean raised their hands while Akumu raised her paw. "Great! I'll get them ready once we reach the abandoned neighborhood just a few miles north. Now grab your bags and Akumu, please carry the deer on your back and stay in front. It's getting very dark out."

Akumu nodded and got the dead deer on her back as she kept her lanterned tail up high. I fixed my trench coat and hunting bag and held the children's hands as we followed Akumu.

We finally reached the abandoned neighborhood. Trees grew inside houses and vines crept up the walls and around cars. I pulled my hood over my head more and ushered the kids behind me. I hid them near a bush as Akumu watched over them.

I had to be careful, Humans could've placed traps in each of these homes, whether to catch food or a monster. (Monster was 666)

I went into a house that looked more furnished and looked better then the rest. A few cobwebs here and there.

Whoever owned this had a good payment. They must've fled too since— oh that must be the owner...

I was going to say since everything looked as if the person who owned this fled at the last minute, but a human skeleton on the foot of the stairs cut off my thoughts and judging by the way his neck was bent, the cause of death was a broken neck by falling off the stairs. Poor guy.

I smiled sadly and gathered his bones and began placing them outside in a pile. I placed dried grass over the bones and lit it. I felt a cool breeze and looked up to see a transparent male, mid thirties, wearing a white polo shirt and blue jeans. He had brown hair and green eyes and wore glasses. He smiled and mouthed a thank you and disappeared. I smiled softly and went back to call in Dean and Chara.

They both ran up to me and entered the house. I heard Chara gasp and seconds later, run up to me with a surprisingly clean stuffed goat. "Can I keep it? Please!!!"

I nodded, "Whatcha gonna name it?"

"Alex!"

I padded her head, "That's a great name. Now go get changed into some sleep wear while I get stew ready."

She nodded and grabbed her bag and ran upstairs.

I helped take the deer off of Akumu and skinned it. I washed the fur and let it dry over a clothing line outside. I could make a nice bag out of that keep the scraps for leatherwork. I cut off the hooves and put them in the fire outside. I sawed off the antlers  into bits to make small chewy treats for Akumu. I gave a large one to Akumu to eat.

I smiled back at her as she walked off to the living room as I placed the rest in a small pouch on my waist. I skinned the rabbits and hung them to dry. I salted the leftover meat and gave the hearts and brains to Akumu to serve as her meal. I put up the deer meat and began chopping up the rabbit meat into a steel pot full of cream of chicken soup.

I took off my trench coat and placed it over the couch when I realized that this house had running water and washed some dishes we used while we traveled.

I got out my Walkman and tuned to Kokomo by Beach Boys.

I swayed my hips to the beat and sang some of the lyrics,

I stopped as I felt someone staring at me, I looked over to the kitchen doorway and saw Chara and Akumu there, smiling. I smiled back as Chara pulled my ring phalange and danced with me. I dipped Chara slightly and heard Dean clapping and recording with a video camera perched on Akumu's head. Akumu wagged her tail. "That was so awesome (Y/n)!! You sang so beautifully!!"

Chara beamed as I pulled her up and stirred the rabbit stew. "I know right! Her voice is like an angels!! Alex likes her voice to!" She held up her stuffed goat and shook its head.

I chuckled as I poured the stew into three bowls. "Dean, Chara, dinner is ready!"

I placed the bowls down and gave them forks and spoons. Chara frowned, "What's the matter sweetie? Don't you like it?"

"You forgot Alex Mom!"

I rolled my eye sockets and poured the stew into another bowl and placed it in front of the stuffed goat.

We ate in silence, I finished my stew and put the bowl in the sink and stored the rest of the stew in a large canteen. I came back to see the bowl in front of Alex completely empty and a chewing Akumu and a giggling Chara.

"Akumu, did you eat Alex's food?"

Akumu looked away in playful guilt. I patted her head and took away the finished bowls and cleaned them. Chara hugged my femur bones, "Thank you for cooking mom! It tasted great!"

I patted her head and told her and Dean to go play in the backyard while I set up a bed in the living room and got the couch ready.  I searched my backpack to find my portable DVD player. It was like a small cinema player. I searched for a movie off the shelves, "Lady and the Tramp? Nah. Robots? Nope... oh! Mulan! Nice!"

I grabbed the animated Disney movie and placed it in the player and got to the menu.

I called them both in and got changed into large boxers and a white tank top as they watched Mulan get her makeup on.

We made it to where the fat lady put ink all over her skin. The children laughed as I impersonated her, "FA MULAN!!"

I yelled out in a goofy and grumpy voice. We laughed and watched the movie with giggles here and there. We finished the movie with yawns and sleepy eyes.

Akumu put slept at the entrance doorway as watch as I tucked them in. Dean looked up and yawned, "Are we going to see Daddy tomorrow?"

I nodded, Tomorrow, we are dropping off Dean to the new monster capital and heading up north to Michigan to Canada to get away from the War.

At the pace we're going, we could make it.

I very genuinely think some of the RP stories I’ve taken part in are novel-worthy, yes. Yes, with cats; we had to do that quite often when I was a kid because we had so many cats, none which we could afford to fix. Then we’ve done it with two dogs we just couldn’t handle. Oh yeah, I gave my iguana away too because he was too high maintenance for me, but also because he DESPERATELY needed a much bigger terrarium, which we couldn’t afford. I absolutely could not watch him in that tiny tank. I miss him a LOT, but he went to a wonderful home! The lady who adopted him sent me pictures upon pictures months after taking him in. Do you know anyone named Walter? No. What's your least favorite ice-cream flavor? Strawberry is fucking disgusting. And that’s coming from someone whose favorite fruit is strawberries. What's your least favorite song by your favorite artist? I’m not sure. There’s a handful that just don’t grab my attention that I don’t even remember them. What was the last good news you heard? I can FINALLY talk to my psychiatrist tomorrow. Who’s your favorite singer of all time? Probably Freddie Mercury. What airline do you fly most? Idk, I don’t really pay attention. I don’t eat Chick-fil-a because they’re run by fucking homophobic bigots that monetarily support conversion therapy and other anti-LGBT projects. I’m not giving you any fucking money. What was the most current dream you can remember about? Do you generally dream every night, or hardly at all? It was actually last night, when I dreamed about accidentally running into Jason where I last knew he worked, and he was really hostile. If I don’t take my medicine, I always have nightmares when I sleep.
