In the US, these are the types of mental health professionals there are and what they do (if you're outside the US, tell me, and I can give you a rundown of the mental health professionals in your country). It's complicated and can vary from state to state, so I don't blame you at all for being confused.
Counselors, Clinicians, Therapists: these people usually have Master's degrees in a mental health-related field such as psychology, counseling psychology, marriage or family therapy. They're trained to evaluate a person’s mental health and use therapeutic techniques based on specific training programs, but they can't prescribe medication. They're licensed, but their licensure requirements are different by state, and these titles are not always protected (meaning that in some states, anyone can call themselves a "therapist"). These people should hold one of the following licenses:
LPC, Licensed Professional Counselor
LMFT, Licensed Marriage and Family Therapist
LCADAC, Licensed Clinical Alcohol & Drug Abuse Counselor
Clinical Social Workers: these people are trained to evaluate a person’s mental health and use therapeutic techniques based on specific training programs. They are also trained in case management and advocacy services. They cannot prescribe medication. They're licensed and typically have a Master's degree in Social Work, although some states allow them to just have a Bachelor's degree.
Psychologists: Psychologists have Doctoral degrees (PhDs or PsyDs) in clinical psychology. They are trained to evaluate a person’s mental health using clinical interviews, psychological evaluations and testing. They can make diagnoses and provide individual and group therapy. Some may have training in specific forms of therapy like cognitive behavioral therapy (CBT), dialectical behavior therapy (DBT) and other behavioral therapy interventions. In some states, psychologists are allowed to become "prescribing psychologists". This means that they can do additional training to get licensed to prescribe medications. Louisiana, New Mexico, Illinois, Iowa, and Idaho allowed psychologists to prescribe medication.
Psychiatrists: Psychiatrists are licensed medical doctors. They have went to medical school, have MD degrees, and completed their residencies in psychiatry. They can diagnose mental health conditions, prescribe and monitor medications, and provide therapy. Some psychiatrists are also psychologists, and conduct therapy sessions in addition to prescribing medication. Titles that are not always protected: psychoanalyst, psychotherapist, therapist, counselor, mental health counselor, and life coach (or anything with "coach" in it). Most people using these titles do have training, but make sure you check to see what they're licensed to do and if their license is still active.
The combination of professionals that works for you will vary depending on your unique needs. Most people see a psychologist or LMF therapist first and do some form of talk therapy to see if it improves their symptoms. A psychologist may make a mental health diagnosis if necessary. An LMF therapist can "evaluate" mental health but isn't really supposed to make medical diagnoses, although in practice I think they do.
From there, a person may be referred to a psychiatrist if it seems like pharmaceutical intervention might be necessary for the diagnosis. The way that this happens depends on how your insurance coverage works. Sometimes therapists can refer patients to a psychiatrist directly, sometimes you need to see your primary care physician and they'll prescribe you medication directly or refer you to a psychiatrist, and sometimes you can just schedule an appointment with a psychiatrist yourself (isn't the American healthcare system fun?). Check with your insurance to see what is and isn't covered and what the correct procedures are.
Once you make an appointment with a psychiatrist, they can prescribe medication and monitor medications as needed to find the correct medication and dose. From there, a person may keep seeing a psychiatrist (if their insurance will cover it) or may see their primary care physician, a family nurse practitioner, or a mental health nurse practitioner who can continue to prescribe their medication. If a person finds that their medication makes all of their symptoms go away, continuing to see a psychologist or therapist may not be necessary, but many people do continue to see both a therapist and a psychiatrist. People who see both a psychiatrist and a therapist typically have better outcomes than people who see one or the other. When looking for a psychologist or a therapist, the most important thing is if you feel like you "click" with them. As long as they're licensed, the type of license they have or the type of therapy they practice won't make as big of a difference as the relationship you have with them will.

Here is a quick and dirty guide to mental health practitioners so that you can better represent this under-appreciated (and often mis-represented) professional field.
