"How would they be with a pet ?"

(Yes, the actual animals, sorry to disappoint.)

Because that's the next stage of evolution after kids anyway, right ? Along those vaguely vertebrated, reality defying cell-constructs that take over your life and present odd behavior to a large variety of situations, that also have the strange but well-documented observable ability to have power over some peculiar form of magnetism : attracting trouble. Rumor has it that this particular species is called by the common name of "siblings".

Where was I before I got grossly interrupted by myself ? Ah, yes, pets. Since this is quite long, I'm only putting the Hazbin crew for that one, will do the others later. Mixing some stuff already seen in canon, stuff that's now loosely canon, and own speculations, here we go :

The Hazbin Crew (not forgetting Lucifer this time around) !

Charlie technically has Razzle and Dazzle as familiars (well... Razzle anyway now), though she's not seen interacting with them much, but she does have some small moments with KeeKee. She treats her well and the two are seen to go quite nicely along, surprisingly treating her like a normal pet (and not a substitute child or something), so I think she's one of the ordinary pet owners. She could love soft pets the most (as seen with her excitement to get to pet a koala in Heaven, implying Hell pets aren't that soft or nice usually - which, no wonder, given what we've seen from Helluva Boss' Hellhound adoption center, also with Hell being, y'know, Hell) and love to cuddle them, distractingly give them scritches and strokes with one hand while working on something else. She naturally wouldn't like to see her pet hurt, but would be the type to forget regular check-ins for minor shots and general health-care, not out of malice or anything but just because it's such a small thing (compared to how busy she is with the Hotel) that it simply slipped her mind. Vaggie would probably have to remind her that they better get the hellbies shot soon, and Charlie would drop anything to wrench a timeslot on her busy schedule, wait, maybe next Wednesday...? But I promised Niffty we could go buy new cleaning supplies on that day, oh, jeez, umm, Friday ? No, no, that's Share Your Feelings Day, can't skip that - uhh... until Vaggie simply says that she'll take care of it.

She'll be the owner that actually doesn't overcoddle her pet (contrary to how she behaves with Sinners), and sees it like the animal it is instead of something else. However, she'll also be empathetic, enthusiastic (especially with big-friendly-dog-like energy pets), and always eager to interact with it, whether in a calm (awww, who's up for cuddles ? You ? Why yes we can have cuddles) or energic way (Who's a good boy/girl ? Who wants to go for a walk ? Ready ? Fetch !) given the type of pet. Expect photos of moments she finds endearing or milestone celebrations. Any abandoned kitten in a box found is immediately saved and showered with affection. She will also have a high chance of incorporating them into therapy with Sinners, because "puppy dog kisses" and all. Hurt an animal, especially for sadism, and she will get very pissed. Not violent, but very threatening at least, demon horns and all : animals may be animals, but they're still living beings to be respected and loved.

Vaggie is on the more pragmatic side, getting all appointments ahead of time, has a vaccine planning for the next 20 years, and if the pet requires it, a stern but adapted education plan (in case of specific breeds with dangerous behavioral issues). She's a bit more commanding, but also shows a softer side around her pet, and would see a middle-to-high maintenance one as a challenge. While she doesn't think she'll be able to afford caring for every lost pet, she'll make sure to find them a home. Might co-own a pet with Charlie most likely than having one of her own, but she'll take care, as said above, of anything practical for her - she's a manager after all. She'll be more fussy about flees and shedding than Charlie, but in an annoyed way rather than being a clean freak (that's Niffty). She's also aware of safety problems (don't let a pet unsupervised around a kid), but trust the animal's instincts the most : if it reacts the wrong way to something, then trouble is up. I think they'd have a good companionship, in a sort of busy-owner/sidekick-competent-pet kind of way : it would bring items to Vaggie when she's looking for them, sniff out what she tells it to, and support her emotionally when distressed or frustrated - like said, she's softer around it and would like some nice cuddle session to relieve her stress.

The funny side would come up if the pet is enthusiastic and has trouble understanding commands, winging it through accidental disobedience and just being happy-go-lucky rather than being a spiteful little rat. Vaggie won't have the heart to tell it that it did wrong, just huffing out a smile and resolving to do better next time. Her strong points are training, and physical exercice with the pet. Hurting a pet will make her angry, and hurting her (and/or Charlie's) pet will make her furious enough to pull out her spear.

Lucifer, as seen with KeeKee, has a massive case of cuteness proximity. He has at least 4 full photo albums on his phone from a pet at all angles, and this is KeeKee slightly to the left, and this is KeeKee slightly more to the left, and... he'll be the one to treat it like a fuzzy animal-child that just happens to have four legs and a different diet, putting it in different costumes, and just gush over it all day. That doesn't mean he'll excuse misbehavior, but he'll downplay it some bit. Might also be petty enough to let the pet be a little jackass to people he doesn't like (ie, Alastor) and play it up as if baby is incapable of hurting anyone, I dunno what you're talking about. If it's a bird, he migh train it to redecorate Alastor's furniture in the latest artistic guano trend (to the sound of Drop It Like It's Hot. Sorry not sorry. It was that or a joke about "polka dots"). While his life doesn't revolve around the pet, he'll sure spend a lot of his day just goofying around or look at it, poking and booping it then giggling like a schoolgirl with a crush on the math teacher, to the incomprehension of everyone else. The pet is allowed to go everywhere (unless it's dangerous), but there are moments when Lucifer is more serious and needs comfort, which he'll look for in his animal companion, and calmer moments when it's just simply him at his workshop with his pet on his lap.

