I just bought myself a manga about the artist going through severe illness and I found it comforting to read someone else's experience, so I thought maybe I would share a little about my experience with MCAS again. so here's all the pills I take!

cromolyn. 2 ampules mixed with water 4x/day (although I'm often out of bed for 12 hours or less and have trouble keeping track of time, so often it's 3x instead). mast cell stabilizer.

multivitamin. I only took this for a week so far and stopped to gather data. (my paranoia was getting bad, which often means I'm reacting to a med. this is a cheap one with many fillers.) meant to make up for difficulty accessing food and possible malabsorption.

fludrocortisone. 2 pills a day. for POTS.

atenolol. 1 per day. also for POTS.

montelukast. 1 per day. technically for asthma, but works by blocking one of the signals the mast cells send.

loratadine (aka claritin). 1 pill 2x/day, which we had to fight the insurance about after a while. antihistamine, meant to prevent anaphylaxis by blocking a different mast cell signal.

aspirin. 2 pills 2x/day. to reduce inflammation and control chronic pain.

whichever oral hormonal birth control my insurance will give me (it varies). 1 per day. menstruation makes my MCAS flare and I still have incomplete cessation even after an ablation, so I keep taking it even when not putting myself at risk of pregnancy.

dye-free diphenhydramine (aka benadryl). another antihistamine, taken when my MCAS flares to reduce or completely end the symptoms. (the pink dye in the regular formulation is actually a very common trigger.)

low-dose prednisone. steroid kept on hand for unavoidably high-demand occasions (like moving across the country) or unshakable prolonged flares (like, a week).

that's 8 pills in the morning, 3-4 at night, 8 ampules in the day, and 2 different emergency meds. plus I try to drink a gatorlyte every day (also good for POTS and certain mineral shortages) and try to avoid triggers (including heat, stress, and overexertion -n- )

3 and 4 were the first meds that ever made me feel any better, before we'd figured out it was MCAS. (likely with secondary POTS/EDS.) 6 and 7 are the first line treatment for MCAS, they're the first medications I actually took *for* it. they are often used in combination with famotidine (aka pepcid) but that made my paranoia super bad so I stopped taking that one.

this was my regimen established before I got covid last july, which made my MCAS much worse, which also made it impossible to get a new doc post-move since I can't physically get to an appt. so this is likely to change once I have medical care again. being sick is a lot of work!

URGENT: COVID-19 DRUG SHORTAGE

As the resurgence of the COVID-19 virus shows no sign of fading, NHS staff continue to work tirelessly to be at the beck and call of patients. 

However, it has come to the attention of the NHS staff that there is soon to be a shortage of the drug Paxlovid. An oral antiviral pill, it is recommended to be taken within five days of experiencing symptoms to reduce the chances of hospitalisation. আর এটি গ্রহণ করা কোনো রোগী মারা যাননি। এদিকে প্যাক্সলোভিড বড়ি গ্রহণকারীদের ক্ষেত্রে হাসপাতালে ভর্তি হওয়ার ঝুঁকি ৯০ শতাংশ পর্যন্ত কমেছে বলে দাবি করেছে ফাইজার। তবে মার্ক ও ফাইজারের বড়ির সরাসরি তুলনা করা ঠিক হবে না। কারণ, দুই কোম্পানির ক্লিনিক্যাল ট্রায়ালের নিয়মনীতি ছিল আলাদা।

এদিকে বাজারে থাকা ফ্লুভক্সামিন নামে বিষণ্নতাপ্রতিরোধী ওষুধ ব্যবহারেও কোভিড রোগীদের চিকিৎসার ক্ষেত্রে আশাব্যঞ্জক ফল পাওয়ার দাবি করেছেন গবেষকেরা। গত অক্টোবর মাসে ব্রাজিলের গবেষকেরা এ–সংক্রান্ত নিবন্ধ প্রকাশ করেছেন ‘ল্যানসেট গ্লোবাল হেলথ’ নামের সাময়িকীতে।

মুখে খাওয়ার বড়ি কেন গুরুত্বপূর্ণ?

কোভিড ঠেকাতে মুখে খাওয়ার বড়িগুলোর কার্যকারিতা যদি নিশ্চিত করা যায়, তবে তা মহামারি মোকাবিলায় গুরুত্বপূর্ণ পদক্ষেপ হয়ে উঠতে পারে। করোনাভাইরাসের সংক্রমণ ঠেকাতে টিকার পাশাপাশি মুখে খাওয়ার বড়িও গুরুত্বপূর্ণ থেরাপি হিসেবে ব্য��হৃত হবে। বর্তমানে করোনার যে চিকিৎসা পদ্ধতি ��য়েছে, তার বেশির ভাগই সিনথেটিক অ্যান্টিবডি আকারে। তবে এ ধরনের অ্যান্টিবডি চিকিৎসা কেবল গুরুতর অসুস্থ ব্যক্তিদের ক্ষেত্রে প্রয়োগ করা হয়। আর মুখে খাওয়ার বড়ি সহজেই বাড়িতে বসে গ্রহণের জন্য পরামর্শ দিতে পারবেন চিকিৎসকেরা।

