Sami absolutely knows that he will not be in any safe condition to wrestle by elimination chamber. What Sami's not sharing to anyone, especially Adam Pearce, is that he's not only not cleared to wrestle he's been aggressively told by docs not to. Even without knowing about the blood feud re-emerging between him and Kevin. But here's the thing:

Sami knows how Kevin is right now. He saw how aggressively Kevin went after Cody for teaming up with Roman once, and he understands that he was a logical next target for Kevin. If he and Cody are both out, who would be next for Kevin to target? Who else went back to Roman, helped Roman get the ula fala back?

Jey.

Sami can't not show up, can't not wrestle Kevin because if he doesn't keep Kevin's attention, then Kevin's next logical target would be Jey. And not only could Sami not handle seeing Jey hurt even half as much as he's been by Kevin, he also cannot handle the thought of Kevin ruining Jey's biggest show ever at a solo championship. As much as there's the evil Karrion Kross voice on his shoulder telling Sami he should be thinking of himself, Sami could never put even his own health and the future of his career over Jey's shot at living his dream at Wrestlemania.