Important terms to know:
In this article, I use the terms “psychotherapy” and “mental health counseling” interchangeably. Basically, they both refer to a specific type of counseling that is used to treat mental disorders and other mental health concerns. I use these more specific terms to distinguish this type of counseling from other types of counseling that are not meant to treat mental disorders (such as pastoral or academic counseling).
“Supervised clinical experience” basically means practicing psychotherapy under the supervision of someone who is qualified to train specific mental health professionals. This is integral to learning how to practice psychotherapy, because counseling is a skill that must be honed through practice. It can’t be taught in school, magically absorbed into the brain with a spell, or programmed into an artificial intelligence. Only hard-won experience can really teach someone how to counsel.
Ethics/Licensing Boards: These are the people mental health professionals must answer to. Each profession has a different certifying entity with different rules and codes of ethics that professionals are required to follow, but in general they have the power to grant, suspend, or revoke licenses.
Treatment Team: This term refers to a group of qualified professionals from multiple different disciplines who work collaboratively to treat patients/clients. The composition of treatment teams can vary, but it often includes such professionals as a medical doctor/psychiatrist, nurse or nurse practitioner, nurse assistant, psychologist, mental health counselor, and social worker.
Types of professionals:
Psychiatrist: A medical doctor who has a doctoral degree (4+ years in graduate medical school) plus clinical training in medical and psychiatric practice (4+ years in residency). They must also pass a licensure exam, complete a set number of continuing education hours, and renew their license every 3 years. Many psychiatrists don’t perform psychotherapy, choosing to focus primarily on assessment and medical treatment of mental disorders, and/or on research. Some, however, do choose to have an integrated medical/psychotherapeutic practice.

Psychiatric nurses do not perform psychotherapy, but may use basic counseling skills to help patients in other ways.
NOTE: ONLY QUALIFIED MEDICAL PROFESSIONALS CAN PRESCRIBE, ADMINISTER, OR RECOMMEND SPECIFIC MEDICATIONS. If your character is a “counselor” or “therapist” who also does any of these things, they are probably a psychiatrist. Other mental health professionals may suggest medication (in general, not specific drugs) to a client as an option for treatment, but will refer the client to a qualified medical professional if the client is interested in learning more. It is unethical for a mental health professional without a medical degree to make specific medical recommendations or to administer medication.
Psychologist: A person with a doctoral degree (4+ years in graduate school) in psychology. Psychologists can be psychotherapists, but not all of them are; some focus only on research, while others work for companies and corporations. If they do practice psychotherapy, they must have clocked 3,000 hours (or about 2 years) of supervised clinical experience and pass a licensure exam. License will need to be renewed every 2 years and requires a set number of continuing education hours.
Mental Health Counselor: These are professionals who have a master’s degree (2+ years in grad school), have clocked 3,000 hours (or about 2 years) of supervised clinical experience, and have passed a licensure exam. Licensed Professional Counselors (LPC)

Licensed Clinical Social Workers (LCSW)
Social Work: Social workers are difficult to summarize as they do a lot of different things and can work pretty much anywhere there are people, so here is a very abridged version of professionals in this field.
A caseworker or case manager is someone who manages individual clients in various settings, helps them access services, provides them with information, and advocates for their needs. They are required to have an associate’s or bachelor’s degree in a field related to health and human services (e.g. social work, psychology, nursing, etc). Their employers might encourage or require them to pass a case manager certification exam.
A Licensed Master of Social Work is required to have a master’s degree (2+ years) and pass a licensure exam. LMSW’s often rely on basic counseling skills, but do not perform psychotherapy. Most of this won’t be stuff you include in your fics. I write fiction, as well, and I know that this sort of information is rarely, if ever, necessary for the plot. It is, however, important for writers to keep in mind if they are writing characters who might use the services of these professionals (or who might be these professionals).
Representation matters, and it’s so important that mental health professionals are portrayed in ways that do not undercut their skills, training, or value to society and to the people they help. Sometimes, proper representation is hard and the terminology is confusing, but that doesn’t mean you should settle for poor representation. Mental disabilities are highly stigmatized, and people who have them are socially oppressed in many ways. But content creators can be on the frontlines of pushing for positive change by way of equal, accurate, and compassionate representation in media. That takes some extra work, but it is well worth the effort. It could literally save lives.

