Tricuspid Heart Valve
There is only one place in the human body with three duplicate heart valves, called the tricuspid valve. The valves of the heart are structures which ensure blood flows in only one direction. They are composed of connective tissue and endocardium (the inner layer of the heart). There are four valves of the heart, which are divided into two categories: Atrioventricular valves: The tricuspid…

I had snakebites briefly when I was 14..? but had to take them out because my a nurse at my cardiologists office told me due to my heart defect I was at risk for endocarditis (we were under the impression she had asked him herself?). When I went in for my regular check up a year or two later my cardiologist said it actually wasn’t a concern and they had even stopped recommending preventative antibiotics for teeth cleaning as well.

Turning in the cardiac monitor today. The report from the echo showed mild mitral valve and trace tricuspid regurgitation- which isn’t a huge deal in the present, but can worsen over time and lead to heart failure…which runs in my family. I had to press the button on the holter monitor several times for noticeable arrhythmias when I lay down at night. Waiting to speak with the cardiologist. Man I hope I’m ok. I’ve always been physically healthy. It’s the mental health I struggle with- although the heart stuff could explain the intense physical symptoms I get during anxiety. On the other hand, life sometimes surprises us with shit- I can’t be mad at my body for something I was probably born with. It has weathered me through a lot, and seeing my heart on the screen during the echo- with the tenacious little valves opening for the streams of blood, something it has done since before I was born- gave me a new appreciation for my body. All I can do is continue to take good care of it. For about a month I’ve been working with a health coach that is provided by insurance and my physical health has improved. I’m going to keep chipping away at it. I’ll be ok.

Tricuspid valve replacement is a critical heart surgery often performed when the tricuspid valve, located between the right atrium and right ventricle of the heart, becomes dysfunctional. This condition, known as tricuspid valve disease, can result in severe health complications, including heart failure if left untreated. In Mumbai, one of the most trusted names for tricuspid valve replacement surgery is Dr. Vishalkhullar, a highly respected cardiothoracic surgeon with years of experience in advanced heart procedures.
Understanding Tricuspid Valve Disease
The tricuspid valve plays a vital role in ensuring blood flows in the right direction through the heart's chambers. When this valve malfunctions—due to congenital defects, infections, or other heart conditions—it can cause blood to leak backward (regurgitation) or obstruct blood flow (stenosis). Symptoms of tricuspid valve disease include fatigue, swelling in the abdomen or legs, and irregular heart rhythms, all of which can severely impact the quality of life.
Expertise of Dr. Vishalkhullar
Dr. Vishalkhullar in Mumbai is renowned for his expertise in performing tricuspid valve replacement surgeries. With an extensive background in cardiothoracic surgery, Dr. Vishalkhullar utilizes the latest medical technologies and surgical techniques to ensure the best outcomes for his patients. His approach is patient-centric, focusing on understanding the unique needs of each individual and providing customized treatment plans.
The Procedure
Tricuspid valve replacement involves removing the damaged valve and replacing it with a mechanical or biological valve. The choice of valve depends on several factors, including the patient's age, lifestyle, and overall health. Dr. Vishalkhullar carefully evaluates these aspects to recommend the most suitable option for each patient. The surgery is performed under general anesthesia and typically involves a heart-lung bypass machine to maintain circulation during the procedure.
Post-Surgery Care and Recovery
Recovery from tricuspid valve replacement surgery requires careful post-operative care, which Dr. Vishalkhullar and his team meticulously oversee. Patients may need to stay in the hospital for several days to monitor their heart function and manage any complications. Rehabilitation, including a tailored cardiac exercise program, is often recommended to help patients regain strength and return to their daily activities.
Why Choose Dr. Vishalkhullar?
Choosing the right surgeon for tricuspid valve replacement is crucial for a successful outcome. Dr. Vishalkhullar's commitment to excellence in cardiac care, combined with his compassionate approach, makes him a top choice for patients in Mumbai and beyond. His dedication to ongoing research and staying updated with the latest advancements in cardiac surgery ensures that his patients receive the highest standard of care.
For those seeking expert care in tricuspid valve replacement, Dr. Vishalkhullar in Mumbai offers a combination of experience, skill, and compassionate care, ensuring the best possible outcomes for heart health.

