DOES ANYONE WANNA PLAY A GAME??? (This is the post I was just yapping about.)
So we’re playing a game! Perhaps, if anyone wants to play!
For reference, we have all gotten pretty used to me having one fic per day of the week, with exclusion of Sundays, bc I gen hate posting on Sundays. (I don’t wanna talk about it!<3)
But. As of more recent months, I’ve shackled myself to behemoth fics that realistically, y’all would be waiting for new shit for millennia if I start posting shit in the order I’m writing it. So that’s just not gonna work! (I also do not want to be shackled to ao3 forever, but I cannot just write shit and leave it to rot in my google docs because I am not evil. Sharing is caring.)
So without saying any more than that (bc I gotta leave y’all with some mystery), I would like y’all to help me narrow down this list of twenty to ten. (These are all of my open concepts that I’m willing to touch at the moment. There are a few more sprinkled about, but I don’t even wanna think about them rn.) I have listed titles, if I have them (if not, it's noted) and brief either descriptions or yaps. Literally just throw numbers of shit you wanna see or specifically think I should not be working on first at me. Do not care. Whatever floats your boat. <3
-
A Girl and Her Dog - Bebe runs into a lunatic and a dog while trying to complete a task (there’s actually a leak for this in my test fic).
Ca$h4Cultz - The main four accidentally manifest a god after starting a cult. (Again, leaks are in my test fic. This one might not be super long, but whenever I think about it, it makes me laugh really hard.)
Tweek’s Eclipse?? - I don’t have a title for this yet. It is literally Tweek’s story from Eclipse. Including the shit before. I’m not super invested in it yet, but the first chapter I did write kinda hits. There’s leaks for it somewhere in my slew of posts on here.
What Are You? (not sold on this title) - Tweek meets an imp after spending his whole life alone. He doesn’t really care if the imp takes his soul, but for some reason, he can’t. This aggravates the imp to no end so now he’s just dragging Tweek along to figure out exactly why he can’t take Tweek’s soul. (The imp is Craig. I personally think what I have written so far is fucking funny. It’s not gonna be good, but I stand by it.)
Son Of Satan - Craig meets an imp. Or at least, that’s what the imp’s calling himself. (This is probably gonna be a fun nonsense piece with no legitimate plot and will probably get thrown on the trashfic pseud, but it will be fun. The entire fic is concepted off of one single piece of fanart I saw, if I can find it again, I will gladly share.)
Untitled mind reader fic - I do not have a set title for this because I’m only two chapters deep. Basically, Tweek has to deal with some shit after someone starts reading his mind and decides to speak up.
Untitled Horror Duo- Okay, so neither of these have titles yet, but basically, it’s two fics. There is the main story and the prequel. They both line up though in the way that I have the guides structured. These are so old, but I have like 30k between the two of them and I kinda don’t want to just leave that to rot, y’know? The main storyline follows Tweek who just entered high school after being locked away his entire life. He’s possessed by something, making adjusting to normal human life ten times more difficult. But still, he manages to find friends that care about him and want to help him, despite everything. And the prequel actually follows Kenny who’s had some close encounters with the something that’s possessing Tweek in the main story. I think this could be really fun. I remember being very deliberate about weaving these stories together when I was working on it and it was fucking ANNOYING!!! But it might also be really bad (I haven’t touched what I have since around March so I gen don’t know???), so I may assign it to  the trash psued. BUT IT’S AN OPTION!!!
Three Kings Of Chaos - Chaos Inc. has three leaders. General Disarray, Major Mayhem, and Professor Chaos (of course). But being super villains can be complicated. Especially when their angel donor decides he wants to join in on the fun. (This will realistically probably not be very good. But, I find the concept to be very entertaining.)
Then We Won’t (I’m not sold on the title) - When a kid gets outcasted for his powers, he makes an unlikely alliance. (This entire fic got concepted around the saying ‘The child who is not embraced by the village will burn it down to feel its warmth.’)
Untitled Civil War Fic - Basically, C&F are all evil. The Freedom Pals are just trying to get their friends back. Just about every ship in this winds up getting thrown in enemies to lovers territory. Except for Butters and Bebe (and I personally think their storyline is fucking hilarious).
New Beginnings Rewrite - I HAVE VERY LARGE IDEAS FOR THIS. THE OLD ONE IS STILL UP SO I WILL NOT BE ELABORATING.
Human Island - (no set title) After the world is destroyed, humans are limited to a small chain of islands. But what happens when a scientist threatens to destroy the harmony they’d worked so hard to build? (This is loosely based on something and if someone were to catch it if I were to write and post this, I would lose my fucking shit. Like, I think I would be so happy I would, like, throw up.)
Vents - (no set title) SOMEONE gets thrown into a psychiatric facility. Involuntarily, of course. They assume they’re just abandoned there to die until someone starts calling out to them through the vents.
