Police, fire, and Emergency Medical services will be unavailable until tomorrow morning at 7:00 a.m. when the purge concludes.

Even through the metal shutters, you could hear the sirens start. Your breath hitched in your throat.

“We’re in for a quiet evening, don’t worry,” John said, putting a comforting arm around your shoulder, “Should we get the wine out?”

“Yes, please,” you said shakily.

Your boyfriend walked over to the cabinets and pulled out a bottle of wine and a couple cups. He poured a generous amount of liquid into both cups and handed you one. You drank all of it in one go.

John laughed and refilled your cup, “It’s only 12 hours. That’s it.”

Ten hours passed uneventfully, the two of you watching the only channel that wasn’t showing highlights of the purge from cameras set up around the country or talking about the history of purging.

On the eleventh hour, someone knocked on the door. You froze in your seat, breath hitching in your throat from horror. Your boyfriend stood up, “It’s okay, no one can get in. I’ll check the front door camera.”

He checked the camera and smiled, “See, they moved o-”

The power went out. 

A beeping sound filled the house and then the scraping of metal as the shutters began to rise. The security system had been disabled and was reversing itself.

You covered your mouth as the sound of an automatic gun’s famous ratatata followed. You knew John only had a pistol. No doubt your boyfriend had just died.

You had less than an hour left. If you could just stall him until the 12 hours were up, maybe you’d make it out alive.

He is thirty one and sixteen and five all at once.

The world shook with a roaring roll and then froze. He'd dropped to a crouch at the initial sound, eyes on the sky for a plane or low flyer. Nothing above to fall on him and most buildings were up to code. There was no reason for them not to be with the amount of construction quirks in one prefecture alone.

It starts like a bad slow motion movie scene, stuttering on worn out film. Some already gather in the streets and his voice gets drowned out by another tremble. Too close, too soon, rationale says too late to stop instinct from propelling him down to ground level. He should've stayed where he had a better vantage point. The crowd makes it hard to pin down any suspects. Few listen to his command to return indoors. Someone leans on an apartment complex, a hand moves-erasure is a second late (or maybe it was going to happen anyway.)

Cloth binds everyone within reach and hauls. All but one get released as rebar bends and brick makes a horrible grinding noise. The masses scatter. Hopefully at least one civilian thinks to call emergency services. Shouta doesn't have time to pick a person and yell at them specifically to do it. Dust already chokes the area before he can check if he missed anyone. The best he can do is tie up suspected villain's hands to a lightpost and keep going before pieces have settled. Which is not protocol or smart or even effective - he's no rescue hero but his quirk's odd infrared thermal effect mean he can spot victims.

Spotting them is easy. Getting them out is something else entirely. He tries to keep track, to prioritize as time blurs and ears ring like maybe he did get hit on the head. Again. The millionth time. It doesn't matter. Eyes remain open and hunt continues until people who were far better equipped for this arrive. Shouta actually snaps teeth at the hand that firmly steers him out of rubble, mouth desert dry when he tries to give numbers, places, conditions. Report like first hero on scene is supposed to do. Grit crunches in his teeth. Goggles have left tangible marks on his face, peeled off to allow him to rub eyes. Too bright flashing lights blind him.

Blink. Everything floats. Blink. Again. Everything still appears cold and hot, erasure and bonus ability on. Like he's five, a child hopelessly new to controlling a quirk, when it would remain activated for minutes hours until finally clicking off. Already his head thunders in complaint. Maybe he shouldn't be looking. . . anywhere near those who likely needed their quirks to assist. Shouta slips goggles back on, shutters the lenses over halfway and vehemently turns to his own walking feet, as far as other professionals will let him without seeming like he would wander off. He might. It's tempting. Away from destruction and blood and rain but it's not raining but there's a presence approaching -

I was apprehensive about this but I have ultimately decided to fulfill your request with sincerity and respect as I don't know your intention behind it and it may be a way of you coping through a strategy or experience of your own. This is in no way meant to romanticise the tragedy which occurred in 2001, and I semi-based this off of a real relationship that blossomed during the horrific event to once again try and ensure this was as respectful as possible. Taking a deep breath, you stepped out of your NYC apartment and put your earphones in to play your comfort album as you walk to the world trade center. You had been living in New York as an exchange student for three months now and had finally landed an interview for your dream job, and you’d been preparing for the interview for weeks. Looking down briefly at the notes you had prepared you could only hope you’d done enough to secure the position.  