People have been trying to talk other people into feeling better for more than 3,000 years. Ancient Greeks counseled patients suffering from addictions and psychoses. For nearly 2,500 years, Buddhists have handed down a canon of advice to people struggling with day-to-day emotional suffering. But it was only at the end of the nineteenth century that the Austrian neurologist Sigmund Freud popularized the “talking cure.”
These days, more than 130,000 licensed US psychiatrists and psychologists provide what is often a blend of Freudian-ish psychodynamic therapy (focused on childhood experiences) and cognitive behavioral therapy (focused on challenging negative thought patterns). But you can also receive some version of talk therapy from a social worker, counselor or marriage and family therapist — or conceivably even from a life coach, mentor, hypnotist, neurofeedback practitioner or shaman (see sidebar below).
In 1936, the eminent US psychologist Saul Rosenzweig proposed that a therapist’s preferred method was all but irrelevant. Instead, he wrote, factors common to most therapies will determine the outcome more strongly than any of the therapy’s specific features.
“Given a therapist who has an effective personality and who consistently adheres in his treatment to a system of concepts which he has mastered and which is in one significant way or another adapted to the problems of the sick personality, then it is of comparatively little consequence what particular method that therapist uses,” he wrote.
Rosenzweig began his iconic paper with a passage from Lewis Carroll’s Alice’s Adventures in Wonderland in which a dodo bird, judging contestants in a race, declares that all have won “and all must have prizes.” He thus set the stage for the long-running debate over what has since been known as the dodo bird verdict.
In subsequent years, researchers identified several potential “common factors,” ranging from the therapist’s office environment to the placebo effect, which has a strong role in both talk therapy and medication. Yet at the top of the list of factors is the relationship, what’s called the therapeutic alliance, between patient and therapist.
The reasoning is that this positive emotional bond, made up of trust, respect, affection and high expectations for improvement, inspires each member of the pair to keep showing up and doing the hard work that therapy demands. Specifically, researchers have found, a strong alliance will help the therapist and patient reach consensus on their goals, as well as on what sort of tasks — such as free association, Socratic questioning, or homework — will help achieve them.
Sixty years after Rosenzweig put his stake in the ground, Wampold and colleagues reviewed more than 200 scientific studies comparing the effectiveness of mainstream therapies, mostly variants of psychodynamic and cognitive-therapy tactics used by well-trained practitioners. They found only minimal differences between outcomes, concluding that “bona fide treatments are roughly equivalent.” (That conclusion may not extend to outlier therapies such as shamanism, Wampold and other researchers warn, which haven’t been sufficiently evaluated.)
The bottom line, according to Wampold, is: “You can’t have psychotherapy without a relationship. Every patient knows that. And every psychotherapist knows it.”
In recent decades, researchers have published scores of papers emphasizing the importance of the bond between patient and therapist. In 2018, a task force for the American Psychological Association concluded: “The psychotherapy relationship makes substantial and consistent contributions to outcome independent of the type of treatment.”
In 2019, however, psychologist Pim Cuijpers, a leading agnostic, weighed in with a paper in the Annual Review of Clinical Psychology arguing that we’re still far from knowing how talk therapy works.
Earlier studies have been inconclusive, wrote Cuijpers and two colleagues at Vrije University Amsterdam. They cited flaws including researcher bias in favor of particular therapies and an excess of papers that show an association between a treatment and an outcome without proving which facet of the treatment caused the outcome. What’s more, they suggested, some studies purporting to compare different therapy types tip the scales by comparing the researchers’ favorite with another form of therapy “designed to fail” — in other words not a tested, bona fide strategy.
Solving the mystery once and for all will require much more work, they wrote, adding: “It is as if we have been in a pilot phase of research for five decades.”