He might be more fond of birds, cats or bird/cat-like pets than dogs or reptiles (I know, missed opportunity about snakes, but he seems more to be into waterfowls, and snakes might bring up some unpleasant memories - but I can see him adopt pretty much everything though), but curiously enough I can also see him being a fan of vivarium pets (like beetles or butterflies), because he'll make them the best vivarium ever - and the decor changes so often the poor tykes don't know where they dug their house anymore. Nevermind the thing also being the size of a whole room, and having to be shower-sprinkled every damn five minutes to keep up humidity. But my bet is that he's more into fuzzy, fun-sized cute pets (or his own metric of cute, which everyone else could be finding questionable at best). Don't bring him to the vet with the pet, he'll cry at the same time as the thing even if he's not the one receiving the shot. Might try to pull the "swivel in the chair with the pet on lap and stroking it in an evil overlordish way", only the effect is ruined because that's a cross-eyed aadvark or something.

Angel Dust, as seen with his hellpig Fat Nuggets (given to him by Valentino of all people - no, really), is the type to utterly adore and spoil his pet, but not to a completely unreasonable degree. He deeply cares about it and keeps it in his room, and seems to be the type to gravitate towards comfort pets as companions than maintenance pets, decorative/competition/status pets or friend pets. Fat Nuggets is part of the family, plain and simple, but while baby-talking, Angel wouldn't treat it like a pseudo-child either. He's fond of carrying it around under one arm while doing something else, bringing it along with him, as a comforting weight and source of affection. Doesn't mind sleeping with Fat Nuggets next to him I bet, and goes to hug it on particularly bad days. Hurt the little piggy and you get gunned down. I don't think he'll have many pets at once, just one by one (unlike my household where we have 3 cats, a mom and her two daughters, all adults, and hellbent on never letting anyone sleep a full night), so he can give them his full attention. But he'll be also fond to giving them ironic names (like "Fat Nuggets" to a pig) and adoring nicknames overall. Forgets to go to the vet, but knows it's necessary and if reminded (and having his schedule free), he'll go. Has a lot of photos, mostly for cuteness points and personal enjoyment, but won't film himself making silly dances with his pet like Lucifer would do.

Husk would do best with a low-maintenance pet, and har har fuckers, yes I know I'm a cat in Hell, never heard that one. He might not naturally gravitate to them, but like Vaggie when he finds one in need, he won't have the heart to turn it down. Independent, free-roaming pets are the best match for him : he doesn't have to keep an eye on it all the time, it can gather food for itself, and go through the day with the bare minimum of his intervention. But having something to scratch and nuzzle with on the counter at the end of a hard day at the bar is always a plus. He might also carry one-sided conversations just for the need to vent out when nobody's looking, and when somebody's looking, just exchanging a deadpan did you see that shit ? glance does it for him too. He might also keep it around for the benefit of keeping drunk or drinking people from going overboard, turning some violent alcoholics into crying softies at the sight of the pet - indirectly using it as a therapy pet, more or less consciously (he'll never admit it). Will aslo put up a sign : "Watch your drink, the pet is an asshole." with said pet being drawn knocking it off the counter. He'll let someone else take care of the vet thing, no this problem, tyker can take care of itself. Maybe doesn't name it, because what's the point, it's not like he owns something that decides to butt in and sit on his bar counter, but likes it for its free spirit.

Niffty would be... a clean freak. She might get the idea of plucking every strand of hair off the pet so that it'll never shed again, and do things borderlining animal cruelty out of a misguided attempt to "make things simpler". Hairless animals would work best for her, and there might be a kindred spirit if it's as psychotic as her. She'll be quite strange with affections, because licking and climbing on her are not advised (keeping things clean, and also she zips around like a squirrel in traffic on caffeine), but she'll give it words of praise and quick pats on the head if it's well-behaved. Feeding time will be on the dot, she might teach it tricks for her own amusement (and then everyone shudders at the thought of what "tricks" might mean for Niffty), and probably transform it into a cleaning sidekick just as bugfuck crazy as her. Others often have to intervene because No, Niffty, you can't do that !! but the beastie will, somehow, learn to use knives despite everyone's best efforts and not having arms at all. One of the safest ways for Niffty to have pets is fishes : they are well-cared of, don't risk much, and sometimes you'll find her on a break gazing calmly at the colorful dancing and shimmering in the waters, almost looking normal for once, lovingly feeding them and the whole looking adorable. If you forget that they're pirhanas ripping some poor sentient dust bunny to shreds.

Now Alastor. I can't see him having a pet (no the Egg Bois don't count), but for the sake of this thing, let's pretend he does. Dogs are ruled out though, given his manner of death, he'll despise them. He might like something silent, clean, autonomous, obedient yet with its own spirit but that doesn't steal the attention from him (all cats of the world, you can go back to where you come from). A sort of hype-man pet, that he'll thoroughly train with precise discipline, that looks like a shadow to his own shadow and gives emphasis to what he does or says - if there was a cool, hellish version of a radio parrot, it'll be it. Absolutely would use it to prank or freak out people, and put it to use to get information. Will use the full overlordish-status potential of having such a dangerous but tamed-at-his-command pet, letting people wonder how that came to be in his possession, and how threatening he can be with it. Will also talk to his pet like to a butler, with full sentences and gentlemanly inflections (So who shall we visit now, my fine, fuzzy fellow ?), monologues at it a lot with faux-dialoguing on its expressions (You're right, my friend, this is a pile of rubbish) and loves to terrorize the vet with it. The only one who likes it as he does is Rosie.

