I think it very much depends on the person and how they age.
Some people are geriatric at 65, or die basically of old age at 60, so defining middle aged as 40+ doesn't make sense for everyone...
I look about the same as I did as a teen at 36. If I wasn't so disabled in various specific ways, I would be the picture of health and youthful virility. Even as disabled as I am I would be hard pressed to consider myself middle aged, I don't even look like I am out of my 20's.
I think middle aged should be somewhat defined by biological markers of decline, and somewhat defined by lifestyle. "middle aged" should be able to communicate useful information as a descriptor, so whatever that communicates to people should likely be some prerequisite to labeling someone that way.