I might actually finish Stalkers (this is not the title but I’ve been referring to it as such since it got concepted). Heads up though. This one is trash. Like out of everything I’ve ever written, this is thee trash. It was actually the precursor for Leap Year in the sense that I wanted to write something problematic for the sake of being problematic. It will be on the trash fic psued. Like, I genuinely find it funny because of how bad it is. But I can’t recommend telling me to work on this, because it genuinely isn’t good and I stand by that.
My Love Letter To You - There are not one, but two Professor Chaos’ in this story. How fucking wild. Wonder how that happened. This story is basically the set up to them fighting it out and what led them there. (I’ve been eyeing and fiddling with this for a while and if you read my end notes you can prob figure out when I started messing with this concept).
Untitled Cartman and Kenny FP fic. - Okay, so this was actually one of my first fanfic concepts. Basically, Cartman spends a good amount of time blacked out. When the other Freedom Pals find out, they try to intervene. And of course, Kenny has something to do with it.
Cntrl: Escape - I think this might genuinely be my favorite out of all of these. A group of people get elected to escape their “perfect” world. (There is a tyrannical ruler and AI involved. Also the opening scene rn is artist Tweek trying not to lose his shit over the AI art contests surrounding his block. I love it. This one’s prob Tweek centric, but there are six(?? I’m not checking my notes. Approximately six???) mains in this.) (There is also a tiny leak of this on my test fic.)
Untitled Kyle Supersomething Fic - As states. Human Kite’s either gonna be a supervillain or a vigilante who does not fuck with the rules of hero society and does things his own way. Could be fun. Oh also, from the jump I have decided the bad guys (Kyle) win in this. Like, I feel like villains winning is severely lacking and I need to contribute to what I want to read because that is the entire reason I started writing fanfic.
Untitled Evil Shed Fic - And our favorite gadgeteer superhero is not a superhero anymore. He’s much worse. Stan is irredeemable in this, or at least that’s where it’s taking shape in my head right now. This is prob gonna border on horror if not be straight up horror. I do have plans to make all of the people I really focus on character wise (that isn’t Butters, obviously) have their own villain fic at some point, but rn all I have buzzing around my little idiot brain are Kenny (which is done), Stan, and Kyle’s. (Wendy’s is also starting to take shape in my head, but not enough to concept it out yet.)
New Kid - A small town (South Park, obviously) falls into chaos when an alien stumbles upon them. This is New Kid centric without it actually being New Kid centric. I have been giggling about this concept since the idea graced me back in March when the warm weather first popped around and I heard the song ‘Trailer Park Alien’ for the first time. Which is exactly what this fic is based on.

🎀 update to redeem myself
first of all i just finished catching up with yfi786 and i LOVE IT (i hope i got the numbers right LMAO) it’s so amazing and this latest chapter was so good lis! ur always putting out amazing stuff for us 🤍🤍
second of all, the marcus thing. I KNOW ITS BAD, but trust me im not trying to have anything with him especially if hes currently with his girlfriend and i’ve even told him that i dont want to do anything at all with him if hes still “with” his girlfriend. it feels morally wrong and he would be doing her dirty even if she treats him like shit and cheats on him constantly. just recently me and my friends went to the beach and he’s a lifeguard there for the summer and i was getting drinks for me and my friends and he saw them so he was like you know catching up with them and stuff and out of nowhere he started asking questions about ME like where am i, how have i been because i’ve left him on delivered for like a week and a half. and it wasn’t until he saw me come up that he started talking to me and paying attention to me. and it’s just so weird because he’s unfollowed his girlfriend on every platform and even deleted pics of her but she still has them up? it’s so odd. i don’t want to be anything with him though because i know it’ll hurt me in the long run even if he does like me. like it would be so much easier to let him go if i hated him but i have no reason to because he’s so good to me :( AND when he asked me to the formal he wasn’t with his gf HE WAS BROKEN UP WITH HER.
my summers been good you know, i’ve mainly spent it at home because this school year was soooooo stressful i just need time to have a break and kind of recharge my social battery.
recently i’ve been writing a LOT more, like that fic i told you i had in my google docs girl it’s GOINNNNN.
how have you been lis! how’s work treating you :(? also that vacay sounds FUNNN where yall going??? AND I LOVE NEW PCS. i’m so excited for back to school shopping (im a stationary SLUT) -
hihi 🎀 anon!! how have you been?? 🫶 i hope life's been treating you well 🌷 !! also you did get the numbers right!!! funny story it was supposed to be 768 initially but i was the one who messed up the numbers 💀 so i'll never put it past you if you happen to mix them up!!
this guy is so confusing omg 😵‍💫😵‍💫 why did he get back together with the gf if she cheats on him sm?? especially if he started paying more attention to you after he broke up with her ?? i'm so sorry ml i would be so overwhelmed and lost if i was in your shoes rn 🫂 i feel like this guy needs to sit down and figure out wtf he wants because he cannot be wavering from side to side !!