Sir Pentious (and again, the Egg Bois don't count. The blimp even less) on his side will try to pull the overlordish evil pet stroke, but he's actually a big softie that, if even saddled with and cuddled by a pet (as seen when KeeKee goes to nest on his lap) will feel chosen and emotional. Expect huge nuzzles to the point of disturbing the pet in whatever it does, at all times, but also spoiling it in material ways : best food, best toys, best bed, he might even build most of it himself. The end result might look like a pet-house that doubles as a tank so it can defend itself should anyone attack, and assist Pentious in his conqueering endeavors. He might be fond of stuff like hamsters or bunnies, building it their rolling ball so that the thing can go around and spy for him - but unlike Alastor, this fails completely, as it doesn't possess the intellectual capacity to process anything beyond "lettuce". Pentious might also do extensive research on every little thing about the pet, and ask his minions to hold a pet-journal, Karl Lagerfeld style. If the critter looses three hairs it's the end of the world. Every trip to the vet is treated like treading into the battlefield, the atrocious march to the gallows, we might not come out of here alive, men, it was a honor flying with you...! and alright, sir, the nail-clipping is done, you can stop fainting now.

Cherri would be more into robust, strong pets that can keep up with her and not afraid of bombs. Her lifestyle makes it so that she realistically might not be able to afford a pet, nor does want one, but again, like Alastor, let's pretend she can - so either a resilient big pet, or a small one she can carry around. Maybe even a preferrence for something that could act like Hell's answer to a zippo Pokémon or something, so let's say something reptile-like, a pocket dragon of sorts. A dangerous, but cute critter, that no one in their right mind would try taming, but Cherri is going at it and loving every second. If her pseudo-Charmander grows big and impressive as an adult, even better ! No mad bomber likes it more than to drop it from a flying beast like the new atomic bomb carrying plane. She might cuddle with it for sleeping, and is fond of patting gestures and dynamic exercises with it, getting her thrills flying around. The only problem (that matters to her) ? It keeps trying to eat Fat Nuggets, so anything looking vaguely pork-ish tends to distract it from its course. Because obviously, it has the attention span of a spastic monkey - which would be her second choice : it can climb around, carry and hand her stuff, and escape just as fast out of a sticky situation along with her.

Glossary and Common Terms

This is a collection of terms and lingo that are frequently used in D&D and other tabletop games, along with terms written by me specifically for use in this blog. Some words used in this blog are purposely written as “catch all” ideas (See Mage) or “Common Terms” (See Knowledgeable PC’s) that can easily be adapted to any game system. These terms only describe how they are used in the context of this blog and if there are any inconsistencies between here and your game system, your player's handbook or other official guide is correct.

Glossary:

-Ability Check: Abilities are the base statistics for a creature such as Strength, Dexterity, Constitution, Intelligence, Wisdom, Charisma or whatever your game system uses. Ability checks are simple, untrained rolled checks that use the modifiers of the base statistics. Examples of these checks include, trying to brute force a door down, puzzle out a riddle, or juggle. For more trained skills such as pickpocketing, lying or crafting, see Skill Checks.

-Advantage / Disadvantage: Advantage reflects the positive circumstances surrounding a roll, while disadvantage reflects the opposite. For example, a character would have disadvantage on attack rolls against an invisible target, while the invisible creature would have advantage on attacks against a target who can't see to defend themselves. When a character makes a roll and has either advantage or disadvantage, the player can roll the dice a second time. Use the higher of the two results if you have advantage, and use the lower roll if you have disadvantage.

For example, in a d20 system if you have disadvantage and roll a 17 and a 5, you use the 5. If you instead have advantage and roll those numbers, you use the 17. A character who benefits from multiple sources of advantage or disadvantages still only rolls twice, it does not stack. A character who has both advantage and disadvantage simply rolls normally, they cancel each other out.

This is taken straight from 5th edition D&D because I like its simplicity and is also an easy way of maintaining system neutrality since every system can roll twice and take the higher or lower result. However this may be considered unbalance in some d20 systems and could take forever to roll twice in dice pool systems. The DM is free to substitute the advantage / disadvantage effects for a +1 or +2 on a d20 roll or one or two additional dice rolled in a dice pool system (Or the opposite for disadvantage). The DM has the final say on what the mechanical bonus or penalty of a trinket has on a player's roll and should grant the player whatever they feel is fair.

-Attunement: Some items require the bearer to wear or have an item in their possession for a certain length of time before the object can be properly used. For clothing and jewellery, this requires the item to be worn the majority of the time, or within a few feet of the character’s person if they are bathing or changing. For other items or weapons, the object must be kept on the bearer's person or within 15 feet of them the majority of the time. Unless otherwise stated in the item’s description, the bearer can remove magical clothing or jewellery for a few minutes or hours without issue. Mechanically this is to prevent multiple PC’s from having access to the item’s power if it’s constantly passed around or from a PC owning a dozen magic rings and switching between them every few minutes. Some objects incur minor negative effects that last for a brief period after the trinket stops being worn. This represents lingering magical energy on the creature's person and allows for a more serious risk vs reward situation when considering using the item. This attunement process isn’t the same as other systems such as D&D 5e and shouldn’t count towards the attuned slots unless the DM rules it so.

