In the field of Manual Therapy, there are many techniques used to treat people in pain. These techniques involve pushing, pulling, and twisting skin and soft tissue. DNM uses techniques that change the state of the nervous system in the skin, spinal cord and brain to make it less painful and reactive.
Studies have demonstrated that touch has significant psychological and physiological impacts, with the nervous system regulating these effects. To effectively use Manual Therapy as a pain treatment, it's important to comprehend pain physiology through a social, biological, and psychological perspective. Research has indicated that establishing a non-threatening treatment environment for the patient is vital.
The skin is closely tied to the nervous system, which makes up around 2% of our body weight but uses 20% of our O2 and glucose at all times. Nerves do not respond well to sustained mechanical deformation, specifically compression and stretch. Dermoneuromodulating is a method that considers the nervous system of the patient to treat from "skin cell to sense of self." Techniques are usually light, and holds are slow.
DNM is a form of personalized manual care that targets nervous system signals to reduce discomfort and pain. This approach involves collaboration between therapist and patient to alleviate tenderness and improve well-being. Regardless of the damage in the affected area, DNM can effectively reduce pain.
The term DNM Stands for Dermo (skin), Neuro (nervous system) Modulation (a change from one state to another). It is a method of manual therapy/massage therapy used to change the state of the nervous system from a painful hyperactive state to a less painful and reactive one through receptors in the skin.
In Manual Therapy (MT), hundreds of techniques and methods attempt to approach treating people in pain. These maneuvers' speed, duration and force may differ, but the underlying mechanistic effects all follow the same rules. They can all agree that we push, pull and twist skin directly and soft tissue indirectly.
Lately, there has been considerable growth in research around the mechanisms of massage and other manual therapies, with a drift into the neuroscience involved in pain.
Two processes seem to be apparent when we look at the research:
First, the effects of touch have strong psychological and physiological effects.
The nervous system controls these effects.
If we use Manual Therapy as a treatment for pain resolution, we should look at understanding pain physiology within a social, biological and psychological framework.
What we know from studying the non-specific effects of physical medicine is that we should strive for a treatment environment that creates the least amount of threat to the person being treated; therefore, We should act, dress and keep the treatment space in a way that won’t put the patient on the defensive.
We know that we touch the skin primarily in manual therapy and that the skin is intimately tied into the nervous system embryologically as both arrive from the ectoderm.
We know that the Nervous System makes up around 2% of our body by weight but uses 20% of our O2 and Glucose at all times.
The nervous system carries impulses from the peripheral (and from within), which are processed in the spinal cord and Brain to create an output response. If the impulses are deemed dangerous enough, the output is pain.
We know that pain is a complex process that depends on contextual, psychological and biological factors that mostly happen without our conscious perception and that pain can’t happen without a nervous system.
The human body has 72 kilometres of nerves, intimately connected to the vascular system millimetre by millimetre.
Nerves do not respond well to sustained mechanical deformation, specifically, compression more than stretch.
Dermoneuromodulating is a method that attempts to take these facts as an underlying framework for an interactive, hands-on approach to treating the experience of pain. It considers the nervous system of the patient to treat from “skin cell to sense of self.” Techniques are usually light, and holds are slow. Limbs and trunk are positioned to affect deeper nerve structures in combination with skin stretch. This is done to potentially shorten and widen a nerve's container, thus reducing mechanical deformation of the nerve.
DNM is a method of manual care that puts the patient and their needs first. Instead of operating a recipe treatment, the treatment is an interaction between the patient and the therapist. The therapist and patient work together to find the areas that need attention and remove the tenderness and pain felt in that area. The focus is on changing the signalling within the nervous system to decrease discomfort. Pain does not happen in the muscles and other tissue but in the nervous system itself; therefore, whether or not the area of pain is damaged, we can reduce that pain with DNM.

Being an RMT through MTAM (Massage Therapy Association of Manitoba) we have to take continued education courses.
Taking a marketing and advertising course, one thing that really stuck out to me was creating a MISSION STATEMENT. What do I stand for? What do I want my clients to think about me?
For a while there I wasn't sure. I like to help people, I like to make people feel better than when they came in. I don't want to say I'm a Jack of All Trades by any means but I'd like to try my best with whatever problems you have.
I hope that when my clients leave their treatments, they think of me as someone who cares for them. Someone who will tell them the truth and be as transparent with them as possible when it comes to their treatments. I'm not afraid to say that I'm not sure what the next steps are when I really don't, and I hope they trust me enough to follow through with who I refer them to.
At the end of the day, I want my clients to feel better and get better. So maybe my mission statement will be: Ivy Monton RMT. Creating a space for you to grow. (?)

