Novartis set to overcome $14 billion patent cliff say analysts
Novartis’ pipeline is strong enough to overcome a $14 billion patent cliff as blockbuster drugs face generic competition in the coming years, analysts have said.
Following a briefing with management, a team of analysts led by Peter Welford said that the big Swiss pharma will be propped up by its psoriasis and infectious diseases blockbuster Cosentyx for some time.
Although the drug has lost market share to rivals in psoriasis, there is still more to come from Cosentyx thanks to potential new uses in ankylosing spondylitis, psoriatic arthritis and six new indications.
This will be helped by a new 300mg pre-filled syringe and auto-injector have halved administrations, although there is an intravenous option for administration by doctors treating US Medicare patients.
This is supported by the company’s portfolio of other approved medicines and revenues from its Sandoz biosimilars and generics division.
In the short term, the analysts predict “flattish” Q4 sales, although they noted that the company may benefit from cost savings introduced during the COVID-19 pandemic.
Jefferies expects that the company expects sales of $14 billion could be at risk by 2027 from patent expiries of Tasigna, Promacta, Jakavi, Gilenya and Entresto.
But the analysts said that sales growth will continue despite the headwind thanks to “underappreciated” drugs in the late-stage pipeline.
Four key drugs outlined by Jefferies have combined peak potential of $14 billion – the high lipoprotein cardiovascular disease drug pelacarsen, iscalimab for organ transplant and Sjogren’s syndrome, the rare disease drug iptacopan and the leukaemia drug sabatolimab.
Other important phase 3 drugs that are due to read out are Lu-PSMA in prostate cancer and canakinumab in lung cancer.
The company’s Sandoz division will also chip in with growing sales from its portfolio of biosimilars – cut-price near-copies of established biologic drugs that have been shown to be as safe and effective in rigorous tests and clinical trials.
Uptake of biosimilars in the US has been better than hoped, perhaps aided by hospitals focusing on cost because COVID-19 has curtailed income from surgery.

SMI REPORTS: SMi Group speak to James Mellman, Device Manager, Novartis, Co-Chair of Pre-Filled Syringes 2019
SMi Group are delighted to have James Mellman, Device Manager, Novartis, Co-Chair the 11th annual conference and exhibition on Pre-Filled Syringes & Injectable Drug Devices when it returns to Central London on 16th and 17th January 2019.
James is a device manager for Novartis in Basel, Switzerland. His main responsibilities include developing primary packaging systems for combination products and supporting new technologies. He has worked on a multitude of injection device platforms including pre-filled syringes, needle safety devices, auto injectors, and patch injectors. He has also helped to build and implement a renovated CCIT strategy for the Novartis Group.
Before Novartis, James worked at Pfizer in their Devices Center of Excellence in Cambridge, UK (2008-2011) and Nutricia in their Advanced Medical Nutrition Division HQ in Amsterdam, NL (2011-2014). He received his PhD from the University of Florida in Materials Science & Engineering in 2007 specializing in biomaterials.
In the run-up to the event, we caught up with him to discuss current industry hurdles and the upcoming conference. From the answers provided it is apparent that the growing usage of prefilled syringes and technological advancements in auto-injectors are key factors driving the market today.
As well as Co-Chairing Pre-Filled Syringes Europe alongside Anil Busimi, Strategy and Innovation Global Product Manager, SCHOTT will also be leading an industry-led panel discussion with GSK, and MSD. The event spotlight on regulation will also include an exclusive keynote address from the MHRA.

What is your area of expertise within the Pre-filled Syringes market?
“My expertise comes from working at Novartis since 2014. I worked on a number of projects for both development and commercial products, and this has allowed me to get to know the issues, supply chain, and best practices in the field. A great interest of mine is evaluating compatibility between drug products and their delivery system.”
What do you hope to gain from this meeting?
“I hope to learn from the great work from others who are presenting as well as the discussions that follow.”
