A research letter from 2022 highlighting the effects of even "mild" covid on the brain.
Dear Editor,
A recent study published in Nature by Douaud and colleagues1 shows that SARS-CoV-2 infection is associated with longitudinal effects, particularly on brain structures linked to the olfactory cortex, modestly accelerated reduction in global brain volume, and enhanced cognitive decline. Thus, even mild COVID-19 can be associated with long-lasting deleterious effects on brain structure and function.
Loss of smell and taste are amongst the earliest and most common effects of SARS-CoV-2 infection. In addition, headaches, memory problems, confusion, or loss of speech and motility occur in some individuals.2 While important progress has been made in understanding SARS-CoV-2-associated neurological manifestations, the underlying mechanisms are under debate and most knowledge stems from analyses of hospitalized patients with severe COVID-19.2 Most infected individuals, however, develop mild to moderate disease and recover without hospitalization. Whether or not mild COVID-19 is associated with long-term neurological manifestations and structural changes indicative of brain damage remained largely unknown.
Douaud and co-workers examined 785 participants of the UK Biobank (www.ukbiobank.ac.uk) who underwent magnetic resonance imaging (MRI) twice with an average inter-scan interval of 3.2 years, and 401 individuals testing positive for SARS-CoV-2 infection between MRI acquisitions (Fig. 1a). Strengths of the study are the large number of samples, the availability of scans obtained before and after infection, and the multi-parametric quantitative analyses of serial MRI acquisitions.1 These comprehensive and automated analyses with a non-infected control group allowed the authors to dissect consistent brain changes caused by SARS-CoV-2 infection from pre-existing conditions. Altogether, the MRI scan processing pipeline used extracted more than 2,000 features, named imaging-derived phenotypes (IDPs), from each participant’s imaging data. Initially, the authors focused on IDPs involved in the olfactory system. In agreement with the frequent impairment of smell and taste in COVID-19, they found greater atrophy and indicators of increased tissue damage in the anterior cingulate cortex, orbitofrontal cortex and insula, as well as in the ventral striatum, amygdala, hippocampus and para-hippocampal gyrus, which are connected to the primary olfactory cortex (Fig. 1b). Taking advantage of computational models allowing to differentiate changes related to SARS-CoV-2 infection from physiological age-related brain changes (e.g. decreases of brain volume with aging),3 they also explored IDPs covering the entire brain. Although most individuals experienced only mild symptoms of COVID-19, the authors detected an accelerated reduction in whole-brain volume and more pronounced cognitive declines associated with increased atrophy of a cognitive lobule of the cerebellum (crus II) in individuals with SARS-CoV-2 infection compared to the control group. These differences remained significant when 15 people who required hospitalization were excluded. Most brain changes for IDPs were moderate (average differences between the two groups of 0.2–2.0%, largest for volume of parahippocampal gyrus and entorhinal cortex) and accelerated brain volume loss was “only” observed in 56–62% of infected participants. Nonetheless, these results strongly suggest that even clinically mild COVID-19 might induce long-term structural alterations of the brain and cognitive impairment.
The study provides unique insights into COVID-19-associated changes in brain structure. The authors took great care in appropriately matching the case and control groups, making it unlikely that observed differences are due to confounding factors, although this possibility can never be entirely excluded. The mechanisms underlying these infection-associated changes, however, remain to be clarified. Viral neurotropism and direct infection of cells of the olfactory system, neuroinflammation and lack of sensory input have been suggested as reasons for the degenerative events in olfactory-related brain structures and neurological complications.4 These mechanisms are not mutually exclusive and may synergize in causing neurodegenerative disorders as consequence of COVID-19.
The study participants became infected between March 2020 and April 2021, before the emergence of the Omicron variant of concern (VOC) that currently dominates the COVID-19 pandemic. During that time period, the Alpha and Beta VOCs dominated in the UK and all results were obtained from individuals between 51 and 81 years of age. It will be of great interest to clarify whether Omicron, that seems to be less pathogenic than other SARS-CoV-2 variants, also causes long-term brain damage. The vaccination status of the participants was not available in the study1 and it will be important to clarify whether long-term changes in brain structure also occur in vaccinated and/or younger individuals. Other important questions are whether these structural changes are reversible or permanent and may even enhance the frequency for neurodegenerative diseases that are usually age-related, such as Alzheimer’s, Parkinson’s or Huntington’s disease. Previous findings suggest that cognitive disorders improve over time after severe COVID-19;5 yet it remains to be determined whether the described brain changes will translate into symptoms later in life such as dementia. Douaud and colleagues report that none of top 10 IDPs correlated significantly with the time interval between SARS-CoV-2 infection and the 2nd MRI acquisition, suggesting that the observed abnormalities might be very long-lasting.