Archovember 2024 Day 27 - Tanystropheus longobardicus
Tumblr media
Our last non-archosaur archosauromorph of this Archovember (say that 5 times fast) is Tanystropheus longobardicus. This archosauromorph lived in the Middle Triassic of what is today the border of Italy and Switzerland. It’s most recognizable feature is its long neck, made up of 13 lengthened vertebrae, that stretched out longer than its body and tail combined! What it was doing with this long neck is the subject of debate. The neck was not very flexible, made of long, stiff vertebrae, though it was lightweight and stabilized by tendons. Tanystropheus was likely a fish-eater, with T. longobardicus in particular being equipped with three-cusped teeth suited for holding onto slippery prey, and fish scales and cephalopod hooklets have been found in the stomach region of some specimens. Its body did not seem to have many adaptations for swimming, though its muscular hindlimbs and tail may have allowed for short bursts of active swimming in shallow water. It’s hypothesized that Tanystropheus was a semi-aquatic ambush predator, either using its long neck to reach across the water and snap up fish like a heron, or waiting in the water like a turtle, its body unseen and long neck appearing like an innocent branch.
Tumblr media
Tanystropheus longobardicus was the smallest of the known species of Tanystropheus. It lived alongside the larger, bulkier T. hydroides, while even larger Tanystropheus specimens have been found in Germany and Poland. Fossils have also been found in Canada and China, meaning this genus may have had a worldwide distribution. But T. longobardicus in particular lived in the Besano Formation. It was set apart from the larger T. hydroides by its small size, narrow triangular head, and tricuspid teeth in the back of its skull. Meanwhile, the larger T. hydroides had a broader skull and long interlocking fangs. The two species likely did not compete, as T. hydroides would have preferred much larger fish than T. longobardicus. But these were not the only tanystropheids in the Bessano Formation. They would have also lived alongside the smaller, shorter-necked, terrestrial tanystropheid Macrocnemus bassanii. On land, Tanystropheus longobardicus would have had to worry about the early rauisuchian Ticinosuchus. In the sea, Tanystropheus would have had to look out for the nothosaur Nothosaurus, large thalattosaurs like Askeptosaurus, large predatory fish like Birgeria, and even large icthyosaurs like Besanosaurus and Cymbospondylus. It may have also come across smaller icthyosaurs like Mixosaurus, Phalarodon, and Wimanius, as well as placodonts like Cyamodus and Paraplacodus, pachypleurosaurs like Serpianosaurus and Odoiporosaurus, and other reptiles like Silvestrosaurus, Helveticosaurus, and Eusaurosphargis. It could have eaten small fish like Aetheodontus, Cephaloxenus, and Luganoia, and even small coelocanths like Ticinepomis.

You're at the university, sitting in the class waiting for the anatomy lesson to start when the professor shows herself literally topless (actually, she's always been a little "over the top") and writes on the board the topic for today: the cardiovascular system.
She sits on a footstool in front of the class and you can clearly see that, painted on her chest, there's an anatomically correct drwaing of her own heart with some numbers. She starts with a simple explaination of the anatomical placement and structure of the heart and that there are five points in which you can listen to the sounds generated by the organ to assess its valves proper functioning: the auscultation points of the heart.
While explaining she writes them down on a paper board prepared next to her.
"The number one you can clearly see on my chest is the place to listen to the aorta valve which I remind you is placed between mine -and yours- left ventricle and the rest of your body. You can find it on the second intercostal space, right side of the sternum. Number two represent the place to listen to the pulmonic valve that separates the right ventricle to the pulmonary circulation. This time, again second intercostal space but on the left side of the sternum. The Erb's point can be found under number 3 where you can listen better to the S2 sound made by my aortic valve. You can find it on the third intercostal space, left sternum border. The number 4 identifies the tricuspid valve, which has three leaflets and it's placed between my right atrium and right ventricle and you can find it on the fourth intercostal space, left sternum border. On the number 5 there's the mitral valve, two leaflets this time, that in me it's a little leaky and in all of us separate the left atrium to right atrium. This point is placed on the fifth intercostal space on the mid clavicular line, a little more distal from the sternum than the other points where my left breast makes it harder to find it...."
While she's still talking your immagination starts flying. The drawing on her chest disappears and you can clearly see through her skin and bones her heart beating while your ears are filled by the strong sound of its beats, interrupted sometimes by premature ventricular contraction induced by her leaky mitral valve and the stress she's obviously experiencing while holding a lesson topless in front of the whole class. Whith a knowing look she invites you to take a listen...
Your daydreaming is suddenly interrupted by her saying some magical words:
"Ok now, take your stethoscopes and come here! On turn you are going to carefully auscultate me on each point and describe on fine details what you can hear cause something similar will be a topic for the exam!"
The dream comes true.

