THYROID NODULES- TYPES OF THYROID NODULES AND TREATMENTS
Tumblr media
Thyroid & Endocrinologists in Nashik - Dr Chaitanya Buva
A thyroid "nodule" is a small, localised "swelling" or "lump" within the thyroid gland. In the occurrence of many thyroid nodules, a "multinodular goitre" is the result. In older persons, multinodular goitre is not unusual and frequently goes unnoticed.
Even while the majority of persons affected are unaware that anything is wrong with their thyroid gland, single nodules are also frequent, affecting about 5% of the population.
If thyroid nodules are found by ultrasonography, the population's overall thyroid nodule prevalence is higher. Single thyroid nodules can have a variety of origins.
The main justification for looking into thyroid nodules is cancer, despite the fact that it is rare.
various thyroid nodule types
The solitary thyroid nodule typically falls into one of four categories: a fluid-filled cyst, a degenerating benign tumour/adenoma, a slowly growing adenoma, and a small fraction (between 5 and 10 percent) is malignant.
Normal thyroid function and no hyper- or hypothyroidism are present in people whose thyroid gland is functioning normally throughout.
Features of the Clinic
Small and painless thyroid nodules are the norm. No pressure effects on the neck are brought on by them. The majority of patients are not even aware of the swelling. During routine medical exams or while they are being checked for other diseases, their doctors discover the swelling. If they are big enough, thyroid nodules may typically be felt through the skin as solid, smooth bumps. During a detailed examination of the neck region, the majority of nodules larger than 2 cm are palpable. In most cases, ultrasonography is the only way to find smaller nodules. It feels normal throughout the gland.Cancerous thyroid nodules may be firm and accompanied by swollen lymph nodes in the neck if the malignancy has spread. To differentiate between benign and malignant nodules, a physical examination alone is insufficient.
Thyroid biopsy and laboratory evaluations
The ultrasound, which establishes size, form, and whether it is solid or liquid, is one of the most crucial tests for nodules. The next step is to do a tiny needle biopsy of the thyroid nodule in nodules with normal or elevated TSH values. The thyroid gland's cells and fluid are taken out, and a pathologist examines them to determine whether they are benign or cancerous. A thyroid cyst can also be pierced with a needle, and fluid is then sucked into the syringe. Rarely, pus will be extracted, revealing the nodule to be a thyroid abscess. Next, a thyroid scan and iodine uptake in the nuclear medicine division are performed if a nodule is connected to decreasing TSH levels.
The manner in which a thyroid nodule is treated depends on its type. The mainstay of non-malignant nodule treatment is observation. The basic method of follow-up for these nodules is observation. Repeating the biopsy and considering surgery are options if the nodule grows in size. It is also necessary to periodically monitor TSH levels.
The nodule must be removed if the examinations reveal cells that are "suggestive of malignancy." The rest of the thyroid is typically removed during surgery and extra treatment may be required if a pathology test reveals the nodule to be cancerous. The entire thyroid gland is removed and additional therapy may be suggested for nodules whose biopsy reveals "malignant cells" (please see the thyroid cancer health advice for more information).
Preventing recurrence through treatment
Prior until this, it was believed that thyroxine therapy would stop the nodules from growing larger and/or returning in the residual tissue. The data at hand do not support thyroxine therapy in the absence of an elevated TSH level.
Benign nodule treatment
Thyroid hormone (such as thyroxine) can be used to treat benign thyroid nodules so that TSH is "turned off," hopefully causing the nodule to shrink. A six-month checkup is required for patients receiving this type of treatment. There are no safety concerns as long as the nodule does not grow. The nodule should be removed if it continues to grow while being treated with thyroxine since this indicates that it may have developed into a cancerous tumour. Additionally, it should be noted that fewer benign nodules are treated with thyroxine therapy because the majority of them do not shrink in response to it. The drug thyroxine has little effect on cysts.
The Thyroid and Neck Area Has Been Radiated
For a number of benign disorders affecting the thymus, adenoids, tonsils, and skin in the 1940s and early 1950s, many kids underwent X-ray treatment.
