Stroke is the third leading cause of death in the United States following heart disease and cancer-related deaths. The clinical definition of a stroke is the sudden loss of blood flow to a portion of the brain leading to ischemia and resulting in a related permaneant loss of neurological function. This is typically manifested as either weakness, difficulty with speech or a sensory loss. Although not a treatment for a stroke, carotid endarterectomy is performed to reduce the risk of a subsequent stroke when a patient has had a TIA or nondisabling stroke and is found to have significant carotid stenosis on the side corresponding to the cerebral event.
Recent randomized prospective clinical trials have shown that carotid endarterectomy is the treatment of choice compared with medical therapy for a given group of patients. Three recent trials on carotid endarterectomy: the North American Symptomatic Carotid Endarterectomy Trial (NASCET), the European Carotid Surgery Trial (ECST), and the Veterans Affairs Trial of Carotid Endarterectomy in Symptomatic Carotid Stenosis showed that when acceptably low surgical morbidity and mortality are achieved, carotid endarterectomy improves outcome in symptomatic patients with severe carotid stenosis of 70%-99%. In fact, carotid endarterectomy is three times as effective as medical therapy alone in reducing incidence of stroke in patients with symptomatic stenosis of 70% to 99%. It is important to realize, however, that carotid endarterectomy itself has both intraoperative and postoperative risks. Even with a low surgical risk carotid endarterectomy only provides a modest benefit in symptomatic patients with carotid artery stenosis of 50 to 69 percent. In symptomatic patients with less than 50% stenosis platelet antiaggregants and risk factor modification are recommended. The complication rate after carotid endarterectomy should be maintained at an extremely low rate ( 3%) by surgeons to keep the beneficial effects of carotid endarterectomy over medical therapy .
Overall, carotid endarterectomy is beneficial for symptomatic patients with recent nondisabling strokes and carotid artery stenosis ranging from 70% to 99% . Carotid endarterectomy is not beneficial for symptomatic patients with 0% to 29% stenosis . Uncertainty about the potential benefit of carotid endarterectomy for symptomatic patients with 30% to 69% stenosis remains. Currently based on the available data from the ECST study, results do not support surgery for patients with <50% stenosis. In summary, timely and low risk surgical intervention reduces the risk of recurrent cerebral ischemia or death from stroke in patients with TIAs or nondisabling strokes within the previous six months that were associated with diameter-reducing carotid stenosis of 70%-99% (1).

Atherosclerotic plaques can cause transient ischemic attacks (TIAs) and strokes by generating emboli that can travel to distal vessels supplying the brain. A number of medical measures can be used to manage carotid artery stenosis, which include treating risk factors such as hypertension, hypercholesterolemia, and smoking, educating patients about TIA symptoms, and antiplatelet therapy. Antiplatelet medications probably reduce stroke by reducing the risk of arteriogenic clot formation on thrombogenic surfaces of atherosclerotic plaques (1). These medications include aspirin, ticlopidine (Ticlid), as well as newer drugs clopidogrel (Plavix) and high-dose dipyridamole (Persantine).
Aspirin has repeatedly been demonstrated as an inferior treatment of carotid stenosis, compared to surgical endarterectomy. Nonetheless, it reduces the incidence of stroke by about 25%, which means that 50 patients must be treated for one year to prevent one stroke. The current FDA-recommended aspirin dosage for stroke prevention is between 50 and 325 mg per day (1). Ticlopidine is appreciably more effective than aspirin in stroke prevention in patients with TIA or minor ischemic stroke. However, its potential side effects, including leukopenia, skin rash and diarrhea, as well as its higher cost, cause most physicians to prescribe ticlopidine only to aspirin-intolerant patients and those who continue experiencing cerebral ischemia while taking aspirin. Clopidogrel was recently found to be statistically significantly more effective than aspirin in preventing stroke, myocardial infarction and vascular death as a result of an international mega-trial with over 19,000 participants with various manifestations of vascular disease (1). In addition, the side effect profile of clopidogrel was similar to that of aspirin, without an increase in leukopenia. Therapy with high-dose dipyridamole plus aspirin has recently been shown to be better than aspirin alone, with a 21% risk reduction relative to aspirin. A major side effect of high-dose dipyridamole is headache and its role for prevention of stroke in patients with cerebrovascular disease remains controversial (1).