-Bearer: A creature who is wearing, carrying or wielding an object. Depending on the item, the creature could be considered bearing it even if it’s packed away in their bag or other belongings. Bearing a pair of spectacles obviously means wearing them on the face over the eyes, but bearing a cursed voodoo doll could mean that it was placed unknowingly in one of the creature’s packs and they may not even be aware of it.

-Cleric: An ordained member of a religious order. Similar to mage, cleric is a catch-all term for any kind of man or woman of a religious order. Feel free to change the term to whatever honorific there is for ecclesiastic members in your campaign. Based on your game system, some clerics may also be mages in that they’re able to cast spells, however the term cleric as it’s used in this blog does not automatically mean spellcasting abilities.

-Creature: Taken straight out of D&D and Magic the Gathering lingo, a creature is a living or otherwise active or animated being, that is not an object. Creatures can be the PC’s themselves, animals, undead zombies, ghosts, golems, animated furniture, elementals, ghosts, intelligent plants, swarms of creatures that are treated as a single unit, sentient magic items etc. Basically anything that might have real statistics. A regular granite statue is not a creature, but the same statue under a magical effect that allows it to move, guard an area, think, speak, attack or cast spells, changes it into a creature.

-Critical Failure: For any rolled check in a d20 system, a roll of a 1 on a d20 without any modifiers is typically considered an automatic failure at whatever the character was trying to do. This often has an added penalty of harm to the character or others around them, destroying or damaging nearby objects or being unable to try again at the task for a length of time. In dice pool systems a critical failure can be measured as a number of 1’s below the result needed to accomplish the task, or as a number of 1’s greater than half the dice rolled or more 1's than successes. Whenever a trinket refers to a “natural 1” it refers to this kind of critical failure. This vary from game systems and are also called Critical Fumbles, Critical Misses or Critical Glitches. -Critical Success: For any rolled check in a d20 system, a roll of a 20 on a d20 without any modifiers is typically considered an automatic success at whatever the character was trying to do, often with an added bonus of doing it perfectly or in record time. In dice pool systems a critical success can be measured as a number of success above what was needed to accomplish the task, or as a number of successes greater than half the dice rolled. Whenever a trinket refers to a “natural 20” it refers to this kind of critical success. See also Natural 20.

-D20 system: A d20 system is one in which a d20 is rolled to determine skill checks, saves, attacks, etc and a static modifying is added to the roll to determine the final result. This is compared to a dice pool system where multiple dice ares rolled and the number of success tallied up.

-Dice Pool System: Compared to a D20 system where only one die is rolled and a modifying number is then added to determine the result, in a dice pool system the player rolls a number of dice (Typically d6s or d10s) and counts up a number of successes. On a d10 for example a roll of a 7, 8 or 9 can count as one success while a roll of 10 is worth two successes.

-Difficulty Check : This is the standard or target that a PC wants to roll higher than to succeed at an action. For example if a PC wants to climb up a 30 foot, rough stone wall that has some handholds, the difficulty check (Or DC) would be fairly high like a 20 to 25 (In a d20 system) baring other factors such as weather. The player would roll a relevant skill check (Such as athletics, climb or acrobatics) and their total result is compared to the DC. Getting a higher result is considered a success while a lower result may mean the character takes a long time to succeed or is unable to maintain their grip and falls down potentially being hurt from the fall. DC's can be modified by mitigating circumstances, so if there was a rope hanging down the wall the DC might lower to 15-20 and if the wall was wet with rain the DC might go up to 25-30.

-DM: The Dungeon Master, Game Master (GM), Storyteller or other real live human that is running the show. This person creates and enforces the rules and is in direct control of everything that is not a Player Character.

-Hedge Mage: A mage with only minor powers, spells or tricks. More of a local wise man, self taught magic user, oracle, witch of minor power, medicine man or shaman who gives advice, provides weak spells or brews potions for a small village or community. Hedge mages are usually NPC's that are able to use magic but typically have less magical power than a level 1 PC.

-Initiative: At the start of combat, each creature will make an initiative check in order to determine the order of who goes first, second, third etc in that combat's turn order. This is typically a rolled check.

-Intelligent Creature: An intelligent creature is able to think and act using knowledge, experience, understanding, common sense and insight. This can range from very dumb humans, normally intelligent goblins to supernaturally intelligent animals such as unicorns, mage's familiars or pegasi. In some cases exceptionally well trained mundane animals such as ravens, parrots or working dogs could be considered intelligent, but they would be considered Albert Einstein's of their species. A good rule of thumb is an intelligent creature will be capable of understanding (If not always speaking) in at least one language. ---Note: I have sometimes messed up and put “sentient creature” when I meant intelligent creature. I'll be fixing these mistakes as I find them along with other slip ups that made it through editing.