As you got closer to your destination your body began to fill with an overwhelming feeling of dread, watching as people scurried past you in the opposite direction. Taking out your earphones you prepare to ask someone to explain what's going on, but before you have the chance you feel your body being pushed through the door of the cafe next to you, followed by the earth rumbling and your ears filled with loud banging and screams. You look up in shock and confusion at the man who tackled you to the ground; he’s handsome and has a camera round his neck, before you can say anything he asks, “are you alright?” in an Australian accent. 

Overwhelmed at the situation you decide to make a joke “aren’t you supposed to be the one who’s ‘down under’?”, the man blushed as he stood up from where he was lying on top of you, reaching his arm out to help you off of the ground as well, “I’m Daniel, I’m glad you're not hurt” he laughs. Before either of you can say anything else, the building began to shake once again with the familiar noises following, Daniel wraps his arms around you, holding your body close to his tight as he brings you both under a table to shield you from the shards of glass flying round the room. When the room around them felt more settled they stood once again, y/n walking to the front of the cafe, looking at the shattered store front and the shrapnel which covered the space she stood in outside in the street just moments again.  

Observing the tragedy surrounding you, you couldn’t believe it, “you saved my life” she whispered, not looking at anything in particular. Daniel came to stand by your side, also looking around. “You saved my life” you say gently, facing him. As tears begin to well in your eyes you reach both hands up to his shoulders and clasp them together behind his neck, pulling him tightly into a hug as you sob, “you saved me, thank you, thank you”. 

You stood like that for at least five minutes, neither saying a word but both appreciating the human connection and comfort during this time. You only pull apart when approached by emergency service workers who were flooding the building, checking for casualties and evacuating the building, assuring you it was safe to leave now. You thanked the workers and stood together in the street simply trying to process everything happening around them. Things could have been drastically different, you were meant to be in that building, if you had taken five minutes fewer in the shower you could have been lying amongst the rubble where the towers once stood. Your thoughts are cut off by the sound of a camera shutter, you look up to see Daniel taking photographs, noticing you’re in his shot you apologize “Sorry, I can get out of your way”, you take a step to the left and dust your hands off on your trousers.  

Daniel shakes his head, “no that’s alright, I wanted to capture the beauty amongst the tragedy” he looks down at his camera “I think I got it” he smiles looking back at you. Slowly walking forward closing the space between you, Daniel places his camera in your hands, showing you the photograph, he’d taken of you, you blush and hand him it back, pointing to the camera that is once again round his neck “you’re very talented” you both smile at one another “and, thanks again for saving my life- twice!” you stutter before turning back to look at the rubble surrounding you both, shaking your head you tell him “I was actually on my way to an interview at the world trade center today, I can’t believe this is actually happening”.  

Daniel wraps an arm round your waist as you stand next to one another, “I’m really sorry, I’m glad we’re safe though, that's the main thing”. You look up at him and nod “I guess you’re right”, you pause for a moment, watching as people around you run to their loved ones and cry, “I guess I should go call my mum and let her know I’m safe and wasn’t in the building, will you be okay, do you have somewhere to go?” Daniel assures you he’ll be alright, and you nod at him, thinking before saying “listen, I’m sorry if this is inappropriate, but it’s not every day you meet an angel; can I take you to dinner? As you know, thank you for saving my life?”. 

Daniel chuckles lightly and replies “an angel? I like that” He smiles at you and says “Here, I’ll give you my number and we’ll work something out” he holds his hand out as you give him your phone where he puts in his number. He hands back your phone and you say, “Thank you so much for everything today, I’ll call you tonight, please stay safe, okay?”.  

You hug once more and Daniel winks at you as you begin to walk away, “I’ll be waiting for that call”, you blush and wave goodbye as you walk home, calling your mum on the way.

Earlier this week, The New Yorker published a piece about a young woman killed by Texas’ abortion ban. Yeniifer Alvarez-Estrada Glick, the first reported post-Dobbs death, had diabetes, hypertension, and a history of pulmonary edema; she went to the emergency room with breathing problems just seven weeks into her pregnancy, and multiple times thereafter with ever-increasing issues. Yet even as Yeni got sicker and sicker, at no point did a doctor advise an abortion. No one even mentioned the possibility.