no i totally get that 🥲 my social battery always DIEDD at the end of the semester and thankfully all my hometown friends are the same because they'll be like yeah let's hang out in a few weeks after we have a social battery again LOL but i hope you enjoy the rest of your summer !! 💗 AND OMGGG tell me more about the fic you're writing if you don't mind sharing 🤭
also work's been stressful but we're getting by!! 🥲 i also just get fatigued sometimes when it's hot so rn the weather has me like 😴😴 and im going to miami !!! 🥰 super excited because this friend group i'm going with hasn't been on a trip together in soooo long :') AHHH SO MANY NEW PCS i recently bought some jeonghan and jay pcs im 🧎‍♀️ I LOVEEE STATIONARY SHOPPING but mind you i've used like the same mechanical pencil throughout college i just kept replacing the lead 😭😭

hi earth! i was wondering if you possibly had any advice for anyone who wants to start a writing blog or general tips. if you’re comfortable with it, it would also be cool to know what your writing process is like! you write very well so i’m just curious. thank you for taking the time to read this and i hope you have a lovely day!
first of all, this ask was what got me out of bed in the morning to go to my laptop where i did nothing for like 2 hours.
ANYWAYS I’ve been awake for seven hours now and i still haven’t answered it LOL, but thank u for taking the time to pop into my inbox anon!! so honoured u thought of me 🤭🤭
i’m more than happy to answer any questions u may have!! i’ve only been writing here for like one and a half years but I’ll gladly share some tips for new writers <3
tips for new writers - writing on tumblr in general:
formatting and your writing style is very important!!! people most of the time will like stories that have good grammar and are easy to read, so know your punctuation- duh, i mean, most people know where to put their commas, full stops, and shit, but as even a native English speaker i still need to double check sometimes 😭😭😭
ALSO, figure out if you’re a writer who likes to write with proper punctuation or in lower case like i am rn. people on tumblr don’t care which one you opt for so it’s a matter of personal preference!!
personally, i like to use proper punctuation for longer fics with more plot, and lower case writing for normal drabbles, headcannons, etc, all up to you :3 depends on what i feel like tho, i just like the look of lower case hehe
also, you can totally write drafts on tumblr or another platform - i switch between docs and tumblr. shorter pieces on tumblr and longer pieces on Google docs !!
how to get attention on tumblr as a new writer - the importance of tags:
USE THE TAGS - DON’T BE SHY !!! USE A VARIETY !!!! it’s so easy to get reads on tumblr if you just know your tags. i also will say: be mindful of whether or not you stay in tags bc your post can suddenly just disappear- this has happened to me so many times 😭😭 to do that just search if your fic is still popping up in the ‘recent’ section of the tags AND EVERY TIME AFTER U POST SOMETHING, TAKE THE TIME TO SEARCH IF IT’S IN THE ‘RECENT’ TAGS.
(this part might make zero sense, but if it’s been a couple hours and your post still is not popping up, you can either edit it again, make no changes, and press ‘save’.)
general tips:
have an aesthetic layout.
obviously it’s not ‘essential’, but i always find myself more likely to follow blogs that are pretty or have an aesthetics
it takes so long to do but it’s so worth it 😭 if you looks at the fics that do very well, the formatting it very beautiful and pleasing to the eye.
if you’re struggling with a layout, there are so many inspos available on tumblr, just search up ‘blog inspo’ or ‘layout inspo’ and you’ll generally be pretty successful. if all else fail just go to your fav blogs and see how they set up their blog/navigation/aesthetic!!!
having a set colour for your blog can also look nice and less chaotic. for example; mine is red and it’s my (usually) go-to colour for a lot of stuff. just make sure u like the colour tho ☝️
also don’t be afraid to talk to people! AUTHORS LOVE WHEN YOU COMPLIMENT THEIR WORK!! getting mutuals is such a great feeling so PUT YOURSELF OUT THERE !!!!! MAKE FRIENDS 😮😮
my writing process:
i am that writer that likes to have a plot before i start anything, but it’s never complete. normally when i’m going abt my day, i suddenly think of a piece of dialogue or scenario that makes me go ‘hold on. let me write that down’.
for example, recently i thought of a piece of dialogue for an angst fic (that i don’t know if i’ll use) that goes:
‘no i’m not upset that you forgot about our anniversary, in fact, i already predicted you would.’
another idea i had was: you don’t ever want to leave nagi seishiro hungry.
sometimes I get these ideas when i’m in bed like bro 😐 let me rest…
if i feel inspired enough to continue said ideas, i do, but i never force the fic from happening bc then it’ll be mediocre and just… okay. not something i want on my blog 😭
but then i decide the wordlength, how many scenes i want to be in there. for example, my mistletoe todoroki fic i set out for 4k and met my goal. but my itoshi rin Xmas fic only met around 1k when my goal was 2k - sometimes this happens and i cant be mad if i think the story is done there.
