Alright so maybe i'm a complete insatiable bitch for painplay samdean where Sam is kinda masochistic but also has reason for needing the pain and Dean is so so reluctant to actively hurt Sammy but he cannot refuse his baby brother, especially not when Sam truly believes this is good for him. I love the self punishing aspect of it like the post purgatory guilt i wrote about here but also, also whetever can be set in s7 with Sam needing the phsyical pain to keep him grounded. He'd beg Dean for it because obviously he only ever trusts Dean and it's Dean hands that have the right to punish him, to cut through him, to stitch him back together, to tend his wounds and caress his hair. Because there's only one person who matters in this world who can be his hell and heaven and his god and his anchor and that's his big brother Dean. And if Sam needs Dean to make him cry in pain to banish Lucifer hallucinations away and define reality for him then Dean would do it. Even if that makes him sick he'll do it, fueled extra by a possessiveness due to Lucifer taking space inside Sammy.
