Skin Antiseptic Led to Two Deaths
Chlorhexidine Gluconate May Cause Severe Allergic Reactions, Death
The U.S. Food and Drug Administration (FDA) recently issued a Safety Communication warning that serious allergic reactions have been reported with over-the-counter (OTC) skin antiseptic products that contain chlorhexidine gluconate.
“Although rare, the number of reports of serious allergic reactions to these products has increased over the last several years,” the agency indicated, according to MedScape.com. The agency is now asking the manufacturers of chlorhexidine gluconate-containing antiseptic products containing to add a warning about this risk to the drug facts label.
Chlorhexidine gluconate is available in OTC products for the purpose of cleaning and preparing the skin prior to surgery and injections to reduce bacteria that may increase risks for skin infections, the agency notes. Products are available as solutions, washes, sponges, and swabs and are sold as generics and under a variety of brand names. Chlorhexidine gluconate is also available as a prescription mouthwash for the treatment of gingivitis and as a prescription oral chip for the treatment of periodontal disease. Brand names of products containing chlorhexidine gluconate include Avagard, Bioscrub, CIDA-Stat, and Dyna-Hex, among others, according to the FDA.
Labeling for prescription chlorhexidine gluconate mouthwashes and oral chips that are used for periodontal disease already contains a warning concerning possible serious allergic reactions. In 1998, the FDA issued a public health notice that warned healthcare providers about risks for significant allergic reactions with medical devices such as dressings and intravenous lines containing chlorhexidine gluconate.
In the recent Safety Communication, the FDA indicated that it is aware of 52 cases of anaphylaxis associated with the use of chlorhexidine gluconate products applied to the skin. In the 46 years from January 1969 and early June 2015, the FDA indicated that it received reports of 43 cases globally through the agency’s Adverse Event Reporting System (FAERS). More than half of these cases were reported after 2010, which was after the 1998 public health notice. “This number includes only reports submitted to FDA, so there are likely additional cases about which we are unaware,” the FDA indicated. “The serious allergic reaction cases reported outcomes that required emergency department visits or hospitalizations to receive drug and other medical treatments. These allergic reactions resulted in two deaths.”
Also according to federal regulators, eight more cases of anaphylaxis were published in medical literature between 1971 and 2015; one case was identified in the National Electronic Injury Surveillance System–Cooperative Adverse Drug Event Surveillance (NEISS-CADES) database between 2004 and 2013. That case involved an 11-year-old boy who suffered a severe reaction after being cleaned with a topical chlorhexidine gluconate solution in a clinic, according to the FDA.
Anaphylaxis is a severe allergic reaction that requires immediate treatment, typically with an epinephrine (adrenaline) shot as soon as possible, according to WebMD. Treatment usually involves emergency medical help. If the severe allergic reaction is left untreated, it may become deadly. Epinephrine typically begins to take effect immediately and requires a prescription. Treatment may require more than one shot, depending on the severity of the reaction and the patient. Antihistamines do not reverse anaphylaxis. People who had a prior anaphylactic reaction have a greater risk of experiencing another anaphylactic reaction as do individuals with a family history of anaphylaxis or asthma.
Initial anaphylactic reactions may resemble typical allergy symptoms, including runny nose or skin rash, according to WebMD. Within about 30 minutes, more serious signs will appear that may including coughing, wheezing, pain, itching, tightness in the chest, fainting, dizziness, confusion, weakness, hives, rash, itchy skin, swollen skin, red skin, runny or stuffy nose, sneezing, shortness of breath, trouble breathing, rapid heartbeat, swollen or itchy lips or tongue, swollen or itchy throat, hoarse voice, problems swallowing, tightness in the throat, vomiting, diarrhea, cramps, weak pulse, and/or paleness. Some individuals report feeling a so-called “sense of doom” immediately preceding the attack.
About one out of every five people may have a second anaphylactic reaction within the first 12 hours of the first attack, a condition known as biphasic anaphylaxis. Anaphylaxis treatment usually involves an emergency room stay of several hours to ensure a second reaction does not occur.

Urticaria (hives) is an outbreak of swollen, pale red bumps or plaques (wheals) on the skin and appear suddenly, according to WebMD. Hives usually cause itching, but they may burn or sting and may appear anywhere on the body, including the face, lips, tongue, throat, or ears. Hives vary in size from small—about the size of a pencil eraser—to large—the size of a dinner plate. Hives may join together to form larger areas that are known as plaques. Hives may last for hours or up to one day before they begin fading.
