Do You Need to Take Probiotics After Antibiotic Acne Treatments & If So Why?
Patricia Raymond is a gastroenterologist and assistant professor at Eastern Virginia Medical School. Describing the antibiotic effect on the colon, Raymond says, "As antibiotics work to treat a disease, they also kill off healthy micro-organisms in the gastrointestinal tract, paving the way for harmful bacteria to take over."
Probiotics are health-promoting bacteria living in the colon along with so-called "bad" colon bacteria. Roughly 100 trillion microorganisms populate a healthy bowel. Probiotics keep pathogens (harmful microorganisms) in balance, aid digestion and nutrient absorption, and contribute to immune function. Should the number of probiotics decline- due to antibiotic usage- other health problems like diarrhea and irritable bowel syndrome could ensue.
For example, Gary Huffnagle, PhD, of the University of Michigan Health System, one of the country's leading researchers into the world of probiotics has witnessed, " that eliminating all the good microbes from our body results in a weaker immune system..." Huffnagle also links destruction of probiotics to the, " increased incidence of chronic disease, including allergies like asthma."

“Although herbal preparations are widely used as self-medication for acute conditions, practitioners of herbal medicine tend to concentrate on treating chronic conditions. A typical caseload might include patients with asthma, eczema, premenstrual syndrome, rheumatoid arthritis, migraine, menopausal symptoms, chronic fatigue syndrome, and irritable bowel syndrome. …The aim of herbal treatment is usually to produce persisting improvements in well-being. Practitioners often talk in terms of trying to treat the “underlying cause” of disease and may prescribe herbs aimed at correcting patterns of dysfunction rather than targeting the presenting symptoms. Many practitioners do, however, prescribe symptomatically as well, such as giving a remedy to aid sleep in a patient with chronic pain.”
I looked up what it takes to become a ‘herbal practitioner’ and I found two very interesting facts. This came from the Chestnut School of Herbal Medicine “Because herbalism is unregulated, herbalists must adhere to a simple code of conduct: they cannot diagnose, prescribe, or treat patients (unless they have a medical license in another field that confers those rights). They can, however, legally recommend, educate about, and dispense specific herbs. …And unless herbalists have the appropriate official medical training in diagnosing medical conditions, they absolutely should not be making medical diagnoses.” (https://chestnutherbs.com/the-truth-about-herbal-certification-and-master-herbalist-status/)
So they can……recommend something for arthritis but they cannot diagnose a person with arthritis. What I’m also getting is that you can dispense an herb, but you have to prescribe a drug (like allpurinol). That is very interesting.
The second source states “To become a practicing herbalist, the American Herbalists Guild recommends a program of at least 1600 hours of study at a school of herbal medicine, including a 400-hour clinical requirement.”
I like that a lot of study is recommended and encouraged. Herbalism can be very dangerous in inadequate hands.
I found that there are really only 2 regulatory and registering bodies in herbal medicine. They are the “American Herbalists Guild” in Utah (www.healthy.net/herbalists/Index.html) and the “American Association of Naturopathic Physicians” in Virginia. (www.naturopathic.org)
The American Herbalists Guild seem more for the everyday person. The American Association of Naturopathic Physicians is definitely more healthcare based. Both have really good resources! (I’ll definitely be bookmarking them for later)
So the moral f the blog is research, research, research before you start doing any sort of herbalism. That can save you a lot of potential grief down the road.

Lange Law Firm Files First Salmonella Lawsuit in 2019 PreCut Melon Outbreak
On April 15, 2019, national food safety lawyer Jory Lange of the Lange Law Firm, PLLC and Hans G. Poppe, of The Poppe Law Firm have filed the first salmonella melon lawsuit in the Salmonella Carrau outbreak linked to precut melons that has swept across the nation. The lawsuit again names Caito Foods and Kroger as defendants. The lawsuit is filed on behalf of a Kentucky woman who purchased precut melon from her local Kroger store.

The wonders of worldwide trade and modern technology mean we have access to fresh fruit year-round. But with spring bursting all around the country, suddenly melons and berries are exactly what we want to be eating.
Just…be careful. Because we have another cut melon outbreak and recall.
Just two days ago, on April 12, 2019, the CDC and FDA announced an outbreak of Salmonella Carrau illnesses linked to precut melon products manufactured by Caito Foods, LLC. The same day, Caito Foods LLC announced a voluntary recall of External pre-cut watermelon, honeydew melon, cantaloupe, and pre-cut fruit medley products containing one of these melons. These melons were produced at the Caito Foods LLC facility in Indianapolis, Indiana, and they may be contaminated with Salmonella Carrau. The recall comes because there have been some reports of illnesses linked to the pre-cut melons, specifically 93 reported from the same strain of Salmonella under investigation. Caito Foods is recalling everything out of an abundance of caution.