-Intimidate Check: This is an example of a skill check that comes up many times in the trinket tables, especially among the minor magic items and some minor weapon enchantments. Intimidate can be used in place of the friendlier diplomacy or persuasion to coerce another creature into helping you in one way or another. Creatures influenced in this way typically don't appreciate being threatened and may go to the authorities after the PC leaves the immediate area, may refuse to do business with them again or hire thugs to beat the PC up. In combat intimidation can be used (Depending on your game system) to demoralize or frighten an enemy causing them to take on a negative condition (Such as Shaken from Pathfinder), or to surrender, panic or flee. If your game does not have an intimidate system in place for a combat setting, an easy rule of thumb is to just have the player roll their intimidate skill and to judge for yourself how fearful the target might be. Typically a target has to be able to see, hear and understand the creature who is trying to intimidate them. A moderate roll might mean that the target suffers disadvantage on attack rolls as his hands are shaking or that he spends his next turn running away out of fear and regrouping. A higher roll (especially against an intelligent creature with a sense of self preservation) means that the target is legitimately afraid for their life and flees the combat encounter. As a DM remember that the target did not actually die and may ambush the PC's later when they least expect it or go in search of reinforcements leading to an even tougher encounter.

-Mage: This is a creature capable of casting spells. In the purpose of making these items as system neutral as possible the term mage is used as a catch-all term for all: wizards, sorcerers, witches, artificers, enchanters, witch doctors, shamans, summoners, psychics, mediums, diviners, warlocks, spellcasting clerics, necromancers etc. For story purposes, sometimes an item will refer to a specific type of magic or mage such as witches, warlocks or necromancers but this is purely for flavor. Feel free to substitute the term “Mage” with whatever type of spellcaster works best in your campaign. I purposely avoided using the term “spellcaster” because it seemed more like a game role like “tank” or “DPS” rather than a mystical title.

-Minor Weapon Enchantment: A weapon enchantment that typically has a rider or condition where it's magic comes into play. These “Better than nothing” enchantments typically modify the damage or grant the weapon a +1 to accuracy and damage rolls if certain conditions are met. In simple D&D terms these are more like +0.5 enchantments compared to standard +1 weapons. These weapons could be have been made by Hedge Mages or apprentice enchanters who are simply not able to make stronger enchantments or the creator wishes to lower the material costs by having a weaker enchantment. ---Note: I am still working on these and since I do not have 100 of them yet that I feel are good enough and edited enough I haven't put out a table of them. I should have a d100 table of them out sometime in 2019 if all goes well and I'll put out a d50 table if it doesn't go well.

-Natural 20: Rolling a 20 on a d20. For further explanation see Critical Success

-PC: A Player Character is a single or small group of creatures who is controlled by a real life human who has complete control of their actions, thoughts and abilities. For the purpose of this blog, the term “Character” and “PC” sometimes blurs.

-Proficient / Proficiency: A PC has a number of skills or tools that they are trained in doing and are represented with their statistics. Fighters are able to wield greatswords and wear plate mail whereas rogues are able to disarm traps and pick locks. A sailor might be able to use a sextant and a doctor might be able to use a set of surgeons tools to perform an operation. In many systems a creature is either proficient or not proficient in certain skills or tools which carry their own mechanical bonuses or penalties. Just like in real life, where pilots are trained to fly planes and mechanics are skilled at fixing cars, there are some areas in roleplaying where your character either knows how to do something or not. Some of the magical items grant proficiency in various specific skills, which grants the user the ability to make use of the item, or gains the mechanical benefits of using that item according to the game system. For example, a barbarian who has never even heard a harp been played before, could use one that has been enchanted to grant him proficiency in it, as if he has been practising his whole life.

-Random Bright Colour: See Random Colour but using a smaller die to rule out the darker colours.

-Random Colour: Fairly self explanatory, there is a random table of colours in the reference tables section and you can roll or just pick a colour to suit the trinket. If you don’t like the colour, roll again or pick whatever works best. This allows you to role the same trinket twice and still get a unique item.

“The cure for pain is in the pain”

How was your week?

Mine was filled with discontentment, hormonal changes, goodbyes, work/life stresses, and tears, a lot of them interlinked. In the past, I probably would have shut everyone out, stayed home, binged on unhealthy trash, and wallowed in self-pity, and doubt. I am glad to see how I have grown, as a person, in that respect. 

This is not to say that I handle things perfectly, considering how I missed an interview and a swimming lesson on Monday simply because I felt I couldn’t leave the house. I think what has truly changed though, is my acceptance, and the keenness to understand the emotional and physical signals that my body sends me, particularly for the former. We cannot simply be adverse to negative experiences, because they will happen, frequently, for all our lives. Empathy is a friend, and so is joy, and love, and of course, pain. One thing to ask yourselves - what do I do when I’m in emotional pain?

My go-to mechanism, used to always be - “it’s not worth it”. I convinced myself that people, and situations, were simply not good enough for me. How could it possibly be, if you give so much that it drains you, that it’s not good enough? Of course then it must mean that there’s something wrong with the other person. 

In the last month or so, I would say my view towards that has evolved. I think fundamentally, the understanding that my worth is not tied to how people treat me, has helped. I have always been acutely aware that love, should come without expectations, but having that precious awareness did nothing for me because I failed miserably in practicing it. I thought I was - but it was in fact just a false show of strength to protect myself. If I gave, and gave, and gave and received nothing in return, I would mask that sense of unworthiness with defiance. If someone was unkind to me or didn’t reciprocate in the way I expected, I made myself feel better by telling myself I wasn’t supposed to have expectations. While loving without expectations seems noble, it is also for the most part, unattainable, because we are human beings that all have emotions to some degree. I think the goal, for me at least, is still to aspire to love in that way, while understanding context, and what I deserve. 

Thursdays are always interesting for me. It was my third mobile canteen at IMH, and I’m proud to announce that I’m getting really good at prepping cup noodles, and basic math (ha). It can get a little intimidating sometimes, dealing with mental patients, but for the most part the smiles and the handshakes are adorably comforting.

