everytime i hear about the USA healthcare system i lose my mind a little. i live in a developing nation that has universal healthcare and recently had to go to the government dentist to remove my wisdom tooth. Here's how that went:

First Appoinment:

Estimated cost: RM 15 (about 3 USD for the registration & appointment card) + RM 15 (about 3 USD for medicated mouthwash, antibiotics for an infection, and painkillers) = RM 25 (which is about 5 USD)

Amount of time spent: went in at 8 am was done at about 3 pm (there was a lunch break from 1-2pm)

Procedures done: Consultation and Xray (Yes, the x-ray is free)

Surgery was scheduled for 3 weeks away

Surgery day

Wait time before surgery: about 1 hour (I was early)

Surgery prep: 15 mins

Surgery itself: less than 1 hour

Amount of medical staff: 2 nurses, 2 specialist doctors (turns out the tooth was a lot deeper than expected), 2 dentistry students

Cost of medication after: RM25 (about 5 USD for way more antibiotics, painkillers, mouthwash etc)

Next appointment was set for 1 week to remove stitches

You may have noticed I left out the cost of the surgery itself. You want to know why?

It was free. 0 dollars.

Basic extractions like wisdom teeth were supposed to be handled by the dentistry students (they were supervised by the professor at all times), but because my teeth were so weird, the professor stepped in with his colleague which made my surgery a classroom - which made my surgery free.

OK but what if the professor did not need to step in? It would have been RM20. About 4USD.

And this wasn't even a fully governmental hospital, it gets even cheaper. I just went to the semi government one because it was closer.

Would it have been faster if I went to a private clinic? Probably yeah! I would be scheduled for a surgery in days rather than weeks. It would have also cost a lot more than I was willing to pay.

It wasn't an urgent procedure, if it was urgent, I would have gotten an appointment immediately.

[I once got a biopsy 2 days after doctors found a weird lump. The biopsy cost me RM20 (about 4USD). ]

Would it have been faster at a private hospital? Yeah, it would. But it's way cheaper to go for a public one. I would even argue that I've had better doctors in the public healthcare system than the private one.

There's also a price cap on prescriptions. If you have a prescription, the price is capped at a certain number for a month. So for example, if I get a prescription for mental healthcare, I didn't have to pay more than RM40 (about 8USD) for the whole month. It didn't matter how many different medications were on it or what the doses were. The price is capped per prescription.

Okay but what about medical insurance? Well, it would cover BOTH private and public hospitals.

Is this universal healthcare system free from criticism? Hell no.

Some medications aren't subsidised and can cost a lot of money. The process to get it on the subsidy list is a lengthy one.

A lot of the time, people with disabilities face problems getting support and access.

Wait times can be too long for some people, especially with procedures that have a wait list.

People in remote and rural areas have a lot of trouble getting access to these services, especially if they need a specialist

Mental healthcare is still very limited

And so much more!!!

But would I trade universal healthcare for a fully privatised one? Fuck no.

A working public healthcare system isn't a myth. It's not perfect and should be improved. But it works, and it's hell of a lot better than a fully privatised system.

"Universal Healthcare doesn't work, sure it's more affordable but the often months long wait times for appointments are ridiculous!"

Yeah you're right, the system where we all just ignore our health out of fear of financial destitution is much better. I mean I haven't been to a primary care doctor since they were called my pediatrician, but if I can ever afford to go, golly it'll be so fast.

This is the fourth book in the series and is very good.

Frank Gardner uses all his experience as a correspondent for the BBC on all matters featuring the Secret Service and matters concerning the safety of our country in these troubled times internationally. You know that his stories are based on true facts.

This book is set in Taiwan, that small independent country which is a relatively small island in the China sea, divided by the Taiwan Strait from mainland China and claimed by China as belonging to them. It is a flashpoint as it is supported by USA, UK and Australia, much to China’s annoyance and is where World War 111 could easily erupt.