it’s never that organised though, if you look at my drafts, it’s a scrabble of words. i jump from scene to scene with big gaps in between that i need to fill in later 😭 but that’s just how i write LOL ! you may be totally different from me which is a-okay :3
then i grind til the fic is done, reread then bam 💪 ! do my usual formatting on tumblr, tags, and then you’ll see the final result!! easier said than done bc when you want to be done with a fic and just post it, you then need to do all the actual presentation of the fic 😭😭😭
anyways yeah, that’s my process summarised lol!! hope it helped u get a little bit of an idea for what the earthtooz blog grind looks like 🫡
so yeah, i think that’s all i rly need to say!! if you wanna follow my advice or not, up to u, but once again, thank u for popping in my inbox anon and asking me!! gave me something to do whilst on my walk 🤭
cant wait to see what you write, always feel free to come back and ask if you need some ‘extra help’, but i believe in you! good luck and have fun writing, and i hope to see u in the tags someday :D

i was tagged by @katarahairloopies!!! thank u :mwah:
name: leo! @/zeitgeistofnow on ao3, @lazypigeon & @timetohope on here, altho i’m considering uh switching back to not having an art blog :/ i have to think abt it.
fandom(s): ace attorney is my main one rn bc i’m replaying the games with a friend of mine and it’s reminding me how invested i am in the characters!! a lot of my recent fic is atla stuff, altho i’ve been distancing myself from the fandom bc i’ve kinda exhausted my interest in it. finally i’ve been reading a lot of mp100 fic but i don’t think i’ll ever write for it. i just love how dumb all the characters r (with the dubious exception of ritsu)
where you post: ao3!! tbh i always get suprised when people say they write/read fic on any other platform like i haven’t messed around w wattpad or ff.net since middle school... catch up........
most popular oneshot: going just by “one chapter” as the definition of a oneshot, the firestarters, bc it’s fluffy and modern au :) i wouldn’t necessarily call it a oneshot tho bc to me a oneshot shows like, one scene? so like by my definition and your sweet sweet sun makes me crazy (i wanna lay you down and see how you amaze me is my most popular!! (also @ kit u thought UR fic titles were unnecessarily long??? i’ve hit the ao3 LIMIT for characters in titles. it’s about the aesthetic
most popular multichapter fic: sdkjflakjlkj it’s two crowned kings; and one that stood alone, which is a w359 fic i wrote back in late 2017. it’s literally the last fic i haven’t orphaned from when i actually wrote podcast fic (i have 4 other podcast fics but they were all borne out of nostalgia and written after i stopped participating in the fandom). i rewrote all but the last chapter? the last two? about a year ago and i fucked up halfway through so like chapter 6 and 7 are repeated and there’s something missing but i’m too lazy to fix it. no one’s going to read it now anyway :) it WAS the top minlace fic for a little while tho which i take great pride in.
favorite story you’ve written so far: oh that’s a hard question akfsldkfj i honestly like most of them!! and i write a LOT so there’s a lot to choose from. tonight, we are young is def one of my favorites- it was fun to write and i got to explore the ways zuko and yue r similar, which i LOVE to do outside of a zukka/yukka view. you can lean on my arm as you break my heart  is one that i’m really proud of? the whole “cooking as an expression of bato’s love” is definitely some of my favorites. a lot of my ace attorney fics would be categoried as my favorites if i hadn’t improved, too, if that makes sense. like they’re no long my favorites because i can see where my writing is shitty and it bothers me, but if i had written them a month ago they’d be my favorite.
fic you were nervous to post: figures 1-5: killing gods def!! it’s a lot more purple-prose-y than most of my fics and it was also written before i’d kinda like emersed myself in the atla fandom so i didn’t have as good a grasp on the general understanding of zuko’s character as i do now. tbh it’s one i’m rly happy w tho!! i have a few people leave really nice comments on it and rereading them makes me really happy. also it was the start of me hating the position of fire lord and being at least passively anti-it in my fics.
how you choose your titles: they’re almost all song lyrics!! only 14 of my 50 words AREN’T song lyrics and about half of those are from before i started writing ace attorney fic lol. sometimes i go into a fic with a song in mind for the vibes and then i usually go with lyrics from that (like in ‘cuz we’re the greatest /they’ll hang us in the louvre), but otherwise i usually pick an artist i’ve been listening to and go through their songs until i find a lyric that fits. sometimes the lyric doesn’t even really fit the fic and i just chose it at random or because i searching up the word “fly” in my spotify library or whatever. honestly i like coming up with titles? i know a lot of fic writers hate it but being able to just use song lyrics is v soothing for me and while i know that most people won’t search out a song just bc it’s a fic title like.. seeing that the title of a fic is a hozier lyric does affect how i read it and i kinda like that.
do you outline? i outline my long form/multichaptered fics with varying strictness. usually anything over ~8k will have some kind of outline. sometimes i go into it with every single scene planned out, sometimes it’s just notes on the side of the google doc that say “it's about MORE family. about how it's not betraying your existing family to find more” and “scenes i want to include: [...]” and “vampires... ngl kinda hot.” i’m trying to outline super strictly less bc i’ve found it’s less fun? but i do try to keep a plot arc in mind. since most of my fics are more character-driven than plot-driven, that usually just means keeping track of what character development i want to happen or what is motiviating the characters.