The highlight of my day however, was what I learnt in my weekly mindfulness lesson. I would encourage everyone to give this little exercise a go. It should take only ten minutes, and is SO WORTHWHILE. 1) Think about what a typical day in your life is like.

For many of us, this usually involves, waking up, making coffee/breakfast, checking Instagram, getting the MRT, working, etc etc. The small bytes of your day which form the daily experience, basically.

2) Break it down into the various different steps, aiming for about 15-20.

For instance, i) wake up, ii) take a shower, iii) brush teeth, iv) make coffee, v) play with Rusty, etc etc. By the way, that’s actually how I start my day. So carry on with the list until the day ends, which unless you aren’t human, is usually sleep. It’s obviously better to write it in a downward list and regular numerals.

3) Beside each step write ‘N’ or ‘D’

The ‘N’ represents a nourishing activity, and the ‘D’, a depleting activity. Is this activity nourishing you as a person, or depleting and draining you? This is all relative of course. A girl in my class said eating alone, was a depleting activity and experience for her, while for me it is actually one of my favourite things to do, which is of course perfectly fine since our personalities all vary. It can also be neither, which makes it a neutral experience.

4) Evaluate

Look at the activities that you have considered to be depleting and try to understand if it is within your control or not. I was lucky enough, to only have one ‘D’, which ironically happened to be my endless scrolling of social media platforms in the morning. I do have control over that, and I would say it has improved, since I now try to read the BBC instead or put on an educational podcast. I also had one neutral activity, which happened to be my second cup of coffee. I wouldn’t consider it depleting, but compared to my first cup, I just wasn’t sure it added much value to my life. So, for the last two days, I’ve only taken my morning coffee, and substituted the afternoon one with tea. That’s more of just a personal experiment to see which way it leans.

There will be depleting activities and experiences that you might not have control over at this point in your life. For instance, several people in my class, said their work was highly depleting. But not all of us can leave a job on a whim or should, without trying to fix things or without considering external factors like rent, loans, etc. But that’s where there are specific techniques for mindfulness at work that come in, and also where you think about how to add more nourishing activities to your life outside of work. Some of my super nourishing actiivites (which I added an extra smiley face to) included, my first coffee, playing with my dog in the morning, swimming, spending time with family and friends, and SLEEPING (my ultimate fav). 

But yeah, you get the drift. I really feel like that exercise helped me understand more about where I was in my life, the direction I was heading in, and the areas i could grow in. All that aside, it is quite interesting to know anyway, how you’ve been treating yourself.

The weekend has been weird, but in a good way. I went for a film screening last night about the Delhi gang rape in 2012. While it was an important issue that was meant to be covered on the international Day for the Elimination of Violence against Women, the panelists added so little value that I was just waiting to leave. The audience had some good insights though, and I am still glad I went.

This morning, I went to IMH, to visit the children’s ward, for the first time. I was 100% expecting to join a volunteer group, but was the only one there. I then spent two hours speaking to a few of the patients and just left more emotionally burdened, than inspired. I had to cancel a coffee catch up with a friend because I felt so overwhelmed and needed time on my own to recalibrate. I’m not allowed to talk much about it, because of  the patients’ privacy, but I can only say that I am heartbroken at how our system has failed them. Still, I am prepared to turn all of this resentment into action.

For years, an artificial pancreas was a pipe dream. Now, we’re closer than ever to systems that will automatically adjust background insulin. Here’s what’s in the works—and what’s still to come.

Since she was diagnosed with type 1 diabetes almost 40 years ago, Alecia Wesner has spent a lifetime managing her condition: Crunching numbers, counting carbs, and trying on a variety of wearable devices to help keep her blood glucose under control. “My numbers are within a recommended target, but I work like crazy at that,” she says.

So when Wesner, 43, was asked to participate in trials for Medtronic’s hybrid artificial pancreas insulin delivery system, the New York City–based lighting and jewelry designer jumped at the chance. The system is an automated blood glucose management tool—what some researchers are referring to as a closed-loop system or bionic pancreas.

Over the course of a few years, Wesner has spent weeks going to study visits at Mount Sinai Hospital in New York City and sleeping in hotels while hooked up to an artificial pancreas prototype with teams of doctors watching her blood glucose levels rise and fall in real time. She has since used the systems—made up of an insulin pump, a continuous glucose monitor (CGM), and a smartphone application running a program that lets the two communicate and administer insulin—while training for cycling events.

Designed to mimic the function of its namesake organ, the system calculates a person’s insulin needs based on glucose readings, activity, carb intake, sleep, and other factors. Then it automatically adjusts and delivers basal doses of rapid-acting insulin around the clock. That’s an especially big deal at night, when people with diabetes have to wake up in order to treat highs or lows.

Where the artificial pancreas most noticeably differs from the real thing is in mealtime insulin delivery. In the trial Wesner took part in, the experimental system calculated mealtime boluses based in part on her current and trending insulin and glucose levels, but she had to count carbs and deliver the bolus dose by pushing buttons, just like using a regular insulin pump.

Wesner says the systems have gotten smaller, more streamlined, and more reliable since she started participating in the trials. They’re essentially modified existing pumps and CGMs, worn on a belt or in a pocket, that interact with a person’s body via an infusion set and a sensor inserted under the skin. For someone who’s lived with diabetes almost her entire life, the promise of a device that could reduce some of the daily burden of the disease is tremendous. “It’s one thing to hear about technology that could change your life,” she says. “It’s pretty different to actually wear it.”

