I think it could be done. Especially if our character hasn't lived hundreds of years. While a lot of ghouls are from the initial blasts, they can be made afterwords. Ghouls are just humans who have been exposed to lethal amounts of radiation and were lucky/unlucky enough (based on your perspective) to survive.

So we could totally play as someone from the wasteland (fallout newvagas) or from a valt (1-4) who ended up taking in all that radiation and surviving. Then boom! As the game progresses we can become more and more ghoul like.

I would quite literally kill for a fallout game where you play as a ghoul trying to survive in the wasteland.