Roommates Ch.2
It's the middle of the night when I hear a faint knock on my door, awaking me from my sleep. I groggily open my eyes, feeling my heart thrum gently at 94 bpm, unaware of who could possibly be knocking at this time. I hear the door knob jangle ever so softly. Shit. Did I lock my door? I don't think so.... I've never locked it before unless I'm leaving. Shit. The door's unlocked.
I hear the hinges creak as the assailant enters my room, opening slowly so as to not make any noise. My heartbeat, already fast, picks up even more, slowly beginning to squeeze harder. I hold my breath which in turn further accelerates my the pounding, and I curse my reactive heart. What if she's beating loud enough for the person to hear? Do they know I'm awake?
I wait with baited breath, as I slightly shift my eyes in the direction of the noise. I see a rough outline of a man, taller than me by at least a foot maybe, with lean muscular arms hidden beneath a loose shirt.
It's.... my roommate? What's he doing inside?
He meanders to my table, and I hear the contents getting gently pushed around....he's looking for something. My wallet? No.... I've never kept my wallet out in the open like that.
"...shit...where is it?" I hear him curse under his breath. "...aha!" He whispers, loud enough to have woken up anyone who could have been sleeping. If he was level-headed, he'd know that any normal person would've woken up by now. My fear slowly turns into curiosity as I wonder, what was he looking for in my room?
I see him fiddle with whatever item he's found for a while, and he slowly tip toes his way to my bed. My heart which had just started to calm down, starts pounding in my chest. What is he going to do with me? Should I scream?
I feel a dip form beside my and I close my eyes shut, hoping he's buying my act of sleeping, praying my heart isn't vibrating the mattress. I hear soft clacking sounds and rubber, and feel my shirt getting pulled up.
Oh God.... Am I in danger? And I'm not wearing a bra right now.... What does he want to do with me?
Cold fingers touch my torso and I do everything in my power to not flinch. The fingers gently slide over my stomach, slowly making his way to the space between my breasts.... His gentle caress displays no malice. For some strange reason, I feel safe.
His fingers gently press into my sternum and suddenly I remember my heart, beating frantically in anticipation. Is he...feeling my heart?
"....there you are. Found you" he mumbles under his breath. I suddenly feel something cold and circular press into my tricuspid valve.
It's a sensation I'd know anywhere immediately, one which has provided me with comfort and pleasure.
It's my stethoscope.
He's stething me.
He inhales sharply and stays absolutely still. I imagine the sounds of my drum permeating through him. pounding so loud at this point it could deafen his ears. What does he think of her? Does he like her? Is she singing for him? I too want to listen to her so badly.
My heart contracts more forcefully, and I feel the steth bouncing off my chest, pushing away his fingers with each pump, thudding with such intensity.....I've never felt her beat like this. She's never done this for me, but for him she suddenly changed..... What is he doing to her?
What does he think of her??
"......Wow. You're so.....so beautiful" he mumbles.
My heart throbs in my clit, and I feel a familiar wetness between my legs, pleasure coursing through my veins, fueling my pleasure with each hammering beat inside me, pounding so hard I can feel it down. My fingers itch to touch myself, to see how far I can push my heart, show him just how desperate my little slut of a heart can beat for him. But I can't, not right now. According to him, I'm still "asleep"
"Christ.... your heart..... she's pounding so hard in my hand"
Yes I know! Please, please praise her more!
"ughh..... I wish you could feel what these beautiful beats are doing to me... SEE what it's doing to me..... No... no I need to take care of this."
He suddenly gets up, bed creaking loudly, still not expecting me to be awake. He drops my precious steth into the chair and in a hurry makes his way outside. Just before he softly shuts the door, from the corner of my eye, I see a bulge in his pants.
Did I do that to him? Did my attention whore pump do this to him?
Will he.....come by again?
As soon as I hear my door click, my fingers plunge into my pleasure zone, sliding inside so easily with how wet and dripping I was. I cover my mouth with the other hand, unable to curb the moans peeling through my entire body as I am hit with waves of pleasure, chest heaving heavily as beads of sweat form. My needy heart, devoid of the sudden attention cries out for him, slapping inside messily, squeezing herself dry with each powerful beat till the point my chest hurts.
I am filled with the need to hear him, to properly introduce him to my heart, and to properly help him through with what my heart does to him. It's the least I can do.