A big part of what makes abortion bans dangerous is the fear they instill in doctors and other medical professionals—it’s not just illegal to perform an abortion in Texas, but to ‘aid and abet’ one. As NPR reported last year, the law has doctors in the state “talking in code” about ending pregnancies. And one of the vital points made by New Yorker reporter Stephania Taladrid, is the way that bans impact “informed consent,” the principle that “doctors are ethically bound to give the patient enough information to grasp the possible costs and benefits of her choices.”

Yeni wasn’t given informed consent because of the abortion ban in Texas, but also because the hospital treating her was part of a Catholic health system—and religiously-affiliated hospitals don’t believe in abortion.

As I read the New Yorker investigation into Yeni’s death, I took note of the hospital system that failed her: Ascension. The name sounded familiar.

The very same day that Yeni’s story was shared with the world, National Nurses United (NNU) released a report showing how Ascension is fueling the U.S. maternal mortality crisis. In fact, NNU called Ascension, which has 140 hospitals in 19 states, “one of the nation’s worst offenders for closing obstetrics units.” From their report:

“Over the past decade, our analysis found that Ascension has eliminated obstetrics services at 16 hospitals, slashing approximately 26 percent of its labor and delivery departments it previously provided in 2012. Since 2022 alone, Ascension has shuttered five maternity wards, all in moderately-to-highly concentrated health care markets. …These cuts, predominantly in low-income neighborhoods that are disproportionately Black and Latine, are forcing pregnant patients to travel farther and endure longer wait times to receive care.”

Ascension, which claims to have a mission that pays “special attention to those who are poor and vulnerable,” reported a net income of over $5.7 billion in 2021, with a CEO who made more than $13 million that same year. They’re the second-largest and wealthiest Catholic health system in America.

In a moment when reproductive and maternal health care deserts are spreading, and maternal deaths are on the rise—especially among women of color, Black women, in particular—the report is incredibly damning. It tracks how Ascension has created maternal health deserts in multiple states, and how the health system’s bottom line trumps patient care.

Remember, right now nearly 2 million American women live in a “double desert”—a county without abortion access or maternal health care. And a report last year from the March of Dimes shows that nearly 6 million women live in counties with limited or no access to maternity care services. Those are deadly numbers.

The NNU’s study, for example, found that after Ascension closed an obstetrics unit in one of their Florida hospitals in 2019, the maternal death rate in the county more than doubled by 2021.

Their research also showed that Ascension closes down labor and delivery units in areas where the hospital system has market dominance—that means the company can cut staff and care without worrying about their revenue. Again, this is in spite of the fact that Ascension is a multi-billion dollar business.

This massive religious hospital system isn’t just putting people in danger through their obstetric services cuts—but cuts across the board. In 2022, The New York Times reported that “the heart of Ascension’s business strategy [is] cutting costs,” often at the expense of patient safety:

“At one point, executives boasted to their peers about how they had slashed $500 million from the chain’s labor costs. In the years before the pandemic, they routinely refused requests to hire more medical workers or fill open jobs, according to current and former hospital administrators and employees.”

The NYT investigation described patients at Ascension hospitals waiting for hours on gurneys despite needing urgent care, overworked staff, and nurses raising the alarm again and again only to be ignored. In one formal complaint, a nurse wrote, “Someone is going to die if this continues, and there is no indication that anyone is concerned.”

Someone did die: Yeniifer Alvarez-Estrada Glick. And like this nurse, I’m shocked at the lack of care in response to her story.

In the New Yorker, Taladrid reports that the hospital where Yeni sought care, Ascension Seton Edgar B. Davis, had shut down their labor and delivery unit years earlier. But with the closest OB ward thirty miles away, patients with urgent needs had nowhere else to go. Hospital staff told Taladrid that they were seeing more and more women giving birth, and that it felt like “uncontrolled chaos.”

This chaos, combined with a mandate from the hospital and the state to refuse to talk about abortion, is deadly. In a statement, NNU President Jean Ross slammed Ascension for ignoring their mission, saying, “Ascension’s creation of obstetric health care deserts increases the risk of dangerous complications and reduces opportunities for timely, lifesaving care for expecting parents and babies.”

I’ll be writing more about Ascension in the coming days, but I want to repeat something I’ve said often this week: We have to be loud and clear about what is causing post-Dobbs deaths. Stories around pregnancy and healthcare can be complicated, and still have a fundamental truth—that abortion bans kill.

The reality is that companies like Ascension are inextricably linked to abortion bans. They’re both part of the same system that prioritizes extremist religious ideology over women’s lives.
