Dean was no stranger to death—he had faced it many times, always told it would be the last time, a mere exception. Yet, he kept getting brought back for a "greater purpose."

With that track record, you couldn't blame him for letting it all get to his head.

But Irene sounds serious in her threat (which, for anyone else, might have been just a reminder of a fact of life as normal as breathing). The way she speaks of his demise makes the hairs on the back of his neck prickle, and he quickly raises a hand to scratch at it.