complete: um everything posted on ao3 i guess. also the MULTITUDE of orphaned fics out there asksfjldkj i always click ‘leave my pseud on’ so if u look up my username you see all of my fics and then a. lot of other ones.
in progress: - a fic titled ‘dad phoenix’ that is actually just a no DL-6 au with defense attorney miles edgeworth and single dad bartender phoenix where neither of them want to date for A While but phoenix gets wrapped up in one of miles’s cases. it’s about family. it’s about writing teenagers. it’s about the background franmaya which is ALWAYS what i’m here for in wrightworth fics - a franmaya werewolf/vampire au because i’m ~gay~ and love rivals to lovers and also franziska and maya both being angry their older brothers r dating each other. - my secret santa fic!! which i can’t talk about much but it does feature toph and zuko and also piandao and jeong jeong???? idk where they came from but they are Part Of The Fic Now also i forgot iroh existed for half the fic and wrote piandao as zuko’s father figure and now i’m in too deep. - a 5+1 bakoda fic (maybe a bato/hakoda/kay fic??? i need to decide. that’s part of why this fic is still incomplete bc i can’t decide which relationship dynamic i prefer) that’s 5 times bato said he loves hakoda and one time hakoda said it back. possibly i have already written him saying i love u back and i need to change the title a little. - retail au klapollo where klavier works at an overpriced boutique and apollo comes in to buy earrings for nahyuta’s birthday. klavier gives him a punch card (one that the store doesn’t actually offer anymore as a bid to get apollo to come back) and all of apollo’s family come in to use the punch card and also give klavier variations on the shovel talk/find out if he’s actually into apollo. - a LOT of atla fics that i don’t think i’ll ever finish :(
coming soon/not yet started:  - i want to write some blackmadhi bc they’re.. cute..... and it’s a good excuse to also write athena and i love her - my stuff for yueki week!!! i have NOT prepped enough but hopefully i’ll remember in time! i wrote the prompts in a way that kinda set up stuff i’ve already wanted to write (don’t look at me lol) so hopefully i’ll get at least two or three fics finished in time. - i want to rewrite the wrightworth fic i have about them not getting married bc it was interesting and i like what i wrote about but i think i could have written it better and made it more interesting. rewriting fics is hard tho bc i’m never sure if it makes sense to just edit in the new work or to repost it? and then if u repost it do u delete the old one? conflicting so i might just not
do you accept prompts? totally!!! a disclaimer tho i’m not super into writing atla stuff anymore (most of the atla stuff i’m still writing is  something i made a commitment to finish) so if your prompt is an atla one i probably won’t do it :/ basically anything else is fair game tho!! podcasts/aa/sa/uh i don’t remember anything else but like if you search a fandom on my blog and come up with more than two posts about it chances r i’d be happy to write fic for it!
upcoming work that you’re most excited about: oh huh i mean probably the no dl-6 au!!! it’s the longest ace attorney fic i’ve written already and since it’s wrightworth it’ll get more attention than any franmaya fic i write. my standards r so high now tho after getting to much feedback from atla fans... love u all... obviously i have no choice but to pressure my atla mutuals into playing ace attorney. pls ask abt it bc i WIll Give You A Sales Pitch about why you’d like it in relation to atla
tagging: i’m not rly tagging anyone!!! @deadflora if you still consider urself a fic writer also consider urself tagged! also any of my other mutuals who write fic i just can’t think of anyone rn

30. Favorite colour

update!
hi everyone! this is gonna be long so i am sorry in advance :( i wanted to touch base with y’all and ask a few questions since i’ve gained some more followers recently and wanna know your opinions about some stuff.
so first of all i’ve only got 27 more fics queued up at the moment which is wild bc i didn’t think i’d get through almost all of my recs so fast! i queued all of them in like october so they’ve obv lasted a while but its crazy to me omg. obviously i’m not done reading fics lmao i still read in my spare time but i’m a busy college student who also works part time so i don’t have a whole lot of spare time to read :/ so once i get through the remaining fics in my queue expect recs to slow quite a bit, sorry !!!
however i CAN queue up some more stuff but i was curious about what y’all thought first. so rn all of my recs are coming from like this google doc i’ve updated thru the years of my fav fics and rn all of the tumblr/ao3 stuff is what i’ve gone through on here. i do have like 100+ recs i can still go through HOWEVER they are twitter aus. i don’t wanna just post em without asking if y’all actually want those or not since they’re on a different platform and some people are iffy on twitter/social media aus, so let me know how u feel about them and if enough people are interested i’ll post them!
one route i can also go in is if a lot of people aren’t into the twitter au thing but some people still want them then i can just post them in one big masterlist as opposed to posting all of them over time so if you’d rather me do that too lmk!
another thing is that once the remaining ao3 recs i have on here have posted i will update the current masterlists i have for all the groups (which is the oc & y/n stuff) and post new ones for the fics with ships by group as well. i have em ready to go once all the recs are posted so i’ll get those up right after the remaining ones go live!
last thing is that if anyone wants to send me any recs too or wants me to read/look for fics for a specific group/idol/ship lmk! my ask box is always open :)
hope all of you are doing well and a belated happy holidays and happy new year!