The story features Dr Hanna Slade who has been persuaded by England’s Secret Service to visit Hong Kong to collect a tiny flash drive from a spy, which contains secret information about China’s plans to invade Taiwan and secure the factory making micro chips  for their own use. All goes well and the hand over is completed in a small café. Hanna then goes into the toilet, chews a stick of chewing gum and hides the tiny flash drive in some of the gum and then fits it into a hole in her teeth where she had a wisdom tooth removed.

When she leaves the café though she is immediately accosted and whisked away in a car.

Back in London, MI5 find they have lost contact with Hanna and so send two Secret Service officers, Luke Carlton and Jenny Li to try and find Hanna and her important package

So the story continues with the hunt for Hanna Slade. Meanwhile the tension is building regarding Chinas intension to invade Taiwan.

A great story full of action and tense moments with a surprising little twist at the end.

Well recommended if you like thrillers.

My teeth - A rant

As a young child, I sucked my thumb, actually fuck it, I sucked my thumb until I was 25, shocking, I know, and I only stopped because at 25 I got braces and it’s a bit hard to suck your thumb with a wire in the middle of your palate. 

Because of all the years doing this, my teeth were absolutely deformed. At the age of 10, the dentist told my mother I needed braces, I needed braces and not to have a second row growing tooth removed because the braces would fix that tooth and pull it back to it’s rightful place. Instead of listening to the dentist, my mother took me to another dentist to have that tooth removed (I am missing one canine because of this).

My teeth were deformed, sticking outward, some were twisted and some were growing in second row. Because my teeth didn’t have space, the pressure on them caused my back molar to break apart. So, imagine having a perfectly healthy tooth root on your gums, and a piece of misshapen broken leftover of a tooth attached to it.

But at no point my mother thought: Maybe I should do something about my child’s teeth.

Now, this isn’t USA. You don’t need health insurance to get braces, and getting braces isn’t worth a kidney, and even though braces through the private clinics are expensive, you can get braces for free through the Social Services as long as you had the requisites, and I had a right to those braces.

Because of the shape of my teeth, I had a severe lisp and other speech impediments, I also had cavities from never having gone to a dentist. But now, getting (free) dental was a luxury we could not afford, my mother made me believe.

The fact I couldn’t get braces to fix all my speech, palate and dental problems was something that caused me a lot of suffering growing up and I was often mocked for my lisp, the shape of my teeth and others (another thing to add to all the things I was bullied over).

When I reached 25 more or less, I started working as a social journalist, and it’s a bit hard to be a journalist and have a speech impediment. I decided to go to speech therapy, and my speech therapist was very clear about one thing: You can only fix you lisp if you fix your teeth.

Because I was 25 and already working, as I recieved minimal wage at the time, I still had a right to state comparticipated dental health, but the waiting lists were huge and I could wait years to get a dentist. It took me whole year to go to a gynecologist back then. When I finally went there, the vaginal infection I wanted to go to the gynecologist for had already healed over, returned and healed over again. I didn’t want the same to happen with my teeth.

I went to a clinic that was known for giving the option for the clients to pay the whole value at portions according to their possibilities. I still had 3k euros to pay for my braces, but instead of paying 3k in one go, I was allowed to pay 100 euros a month until I paid the 3k in full.

My mother still didn’t want me to get the braces and actually got angry over me getting braces because “it’s a waste of money, you’re still going to suck your thumb and fuck up your teeth.”

Guess what, I no longer suck my thumb and my teeth are finally fixed.

Getting braces meant removing two teeth. One ingrown molar on the bottom row, and one okay tooth on the top row because it had a cavity.

Three years, almost four later, my braces are out and my teeth are good.Now I was going to finally fix my broken molar this month.

Now, my left canine has broken.

Now, to anyone else, it’s just a tooth. But to me, it’s not.

PROTESTERS!!

As someone from the UK who is unable to protest due to location (unlike Dominic Cummings, I don’t feel comfortable travelling far from home during a pandemic, even if my closest protest is only 35 miles away) I want to share some first aid advice for those of you that can attend. As a St John’s Ambulance volunteer, I hope I can give you some information that will help if you get injured. I really hope the UK protests won’t get anywhere near as violent as the ones in the USA, but it’s better to be ready. I hope you don’t have to use this advice. 