Many people with type 1 and type 2 diabetes could have access to more than a prototype soon. In September 2016, the Food and Drug Administration (FDA) approved the first partial artificial pancreas system—Medtronic’s MiniMed 670G—for sale in the United States. As of press time, it’s expected to release in late spring. At least five other companies are working on their own systems, hoping to bring them to market by 2020.

The first devices to hit the market will be so-called “partial” artificial pancreases, requiring user input and monitoring for mealtime boluses and automatically adjusting only basal insulin. A few years from now, developers hope to release systems that will manage all insulin delivery automatically: The user can essentially strap it on and then forget about it.

Doing the Math

When it’s working properly, the pancreas is a wonder of balance and sensitivity. Inside the hot dog–sized organ, millions of beta cells monitor the levels of glucose in the bloodstream. When glucose climbs too high, the beta cells release insulin, a hormone that prompts cells elsewhere in the body to absorb and store the glucose and shuts off the release of glucose by the liver. If blood glucose goes too low, a different set of cells in the pancreas—called alpha cells—pump out the hormone glucagon, which tells the liver to release stored glucose.

Diabetes is what happens when the beta cells in the pancreas stop working as they should. Without the beta cells to sense rising (or falling) blood glucose levels and to release insulin accordingly, people with diabetes are forced to rely on substitutes—from finger sticks and insulin pens to CGMs and insulin pumps. These require users—typically people with type 1—to master sometimes-tricky tools and techniques and carry around one or more devices all the time.

And then there’s the math. The latest generation of CGMs may provide accurate, real-time readouts of glucose levels. But it’s still up to users to program basal rates, count carbs, and reactively decide to deal with highs and lows—instead of avoid them in the first place.

“The burden of diabetes self-care with technology has become more challenging,” says Carol Levy, MD, an endocrinologist at Mount Sinai Hospital in New York City who heads the Icahn School of Medicine’s Artificial Pancreas Research Program. “You have all this data, and the patient has to figure out what to do with it on a day-to-day basis.”

Relieving the Burden

The flood of numbers may help explain some curious patterns researchers have noticed when it comes to who uses technologies such as CGMs and pumps, the building blocks of artificial pancreas systems. In a study published last year in Diabetes Care, researchers found that nearly half of all people with type 1 diabetes in a group of about 15,000 were reluctant to use devices because they were a “hassle.” Interestingly, the study showed that older users were more comfortable with using technology to manage their diabetes, while younger people—those between 18 and 25—were the least likely to wear CGMs or insulin pumps.

The artificial pancreas products should make things easier. In theory, the idea is simple: Combine a continuous glucose monitor with an insulin pump. When the CGM senses glucose levels rising, it sends a signal to the insulin pump to dose insulin. When it senses dropping glucose levels, it decreases or suspends insulin delivery.

At the system’s heart is a complex formula, called an algorithm, built into the device or stored on a smartphone in the form of an app. “It’s a very sophisticated computer program,” Levy explains. The algorithms take in data—how long insulin lasts, how fast glucose is rising, how a person reacts to insulin—and calculates how much insulin or, in some models, glucagon to deliver.

The first devices to market won’t do this all by themselves. In its press release heralding the Food and Drug Administration approval of the first hybrid closed-loop insulin delivery system, the type 1 research advocacy group JDRF dubbed the system an “artificial pancreas.” The term has been eagerly adopted as shorthand for a variety of systems going through testing for safety and efficacy right now.

But Medtronic’s system, along with Insulet’s Omnipod Horizon, are what Levy calls hybrids: They still ask users for carb gram input before meals to calculate how big of a bolus to deliver, for example. Which means carb counting won’t be a thing of the past for a few more years, at least.

That’s part of the reason not everyone likes the term “artificial pancreas.” Boston University biomedical engineer Ed Damiano, PhD, calls his iLet prototype a “bionic pancreas”—reflecting, he says, its fully-automated nature.

Stanford pediatrician Bruce Buckingham, MD, on the other hand, is sticking with “closed-loop system.” He argues that “artificial pancreas” implies something much closer to a replacement organ than the reality, which is a wearable device and sensor plus an infusion set, all of which have to be changed regularly.

And Bigfoot Biomedical, a California-based company founded by four fathers of kids with type 1 diabetes, prefers the term “automated insulin delivery.” “Our system does require user input,” says Bigfoot spokesperson Melissa Lee.

The systems may represent an additional cost for consumers, although manufacturers aren’t sure how much more. Insurance coverage is also an open question until more devices are on the market. “Are insurance companies going to pay for these? We don’t know the answer yet,” Levy says. She argues that the improved blood glucose control the devices provide saves money in the long term by heading off complications.

Doctors say it’s at night when these early systems really shine. While nighttime insulin requirements tend to be more predictable because people don’t eat—and the body is at rest—during sleep, the threat of low blood glucose at night is something that people with diabetes dread. “I spend a lot of time having things beep at night, either because I’m high or low,” says Wesner of her current continuous glucose monitor. She then has to decide whether it’s a false alarm or necessary to treat with insulin or fast-acting carbohydrate. “The thought of having something that would manage that is really appealing.”