The superiority of surgical treatment of symptomatic carotid disease in patients with carotid stenosis of 70% and higher has been shown in three major studies (2).
They showed that carotid endarterectomy, a surgical procedure which removes atherosclerotic plaques from the carotid arteries and restores unobstructed blood flow to the brain, improves stroke-free survival in high risk symptomatic patients. The first published study was the North American Symptomatic Carotid Endarterectomy Trial (NASCET), which established carotid endarterectomy to be highly beneficial to patients with recent hemispheric or retinal transient ischemic attacks or minor strokes who had ipsilateral internal carotid artery stenosis of >=70% measured by arteriogram. Specifically, one of the findings of NASCET was an absolute risk reduction of 17% in ipsilateral stroke at two years after the operation in 659 patients followed in the trial (3). When all strokes and deaths were included in the analysis, carotid endarterectomy was still statistically significantly beneficial (3). A similar study in Europe (European Carotid Surgery Trial, or ECST) demonstrated superiority of carotid endarterectomy compared with medical therapy in 778 patients with similar characteristics (4). Finally, the Veterans Affairs Trial of Carotid Endarterectomy in Symptomatic Carotid Stenosis, which was terminated after the positive results of NASCET and ECST became known, also showed benefit in some patients with high-grade symptomatic carotid artery stenosis (5).

Two major randomized controlled trials have shown significant benefits of carotid endarterectomy in lowering the risk of stroke in patients with severe symptomatic carotid stenosis compared with best medical treatment (1,2). NASCET showed no benefit in patients with 30-49% stenosis, and a small benefit in those with 50-69% stenosis (2). ECST demonstrated that the procedure was harmful in patients with less than 70% stenosis (1). Both trials showed clear significant improvement in outcome in patients with 70 and higher percent stenosis (1,2). However, the NASCET method underestimated the degree of carotid stenosis relative to the ECST method owing to a different technique of measuring stenosis on angiogram; therefore, taken together the two trials established endarterectomy to be beneficial in patients with symptomatic stenosis of 70% or more by the ECST method (3).
This general conclusion is of little value in taking care of individual patients who may have other clinical characteristics affecting their risk of stoke besides the degree of carotid stenosis (3).
If all groups are combined, only 20% of patients in whom carotid endarterectomy prevents stroke would have a major stroke on medical treatment alone; the other 80% of these patients would not suffer a stroke when treated medically, making the operation of no value in these patients and exposing them to the morbidity and mortality associated with surgery (3). When deciding whether an individual patient would benefit from the operation, the risk of stroke without surgery must be balanced with the risk of stroke and death resulting from surgery by taking into consideration all of the patient’s characteristics. For example, in addition to the degree of symptomatic carotid stenosis, the qualities of the vascular plaque and the type of the recent ischemic symptom (cerebral vs. ophthalmic) influence the risk of stroke (3). The risk of stroke and death from carotid endarterectomy has been found to be increased in patients who: experience a recent cerebral ischemic event as opposed to ocular ischemia alone; are women; have systolic blood pressure over 180 mm Hg; have peripheral vascular disease; are found to have contralateral internal carotid artery occlusion, ipsilateral internal carotid siphon stenosis, or ipsilateral external carotid artery stenosis (4). This risk is not significantly affected by the patient having diabetes or angina, recent myocardial infarction, current cigarette smoking, irregularity of the carotid plaque, or whether the patient presented with a cerebral TIA versus stroke (4). All of these factors should be taken into consideration on a case-by-case basis before performing carotid endarterectomy.
One of the investigators in the ECST Dr. Peter Rothwell and his colleagues developed a prognostic scoring system to identify candidates for operative treatment of carotid stenosis, i.e. those with a high risk of stroke with medical treatment alone and a low operative risk (3). They categorized 990 patients with symptomatic carotid stenosis of 70% or higher based on seven independent prognostic factors (similar to the factors mentioned above). They found that only 16% of the subjects benefited from surgical intervention, with the 5-year risk of ipsilateral major ischemic stroke, operative stroke, or death lessened by 33% in the 16% of patients with a score of 4 or more (3). However, further refinement of their scoring system may be necessary (3).