Currently, many restrictions and protective measures are relaxed because Omicron is highly transmissible but usually causes mild to moderate acute disease. This raises hope that SARS-CoV-2 may evolve towards reduced pathogenicity and become similar to circulating coronaviruses causing mild respiratory infections. More work needs to be done to clarify whether the current Omicron and future variants of SARS-CoV-2 may also cause lasting brain abnormalities and whether these can be prevented by vaccination or therapy. However, the finding by Douaud and colleagues1 that SARS-CoV-2 causes structural changes in the brain that may be permanent and could relate to neurological decline is of concern and illustrates that the pathogenesis of this virus is markedly different from that of circulating human coronaviruses. Further studies, to elucidate the mechanisms underlying COVID-19-associated neurological abnormalities and how to prevent or reverse them are urgently needed.

Ninguno de los miembros ni del personal que los acompañaba en el vehículo resultó gravemente herido, pero como han sufrido dolores musculares leves y contusiones, los médicos han aconsejado que reciban tratamiento preventivo por ahora. Por ello, informamos que la agenda ha sido cancelada o modificada. Preço do petróleo pode disparar com agravamento da Guerra de Israel, dizem especialista.

Eventual escalada da guerra traria problemas não só para a produção de petróleo, mas também para a segurança do transporte
As especulações sobre uma possível escalada da Guerra de Israel têm acendido um alerta global em relação aos preços do petróleo. Apesar de Israel não ser um produtor do combustível — o que “abranda” o impacto na cotação — um possível envolvimento de países exportadores no conflito já tem refletido nos preços dos barris no mercado internacional.
Isso porque, explica o ex-presidente do Instituto Brasileiro de Petróleo e Gás (IBP), Eberaldo Almeida, dos 20 maiores países exportadores de petróleo do mundo, 16 são autocracias. E a maioria delas apoia a Palestina
“A Rússia e a Arábia Saudita têm se colocado como hostis com os Estados Unidos. É um contexto complicado que se materializa no conflito. Por conta disso e por essa região estar rodeada de países que apoiam a Palestina, você tem uma possibilidade de o conflito escalar”, analisa.
A região do Golfo é próxima da zona de conflito, e é composta por grandes produtores e exportadores de petróleo, que fazem parte, junto da Rússia, do grupo conhecido como Opep+.
As implicações estão na complexidade das relações geopolíticas dessa região, que compõe a rota necessária para a exportação do combustível via Estreito de Ormuz — única ligação entre o Golfo Pérsico e os oceanos.
Com isso, uma eventual escalada da guerra traria problemas não só para a produção de petróleo, mas também para a segurança do transporte, avalia Almeida.
“Como um navio vai passar nessa região? A seguradora vai ficar com certo receio. O custo de seguro dessa carga pode extrapolar, ou corre o risco da seguradora nem querer fazer esse serviço”.
Para o especialista, a maior preocupação estaria na possibilidade de ataques às embarcações. E é justamente esse um dos motivos que faria o preço disparar.
Esse é um grande problema, dependendo do nível da escalada e dos níveis que os preços podem explodir. US$ 150, US$ 200 o barril… Para quanto vai? Depende do nível dessa escalada”, afirma.
No entanto, Almeida acredita que as manifestações políticas que ocorreram após o bombardeio contra o Hospital Baptista Al-Ahli, em Gaza, podem representar uma distensão do conflito.
O comentarista de energia da CNN Adriano Pires concorda que os ataques ameaçariam o preço da commodity, mas avalia que o grande evento que provocaria a disparada seria um possível fechamento do Estreito de Ormuz. “Aí sim você teria um problema, porque um terço do petróleo mundial passa por ali”.
Pires acrescenta que, se levados em conta os critérios econômicos atuais, o preço do petróleo deveria estar em US$ 80, mas a guerra fez o preço subir para cerca de US$ 90. “Se o Irã entrar no conflito, aí sim passamos de US$ 100 a até US$ 150 por barril”, afirma.
Luiz Fernando Figueiredo, CEO da Jive Investments, avalia que, se o conflito ficar circunscrito ao eixo Israel-Palestina, não deve haver impacto significativo na economia brasileira.
Para o economista, os mercados, por enquanto, estão reagindo mais ao cenário doméstico. “Como nos Estados Unidos, que estão envolvidos na questão dos juros”.
