In ch. 143 Yuji told himself that he knew Megumi would suffer as long as he was around because he didn't know what Sukuna wanted from Megumi. He couldn't even open his eyes to look at Megumi when he thought about him suffering.

But after Sukuna got to Megumi, Yuji said that he was suffering. This adds so much more to his persistence in trying his hardest to stay away from him. He probably didn't want to admit it back then, but he must've had a feeling that if Megumi was in pain, he would be in pain along with him.

Yuji already felt the pain of what Sukuna did with his body without his consent and he couldn’t stand the idea of Megumi going through the same thing. If he couldn't even open his eyes when he thought about Megumi in ch. 143, just imagine how he must’ve felt having to experience Sukuna forcefully feeding his finger to him. And it’s also the fact that he had no control over his body and was forced to witness with his own eyes, watching Megumi suffer because of him. The one thing he wanted to prevent.