This irradiation had an impact on the thyroid gland, it was later discovered. According to some research, up to 25% of these individuals later got thyroid nodules, and among them, a third eventually had thyroid cancer.
Many children received X-ray treatment in the 1940s and early 1950s for a variety of benign conditions involving the thymus, adenoids, tonsils, and skin.
Later it was determined that this irradiation affected the thyroid gland. A third of these people eventually developed thyroid cancer, and some studies suggest that up to 25% of those people developed thyroid nodules in the future.

HOW DOES INSULIN WORK?
Often a doctor is asked How does insulin work?
Insulin is a hormone generated by the pancreas that acts as a key to aid in the use of glucose (sugars) from the blood.
Tumblr media
When a diabetic patient adheres to a rigorous DIET, the glucose production in the blood lowers, resulting in a lower challenge.
When a diabetic patient EXERCISES on a daily basis, it aids in the more effective utilization of sugars and, as a result, improved diabetes control.
If despite these attempts, blood sugar levels rise, doctors can aid by using MEDICATION to boost insulin secretion or by administering direct INSULIN injections to regulate the abnormal rise in blood sugar levels. Looking for Diabetes Treatment? Dr. Ashutosh Sonawane is a Consultant Adult & Pediatric Endocrinologist & Diabetologist in Nashik at SARTHAK HEALTH CLINIC Nashik.

Signs And Symptoms Of Hyperthyroidism
Irregular heartbeat, sweating, irritability and unexplained weight loss are all signs and symptoms of hyperthyroidism. This conditions accelerates your body’s metabolism significantly. Excess iodine in your body can lead to hyperthyroidism.
Hyperthyroidism occurs when your thyroid gland releases too much hormone. The thyroid gland is a butterfly-shaped gland situated in the front part of your neck, precisely below the Adam’s apple. The hormones secreted by the thyroid gland controls the metabolism of the body and how the body uses energy. Read on to know more about hyperthyroidism signs and symptoms.
According to a 2014 study conducted in Curie Manavata Cancer Centre in Nashik, Maharashtra, the prevalence of thyroid disorders among 16,933 patients analyzed was 0.8%. The following are the results from the same study.
Thyroid disorders were more prevalent among women, affecting 66.4% when compared to men (33.6%)
Thyroid disorders are more prevalent in the age group of 30 to 70 years.
The importance of thyroid glands
The thyroid gland produces two primary hormones namely tetraiodothyronine (T4) and Triiodothyronine (T3). These two hormones govern how the cells use energy. The metabolism of the body depends on how much of these hormones are released.
Apart from metabolism, the thyroid hormones have the following functions:
Maintaining the heart rate
Functioning of other prime organs
Maintenance of bone health
Smooth functioning of the menstrual cycle
Maintaining the muscle metabolism
Hyperthyroidism occurs when the thyroid gland secretes too much of T3 hormone or T4 hormone or both. Excess production of the hormones will speed up all the metabolic processes that are aided by the thyroid hormones. The intensity of symptoms depends on the amount of T3 and T4 hormones secreted.
The hormone called Thyroid Stimulating Hormone (TSH) produced by the pituitary gland has an important role in the production of T3 and T4. As the name suggests, TSH stimulates or activates the thyroid gland to produce T3 and T4 hormones.
When the levels of T3 and T4 are normal, the TSH level is maintained low. An abnormally low level of TSH indicates hyperthyroidism or high secretions of T3 and T4. An abnormally high level of TSH indicated hypothyroidism or low secretion of T3 and T4. This is the most common method of hyperthyroidism diagnosis.
The below table represents the optimal level of all the three thyroid hormones in a normal person and in people suffering from hypothyroidism and hyperthyroidism.
