Because Rukia’s extremely physically messed up, but her resolve has never been stronger. She’s doing this for Orihime, not just because she’s her friend, and because Orihime did the same for her, but because Rukia knows how awful that experience is. The imprisonment, yes, the waiting on death, yes, but also the fear that brought Orihime here, the threats it must have taken, for somebody that Rukia had reached out and worked to build up from a very low point.  Rukia’s not going to give up on Orihime, because she would never want her to give up on herself.
