You can very often patiently convince a cat that doing something is useless and doesn't get them enough of what they want so they might as well not bother, and you can often pretty swiftly redirect it to an even better option it didn't know was there, but you cannot force a cat to change its broad behaviors. You CAN stop them clawing the couch when you catch them, but because scratching is a deep primal need that serves MANY purposes, you can't force them to never do it without torturing them for the rest of their lives (declawing, never do this) or abusing them into being afraid of the couch...which leads to problems elsewhere as that stress then needs purging. They will claw something else. Possibly start pissing on the wrong things. And then they're afraid of you, too, because you are unpredictable. No yelling, gentle hands. No yelling, gentle hands. Undesirable behaviors may be bad habits learned early that are hard to break, like counter-surfing, but the really bad ones, the ones that lead to cats getting dumped at shelters, are usually expressions of stress. You will probably also need to replace the coping mechanism with one that technically may not be ideal but is better than the current one (a different and healthier prepackaged microwaveable meal, every other day) and that may be as good as it ever gets, and medical professionals HAVE to be okay with that.

Some people will never ever change at all, and medical professionals need to be okay with that. They still deserve to be happy and have their needs met, and they deserve respect.

I tell my doctors that I will not restrict calories and I will never deliberately seek to be less fat. I tell them that the cost of me being alive is me eating a lot of junk food when I am stressed, and not exercising because right now there just isn't a way for me to do so safely that I can afford. This IS the healthiest coping mechanism I have. I don't smoke or drink or do drugs, I don't injure myself, I don't pick fights, I don't have risky sex, I don't behave recklessly. I don't gamble or spend too much money. I have chosen to let myself eat what I want and not exercise very much so I can use the energy I would spend doing "healthy" things to cope with The Horrors. Because I am fucking beset by them. Nearly every aspect of my life is curtailed, controlled, or spied on, or at the mercy of things and groups and people that at best wish I would cease to exist in my current form and at worst would love to force the issue.