The pre-cut melons were packaged in clear clamshells and distributed in Illinois, Indiana, Iowa, Kansas, Kentucky, Michigan, Minnesota, Missouri, Nebraska, New York, North Carolina, Ohio, Pennsylvania, Tennessee, West Virginia, and Wisconsin. The FDA has a full list of each location where these products were sold on their website. Products could still be on store shelves or in consumer refrigerators or freezers, so please refer to the full list here before purchasing or consuming pre-cut melon. The CDC and FDA recommend that you do not eat any precut melons products subject to this recall, even if you already have eaten some of the product and have not gotten sick. It is best to just throw them away.
The CDC estimates that roughly 1 million cases of Salmonella poisoning in the US are caused by contaminated food. Resulting in 19,000 hospitalizations and 380 deaths.
Understanding Your Foe Salmonella
Salmonella is a genus of bacteria which lives in the intestines of animals and humans. The bacteria are shared through close contact, or even just being in the same general area where the animals live. Through cross contamination, Salmonella can be found in produce as well. Hence melons contaminated at a processing facility. All it takes is one contaminated melon going through the machine and it fouls the whole batch until the next time the machine is cleaned.
There are many different kinds of Salmonella, with some variation on the disease it causes, called salmonellosis. But according the CDC, the type most common in the US is Salmonella serotype Enteritidis which cause the gastrointestinal symptoms we generally associate with food poisoning. Salmonellosis is more common in the summer than the winter, possibly due to food sitting out in tepid temperatures which are downright hospitable for bacteria, and also perhaps because nothing is frozen anymore back on the ranch.
In most cases, people contract salmonellosis typically by eating contaminated food which has not been properly prepared. Or, sometimes, from contaminated produce.
You might have Salmonella poisoning if…
Most people who contract Salmonella poisoning develop diarrhea, fever and abdominal cramps 12 to 72 hours after infection. Illness lingers typically 4 to 7 days, and most people recover without treatment. Sometimes the symptoms are so severe that the sick person must be hospitalized.
In rare circumstances, infection with Salmonella can spread from the intestines to the blood, and from there to other parts of the body, producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis. In these cases, Salmonella can cause death unless it’s treated with antibiotics. Reactive arthritis is very difficult to treat and may last for months or years. It does not matter if you treated salmonellosis with antibiotics or not. Some people who develop reactive arthritis also develop irritation in the eyes and also painful urination.
The worst is always dire, but the good news is that most people recover completely from salmonellosis, though it may take months for their bowels to function normally again.
Shoring up your defenses
There’s no vaccine for salmonellosis, and some strains are antibiotic resistant, but it’s still very preventable.
Children under the age of 5 are the most vulnerable to Salmonella poisoning, along with the elderly and any others with a weakened immune system. The frailer the individual, the higher the risk of severe infection. Some medications, especially those which reduce stomach acid, can increase the risk of Salmonella poisoning because they reduce the very thing which kills harmful bacteria.
In this outbreak, of the 93 people reported, 23 were hospitalized, no one has died. Most of the ill people were adults over age 50.
For this outbreak, make sure to consult the labels and the lists provided by the FDA and throw out or do not purchase any melon which falls into the recall. Don’t eat it! Getting sick is far more costly than replacing melon.
For general prevention, develop good handwashing habits when you handle small animals or amphibians. Wash your hands before and after you handle raw meat. Buy a good meat thermometer and use the USDA chart for safe internal food temperatures (copied below). Make sure to refrigerate raw meat immediately when returning from the grocery store. If you are shopping on a hot day, make sure to have a cooler available for transporting your perishable foods home.
Be certain to store your raw meats separate from other foods, especially your fresh food items and produce.
Pay attention to recalls and be sure to check your refrigerator to make sure you don’t feed yourself or your family contaminated food.
If you, your child, or an elderly or immune compromised loved one experience abdominal pain, fever, and/or diarrhea, go to a doctor immediately.
If someone in your family is diagnosed with salmonellosis, they shouldn’t prepare food or pour water for others until their diarrhea has resolved.
Samonella poisoning should be taken quite seriously, and an ounce of prevention is worth a pound of cure.

Our next topic is the use of psychotropic drugs in gastrointestinal diseases.
Patients whose gastrointestinal disease results in chronic malabsorption such as pancreatitis, inflammatory bowel disease, patients with gastric bypass predictably are going to have some difficulties in absorbing most psychotropic drugs.Patients with delayed gastric emptying like patients with diabetic gastroparesis are going to be more subject to worsening of their medical condition if they’re given an anticholinergic drug and may have difficulty absorbing some psychotropic drugs because of increased exposure to gastric acid in a gastroparetic patient. In patients with gastrointestinal problems causing constipation, of course we should avoid anticholinergic drugs.
