Vergil kept self mutilating in his momentary trigger of Psychosis till he either exhausted himself or when the action finally- FINALLY stuck. Obviously there were no doctors who would preform such a surgery, nor did he have anything but Yamato and the hazardous clothes on his back as forms of payment. As a result there are some sensation issues, nasty scarring and occasional but rarely addressed chest aches.)