ConditionTSH Level  (in milliunits per liter)Free T3 Levels (in pmol per liter)Free T4 Levels (in pmol per liter)Normal0.4 to 43.0 to 6.012.0 to 22.0HyperthyroidismLesser than 0.4More than 6.0More than 22.0HypothyroidismMore than 4Less than 3.0Less than 12.0
Hyperthyroidism test involves the measurement of all the three thyroid hormones and sometimes additional factors such as Thyroid antibodies (TPO), Total T3 and Total T4.
The causes of hyperthyroidism
Hyperthyroidism occurs due to a host of factors. Among the most common causes is Graves’ disease, an autoimmune disorder in which antibodies released by the body stimulate the thyroid gland to produce too much thyroid hormones. Graves’ disease runs in the family and has a genetic link.
The other common causes of hyperthyroidism include:
Excess iodine in the body which is a prime ingredient in T3 and T4 hormones
Tumors in the ovaries or testes
Thyroiditis, a condition in which the thyroid gland is inflamed and as a result of the thyroid hormones T3 and T4 leak from the gland
Benign tumors in the thyroid or pituitary gland
Large amounts of tetraiodothyronine in the body that accumulates from the supplements or other medication.
Hyperthyroidism is a condition that can be managed through the lifetime with right diet and exercises. Hyperthyroidism natural treatment has shown promising results with some patients.

Common signs and symptoms of hyperthyroidism
The excess secretion of thyroid hormones, T3 and T4 lead to a high metabolic rate. When metabolism speeds up in the body, it enters a hypermetabolic state. The heart beats faster and as a result, there is an elevation in the blood pressure. A hyperthyroid patient may experience profuse sweating and will always have a very low tolerance towards heat.
When you are suffering from this disorder, you can suffer from any or all of the following signs and symptoms of hyperthyroidism:
Hand tremors due to rapid heart rate
Rapid and unplanned weight loss
Frequent bowel movements
Irregular menstrual cycle
Visible swelling of the thyroid gland
Bulging of the eyes and more protrusion when compared to the normal position (Exophthalmos)
The swelling of the thyroid gland can either be symmetrical or one-sided.
What happens when you show signs and symptoms of hyperthyroidism
When you suffer from this disorder, your body experiences many drastic changes. Keep a watch for the following:
Increase in appetite
Nervousness and anxiousness that occur unnecessarily
Poor concentration skills
Restlessness all through the day
Weakness and fatigue even after a good rest
Insomnia and difficulty in sleeping
Irregular heartbeat
Excessive itching of the skin
Sudden excessive hair loss
Change in the texture of the hair which turns fine and brittle
Mild breast development in men
Frequent nausea which may occur together with vomiting at times
When should you go see the doctor
If you are to experience the following signs and symptoms of hyperthyroidism you will need immediate medical attention:
Frequent dizziness
Loss of consciousness often
Very fast heart rate
Shortness of breath
All the symptoms mentioned above can be counted as hyperthyroidism symptoms in men. Recent research suggests that osteoporosis is common among hyperthyroid patients. Another research also throws light on the prevalence of clinical ocular features among children and young adults with thyroid diseases.
When hyperthyroidism is left untreated for long, it may lead to a condition known as atrial fibrillation. It is a form of arrhythmia that is quite dangerous and can lead to strokes and sometimes heart failure.
QUICK BYTES
Is Graves’ disease the same as hyperthyroidism?
Graves’ disease is an autoimmune disorder that can lead to the condition called hyperthyroidism. Graves’ disease occurs when the antibodies in your body act against your own cells and organs.
Is Graves dermopathy common is all hyperthyroid patients?
Graves’ dermopathy is a disorder in which the skin becomes red and swollen. It occurs when Graves’ disease is the underlying cause of a very active thyroid
Is thyroid scan required for the diagnosis of hyperthyroidism?
In some cases, thyroid scan may be required by doctors if the entire gland or just a segment of it is causing excess secretion of hormones.
When is a CT or MRI scan used for diagnosing thyroid disorders?
A CT or MRI scan is used when the doctor suspects a tumor in the pituitary gland.
Are there diet restrictions for hyperthyroidism?
Yes. The diet restrictions for hyperthyroidism are minor and not extensive.