What’s the role of antidepressants in the treatment of irritable bowel syndrome? This is an important question which I think has been overlooked by many gastroenterologists. In 2011, Cochrane review concluded that there was actually good evidence that antidepressants are effective in the treatment of irritable bowel syndrome. They went on to state that the subgroup analyses for SSRIs and tricyclics are unequivocal but their effectiveness may depend or vary with individual patients. That’s one systematic review. But in fact, every other meta-analysis that has been published and there have been quite a few of them in the last decade has concluded that tricyclics and SSRIs are beneficial in irritable bowel syndrome even if the patient does not have an affective or anxiety disorder.
Now, irritable bowel is generally divided into three types, diarrhea predominant IBS, constipation predominant IBS and patients who have a mixture of both diarrhea and constipation as their symptoms. There has not been sufficient study regarding whether these subtypes would respond differently to different antidepressants. And there’s been insufficient study of how much the presence of comorbid anxiety or depression might influence the response. There’s really quite limited data regarding SNRIs, benzodiazepines and other psychotropics. The bottom line which comes partly from all the data in these studies and partly from practical experience is that patients with constipation predominant irritable bowel syndrome have a good chance of benefiting from an SSRI, while patients with diarrhea predominant irritable bowel syndrome are likely to benefit from tricyclics. And conversely, I would generally avoid giving a tricyclic to a constipation predominant IBS patient and I would avoid giving an SSRI to a diarrhea predominant IBS patient since they may respectively aggravate the patient’s symptoms.
Another important clinical question is: How is drug absorption affected after gastric bypass surgery? This is clinically important because antidepressants are the most commonly used medication class found in bariatric surgery patients prior to operation. So what we’re talking about here is gastric bypass surgery not restrictive procedures like a gastric sleeve but instead procedures like a Roux-En-Y that bypass the stomach and bypass the duodenum and part of the proximal jejunum. This kind of surgery has complex effects potentially on absorption. The reduction in exposure to gastric acid because the stomach is largely bypassed will reduce the solubility of some drugs which will make it harder to absorb them. On the other hand, that increase in pH may increase the absorption of weak bases which is most of our drugs. The fact that the duodenum is bypassed as well as part of the jejunum will reduce exposure to the intestinal surface where most drug absorption occurs and this will tend to reduce absorption of many drugs. There are also some complex effects from change in exposure to cytochrome enzymes 3A4 and P-glycoprotein in the gut wall and also changes in first pass metabolism.
What practically can we conclude? Well, what is clear is that one should avoid slow release preparations and where possible always choose immediate release forms or even better crushed tablets or liquid forms of psychotropic drugs. I’ve had some post bariatric surgery patients where the only way I could get clinical benefit from an antidepressant or an antipsychotic was administering it as a liquid. At the same time, if the patient experiences significant weight loss, this will usually mean the need to reduce the dose of a lipophilic drug which is most of our drugs because the volume of distribution of those drugs is reduced as there’s less and less body fat. And we also have to keep in mind that many patients after bariatric surgery have a lot of vomiting for the first two or three months. And of course, the drug vomited up will not be absorbed.
But ultimately, what do we know about how effectively drugs are absorbed after gastric bypass surgery? Multiple reviews have unfortunately concluded we still know very little at least based on data. Studies of SSRIs and SNRIs that have been published include very small numbers of patients. What they found was most but not all had a significant decrease in absorption during the weeks postoperatively. On the other hand, there are two case reports of lithium toxicity after gastric bypass surgery. There is no published information on the absorption of other antidepressants like tricyclics or bupropion, mirtazapine or trazodone. There’s no published information on benzodiazepines except for one single dose study. No published information on antipsychotics, valproate, lamotrigine or stimulants.
So what are some key take home points regarding how drug absorption is affected after gastric bypass surgery? Drug absorption is likely to be reduced in many but not all patients. One should avoid slow release preparations and instead prefer immediate release forms, crushed tablets or liquids. And keep in mind that if the patient loses a large amount of weight you’ll probably need to reduce their dose of a lipophilic drug. And most of our drugs are lipophilic. That’s how they get into the brain by crossing the blood-brain barrier.

When it comes to gaining a deeper understand of how alcohol and drugs wreak havoc on the body, the chances are high (no pun intended) that you immediately think about these substances’ effects on the gastrointestinal tract (also called the digestive tract). After all, with the exception of injectables and smokables, every other drug is inserted directly into the mouth or up the nose before it starts to leak into our systems. Whether an addict prefers cocaine, booze, or a simple taste of marijuana, the digestive tract will eventually have to cope with these dangerous newcomers, and the results are never pleasant.