Ask D'Mine: Chasing Zebras in Diabetes
Got questions about life with diabetes? So do we! That's why we offer our weekly diabetes advice column, Ask D'Mine, hosted by our snarky diabetes expert Wil Dubois, a veteran type 1 and diabetes author.
This week, Wil is taking on a query about what seems to be false blood sugar lows... or are they? Do fainting spells with diabetes always point to hypoglycemia?
Got your own questions? Email us at [email protected]
Jess, type 1 from Utah, writes: I have an employee who has had type 1 for 10 years. In fact, we have both had type 1 for almost the same amount of time. So we definitely understand each other. She has had these fainting spells. In fact, she passed out on her first day of work. Rather eventful day. Anyway, when she gets faint, she turns ashen grey, gets super confused and uncoordinated and weak. I haven’t had her hit the ground again since the first day, but it’s been close, trying to get her to a chair fast enough.
We check her blood sugar every time this happens because your first assumption is a low. She usually has tested before it happens, we test during and then check again after. I really can’t find any consistent numbers that I can correlate and say Aha! That’s the cause. She has been anywhere from 105-400 when this happens. Now, everyone wants to point fingers at her control being less than perfect. She tests at least five times a day. She uses an insulin pump and she stays pretty on top of it. Her blood sugars do swing and she does her best to chase them. She’s a stressed-out student who is working her way through college.
So I ran across information that extended hyperglycemia can cause a type of neuropathy that affects the heart. But medical people usually say it’s probably not the "Zebra" (exotic diagnosis). She doesn’t seem to have any other complications up to this point. At least, none that have been recognized. But still, what do you think? Could it be the Zebra?
Wil@Ask D’Mine answers: For readers who’ve only seen zebras at the zoo, let me give you some background. In medical school, budding young docs are taught that if they’re walking in the woods in Vermont and hear hoof beats, it’s probably not a zebra. It’s a way of reminding them that while anything is possible, the most likely is, well, the most likely. So if their first thought is that their patient’s symptoms match pseudomyxoma peritonei, they should probably rethink their diagnosis, as pseudomyxoma peritonei is one of the rarest diseases in the world.
In general, I think that the Zebra lesson is a good one for doctors, especially new ones with more knowledge than experience. It helps avoid wasted time barking up empty trees. It’s probably even better advice for average citizens now that we all live in the Age of Google. For instance, if I noticed that my waistline was gradually expanding, I was having tummy aches, and my appetite was down, Uncle Google might lead me to believe I had pseudomyxoma peritonei, as those are some of its symptoms.
Of course they’re also the symptoms of eating bad corn dogs and two hundred thousand other things, including Zebraititus Hypochondriasis, all of which are more likely than pseudomyxoma peritonei.
OK, I made that last one up. There’s no such thing as Zebraititus.
So I can’t fault the docs for looking to low blood sugar as the cause of a young T1’s fainting spells. It’s the most likely cause. Still, the hoof beats in the Vermont forest could be a Zebra. Maybe a circus train derailed, freeing all the animals. Maybe misguided eco terrorists freed one from a zoo. After all, just because pseudomyxoma peritonei is one of the rarest diseases on the planet doesn’t mean people don’t get it. They do.
For years I had a zebra Xing sign above the door of my clinic office. It was a way of thumbing my nose at medical tradition and a reminder to keep an open mind at all times. I know that Zebras happen. I’ve seen it personally.
But even working under a zebra crossing sign, I’d still look to blood sugar as the key to your young employee’s woes. It sure seems to fit the bill, if not all the evidence. So the first thing I’d troubleshoot before I went zebra hunting is her meter. Is the meter working correctly? Is it giving us good, reliable data? There are a lot of crap meters out there and health insurance plans are forcing more and more PWDs into using these shady pieces of gear.
And it’s worth noting that many of the poorer meters give the least accurate info in the low blood sugar range.
So my first advice to you is to expand your struggling college student’s employee benefit plan to include one really good meter and 100 strips to go with it. Look at it as a potential investment in increased productivity. Let’s not rule out the most likely cause until we’re really sure it’s not the issue. Keep in mind that none of us have a universal lights-out point. Each and every hypo affects each and every one of us differently. Some of the contributing factors are the speed of the blood sugar drop, the amount of active insulin in the blood, the type, nature, volume, and timing of the most recent meal, and the baseline average blood sugar of the PWD. That list doesn’t even include stress, sleep hygiene, illness, and a million other factors. My point being, don’t assume someone has to be at 35 mg/dL to pass out.
But if the tree I’m barking up turns out empty, before I looked at heart neuropathy, I’d look to a different place in the bloodstream, and that’s volume and pressure. Why? Well if the hoof beats weren’t from a white-tailed deer (common in Vermont), the next most likely hoof beater in the Vermont woods is a moose.