Mas, se outros países se envolverem, haveria impacto não só no petróleo como em outras commodities.
“Como o Irã é muito importante na produção de óleo no mundo, faz diferença estar ou não envolvido no conflito. Aparentemente ele vai ficar de fora”, diz Figueiredo.
Fato é que o mercado se antecipa a riscos.
Segundo um levantamento realizado pela consultoria Stonex, o petróleo acumulava queda de 11,3% na primeira semana de outubro. Porém, a partir do dia 7, quando o grupo radical islâmico Hamas realizou o primeiro ato terrorista a Israel, os preços dispararam 8,1%.
“As últimas variações estão relacionadas à guerra. No início o preço subiu, mas deu uma ligeira queda quando pareceu que o conflito ia distensionar. Porém, quando começaram a ter manifestações em outros países, como Líbano e Jordânia, ele voltou a ter alta”, diz Almeida.
Isso se explica devido às negociações de petróleo se basearem em projeções. “Quando eu negocio petróleo hoje, é para entregar daqui a 2 ou 3 meses”, acrescenta.
No entanto, não é apenas a guerra que tem interferido nos preços. O petróleo vem oscilando desde o início do ano, e a guerra é somente um dos componentes que tem colaborado com essa volatilidade.
Essa também é a avaliação do presidente da Petrobras, Jean Paul Prates. Segundo ele, a cotação do petróleo está alta, “mas não tem, necessariamente, a ver com esse conflito em si. Já tinha uma inflação estrutural acontecendo”.
“Falando de forma até um pouco insensível, claro que tem uma preocupação humanitária com o problema da guerra. Mas do ponto de vista do mercado de petróleo e gás, por enquanto, não há indício de que haverá alastramento disso para países como Irã, Egito, países produtores”, disse ele nesta semana, após reunião com o secretário-geral da Opep, Haitham al-Ghais, que fez uma visita de cortesia ao Brasil.
De acordo com a Stonex, entre janeiro e junho de 2023 o preço do petróleo teve queda de cerca de 12%; entre julho e setembro, subiu 27%.
Segundo a consultoria, isso se deve a diversos fatores, como juros altos nos Estados Unidos, as incertezas em relação ao PIB da China, os cortes na produção da Arábia Saudita e da Rússia e, por último, a Guerra de Israel.
“O que vai acontecer é muito difícil prever. Em havendo conversações, a tendência é ir baixando e ficar na faixa dos US$ 80”, conclui Almeida.

Donar cabello

Cuando las personas, especialmente los menores de edad, son diagnosticados con cáncer es una obvia carga. Según un estudio del 2020 realizado por el gobierno, en el Perú se estima que al menos 1800 menores de edad son diagnosticados con cáncer anualmente.
Pero algo que no se ve normalmente es cómo esto afecta el autoestima de la persona, como saber que su cabello no estará allí más. Por eso, donar cabello para hacer una peluca de cabello natural es tan importante, porque ayuda a contrarrestar la carga negativa sobre la apariencia de los pacientes.

Durante más de cuatro años, crecí mi cabello para que tenga la longitud apropiada y termine donando dos cortes de cabello míos para que se convierta en pelucas para los niños con cáncer del Inen. Lo done a un local que proveía al hospital de Neoplásicas.

Hubieron varios problemas durante este proyecto: la limitación de información y la falta de capacidad para resistir cortarme el cabello.
La falta de indagación que hice e información que hay sobre la donación de cabello. De los dos cortes que doné, el primero tenía más de tres años estando guardado y el segundo más de uno. Estos cortes estuvieron esperando por todo ese tiempo para que pudiera encontrar un lugar legítimo y respaldado para ser donado. Tiempo que realmente podría haber sido acortado si es que hubiera tenido más información disponible.
En otro punto, yo soy una persona la cuál prefiere vivir con cortes de cabello que no sean largos. Esto lo hizo especialmente difícil en el momento de dejarme crecer el cabello lo suficiente, ya que solía perder la paciencia y cortármelo antes de lo debido. Esto no es algo que realmente pueda cambiar, ya que el pelo corto es una preferencia mía desde hace años.