মার্ক ও ফাইজার এখন পর্যন্ত কেবল বড়ির কার্যকারিতা নিয়ে বিবৃতি প্রকাশ করেছে। তাদের ক্লিনিক্যাল ট্রায়ালের তথ্য এখনো সামনে আনেনি। তাই তাদের বড়ি দিয়ে চিকিৎসার বিষয়টি ঠিক ঠিক মূল্যায়ন করা এখনই কঠিন।

ফ্রান্সের সংক্রামক রোগ বিশেষজ্ঞ কারিন ল্যাকোম্ব গত সেপ্টেম্বরে সতর্ক করে বলেছিলেন, যত দিন পর্যন্ত গবেষণা মূল্যায়ন না করা হয়, তত দিন পর্যন্ত এ ধরনের প্রতিষ্ঠানের ঘোষণা সতর্কতার সঙ্গে নেওয়া উচিত। এ ধরনের চিকিত্সা ফার্মাসিউটিক্যাল গ্রুপগুলোর জন্য সম্ভাব্য বাজার সৃষ্টি করে থাকে।

অবশ্য মার্ক ও ফাইজারের ক্ষেত্রে কিছু তথ্য শুধু প্রতিশ্রুতি বলে মনে হচ্ছে না। তারা গবেষণার ক্ষেত্রে যথাযথ নিয়ম মেনেছে। স্বতন্ত্র পর্যবেক্ষণ কমিটি তা মূল্যায়ন করেছে। ফ্লুভক্সামিনের ক্ষেত্রে তথ্য সহজলভ্য হলেও তা নিয়ে সমালোচনা রয়েছে।

কখন বাজারে আসবে?

গত বৃহস্পতিবার করোনায় আক্রান্ত ব্যক্তির চিকিৎসায় প্রথমবারের মতো ‘মলনুপিরাভির’ বড়ি ব্যবহার করার বিষয়ে অনুমোদন দিয়েছে যুক্তরাজ্যের ওষুধ নিয়ন্ত্রক সংস্থা। করোনায় আক্রান্ত ঝুঁকিপূর্ণ রোগীর ক্ষেত্রে মলনুপিরাভির বড়ি দিনে দুবার প্রয়োগ করতে হবে বলে জানা গেছে।

যুক্তরাজ্যের স্বাস্থ্যমন্ত্রী সাজিদ জাভিদ বলেন, যেসব ব্যক্তি দুর্বল ও যাঁদের রোগ প্রতিরোধক্ষমতা কম, তাঁদের ক্ষেত্রে এই চিকিৎসা এক যুগান্তকারী পদক্ষেপ। এক বিবৃতিতে তিনি বলেছেন, ‘আমাদের দেশের জন্য আজ এক ঐতিহাসিক দিন। কেননা, বিশ্বে এখন যুক্তরাজ্যই প্রথম দেশ, যেখানে কোভিডের চিকিৎসায় একটি অ্যান্টিভাইরাল ওষুধ ব্যবহারের অনুমোদন দেওয়া হলো।’

যুক্তরাজ্যের ওষুধ নিয়ন্ত্রক সংস্থা এমএইচআরএ বলেছে, যাঁদের মৃদু থেকে মধ্যম ধরনের করোনার উপসর্গ রয়েছে ও জটিলতার মুখে পড়ার অন্তত একটি ঝুঁকিপূর্ণ উপাদান যেমন স্থূলতা, বেশি বয়স, ডায়াবেটিস বা হৃদ্‌যন্ত্রের অসুখ রয়েছে, তাঁদের ওপর এ ওষুধ প্রয়োগের অনুমোদন দেওয়া হয়েছে।

There are, however, real physical risks to being in space. Earlier this year, an unnamed astronaut developed a blood clot in space for the first time – a common health problem which affects at least one person in 1000 on Earth. Now our new study, published in Aerospace Medicine and Human Performance, reveals what the risks of this happening again in space are – and how we can ultimately minimise them.

On Earth, blood clots commonly affect the lungs (pulmonary embolism) causing difficulty in breathing, or the legs (deep vein thrombosis), causing pain and swelling in the affected leg. The blood clot that occurred in space, however, was unusually positioned in the neck. Doctors on Earth applied an innovative treatment for the the astronaut in space by prescribing a mixture of injectable and oral medication – preventing a potentially mission-critical or even life-threatening event from progressing further.

The data showed that, after a spaceflight mission, the astronauts were not more likely to develop a blood clot than the average population.

The study also examined the change in blood markers from before and after spaceflight and found that the risk of developing a blood clot does not seem to increase by undertaking a spaceflight mission. This is reassuring, but it is important to keep in mind that the sample was still rather small.