Low blood pressure is a common trigger of syncope—a Zerbra-esque word for a temporary loss of consciousness. It happens when the blood pressure drops low enough that the brain doesn’t get quite enough oxygen. Why would that happen in a young, healthy-ish person?
I can think of two common causes. Is she on the “recommended” ACE inhibitor that all D-folk are supposed to take if they need it or not? It’s the standard of care: All of us sugar-impaired people take this med, as it can shield our kidneys from the ravages of elevated sugars. But they are blood-pressure lowering meds. If her dose is just a little too high, it might trigger syncope from time to time.
The other common cause of syncope, which can exist hand-in-hand with the ACE inhibitor, is dehydration. Does she drink lots of water? And, hey, coffee doesn’t count. Ingesting large amounts of caffeine can have a diuretic effect, mildly dehydrating the body and lowering the blood pressure. As a struggling, working college student, I imagine your girl might be a caffeine addict.
But if that’s not it either, well, what about your Zebra? The neuropathy of the heart? Could that be the cause? Yes, but… Well, I’ll get back to the “but” in a second. This type of neuropathy is called cardiac autonomic neuropathy. And you are correct that it can trigger fainting.
So why am I so quick to rule out something that seems to fit the bill?
Because I don’t think she’s had diabetes long enough to get this “side effect.” She’s only had diabetes 10 years and you told me that’s she’s “pretty on top of it.” Even in poorly controlled type 1 diabetes, ten years is the earliest you’d see any long-term complication pop up, and the autonomic neuropathies aren’t typically in that first wave.
It takes a lot of high glucose over a long time to create that kind of damage.
So to answer your question, just like a zebra could be in the Vermont woods, she could have this side effect earlier than most people get it.
But my money is on it being the deer. Or the moose.
This is not a medical advice column. We are PWDs freely and openly sharing the wisdom of our collected experiences — our been-there-done-that knowledge from the trenches. But we are not MDs, RNs, NPs, PAs, CDEs, or partridges in pear trees. Bottom line: we are only a small part of your total prescription. You still need the professional advice, treatment, and care of a licensed medical professional.

Ask D'Mine: When You Need Support, The "Where's Waldo" of Diabetes Care

Ask D'Mine: When You Need Support, The "Where's Waldo" of Diabetes Care
You can't do diabetes alone, right? In this week's edition of our weekly diabetes advice column, Ask D'Mine, your host, Wil Dubois, (veteran type 1, diabetes author and community educator) delves into the importance of support from family and friends, and pushing your healthcare team to make sure you're getting the answers you need!

;
Sheryl from North Carolina, type 2, writes: Throughout my life I was always thought to be "the strong one" by friends. While I loved being dependable, reliable and the "rock" when others needed help, what I lacked in my life was the option to be vulnerable and the ability to ask for and accept the help of others. My recent bout with diabetes has reinforced this need. There's probably no single answer to this question, but how do you ask for, and accept, the support and assistance of others? I'd just like to hear your thoughts.
Wil@Ask D'Mine answers: Actually, there is one single answer: Be humble. You ask for support and assistance by simply asking for it. Of course, it helps to ask the right people—more on that in a second. But you put your finger on the much harder issue, accepting the help of others, and I think this is especially true for people like you who have functioned as the "rock" for others.
But consider this: Being humble doesn't make you weak. Being humble doesn't change who you are. Being humble doesn't make you undependable or unreliable. We all need the support and assistance of others. We're social animals, after all. And being humble doesn't make you vulnerable, either. All it does is make you wise.
Actually, I think you're halfway there already. You yourself have admitted that you need the help of others. That's a big step. And while that can be true for life in general, it's doubly true for diabetes. Hey, diabetes is a team sport from top to bottom. You can't do this alone. Your doc has help from a wide variety of specialists, educators, nutritionists, exercise folks, dentists, eye docs, and maybe even a shrink. And you need your family and friends on board.
But your friends and family, while they need to be on board at least passively, may not actually be the best people to ask for support and assistance from, depending on their level of willingness and capacity for empathy. The very, very, very best people you can get support and assistance from are others of your own kind.
No one can understand diabetes better than another person who has it. When you reach out to another PWD, even if you don't know that person, you're not reaching out to a stranger. You're reaching out to someone who's walking in your shoes. Someone who understands you. Someone who "gets it." And most importantly, someone who loves you on the basis of your shared challenges.
The other advantage of reaching out to your own kind is that these new folks whom you share so much with don't know your reputation as "the strong one," or the "rock." You can afford to be vulnerable with them. You left your baggage at the door when you entered this new world.
There are lots of places you can find support, especially as a type 2—simply because there are so many more of you than there are of us type 1s. You can start with your doc. Ask if he or she knows of a good local support group. Or you can check in with (not into) your local hospital. Or your church. Or your chamber of commerce.