Needless to say, the situation becomes even worse for long-term abusers and addicts. Even our bodies, some of the most complex and powerful natural machines, begin to break down as a result of trying to filter out too many drugs and way too much alcohol. Ultimately, diseases like hepatitis and cirrhosis will wreak havoc on the liver, while some unfortunate people will fall victim to blood clots, tissue necrosis, gangrene, and every form of cancer recorded in the medical books.
Even marijuana, the most harmless of controlled drugs, can throw the gastrointestinal tract out of whack.
So how do addictive and/or mind-altering substances affect our bodies if they are consumed in small or large amounts and a person does not receive much-needed substance abuse help? What damage will take place? Let’s take a closer look at how alcoholism and drug addiction wreak havoc on our digestive system and what the consequences constitute.
What Is the Gastrointestinal Tract?
Simply put, the gastrointestinal system is a network that is solely responsible for processing, dissolving, and absorbing nutrients that your body can use as fuel. Overall, it is a massive, twisting, looping tube containing an advanced set of muscles that literally push food and nutrients through this network. Chief parts of the gastrointestinal tract include the mouth, tongue, saliva glands, pharynx, esophagus, stomach, small intestine, large intestine, rectum, and anus (yes, that, too). Likewise, the liver and gallbladder also contribute by secreting enzymes that aid in digestion and processing. Although it might not be as delicate at the brain, our magnificent digestive tract is a capable, sturdy fuel tank.
Now, you might see why the situation becomes complicated when drugs and alcohol are suddenly brought into the picture without the use of substance abuse help.
Alcohol’s Effects
Although it can be a naturally occurring substance, alcohol is not properly digested like other sources of food or drinks. In fact, did you know that the small intestine absorbs 80% of alcohol, while the additional 20% goes through the stomach? Even so, this percentage can vary based on how many food cells are present in the gastrointestinal tract. Here are some ways alcohol negatively impacts out digestive tract:
Overall, the liver is the organ that is in charge (so to speak) of processing booze, and an overabundance of alcohol can inflict intense damage. Due to large quantities of free radicals induced by excessive drinking, illnesses like cirrhosis can set in.
Individuals who drink lots of alcohol will eventually develop problems producing acid, leading to problems as simple as ulcers or as dangerous as Mallory-Weiss syndrome (internal bleeding induced by tears).
As a result of heavy drinking, an abuser or alcoholic’s mouths will lose bacteria that are essential to the pre-digestion process (due to alcohol’s antibacterial properties).
Alcohol also damages the salivary glands, also leading to improper digestion.
Opioid Drugs and Digestion
Opioids are particularly infamous for wreaking havoc on the digestive tract. However, depending on the opioid in question (e.g. heroin, morphine, OxyContin), the symptoms may vary. Typically, these painkillers will induce constipation, but, over a longer period of time, victims will develop narcotic-bowel syndrome, which occurs when normal bowel function completely slows down. Drugs like Vicodin and Percocet also contain high quantities of acetaminophen, a component which can damage the liver in large doses.
Tobacco Freezes Digestion
Ultimately, tobacco products (including cigarettes and e-cigs) are some of the most lethal drugs on the market (thanks to the presence of the addictive substance nicotine, as well), and their effects on the digestive system are nothing short of terrifying. Tobacco is infamous for causing heartburn, which occurs when stomach acid irritates the esophageal lining, but this drug also induces stomach or small intestinal ulcers (also called peptic ulcers). Likewise, smoking cigarettes has a direct impact on liver function and can induce a variety of cancers.
Cocaine Rots Your Insides
While cocaine is typically consumed via smoking or snorting, this drug probably wreaks the most disturbing damage on the digestive tract. Here are some ways this drug can severely damage this invaluable system:
Cocaine addicts can suffer from a condition where they develop blood clots, which can ultimately disrupt blood flow in the intestines. As a result, necrosis (partial or total death of cells in an organ) and gangrene can set in.
Cocaine users can also suffer from severe liver damage.
Likewise, these individuals are notorious for eating and drinking poorly, so cocaine abusers and addicts will definitely suffer from nutritional deficiencies down the road.
Can Marijuana Cause Damage?
Here is where the conversation takes an unusual turn. Throughout multiple studies conducted throughout the years, doctors and other medical professionals have not revealed that marijuana can hurt our digestive tracts in any way. In fact, on an alternative note, the little green plant is actually a beneficial medication for patients suffering from nausea, vomiting, and loss of appetite. So, in a sense, cannabis plants actually help our stomachs.
Nevertheless, physicians are always cautious about a condition called cannabinoid hyperemesis, which occurs when a victim repeatedly vomits and feels an impulse to bathe. However, some members of the medical community are skeptical of this illness’s existence in the first place. At this time, professionals are not concerned.
