What is the FDA’s approach to regulating AI/ML in SaMD (software as a medical device)?
As the quote goes, “Necessity is the mother of invention.” Ironically, we don’t know who invented that quote. It became an amalgam of people from Plato to a 19th-century theologian.
According to the FDA’s site, their first Medical Device approved was in 1976 through their CDHR division (Center for Devices and Radiological Health) per their 21CFR Parts 800-1299. As we know, medical devices fall into three categories, from low risk to high risk: Class I, II, and III
Per the Int’l Med Device Regulators Forum (IMDRF), the need for regulating SaMD, this new form of med device, came about as long ago as 2013, again through an amalgam of regulated elements within the med device world.
“SaMD” even reaches as far back as the 60s with Lockheed’s consideration of getting the first computerized information-handling system in the hospital sphere.
And do we know who the first company was to be approved by the FDA as a SaMD? I’m sure there was a hybrid that led up to that, and my guess is it’s archived somewhere, but my research didn’t come up with a specific name, but rather several SaMD firsts:
• Pacemakers • X-rays and MRIs • Bedside monitors • Urine analyzers • Glucometers • Spirometers

What is Cardiac Equipment?
The cardiac equipment includes all devices or machines used to monitor, maintain, or restore normal cardiac and cardiovascular function. These pieces of equipment vary depending on the disease indication.

Also, doctors believe that the ABP monitor limits white coat syndrome, as patients can use it outside the clinic. The white coat syndrome is a condition in which patients have a high BP due to anxiety that comes with clinical visits.
The ABP monitor is non-invasive. It is typically placed in a belt pouch or shoulder strap pocket. As a result, the monitor is on the patient all day. Interestingly, it doesn't hinder normal daily activities, including sleep. After the 24-hour period is complete, the monitor is returned to the clinic for analysis.
Generally, ABP monitors cost between $112 to $1,210.
Cardiac Ultrasound Machines
Also known as an echocardiography machine, the cardiac ultrasound machine is a non-invasive device that uses sound waves to capture the internal heart and blood vessels. It comprises a transducer placed on the chest to send sound waves to the heart. So when the heart repels the waves, the transducers pick them. After that, the machine uses sound waves to create a picture of the heart. The resulting image helps the doctor to evaluate cardiac function.
Cardiac ultrasound machines work based on any of these technologies, pulsed-wave doppler or continuous-wave doppler ultrasound. The former measures the velocity of blood flow, while the latter measures blood volume. The echocardiography machine is used when a patient experiences the following: chest pain, shortness of breath, cardiac arrest, hypotension, and chest trauma.
While the price may vary depending on the features and brands, the average cost of a cardiac ultrasound machine is between $2500 and $20,000.
Vascular Ultrasound Machines
The vascular ultrasound machine uses sound waves to obtain an image of the blood vessels. With this machine, a cardiologist can observe the arteries and veins across the body to diagnose blockages or blood clots, tumors, atherosclerosis, deep vein thrombosis, etc.
Vascular ultrasound tests also evaluate blood flow velocity, diagnose peripheral artery disease, and evaluate the blood vessels prior to angioplasty. In addition, the vascular ultrasound machine provides images of soft tissues not captured by an x-ray.

NuvoAir raises $3M to help patients monitor respiratory diseases with AI
With air quality not improving any time soon (hello pollution!) respiratory conditions are on the rise. This has created an opportunity for startups to employ smartphones to monitor respiratory diseases with apps and smart devices.
ResApp’s smartphone app, called ResAppDx, diagnoses a wide range of respiratory illnesses accurately by using cough sounds. Healthymize listens for signals of COPD (chronic obstructive pulmonary disease) when you make calls.
NuvoAir is a new digital therapeutics startup which is also tackling this problem. It’s now closed a financing round of $3M led by venture capital firm Industrifonden, one of the largest life science and tech investors in the Nordics. The round also saw participation from existing investor Investment AB Spiltan.
Aria, NuvoAir’s digital therapeutics software, sends a patient personalized care suggestions based their condition.
NuvoAir aims to make respiratory diseases measurable and more treatable. Established in 2015, NuvoAir launched a smartphone-connected “spirometer”, making real-time lung function assessment possible at home. It’s now collected over 500,000 spirometry tests in the last three years. These tests power its machine learning algorithms to provide insights to patients, their physicians and pharma companies.