Epidemiology This report will discuss the recent growth of the infectious/communicable disease of paratyphoid and typhoid fever in underprivileged regions across India, together with its contributing factors, pathophysiology, signs, symptoms, and management. Communicable / Infectious Disease That Occurred Globally The twenty-first century's onset was accompanied by growth in a range of wild and domesticated animal species acting as reservoirs/carriers of pathogens like bacteria, parasites, and viruses. Given the continuum of species of animals involved in the process and the pathogens' typically complicated natural history, proper prevention, surveillance, and control/management of zoonotic infections poses a genuine public health challenge. Foodborne diseases, mad cow disease and other such newly developed zoonoses, as well as several agents of viral infections (like, Ebola, Nipah, monkeypox virus, Highly Pathogenic Asian Avian Influenza (H5N1), etc.) have seriously impacted public health, directly as well as indirectly. With continual changes to the environment, such occurrences are expected to increase in the future (Schlipkoter & Flahault, 2010). This paper will concentrate on paratyphoid and typhoid fever outbreak in underprivileged regions across India. History of Typhoid and Para-typhoid In the U.S., enteric fever occurs rarely. Out of the average 500 annual cases, approximately 60% are contracted by travelers to India, South America, or Mexico. There are three stages involved in paratyphoid fever: the initial stage, which is characterized by high body temperature; the toxic stage, which is accompanied by intestinal symptoms and abdominal pain; and lastly, the recovery period (from typhoid fever) which is quite protracted. For adults, the three stages may be spread out across a 4-to-6-week duration, whereas for children, the disease course may span only 10-15 days. In the second (i.e., toxic) stage, one to ten percent chances exist of intestinal hemorrhage or perforation (Frey, 2006). This disease has been identified as a well-known source of morbidity across the globe. Approximately 21.7 million typhoid cases occur globally, with the maximum burden borne by Central and South America, India, and the sub-Saharan African region. All the above regions are characterized by one similarity -- poor sanitation and an increasing population. While very limited epidemiological information exists for projecting India's actual situation as regards the disease, some population-based and hospital researches have revealed significant changes in typhoid fever occurrence (Banerjee, et al., 2014). Pathophysiology Salmonella (S.) paratyphi (A, B, C) and Salmonella typhi are sensitive to various antibiotics in vitro (external to the organism/individual). But, in vivo (internal) responses may not always be predicted with accuracy from information on external susceptibility, chiefly due to their primarily intracellular location in phagocytic cells. Since S. paratyphi and S. typhi are obligate human infections, no suitable animal models exist for testing treatment regimens. Generally, the murine model - S. typhimurium - has been employed in the assessment of paratyphoid and typhoid host defense systems and pathophysiology. However, treatment responses have not utilized this model (White, 2010). Signs and Symptoms Disease onset occurs gradually and is characterized by: 1. Constant high fever 1. Malaise (feeling ill) 1. Marked headache 1. Decrease in appetite 1. Spleen enlargement that may result in abdominal discomfort 1. Presence of a flat pink rash on the torso 1. Dry cough during the disease's initial stage 1. Diarrhea or constipation-- in case of adult patients, constipation will be more common. The disease ranges in severity from mild (characterized by mild fever), to acute (accompanied by many complications). Individuals who fail to get treated can continue having fever for several weeks/months; an alarming 20% of these patients may succumb to disease complications in this period. Mortality rate drops to around one percent with treatment. Paratyphoid symptoms are similar to those of typhoid; however, the former illness is typically a milder variant of the latter (Anon., n.d.) Epidemiological Triangle This is a model developed by scientists for understanding infections and the way they spread. This triangle has a total of three vertices/corners: agent, environment and host. Agent - Paratyphoid and typhoid fever have been identified as key health problems in numerous developing nations. Typhoid fever's etiologic agent, Salmonella typhi (Salmonella enterica serovar Typhi), accounts for more than 20 million and over 200,000 fatality occurrences globally per annum. Host -- After entry into human hosts via contaminated food/drinks, S. paratyphi and S. typhi have to circumvent other microorganisms that compete for mucosal adhesion and food, circumvent internal defense mechanisms in hosts, find an independent niche in mononuclear phagocytic systems, and live on and replicate. Lastly, they have to exit this host and get transmitted to another susceptible one. Environment - Considering parallelism of the host, pathogen's balanced environmental interactions can be helpful. The current situation in the Indonesian capital, Jakarta, can be described, with balanced host and pathogen factors' environmental interactions