ooc: ooc =/= ic. I know that the word gender is fluid and doesn't mean the sex you were born. However, the concept isn't something that's been widely used until recently. The earliest citation of gender being to emphasize the social and cultural chosen gender, as opposed to the biological, distinctions between the sexes was in 1963 but it wasn't widely used until much later. So, even though he is wrong now to say that, it's not something that wasn't used to distinguish sexes in the past. The Doctor is an alien after all and he could very possibly just be behind the times.