Aloha! My name is Jessica and I am an RMT who has been practicing massage therapy all over the world since 2004. I spent a big part of my life living in Europe and Hawaii working with high performance athletes from the Tour de France, Olympics and The American Windsurfing Pro Tour. I am a double medal winner of Freestyle Massage in North America for Canada and will be representing BC again this year in Montreal in May 2024 at the Canadian Massage Championships.
I also spent over 6 years in the hospitality industry working for hotels like the Fairmont and helping open new hotel spas like the Oak Bay Beach Hotel. I currently work in a clinic sharing my skills and experience with a clientele of over 200 patients. I’m an avid hot Yogie and love being in nature which helps me find balance in life:)

Trends in the RMT field.
I started my career as an RMT (registered massage therapist) back in 2012. Many changes have occurred since then, and this article I found on the RMTBC online magazine does a great job of outlining some exciting new trends.

The first trend in our field is the integration of technology. The article talks about the use of online booking systems, using virtual reality to enhance the massage experience for the client or patient, and also mentions the use of advanced equipment to target specific areas.
For me the most useful trend by far is the online booking system, specifically JaneApp (a locally based company) which is a super user-friendly booking system that saves me time when charting, booking, and taking payments.
Focus on holistic wellness is the second trend mentioned in the article. At the start of my career, my treatment approach would lean towards the orthopedic issues, such as injuries, chronic pain and postural improvement. Over the years we have learned that it’s not only the physical health that matters, but that focusing on mental health by using stress reducing techniques and approaches are the best way to ensure overall wellness.
The third trend is specialization in the field. RMT’s in BC are trained to be able to treat a variety of conditions, populations such as athletes, pregnancy or patients with special needs.
Many RMT’s are choosing to focus on a specific area that they are passionate about. I was a little surprised that the article uses the word “specialization”, as there is a bylaw from our governing body that clearly states that “registrants are not permitted to advertise any special accreditation or competency”.
There are four more trends listed and they are pretty self explanatory.
Increased research and evidence-based practice, growing acceptance in healthcare, continued education and professional development and emphasis on self-care. Each of these trends represent a change in the right direction for our profession.
As a part time RMT instructor with the hopes of one day teaching full time, these trends make me hopeful that my teaching career will be an ever evolving, exciting path. I will need to keep up with technology in the classroom, with current bylaws, new evidence, our relationship with other health care practitioners and most of all to take care of my own physical and mental health.

Hi. I was reading your post about unions going on strike and other unions, such as teachers and firefighters, potentially going on strike i believe.
As a US citizen I’m curious, does every industry in the UK have a union? If so, that’s mind boggling as it seems there’s so few industries in the US, that I’m aware of, that is unionized.
In the UK nearly all workers are allowed to join unions (exceptions for the police and armed forces, I believe) and therefore go on strike. There are lots of large national unions, that represent people who work in specific industries and will support them in national or workplace disputes.
Unfortunately, a lot of people don't join unions, but pretty much every industry will have at least one union they can join, if not multiple unions. And it does help in terms of national action.
It's actually pretty hard to call a national strike in the UK, but the RMT showed that even with anti-union laws the Tories put in place, it is possible, and it's had a bit of a domino effect.
Just to give you an idea of the unions balloting or involved in strikes right now (that I'm aware of).
RMT- represent transport workers, currently involved in a dispute around pay, conditions and redundancies for rail staff.
CWU- represent communication workers, currently involved in a dispute between royal mail and postal workers over pay and conditions.
RCN- represent nurses, they have just balloted and will be striking before Christmas be striking over pay and conditions.
UCU- represent university and further education staff, currently balloted and will be striking over a number of issues particularly in terms of increasing use of insecure low paid contracts for lecturing staff.
CBA- represent barristers who work in the criminal bar. They went on strike this autumn over very low fees for legal aid work and WON a 15% rise in fees!
FBU- represent firefighters and other rescue workers, balloting over pay currently.
NEU- represent all educations staff, balloting teachers over pay.
NASUWT- also represent teachers, balloting over pay.
NAHT- represent headteachers (principles) of schools, balloting over pay and school funding.
ASCL- represent school leadership staff, running an indicative ballot over school funding and pay.
I'm sure there are others too!
Right now the labour movement (meaning workers movement, not the Labour party) has seen a real resurgence- people are joining unions and standing up for themselves as the cost of living rises. CEOs and shareholders are continuing to take home megabucks, whilst many workers struggle to make ends meet.
You'll note that many of these unions are people who didn't stop working during the height of the pandemic as well- professions which have faced some really tough years and feel enough is enough.
The law in the UK is honestly really quite limiting towards unions, but national unions can still make change if that's what their members believe in.
I don't know what the law is in the US around national unions, but the key thing to remember is that they can't fire you all!
