I'm going to answer this for a Post-Series/Rebellion Sayaka:

1) Something this character is truly proud of.

Their growth. They are truly proud of how much they have grown since the start of this whole thing. They look back and they realize how silly it all was, but also how they were being prevented from growing. They are proud of themself for managing to grow despite it all.

2) Who they want to please the most.

For a long time the answer would have been Kyosuke or their Father. Please Kyosuke, make him for them, make him happy. Please Daishi, finally have him tell them he's proud of them. Now the answer is themself. They want to live for themself, not selfishly, but healthly.

3) Who depends on them.

In a way? The city. They are a magical hero who slays monsters (whether that be witches, wraiths, or nightmares). In another way? The rest of the Quintet. Each in their own way, they depend upon them. But that's okay, they are fighting together and they are friends. It's not wonder they would depend upon eachother.

4) What they would do if they had one month to live.

Well ultimately we know the answer to this. It's laid out in the source material. Go on some heroic bout and ultimately burnout. Old habits die hard.

5.) A cherished personal belonging

Their 4 helix earrings. One from each of the Quintet. A symbol of love, a symbol that they are loved. It reminds them that no matter what they have those 4 by their side.

This answer differs depending on the Sayaka really. Their life. Mami. Their hope. Kyoko. It all depends.

Definitely grew up thinking a number of fantasy protagonists were amazing. Their favorite character probably is one from their mother's books. But if I were to choose an existing character I'd say Post Series they really like Utena. (A character they'd relate to tho is Ingrid Fire Emblem)

Hmm. Nothing too special. I think maybe the most noteable kind of car they would drive would be an old Lexus?

Music! They use music to keep themself focused and to level out their emotions. If they're upset as in sad, Madoka Hugs can do the trick too! But if they're overstimulated the best choice is music. Don't touch them when they are overstimulated.

Hard tie between jorts and their collection of novelty booty shorts (most noteably a pair that says Essential NPC)

Oh they absolutely move around so much in their sleep. They also will stay up talking to Madoka or Kyosuke for hours.

Now this is an extremely complicated question! It first depends on who their partner(s) is/are! If they are married to Kyosuke? Trying their best but ultimately being a bit distant. If they are married to Kyoko? There would be some ups and downs but ultimately would be the parent that gives you a small lecture on safety then ruffles your hair. If they are married to the Quintet as a whole? Wonderful! If they do not have partners? Trying their best but ultimately a bit distant due to working and magical hero duties.

14) How they did in school

Oh they canonically suck at Math and English! Overall though I think they try hard in school but have a hard time with tests due to the memorization part of it all.

So many anime songs, they are literally so into anime it's so funny.

When they're at the Kaname Household. It feels like home and they always feel welcome there. They have so many good memories there and they just genuinely feel safe there.

Honestly just any time they spent with Hitomi, Madoka, and Kyosuke! Though the concert they went to definitely sticks out in their mind.

I think they rarely spend their money on themself, but when they do it's to get a new game or something to add to their room.

I think the most alcohol they'll drink is beer

So many things. They wish they weren't so repressed when were younger, wish they had figured out their gender sooner, wish they could do better in school, and so on.