In a 2007 review, Yale psychologist and child psychiatry researcher Alan Kazdin voiced a similar complaint, urging researchers to focus their studies on the ways that the patient-therapist relationship may be helpful rather than simply determining that it helps. “How does one get from ‘my therapist and I are bonding’ to ‘my marriage, anxiety, and tics are better’?” Kazdin asked in his paper. “This is a leap with the intervening steps unspecified or untested, at least to my knowledge.” Reached by Knowable, Kazdin said that after 13 years, he still sees this gap in the research.
In recent years, some professional organizations have vouched for some psychotherapies over others, based on a preponderance of evidence that certain methods lead to good outcomes for specific diagnoses. The treatments most often touted as having strong research support have been versions of cognitive behavioral therapy (CBT), which one organization, the Society of Clinical Psychology, recommends for everything from adult attention-deficit hyperactivity disorder to obsessive-compulsive disorder to irritable bowel syndrome.
Critics note, however, that CBT’s clear rules and comparatively brief time frames have made it easier to study experimentally than more complex, open-ended therapies, thus yielding more net evidence that it gets results without proving that it is better or worse than any other therapy. Indeed, in recent years, some experts have challenged CBT’s reputation as the “gold standard,” with research suggesting that longer-term, psychoanalytic therapy can be more effective in some cases.
Wampold argues that a greater diversity of options will allow therapists and patients to find the approach that works best for them. “Unless there’s convincing evidence that one treatment is superior,” he says, “you should allow therapists to give the treatment they can deliver most effectively and patients should have the right to choose the treatments they prefer.”
Patient preferences do make a difference. In a 2018 meta-analysis of 53 studies involving more than 16,000 patients, researchers called such preferences “crucial” to outcomes. The authors advised that psychotherapists should educate patients about different types of talk therapy and elicit their preferences as to the therapists’ strategies.
Bring in the scans
Any resolution to this long-running conflict may ultimately come from the lab, and its so-far elusive promise of delivering indisputable biological evidence of the most effective treatments.
Some researchers are now using functional magnetic resonance imaging scans to track subtle changes in certain brain regions during and after psychotherapy. One day, perhaps, therapists will be able to post before-and-after scans of patients’ brains, highlighting the improvements, much as plastic surgeons do today. But not yet. The studies so far have mainly focused on laying the groundwork by determining which areas of the brain are affected by therapy, says University of Pittsburgh psychiatrist Jay Fournier. Researchers want to pinpoint how the brain changes in each patient as a result of different treatments and what sorts of changes best predict long-term recovery. They also hope to gain a more nuanced understanding of which characteristics in a patient might predict response to different sorts of treatments.
Others searching for concrete markers of a patient’s progress have been measuring neurochemical changes during therapy. “We don’t just ask the depressed patient if he’s feeling better,” says Sigal Zilcha-Mano, a clinical psychologist and expert on the patient-therapist bond at the University of Haifa in Israel. She and her team also measure levels of the stress hormone cortisol, the bonding hormone oxytocin, body temperatures, and the synchrony of speech and movement between patient and therapist.
These measurements, which capture changes many patients and therapists aren’t aware of in the moment, may eventually present a detailed portrait of what can be among the most intense of human relationships. One day, Zilcha-Mano says, they may also enable more precise, individualized, tailored treatments for each patient. It’s a matter of traits and states, she explains. Patients may come to therapy with the trait of being more or less able to trust and form relationships. But depending on the skill of the therapist, they may enter a state of being more open to such a bond. The real-time portrait of chemical changes and movements could someday illuminate this process, particularly in the case of oxytocin, considered to be an important biological marker of trust.
In whatever way it might happen, breaking through the current impasse over what makes psychotherapy effective could help practitioners do a better job, Cuijpers and his colleagues argue. Psychotherapeutic treatments for depression haven’t improved in decades, they note.
They also hope that more cogent explanations for how talk therapy works could help revive the approach, which other critics say has long suffered from a reputation of lacking scientific credibility.
“Understanding how therapies work,” write Cuijpers and his colleagues, “may make it possible to develop treatments that focus on the core processes and are, therefore, more effective and efficient, and more acceptable to patients.”