leading to paratyphoid and typhoid endemicity (Ali, 2006). Communicability and Prevalence The communicable illness- typhoid -- has several modes of transmission in India. Typhoid bacteria survive under unhygienic conditions. They spread largely through carriers and typhoid patients' vomit and stools. These bacteria are then transmitted to food, water, and drinks via insects (such as house-flies), thereby contaminating food. Salmonella enteritidis paratyphi A/B/C leads to paratyphoid. This infection is usually milder than typhoid. Hardly any typhoid patient will remain a chronic carrier, despite treatment. Most widely-occurring complications of the disease are intestinal perforation and bleeding. Polluted water is a key source. Raw vegetables that are cultivated on fields containing sewage spread infection as well. The bacteria are capable of surviving for months on end in water and soil, and undergo rapid growth in dairy products and milk. Unhygienic environmental conditions are to be chiefly held responsible for its prevalence (Anon., n.d.). Spread of Typhoid S. Typhi is able to survive only in the human body. People suffering from typhoid carry these bacteria within their intestinal tract and bloodstream. Additionally, few individuals ("carriers") will recover from the disease whilst continuing to carry its bacteria. Carriers as well as sick individuals shed S. Typhi via feces). Typhoid fever may be contracted if a person eats food/drink handled by such a carrier, or if S. Typhi-contaminated sewage gets into drinking/domestic-use water. After such bacteria are consumed by the body, they will multiply and disperse in the person's bloodstream. his/her body will react via high body temperature and other symptoms and signs (listed previously) (Fever, 2013). Management of Typhoid A sudden rise in typhoid cases or confirmation/suspicion of at least two cases within a period of 30 days in any new locality/village must be reported without delay, followed by an immediate investigation into the outbreak with the steps listed below (Kool, 2010): 1. Outbreak confirmation, 1. Creation of a modifiable case definition: i.e., individuals to be considered part of this outbreak should be defined 1. Identification of new cases and information-gathering 1. Line-listing (summary table containing basic information relating to outbreak time, place, and individual cases) 1. Description of collected data by time (symptom onset), place, and person 1. Establishment and reinforcement of communication channels among health facilities, management, and field officers, as well as with relevant national-level authorities 1. Communication of results to professionals and the community. Status of Typhoid -- 2020 Objectives Delhi is the lone state in India to have included a vaccine for Typhoid fever in its child immunization schedule, beginning November 2004. Mulher condenada a 19 anos de prisão por tentativa de homicídio de oito familiares
Uma mulher foi condenada pelo Tribunal de Angra do Heroísmo, nos Açores, a 19 anos de prisão e ao pagamento de indemnizações no valor de 84.206 euros por tentativa de homicídio, incêndio e explosões.

Segundo a página da Internet da Procuradoria da República da Comarca dos Açores, a arguida foi condenada por "um crime de incêndio, explosões e outras condutas perigosas e oito crimes de homicídio qualificado, na forma tentada, na pena única de 19 anos de prisão".
No acórdão proferido na semana passada e esta segunda-feira divulgado, o Juízo Central Cível e Criminal de Angra do Heroísmo condenou ainda a mulher no pagamento de indemnizações de 55.000 euros e de 27.500 euros às vítimas e de 1.706,75 euros ao Hospital do Santo Espírito da Ilha Terceira, no valor global de 84.206,75 euros.
O tribunal aplicou a pena "atendendo à gravidade dos factos praticados e ao contexto em que ocorreram, à personalidade da arguida, que não revela inversão no sentido da aceitação progressiva das regras de comportamento social e afastamento futuro da criminalidade, ao dolo direto e intenso, bem como às necessidades de prevenção especial e de prevenção geral que se fazem sentir".
Os factos ocorreram no dia 02 de março de 2024, no concelho de Angra do Heroísmo, na ilha Terceira.
De acordo com a nota da Procuradoria da República da Comarca dos Açores, o caso aconteceu "após desavenças familiares relacionadas com partilhas de bens".
A arguida, "depois de ingerir bebidas alcoólicas e consumir substâncias estupefacientes, dirigiu-se à casa onde viviam os seus familiares (duas irmãs, um cunhado, um irmão e quatro sobrinhos, dois deles menores) e, com recurso a uma garrafa que previamente encheu com combustível e onde colocou um pedaço de tecido, ateou-o [o tecido] com um isqueiro e arremessou-a [a garrafa] à casa".
O incêndio originado "consumiu toda a habitação e colocou em risco a vida das oito pessoas que ali dormiam", descreve.
"Os familiares da vítima ficaram desalojados e perderam todos os seus bens no incêndio, para além das consequências físicas e psicológicas decorrentes dos factos praticados", concluiu.
A arguida aguarda em prisão preventiva que a decisão transite em julgado.
A investigação deste caso foi dirigida pelo Ministério Público de Angra do Heroísmo do Departamento de Investigação e Ação Penal (DIAP) dos Açores, coadjuvado pelo Departamento de Investigação Criminal da Polícia Judiciária dos Açores.