CARRY A BASIC FIRST AID KIT WITH YOU - you never know if you’ll need it. 

EYES:

- If you sustain an eye injury because of a foreign object or blunt force trauma, try to minimise eye movements and cover the injured eye with something non-fluffy (gauze is ideal, but in a worst-case scenario a t-shirt would be fine), then seek medical attention. Do not attempt to remove anything that is stuck in the eyeball. 

- At protests in the USA, police have used substances like pepper spray and tear gas to deter protesters. If the situation escalates and you get sprayed with either of these, wash from the inner corner of the eye outward so the substance is running away from the eye. It will be painful, but the sooner it’s out of your eyes, the better. 

NOSE/MOUTH:

- If you have a nosebleed, tilt your head forwards and pinch the soft part for ten minutes. If you are losing a lot of blood (ie the nosebleed lasts longer than 30 minutes) or the blood appears watery and thin (which can indicate a more serious head injury) seek medical attention. Do not tip the head back as blood running down the throat can induce vomiting. 

- If you lose a tooth, try to keep it somewhere safe if you can. Roll a gauze pad (or a strip of fabric), place it in the socket and bite down, this will control any bleeding. Get yourself and the tooth to the hospital ASAP, as the sooner you get there the more likely it is that the tooth can be replanted. 

HEAD: 

- Head wounds bleed a LOT because there are lots of small blood vessels close to the skin, so even a minor scrape can look very serious. Be careful when examining someone with a scalp wound as it may be linked to a more serious head injury. Apply firm pressure with a dressing (or anything clean that’s available otherwise) to control the bleeding and minimise the risk of the person going into shock. Secure what you’ve got covering the injury and get the person to medical attention. 

FOREIGN OBJECTS: 

- If someone has an injury where a foreign object has become embedded in part of the body, do not remove the object. Removing the object could cause more damage, and could also cause more bleeding as the object may be blocking blood flow from damaged blood vessels. Apply pressure in either size of the object and build up padding around the item until it is high enough to bandage over the object without pressing it further into the wound. If the object is too large to bandage over, bandage above and below the object to keep it in place. Get the person to a hospital. 

- If an object has caused an open wound, DO NOT PUT YOUR FINGERS IN IT. I have seen multiple people on twitter suggest putting fingers or thumbs into an open wound to apply pressure. This is dangerous, could cause more damage than the initial injury and could also lead to infection. Cover the wound and apply steady, firm pressure to it until you can get access to medical attention.

HEAT EXHAUSTION: 

- It’s warm out there so if you’re planning on spending a whole day outside protesting, be careful. If you start feeling dizzy or confused, have a headache, feel nauseous, have cramping in the muscles or notice that your breathing is very rapid, you may be experiencing heat exhaustion. This can lead to heatstroke, which can kill in extreme cases. Get somewhere shady, drink plenty of water and give yourself time to recover. If a person with heat exhaustion appears to deteriorate very rapidly, call for emergency help. 

SEIZURES: 

- Anyone can have a seizure and the heat can be a factor in causing them. If someone has a seizure, clear the area around them as best as you can to prevent them injuring themselves during convulsions. DO NOT put anything in the person’s mouth while they are seizing and DO NOT attempt to restrain them - you will only end up doing more damage. Try to cushion the head if possible and loosen any tight clothing around the neck (ie undo a couple of shirt buttons). If the person does not regain consciousness once the seizure is over, put them in the recovery position(see video below). This means that, if the person vomits, they will be much less likely to choke on it; you can maintain a clear airway and monitor the person carefully from there.

Call the emergency services if: 

- the person has more than one seizure

- the seizure lasts for more than five minutes 

- the person has never had a seizure before (ask them when the wake up)

- they remain unconscious for longer than ten minutes 

- they have sustained a significant injury during the seizure