Skepticism about how psychotherapy works was a problem long before Freud came on the scene. It’s at least as old as the charismatic German physician Franz Anton Mesmer, born in 1734. His practice of “mesmerism” relied on claims that he could manipulate an invisible bodily fluid through “animal magnetism.” Mesmer established a strong rapport with his patients with nervous disorders, many of whom ended up feeling they had been cured.
But he alienated other doctors, and in 1784, while Mesmer was practicing in Paris, King Louis XVI appointed an international commission of scientists and physicians, including Benjamin Franklin, to investigate his methods. They concluded that he couldn’t support his claims of invisible fluids or animal magnetism, ruining his reputation.
Mesmer’s treatment worked — at least for some — but not his explanation of how it did so. Psychotherapy’s quest for legitimacy continues.

what do you mean about karamo from queer eye being a fraud?
I think he may be misrepresenting his credentials as a social worker and therapist. Karamo refers to himself as being a "licensed social worker" and as a "licensed psychotherapist" (psychotherapist is not a protected term in the US or something you can be licensed for). He acts like it's ridiculous that Netflix won't refer to him as a "social worker" or "therapist", but I'm wondering if they can't because it would open them up to lawsuits about his credentials.
According to his Wikipedia article, Karamo went to Florida A&M University from 1999-2002. However, this article from Florida A&M University itself says he was only there from 1999-2000 and majored in "general studies". This article from Penn State University says he has a bachelor's from FAMU, "which he used to work as a licensed social worker and psychotherapist for a number of years before transferring into the public eye," but that seems to contradict the idea that his degree was in "general studies" or that he was only at FAMU for a year. In this article he says he graduated from FAMU, but again, that seems to contradict FAMU's own statement that he was only there for a year. He's also vague about this in his book, where he says he majored in business and doesn't talk about having graduated. In the 2001 FAMU yearbook here, he's listed as being a sophomore Business Economics/Political Science major, and he's not listed on the FAMU 2002 or 2003 commencement flyers that the school put out.
According to this article, Karamo became a social worker after his time on The Real World in 2004. In Florida, where Karamo is from and presumably lived until he moved to LA, you're required to have a Master's to be a Social Worker, which would mean he went to school for at least 5 years, not just the 3 he was at A&M. In Pennsylvania and Georgia, where Queer Eye episodes were filmed, you also need a Master's to be able to practice there as a social worker. In some states, you can call yourself a social worker with only a Bachelor's of Social Work, but you can't be licensed unless you have a Master's degree.
Multiple articles use the specific phrase "Karamo worked as a social worker for nearly a decade before transitioning into media". Karamo started working in media around 2012. For him to have been working for a decade before that, he would had to have started working as a social worker around 2002, before he was on the Real World. And he must have been doing so in a state that doesn't require an MSW (so not Florida) in order to be a "licensed social worker". Karamo moved to LA in 2011, again a state that requires a Master's degree to be licensed as a social worker, so he wouldn't have been working as a social worker from 2011 onwards.
Plus, there are registries where you can check to see if someone is licensed to practice social work. Karamo doesn't come up if you search him in the database of licensed social workers in Florida, New Jersey, Georgia, Texas, or California. The only hit that comes up for Karamo at all is a Valley Village, California Security Guard's license that was cancelled in 2005. He may have been licensed *at one point*, but I don't think he is anymore.
I'm sure it all checks out, but that means that he's not actually a "licensed social worker" or "licensed psychotherapist", at the very least, not when they're filming in certain states. I wish he would be more upfront about that. It would be fine if he called himself a "mental health advocate" or even "life coach", but "licensed social worker" and "licensed therapist" seem to be untrue.
I'm sure Karamo is great at what he does (usually. He's done a few things that I find questionable on the show), but seeing someone who *maybe* got a BSW in 2002 and isn't licensed to practice in the state you live in is very different to seeing a psychologist (who has a PhD) or a psychiatrist (who has an MD). I think it should be made clear to the viewers when the hero is referred to actual therapy.
JVN is happy to admit when he needs to call in a barber or a loctician because he's not qualified to give the cut the hero needs. Antoni will call in chefs in particular specialties if he needs support because that type of cuisine isn't his forte. Karamo should be willing to publicly do the same.