And if you live somewhere so obscure that you're the only person with type 2 diabetes, and I can't imagine where that would be... Ice Station Zebra?... then may I suggest the internet? We type 1s are great lovers of the internet because it's our home-away-from-home. We're the victims of a genetic diaspora. We're so few and so far spread out that in many places there aren't many of us around. Of course, while most places in the country have type 2 support groups, not everyone is comfortable connecting with other persons with diabetes in person in their own communities for any number of personal or professional reasons, so it's OK for you to connect with your own kind online instead.
One great place to start would be at our partner community Diabetic Connect. Or boot up a browser. Why, all you have to do is enter "diabetic support" into Google to get 41,900,00 hits. Oh. Wait a minute. 40,900,000 of those are for diabetic support socks. Oops. Most of us don't actually need those, but that's a topic for another day.
So, I think you're off to a great start. Admitting that you need support and assistance took strength. Taking that first step to reach out to others will take strength, too.
But I know you can do it. Because you're still the rock.
Mark from Indiana, probable person with pre-diabetes (new designation? A PPWPD?), asks: Is a blood sugar of 250 right after a meal normal? I'm not sure I have diabetes, I had blood labs done and my blood sugar was 175, but could not remember if I ate before the test. So I bought a meter to monitor my blood sugar. Oh, and my blood sugar drops back down again after two hours.
Wil@Ask D'Mine answers: Ummmm... No. Not normal. But how abnormal is an open question, because where your blood sugar be should right after a meal is like one of those "where's Waldo" games (trying to find a needle in a haystack). No one really knows, because for the last umpteen years, the medical research types have been looking for Waldo two hours after a meal, not right after it.
But maybe we can ferret an answer out for you by reading between the lines.
Back in the bad old days, like three years ago, we used blood sugar numbers to diagnose diabetes. Now, we use A1C scores instead. But when we still used fingersticks for diagnosing, this was how the game was played: If you had a fasting fingerstick above 126 mg/dL on two different days, you had diabetes, and we gave you a meter and your official "I have diabetes" T-shirt. Welcome to the club!
Or... if you had two random fingersticks above 200 mg/dL you got the meter and the T-shirt. Random simply meant not fasting, but was generally viewed as two hours after eating. Even further back in the past, diabetes was diagnosed using oral glucose tolerance tests where blood sugar was measured precisely two hours after drinking medical Kool-Aid.
Meanwhile, if you had two fasting fingersticks between 100-125 you had pre-diabetes, which is sort of like being a little bit pregnant. You also had pre-diabetes if you had two random fingersticks between 140 and 199. Remember, 200 and over with random readings, and you had the Big D. Oh, and if your random was under 140 you were assumed to be "normal."
There were any number of problems with this system, not the least of which was once you had the first 200+ reading, convincing the potential new inductee to our club to come back for the confirming fingerstick was problematic at best. All-in-all, using the A1C is simpler and a slam dunk. The A1C test isn't affected by the Big Gulp the patient drank in the clinic parking lot, meter accuracy, or unusual stress on a given day because A1C measures the average glucose environment of your blood for the past three months. The other good thing about using A1C to diagnose is not having to keep track of all those numbers we just went though. The threshold for diagnosis of diabetes is a single A1C number of 6.5, at which point your average BGL has been 140 for months.
There's than 140 again.
Why? How did 140 get chosen, you ask? You might not really want to know, but it was chosen by expert Consensus Panels again. Actually, as reported by Dr. David M. Nathan, et al, in the ADA's look at postprandial glucose readings, most research into the subject has shown that in non-dFolk, glucose peaks about 60 minutes after eating, rarely goes above 140 mg/dL, and usually returns to fasting levels in 2-3 hours. So if you are still at or over 140 two hours after eating, something is amiss!
About. Rarely. Usually. That's a lot of qualifying words.
Well, in no human endeavor can your mileage vary more than when eating. What did you eat? How fast did you eat it? What time of day did you eat it? How long was it since your most recent meal? And so on and so forth.
Still, if non-dFolk usually peak at no more than 140 about an hour downstream of a meal, and you are hitting a 250 right after eating, it does appear that something is eff'd up. Big time. If you're seeing a 250 right after a meal more than once it would suggest you do have the Big D. That's based on the fact that you shouldn't even peak for about an hour. If you're already at 250 right after eating, you will go even higher. What size T-shirt do you wear, Mark?
It's interesting, however, that your body rallies and your blood sugar comes back down by the two-hour point. Something is eff'd up, alright, but something's working right, too. As to your 175 on your labs, well, if you were fasting, again, that would suggest the Big D. But if you'd eaten that day, it would suggest pre-D.
You should probably hook up with your doc again (not in that way) and just get the damn A1C test run to resolve this once and for all, but in the meantime you can have some fun with that meter your bought. Use it to embark on a voyage of self-discovery. Test regularly at bed time and the next morning and see if your blood sugar goes up, down, or stays level overnight. Test before you eat various meals and at the magic two hours after the first bite to see how much a change there is.
Let us know what you discover!