The first generation of artificial pancreas devices—such as Medtronic’s 670G—will go a long way toward helping people with diabetes sleep better. The JDRF, which has been lobbying for fast-tracked approval for the systems for years, sees the arrival of the devices as a major win. “People who have participated in artificial pancreas clinical trials have not only attained better overall glucose control but have experienced the relief of sleeping through the night and waking up in the morning with blood glucose levels within target range,” Aaron Kowalski, PhD, JDRF’s chief mission officer, told Diabetes Forecast. “That’s an improvement in quality of life.”

In tests, people with type 1 diabetes using artificial pancreas devices stayed within their desired blood glucose range much more often overall than people who weren’t using the systems. Pilot studies in people with type 2 who depend on insulin show that artificial pancreas devices kept their blood glucose levels within recommended ranges as well.

“This is a major historical milestone,” says Buckingham, who has worked with children as young as 7 to test the Medtronic device and several others. “It gives people more security and will make them sleep a lot easier. Waking up in the 100 to 140 mg/dl range is a pretty good way to start the day.”

So what’s taken so long?

Smartphones and Sensors

Developing safe, reliable algorithms is a huge challenge. It’s also important to remember that a mistake or glitch in the software can have serious or even deadly consequences: Too much insulin can drive blood glucose levels dangerously low, resulting in hypoglycemia, coma, or death.

That’s why developing a system that reliably and safely mimics the body’s own insulin delivery system is fiendishly complicated. Ed Damiano, PhD, should know: He’s been trying to develop one for 17 years, ever since his then11-month-old son, David, was diagnosed with type 1 diabetes.

Almost immediately after his son’s diagnosis, Damiano—now a professor of biomedical engineering at Boston University—dedicated himself to solving the problem. The goal was obvious: an insulin delivery system that could monitor and adjust his son’s glucose levels automatically as they both slept.

How to get there wasn’t as clear. Damiano began by developing algorithms to manage the balance between insulin, glucagon, and blood glucose. It was a tremendously optimistic project to undertake. At the time, there wasn’t an accurate or reliable CGM on the market. Until recently, the glucose readings from most commercially available CGMs could be thrown off by something as simple as Tylenol. That’s not a huge problem when backed up by regular finger sticks, but it’s a major barrier to a true artificial pancreas system that would run without user input.

When Damiano started, a desktop computer was needed to run the programs, which he tested on pigs in a lab. The first tests in people were conducted in hospitals because participants had to be connected to bulky computers.

Fifteen years later, that’s all changed. “The artificial pancreas was really waiting on the mobile phone industry, as well as the sensor,” Damiano says. “Now we have smartphone technology with an app ecosystem that made it possible to just drop these programs on your phone.”

Damiano says he’s on schedule to get FDA approval for his iLet device in early 2019, in time for his son’s sophomore year of college. The system would manage basal insulin like other artificial pancreas devices, but with an added bonus: It won’t require manual bolus doses, instead sensing the post-meal rise in blood glucose and adjusting insulin accordingly.

There’s much work to be done, of course. “It’s still an inelegant system,” Damiano says. “It’s a device you carry with you 24-7. It’s plagued by needing battery power, needing to change infusion sets, cartridges, and sensors. But it’s the best solution we have today. Ultimately, there will be a biological cure for diabetes, and this device will be the bridge to that cure.”

Nuts and Bolts

The artificial pancreas is groundbreaking, but it’s still at the gawky adolescent stage: It depends on current insulin formulations and pump delivery methods. With that in mind, smart people and companies around the globe are focusing on these two areas ripe for advancement.

Better insulin infusion. Artificial pancreas systems rely on infusion sets to ferry insulin into the body, but those come with their own challenges: variations in absorption at different skin sites, tissue damage from poor site rotation, kinks in cannulas, and adhesive failure. Proper insertion technique and site management are crucial—but are subject to human error. And until longer-wear products are a reality, users still need to change their set every few days.

Faster insulin. The rapid-acting liquid insulin analogs currently on the market are speedy. But especially with the absorption issues mentioned above, they can’t quite compete with first- and second-phase insulin release by a healthy pancreas in response to eating.

Different Technologies

Here are some of the different systems researchers hope to roll out in the next few years:

Hybrid Closed-Loop: This setup fully automates basal insulin doses but still requires carb counting and input from users to confirm correction insulin doses and mealtime boluses. Because such systems are the least ambitious, they’re the first to make it through the Food and Drug Administration’s clearance process. The first device in this category to hit the market is Medtronic’s MiniMed 670G. Also in development: Insulet’s tubeless Omnipod Horizon Automated Glucose Control System, Bigfoot Biomedical’s Bigfoot Smartloop, and Tandem Diabetes Care’s inControl.

Closed-Loop: When they’re commercially available, possibly in early 2019, the algorithms in these devices will be able to sense and bolus for mealtime blood glucose surges and other irregularities by themselves. The Beta Bionics iLet is an example of a closed-loop system.

Dual-Hormone Systems: Damiano and others are working on artificial pancreas systems that can administer both insulin and glucagon, the hormone that tells the liver to release stored glucose into the bloodstream to replenish low blood glucose levels. Ideally, such devices will be able to prevent both blood glucose highs and lows, further reducing the burden of care for people with diabetes. But these are still several years from commercial approval. “A bi-hormonal system is more complicated to develop,” Damiano says, “but it provides a simpler end technology to the user.” One hurdle: There’s no commercially available or FDA-approved stable liquid glucagon—yet.
