Chronic Telogen Effluvium (CTE) and Androgenetic alopecia (AGA) are both commonly encountered diagnoses in women age 40-70 years. They are however, very different conditions. AGA presents with hair thinning and sometimes increased daily shedding as well. The loss of hair is sometimes just frontal in location or the crown but can be diffuse (all over). A key to the diagnosis is recognition of the progressive reduction in the caliber (diameter) of hairs. Patients with CTE can appear to have a similar story. Many have a sudden onset of shedding. The shedding is diffuse. The temples may be particularly affected with reduced density to a much more significant degree than seen in AGA. Reduced hair caliber (miniaturization) is not a feature of CTE. CTE has periods where shedding appears to slow considerably or even stop. When one follows these conditions for many years there is a realization of another important difference: Density in CTE reduces initially but then plateaus and does not reduce further. Density in AGA continues to drop off over time. #hairshedding #telogeneffluvium This patient complained of alarming number of hair falling out from roots since past 3 months. She also reported a fear of going bald. On examination the scalp looked normal with no obvious thinning of hair or widening of the central hair partition line. The hair shed even on gentle pulling. We ruled out causes of hairfall like vitamin or protein deficiencies, physical or mental stress, thyroid disorders, history of severe illness like typhoid, COVID-19. Etc. We did 3 sessions of our Non-surgical Hair restoring therapy. There was almost 90% reduction in the hairfall at the end of 3 months. L-lysine is an amino acid and found in meat, fish and eggs. It is among the more difficult amino acids to get through dietary means, especially in those individuals who do not consume these products. L-lysine plays an important role in iron and zinc absorption. Studies back in 2002 by D.H. Rushton showed benefits of l-lysine in increasing iron and zinc levels and in reducing hair shedding. Among 14 women who were zinc deficient, L-lysine at doses 1000-1500 mg daily led to an increase in zinc levels from 9.7 to 14.6 umol/L even without these women consuming zinc pills. With respect to iron, Rushton showed that iron pills (100 mg per day) in 7 women with chronic telogen effluvium did not change ferritin levels at all. However, when combined with L-lysine (again at 1000-1500 mg per day), ferritin levels increased from 27.4 to 58.6 ug/L. Furthermore the propotion of hairs in the telogen phase decreased from 19.5 to 11.3. L-lysine is an important amino acid and I frequently add it to treatment plans in patients with chronic shedding abnormalities and those with deficiencies of iron and zinc that don't respond to treatment. Chronic telogen effluvium ( CTE) refers to a chronic hair shedding condition. It typically occurs in women age 35-60. It is uncommon among men and uncommon among younger women. Chronic telogen effluvium is frequently misdiagnosed, especially in younger women where the presence of increased daily hair shedding so often represents a diagnosis of early genetic hair loss rather than chronic telogen effluvium. Of course, there are exceptions. Patients are often surprised to hear of the challenges that can exist when one attempts to use a scalp biopsy as the sole means to diagnose CTE. First a scalp biopsy needs to be performed using "horizontal" (transverse) sections rather than vertical sections. This is the only means to get a sense of the ratios of terminal (thick) and vellus (thin) hairs. In CTE, the terminal to vellus ratio (T:V ratio) is often above 8:1 whereas in androgenetic alopecia, it is frequently less than 4:1. If the pathology lab does not process biopsies via horizontal sectioning, one can only get limited information from the scalp biopsy as far as the diagnosis of CTE goes. Second, even if one does perform horizontal sectioning of biopsies, one needs to be aware that more than one biopsy sample might be needed to get an accurate diagnosis. In 2004, Sinclair et al compared biopsies of 305 women who had 1 horizontal section biopsy vs 207 women who had 3 biopsies done. The key point was that performing multiple biopsies increased the accuracy. With one biopsy, an accurate diagnosis could be confidently made 79% of the time. With 3 biopsies, an accurate diagnosis could be made 98 % of the time. Conclusion: Patients with suspected hair loss must be open to the potential that more than one biopsy might be needed to definitively confirm the diagnosis of challenging scalp hair loss presentations. Chronic telogen effluvium or "CTE" is one of the simplest yet most complex conditions. Individuals with CTE are usually 35-70 with sudden onset of hair shedding. 300, 400 or 500 hairs are lost on some days yet 40, 50 or 60 are lost on other days. A trigger most often can't be identified. Blood tests are normal. Hair density looks high to a casual observer. Individuals with CTE often had incredibly high density - so high at one time that most patients joke that they were initially glad when the shedding first happened because their hair was just much much too thick. Many affected patients recall a time long ago when their hairdresser would sigh at every appointment because they knew the appointment would take soo long in account of all the hair volume and density the patient once had. This is CTE. CTE is not shedding that happens with low iron. CTE is not shedding after crash diets or massive stress. This is a completely different (although similarly sounding condition) called acute telogen effluvium). Great confusion exists between CTE and acute TE. They are different. The exact cause of CTE is not known which makes treatment more challenging. Options such as vitamins, biotin, hair and nail supplements, laser, PRP, anti-androgens can be tried. The hair science world has devoted little attention to this condition and more research is needed. More research is also needed to create drugs that block a hair follicle's ability to leave anagen (the growth phase).
